[Cardiotocography -- proposal of system solution (author's transl)].

A system solution with new technical details and new demands on space distribution is proposed for the use of antenatal cardiotocography and cardiotocographic monitoring of labour. Bedside equipment will be reduced and widely shifted to a centralised control room by introduction of the system. All installations are of high flexibility to leave room for future developments in medical engineering and for necessary services. -- Large-scale use of the new technique with capacity utilisation of the hardware involved will be ensured by appropriate upgrading of labour room personnel as well as by techno-organisational improvement of work and minimisation of expenses on diagnosis and monitoring. The measures proposed work according to expectation and lead to good perinatological results.

[Case history of acute fatty degeneration of the liver due to pregnancy-relevance to differential diagnosis and therapy (author's transl)].

Reported in this paper is the case of a patient, 19 years of age, with twin pregnancy who developed acute pregnancy-caused fatty degeneration of the liver with typical clinical symptoms in the 34th week of pregnancy, following nine weeks of hospitalisation for tocolysis treatment. The presence of a severe hepatorenal syndrome due to late gestosis, pregnancy cholestase or medicamentous intoxication is discussed for differential diagnosis. Suggestions are made as to high-dosage Partusisten long-time tocolysis which, possibly, might have been an additional hepatotoxic stress factor in concomitance with late gestosis.

Insulin response in skeletal muscle and fat cells of the genetically obese Zucker rat.

Isolated fat cells derived from 10-wk-old Zucker obese rats utilized substantially greater amounts of glucose per cell in the presence or absence of insulin than those from lean rats. Initial rates of deoxyglucose or 3-0-methylglucose uptake in fat cells from Zucker obese rats were also 5--10 times greater than those observed in cells from lean rats. However, while 240 microU/ml insulin elicited a maximal response in fat cells from lean rats, this dose of hormone was only about 50% as effective as 24 microU/ml insulin in stimulating glucose metabolism or hexose transport in obese rat cells. This apparent rightward shift in the dose response-relationship could not be adequately explained on the basis of decreased insulin receptors since (125I-) insulin binding per fat cell was increased 2.5--3-fold in obesity, while receptor density on the cell surface in obesity was decreased only slightly. Soleus muscles from obese Zucker rats exhibited decreased basal rates of D(5-3H)glucose conversion to glycogen and H2O compared to those of lean controls. While the percent increase in glucose metabolism due to a supermaximal dose of insulin was similar in soleus muscles of lean and obese Zucker rats, a blunted response to a submaximal insulin dose was observed in muscles from the latter animals. This rightward shift in the dose-response relationship was also observed when deoxyglucose uptake was monitored in soleus muscles from obese rats. Binding of (1251-) insulin to soleus muscles at a medium concentration of 57 microU/ml was significantly decreased in obese compared to lean rats. We conclude that (1) fat cells do not contribute to the insulin resistance of 10-wk obese Zucer rats since glucose utilization is higher in these cells at all concentrations of insulin tested, (2) obese Zucker rat soleus muscle metabolism is defective in two respects--imparied basal glucose utilization and a rightward shift in the insulin dose-response relationship with respect to hexose transport, and (3) this latter defect involving decreased sensitivity of muscle to insulin appears to result from a marked decrease in cell surface receptors for the hormone.

[Cardiovascular preventive medical check-ups in the large industrial plants of Erfurt. 1. Implementation of the screening phase].

During a medical check-up for the recognition and treatment of patients with diseases of heart and circulation or their early stages and of patients with factors of risk in two large factories of Erfurt altogether 6,646 men and women were examined. The screening was carried out in November 1975 and 1976 in immediate nearness of the working place. The times during which could not be worked were thus reduced to a minimum. Some organisational problems and their solutions in the preparation and performance of the screening phase as well as first results are reported on.

[Injury hazards in deliveries using the Shute forceps (analysis of 1016 forceps deliveries)].

Report about 1016 deliveries by means of the parallel forceps (W. B. Shute). This group contains pathologic-anatomical results of 30 stillbirthes or died newborns. Accordingly to the classification in subarachnoidal, subependymal or subdural hämorrhage there are only in two cases signs of obstetrical brain damage. The hypoxia is the most important reason of intracraniale haemorrhage. In order to an objektive indication, without progredient signs of fetal hypoxia the extraction by means of parallel forceps is not dangerous. We have used the instrument for extraction as well term as preterm infants with good results.

[ADP system for automatic production of physician's letters as well as statistics analyses in the area of obstetrics and perinatology].

Report about an electronic data processing system for obstetrics and perinatology. The developed data document design and data flowchart are shown. The continuously accumulated data allowed everytime a detailed interpretation record. For all clinical treated patients the computer printed out a final obstetrical report (physician report). By means of this simple system there is an improvement of the information and the typewriting work of medical staff has been reduced. Cost and work-expenditure for this system are limited in relation to our earlier hand made procedure.

[Automatic tocography using the Cardiff infusion system under cardiotocographic control with the fetal monitor BMT 504].

Clinical results of the induction of labour by means of the Cardiff-equipment. The fetal monitor BMT-504 (VEB Kombinat Mebgerätewerk Zwönitz) has been used as cardiotocographic control unit. Labour has been induced with oxytocin (n = 38) or methyloxytocin (n = 30). 50 deliveries, which where induced by means of intravenous oxytocin infusion enables to be comparate the results. Using the automatic infusion the average induction-delivery interval was 4 hours 14 minutes, the average dosage of tocergic drugs was 2 IW oxytocin or 20 mug methyloxytocin. Using oxytocin for labour induction we found an increase in obstetric operative frequency caused by fetal distress. There where an increase in basal tone of uterine activity in this patients. Therefore we recommend methyloxytocin for the labour induction of high risk pregnancies. The safety factors, which are incorporated in the Cardiff-equipment have many advantages in obstetrical routine work, to determine fetal distress at an earlier stage. The results presented show that with this automatic infusion system labour can induced with even greater efficiency and safety. The equipment is very useful in accordance to the continuous increasing number of planned deliveries.

[A central fetal monitoring system in the labor room].

Report about central obstetrical monitoring system (Biomonitor Kombinat VEB Messgerätewerk Zwönitz). The following bed-side monitors have been used: BMT-504 (Kombinat V-B Messgerätewerk Zwönitz), HP 8020 A (Hewlett Packard), FM-3 (Kranzbühler). The FECG and CTG or other fetal, maternal and neonatal functions from six single obstetrical rooms have been registrated in a central work room by means of a 6-channel-recorder (6-NEK-4) and a 6-channel-scope (USG-101). The mother and newborn can be supervised by means of television. This central fetal monitoring system gives the following advantages. There is no disturbance of the patient because the recording and warning events takes place in a central work room. The service of the recorders and scopes is simplified. The room-saving arrangement of bed-side monitors on instrument brackets. Recorders and instrument trolleys have been spared. Preparation of CTG computer analysis. The continuous cardiotocography of all labors (2000/year) by means of the central fetal monitoring has been realized with two midwifes and one physician around the clock. The installation of a fetal central monitoring system is for reconstruction or construction of obstetrical departments to take in consideration.

[The significance of fetal electroencephalography for the diagnosis of fetal disorders].

In typical cardiotocographic and bloodgasanalytic changes indicating fetal hypoxia the fetal electroencephalography shows frequency slowing, voltage suppression and spike activity. This EEG tracings simultaneously appeared with first cardiotocographic signs of fetal distress. It was not possible to recognize disturbances of fetal oxygenation in an earlier stage by EEG monitoring. Fetal EEG is of difficult technique and interpretation. In so far cardiotocography is of higher security, sensibility and easier handling for fetal monitoring. With regard to future improvements in electroencephalographic techniques it seems possible to get additional and more accurate informations for fetal monitoring and about perinatal morbidity.

[Comparative studies of the value of impulse-reception methods of prenatal cardiotocography].

102 pregnant womens have been controlled by means of the various three methods of the antenatal CTG (phonocardiography, ultrasonography and indirect fetal ECG) between the 22th to 41th week. The registered time for each method was ten minutes. Between the 27th to 33th week of the pregnancy the ultrasonography gives the best results. The phonocardiography gives after the 32th week, especially after the 37th week, increasing technical unobjectionable tachograms. There was a good tachografic performans between the 22th to 27th week and after the 35th week by means of indirect fetal ECG. Between the 27th and 34th week this method refuses. The interpretation of indirect fetal ECG or phonocardiotocography, if they are possible, shows clear advantages in antenatal diagnostic of fetal condition.

[Comparison of various screening methods for the diagnosis of chronic ischemic heart disease].

The screening phase of a multifactorial intervention study which is carried out under the direction of the WHO and the aim of which is the influence on mild forms of hypertension, hypercholesterolaemia and slightly disturbed glucose tolerance was supplemented by an electrocardiogram after work under the conditions of screening. The questionnaire contained in the investigation program for the statistical establishment of a typical angina pectoris after work according to Rose and the modified electrocardiographic test after work were tested concerning their evidence in the early recognition of a coronary sclerosis. As reference test served the Watt-step-test recommended by a group of experts of the WHO. An extensive correlation between screening after work and Watt-step-test was found. Questionnaire according to Rose and gradual load, however, had a smaller correspondence. The result of other investigations was that the distribution of the factors of risk does not give any clear reference to the existence of a latent coronary insufficiency.
