Alpha 1-antitrypsin, alpha 1-antichymotrypsin, and alpha 2-macroglobulin in human gastric carcinomas: a retrospective immunohistochemical study.

One hundred twenty-six gastric carcinomas (68 advanced cancers and 58 early cancers) were examined immunohistochemically for alpha 1-antitrypsin (AAT), alpha 1-antichymotrypsin (ACT), and alpha 2-macroglobulin (AMG) within tumor cells. The incidence of these three protease inhibitors was markedly higher in advanced than in early cancers, regardless of the histologic type of gastric carcinoma. In advanced cancers the incidence of both AAT and AMG was significantly higher in well-differentiated adenocarcinomas than in poorly differentiated adenocarcinomas, but no difference was observed in the expression of ACT between these two types of advanced carcinomas. Eighty per cent of the AAT-positive advanced carcinomas had ACT, and 40 per cent of these tumors also contained AMG. The two-year survival rates clearly indicated that well-differentiated adenocarcinomas with AAT have worse prognoses than well-differentiated adenocarcinomas without AAT, but there was no relation between the expression of ACT or AMG and prognosis. These results strongly suggest that the presence of protease inhibitors in gastric carcinomas is related to the invasive growth of the tumors and that AAT is a tissue tumor marker of well-differentiated adenocarcinomas of the stomach. It may also serve as a biologic marker of high malignancy in patients with these gastric cancers.

Rhabdomyosarcoma of the prostate--report of a case with an immunohistochemical study of the neoplastic rhabdomyoblast.

A case of rhabdomyosarcoma of the embryonal alveolar type arising from the prostate in a 6-year-old boy was reported with a immunohistochemical survey. Tumor cells in the biopsied inguinal lymph node were weakly positive for myoglobin and CPK-mm and showed positive staining for neuron specific enolase (NSE). At autopsy, an immunohistochemical examination demonstrated that the tumor cells, especially differentiated elongated cells, were strongly positive for myoglobin, CPK-mm and NSE. An enzyme immunoassay for three distinct subunits of NSE revealed a 320 fold increase in the beta subunit of enolase which is specific for heart and skeletal muscle. The diagnostic significance of immunohistochemical methods for rhabdomyoblasts was discussed.

Growth and functional behavior neuroblastomas in man and nude mice.

Nine human neuroblastoma were serially transplanted in nude mice. These tumor lines were established from the patients older than 1 year 10 month year old. None of the tumors taken from under the age of 1 year and 7 months grew in nude mice. All transplanted tumors produced catecholamines in varying degree. The growth of low catecholamines producing tumors was apparently aggressive compared with high catecholamines producing tumor lines. These results may indicate that tumor aggressiveness decreases with increased functional differentiation in human neuroblastoma in nude mice. The contents of catecholamine was closely correlated with the number and size of secretory granules in tumor cells. This human neuroblastoma/nude mice system may prove to be a useful tool for basic research on the biological behavior as well as on the cytological and functional differentiation of human neuroblastoma.

Functioning paraganglioma in the posterior mediastinum.

An unusual case of a functioning paraganglioma originating from the posterior mediastinum is reported. The main indications in the 21-year-old patient were hypertension and an abnormal roentgenogram of the chest. Blood and urine catecholamine assay confirmed the diagnosis of norepinephrine-secreting tumor, which was covered by the parietal pleura and attached to the sympathetic trunk. In the literature we could find reports of 25 patients with paraganglioma arising from the sympathetic trunk in the posterior mediastinum, 8 of whom had some symptoms and only 3 of whom were assayed for catecholamines. We diagnosed the present tumor as functioning aorticosympathetic paraganglioma according to the new classification and terminology suggested by Glenner and Grimley [1].

Evaluation of usefulness of serial AMHTS--from the viewpoint of inpatients in our hospital.

We studied the relation between admission after check-up and check-up frequencies of AMHTS examinees in our hospital in order to evaluate the usefulness of serial check-ups. Follow-up studies were performed on a total of 16,630 examinees and continued until March 1982. There were 558 inpatients, 3.4% of the total. The main reason for their admission was cancer, followed by diabetes mellitus and chronic hepatitis. It was shown that the admission rates of examinees decreased as the check-up frequencies increased the examinees under 60 years old, but not for those over 60 years of age. In addition, both admission and cancer rates of AMHTS examinees decreased on the whole as their check-up frequencies increased.

Experimental study on monocrotaline induced pulmonary hypertensive rats. (1) Effect of long-term injection of immunosuppressant.

To investigate the involvement of the immune mechanism in the development of pulmonary hypertension in rats given monocrotaline, pulmonary hypertension was induced in rats and the effect of daily injection of prednisolone was studied. Fifty six Sprague-Dawley rats (4 week-old males, 50-60 g) were divided into four groups as follows: (1) injection of monocrotaline alone (A: 17 rats), (2) injection of monocrotaline + long-term injection of prednisolone (B: 22 rats), (3) long-term injection of prednisolone (C: 7 rats) and, (4) long-term injection of physiological saline solution (D: 10 rats). The survival time was prolonged, and slightly better results in the pressure of the right ventricle, hypertrophy of the right ventricle and lesions of lung tissue were obtained in B than in A. However, the rise in the RV pressure the hypertrophy of the right ventricle in B were more pronounced than in C and D, and findings in the lung tissue closely resembled those of A. Judging from the above, it is certain that long-term injections of prednisolone improve the lung tissue lesions and prolong the life of monocrotaline induced pulmonary hypertensive rats. However, it is unlikely that the immune reaction plays a leading role in the development of pulmonary hypertension in the rats.

Acute irreversible rejection after withdrawal of azathioprine in renal allograft recipients.

Azathioprine was withdrawn from five recipients after renal transplantation because of hepatic damage. In three out of five cases, clinical courses were uneventful for more than 1 year after withdrawal (Group A). In the other two cases, rejection episodes occurred 3 and 5 months after withdrawal (Group B). Differences in histocompatibility, the interval between the operation and withdrawal, and the maintenance dosis of prednisolone were not practically striking in either group, but rejection episodes occurred early in group B. We evaluated the results and considered the mechanisms of tolerance after withdrawal of azathioprine because of hepatic damage in renal allograft recipients.

Clinical considerations of renal allograft pathology.

Eleven out of 31 cases of living renal allografts lost their graft functions. Causes of graft loss were acute rejection in eight cases chronic rejection in one, graft rupture in one, and death due to meningitis with good graft function in one case. Five renal allografts and two biopsy specimens obtained after acute irreversible rejection and one renal allograft due to acute tubular necrosis an rupture were reviewed. We evaluated the correlation between these findings and the irreversibility of renal graft functions. All removed allografts and biopsies demonstrated interstitial edema and cell infiltration. Vascular pathology due to acute irreversible rejection revealed a high level of severity with fibrinoid necrosis and luminal obliteration. These findings suggested that graftectomy and cessation of immunosuppressants were indicated.

Diseases and cancer rate of AMHTS examines in the Tokai University Hospital.

Studies were conducted on 10,261 subjects who had undergone checkups in an AMHTS (Automated Multiphasic Health Testing and Services) Center in the Tokai University Hospital. The breakdown of the screening at the time of the examinations was 48% abnormal and 52% borderline or normal. The disease ratio obtained in follow-up survey averaged 16% and increased as the subjects became older. There were 68 cancer patients, 0.7% of the total number of examinees, and it was found that 88% of the cancer cases definitely diagnosed as cancer within one year after the examinations received these diagnoses within three months from the day of the examinations.
