Respiratory function in the muscular dystrophies.

The purpose of this investigation was to compare the respiratory function of patients with different clinical types of muscular dystrophy (MD). A total of 190 patients representing facioscapulohumeral MD (n = 20), limb-girdle MD (n = 50), Becker MD (n = 20), Duchenne MD (n = 90), and the "intermediate" type of MD (n = 10) were studied using simple spirometric respiratory function tests. The respiratory modifications observed in the 3 adult forms indicate that pulmonary function was almost normal. Respiratory function in Duchenne muscular dystrophy (DMD) was always characterized by a restrictive syndrome which severely impaired pulmonary function. The vital capacity (VC) underwent ascending, plateau, and descending phases during the course of the disease. The VC at the plateau stage may be used as an estimate of life span. The most interesting finding was the high degree of variability in the restrictive syndrome associated with DMD. In the DMD group 30% presented a severe restrictive respiratory syndrome associated with a life span of less than 20 years (Type 1), 40% had a serious restrictive syndrome and variable mortality (Type 2), and the remaining 30% had a moderate restrictive respiratory syndrome (Type 3).

Rectal malacoplakia in renal transplantation: MR features.

This report describes the CT and MRI features of rectal malacoplakia in a renal transplant recipient. The lesion appeared strongly hyperdense (105 HU) on unenhanced CT and demonstrated hypointense signal on T1- and T2-weighted MR imaging with slight homogeneous enhancement after gadolinium injection. These characteristics distinguish this lesion from other more common neoplastic, inflammatory rectal lesions that are typically isointense on T1-weighted imaging and enhance variably after gadolinium injection. Characteristic intracytoplasmic siderocalcific spherules in malacoplakia can produce suggestive features on unenhanced CT and MRI.

[Determination of correlations between the main pharmacokinetic parameters and physiopathological factors of flecainide in the elderly].

The purpose of the study was to evaluate the correlation between pharmacokinetic and physiopathological parameters of flecainide in elderly population. Pharmacokinetics of flecainide was determined in seventeen patients, aged more than 60 years, with clinically documented ventricular extrasystoles. Patients received 1.5 mg/kg flecainide either by intravenous administration or by slow perfusion. In elderly population the elimination half-life, the mean residence time and the volume of distribution of flecainide are increased and the plasmatic clearance is reduced. All of these parameters show an important interindividual variability. The relationships between pharmacokinetic and physiopathological parameters were evaluated with multivariate analysis. The correlation obtained (89.86% for total clearance, 64.64% for volume of distribution, 42.59% and 31.12% for elimination and distribution half-lives) provide a relatively good determination of pharmacokinetic parameters of flecainide in aged patients.

[Findings by baryum enema in granulomatous colitis and ileocolitis. A review based upon 60 cases (author's transl)].

Radiological aspects of colonic involvement in 60 patients whose disease has been followed up during several years are described. The typical radiological findings have been observed with their usual frequency. This study also confirmed the persistancy of the radiological findings in the chronic stage and the frequency of recurence in the two first years after surgery. Extension of disease has not been uncommon in the series, even without surgery. This progression was different, according to the initial location of disease: in the case of initial ileal lesion, extension occurred towards the adjacent caecum and right colon in sequence; in the case of initial colitis or ileocolitis lesion, extension occurred at a distance in other colonic segments.
