A 35-year-old patient who presented with recurrent chest infection, pulmonary oedema and cardiac failure was found to be grossly hypocalcaemic owing to previously undiagnosed hypoparathyroidism. The cardiac failure was not easily relieved by digoxin and diuretics but it quickly responded when the plasma calcium was restored to normal with dihydrotachysterol. With dihydrotachysterol as sole treatment for more than 2.5 years he had normal exercise tolerance and no features of cardiac failure.

The field of consultation-liaison (C-L) psychiatry has developed a variety of research interventions to try to improve the recognition and treatment of mental disorders in primary care. The authors review the results of three generations of randomized trials of mental health interventions developed by C-L psychiatrists. In general, these trials have been more successful in improving detection of mental illness and increasing the application of mental health treatments by primary care physicians than in changing patient outcomes. The results of these randomized trials are interpreted by using models developed in the health services field.
PURPOSE: To evaluate the levels of ofloxacin (Ocuflox) in human corneal tissue in patients exposed to various dosing regimens and application methods. SETTING: Corneal Consultants of Indiana, Indianapolis, Indiana, USA. METHODS: Forty patients meeting inclusion criteria were recruited from persons scheduled to have penetrating keratoplasty (PKP). Patients were placed into one of two study groups of 20 each based on dosing regimen and application method. The SAS GLM (General Linear Models) procedure was used to test for differences in mean Ocuflox tissue concentration among study groups and for interaction effects of preoperative diagnosis, trephine size, dosing regimen, and corneal integrity. RESULTS: Corneal tissue concentrations of Ocuflox were elevated significantly by varying drug application methods and dosing schedules before PKP. Eyes receiving medication every 15 minutes for 4 hours had significantly higher levels of Ocuflox than those receiving the drug in six doses over 24 hours before surgery. Ocuflox concentrations exceeded the MIC90 (minimum inhibitory concentration) for key ocular pathogens in essentially all cornea specimens studied. CONCLUSION: Ocuflox penetration was improved by administering it in a controlled setting at 15 minute intervals over 4 hours. The drug provided high tissue levels in both the cornea and aqueous humor, which together with its broad range of antibacterial coverage, should make it a good choice for prophylactic treatment.
PURPOSE: The authors document the degree of in vitro shortening of the Wallstent when expanded to different diameters and demonstrate the clinical importance of this property. MATERIALS AND METHODS: Forty-two-, 68- and 94-mm size Wallstents were expanded within cylinders that were 6, 8, 10, and 12 mm in diameter, and their lengths were measured. RESULTS: Progressive stent shortening occurred between diameters of 6 and 10 mm. Extreme shortening occurred at overdilation to 12 mm: At this diameter a 42-mm stent is 24.5 mm long (vs 46 mm at a 10-mm diameter), a 68-mm stent is 40 mm (vs 65 mm), and a 94-mm stent is 64 mm (vs 95 mm). CONCLUSIONS: Specific knowledge of stent lengths at different diameters is required for proper stent choice and accurate deployment. Over-dilation to a 12-mm diameter is useful in correcting excessive stenting and resistant stent narrowings, but it can result in excessive shortening requiring additional stent placement.
Acid phosphatase was measured in the semen of normozoospermic, oligozoospermic, azoospermic and asthenoteratozoospermic men: there was no significant difference between oligozoospermic and control groups whereas we observed an increase in enzyme levels in the azoospermic group and a reduction in the asthenoteratozoospermic subjects. Furthermore, an isoelectric focusing method for the separation of isoenzymes was developed. Only prostatic isoenzymes 2 and 4 were found in semen. Isoenzyme 2 sialylation profile was determined in the four groups. The sperm count did not influence this profile. These findings are discussed.
