[Percutaneous hepatic biopsy puncture in patients with hemostasis disorders. A new technique].

In order to avoid the inconveniences of the trans-jugular liver biopsy, a modified percutaneous liver biopsy with embolization has been performed in 27 patients with chronic hepatic diseases with or without hemostatic disorders. By means of a sheath and a Menghini or Tru Cut needle type, an embolization of the needle tract was carried out utilising coagulations factors (I, IIa and XIII). All procedures were laparoscopically visualised and the "Hepatic bleeding time" measured. With or without embolization, the "Hepatic bleeding time" found was 0-6 seconds and 2-6 minutes, respectively. This modified technic is a safe and effective method for high risk bleeding patients and no hemorrhagic complications postprocedure observed.

Renal sodium handling abnormalities in hypertensive and normotensive patients with a family history of hypertension.

We measured the effective renal plasma flow, the glomerular filtration rate and the fractional excretion of sodium under low and high sodium intakes in the normotensive offspring of hypertensives and in essential hypertensive patients. During the high sodium intake, 57% of the offspring and 42% of the hypertensive patients showed an increase in effective renal plasma flow and fractional excretion of sodium. In the remaining hypertensives, enalapril improved the renal haemodynamic and sodium-handling responses. Enalapril also reduced the blood pressure in all hypertensives, whether they showed a normal or an abnormal response to a high sodium intake. In the hypertensive patients with an abnormal response to a high sodium intake, the intra-erythrocyte sodium content was higher than in the normal responders. Thus, abnormal renal haemodynamic and sodium-handling responses to a high sodium intake were observed in 50% of the hypertensive patients and the offspring. The high intra-erythrocyte sodium content observed in the hypertensive patients suggests that extrarenal alterations in sodium handling were present.

Use of enzyme-linked immunosorbent assay for the determination of serological relationship between two comoviruses.

ELISA was used to determine the degree of serological relationship between red clover mottle (RCMV) and broad bean stain (BBSV) viruses and the optimal conditions for the differentiation of the two viruses by ELISA were established. The titres of homologous and heterologous antibodies determined by ELISA were 100- to 200-fold higher as compared with the ring precipitin test. Antigen concentrations of 0.15 and 0.075 ng/ml, respectively, could be detected. The reactions in ELISA depended on the concentration of antigen, of IgG used for coating and of conjugated IgG. In anti-RCMV IgG, the ratio of group-specific to species-specific antibody as determined by ELISA was from 1:1 to 1:2, while in anti-BBSV IgG this ratio was from 1:4--1:8. The differences between homologous and heterologous reactions in ELISA suggested a similar ratio of antigenic determinants in the two viruses.

A statistical model for pain in patient-controlled analgesia and conventional intramuscular opioid regimens.

A statistical model was developed: 1) to compare the efficacy of patient-controlled analgesia (PCA) and traditional intramuscular (IM) opioids for pain relief in 40 patients after total knee replacement and, 2) to evaluate pain cycles associated with each technique. Hourly visual analog pain scores were subjected to two-way analysis of variance (ANOVA) and time-series analysis. Hourly verbal analog pain scores were used to determine predominant pain levels. According to ANOVA, PCA was no more effective than were IM opioids. Time-series analyses documented a complete cycle of pain every 5.3 hours in patients receiving IM opioids but no pain cycle with use of PCA. Analysis of PCA verbal analog scores demonstrated self-administration of opioids to "moderate" levels of pain relief with use of PCA, not to complete analgesia. These results suggest that certain patients may not envision complete postoperative analgesia as being possible. Hence, they self-administer opioids for pain relief with PCA according to their expectations. Population characteristics may modify PCA efficacy. These characteristics should be delineated and the use of PCA targeted to appropriate patients.

After-cataract in the rabbit eye following extracapsular cataract extraction--a wound healing reaction.

The cellular response in lens remnants was studied after ECCE in the rabbit eye. Following capsulotomy, the lens content was gently exprimated. No aspiration/irrigation was performed. Degenerative changes were found in the lens fibers and lens epithelium at the equator. Proliferation of lens epithelium started early. Lens fibers break-down almost ceased when lens fibers had been enclosed by proliferating epithelium, that also formed new foci for lens fiber formation. In the opening between the anterior and posterior capsule fibroblastlike cells emerged proliferating on to the posterior capsule.

Coxiella burnetii antigens for detection of Q fever antibodies by ELISA in human sera.

Different concentrations of chemically-treated (by potassium periodate oxidation or mild acid hydrolysis) purified phase I Coxiella burnetii (C.b.) corpuscles and natural (untreated) purified phase I and phase II C.b. corpuscles were compared by ELISA for detection of both phase I (directed to antigen 1) and phase II (directed to antigen 2) antibodies in human Q fever convalescent sera. As to the absorbance values the most sensitive was the antigen obtained by mild acid hydrolysis (0.1 mol/l HCl of phase I corpuscles for 30 min at 100 degrees C) followed by phase I corpuscular antigen (treated with 0.01 mol/l potassium periodate for 4 hr at 45 degrees C). The natural phase I (the 3rd egg passage) and phase II (the 162th egg passage) C.b. corpuscles gave lower absorbance with some sera not even distinguishing the 6th egg passage (EP6 or EP3 in phase I) from EP 162 or from negative controls.

Some structural, biochemical and biophysical characteristics of L-929 cells growing in the presence of hyperosmotic sorbitol concentrations.

Mouse L-929 cells (a fibroblast-like line) were transferred from normal growth medium to one supplemented with 0.3 M sorbitol, doubling the normal external osmotic pressure. After a short lag phase and minimal cell death, the cells began to grow, and the growth rate reached that of controls after about one week. These chronically grown cells (S) have been compared to those of control cultures (C) with regard to general morphology, ability to reverse when returned to normal condition, water content, volume and selected metabolic parameters. S-cell cultures exhibited considerable heterogeneity but most contained vesicle-like cytoplasmic structures, sometimes in abundance. These structures do not appear to be completely bounded by membranes, but that is uncertain. S cells become larger and contain more water than C cells; however, the ratio of total water to total dry mass is indistinguishable from controls suggesting regulation at that level. S and C cells were found to be remarkably similar, on a per cell basis, with regard to their rate of respiration and the incorporation of glucose into metabolites and macromolecules. These results are interpreted in terms of current views on the composition and organization of the aqueous compartments of animal cells.

Thromboembolic complications of the Jarvik-7 total artificial heart: case report.

The Jarvik-7 total artificial heart (TAH) has now been implanted in seven patients, with four (57%) suffering cerebral embolic events. Functional recovery in two patients was impaired by subsequent intracranial hemorrhage from anticoagulant therapy. The findings of a Jarvik-7 TAH explanted following a cerebral embolus are reported here. Thrombotic deposition was seen in the inner crevice formed between the two components of the aortic quick connector. In addition, deposition was seen around all four valves in the crevice formed between the valve ring and the plastic valve housing. On the atrioventricular valves, the deposition in this crevice was most severe on the outflow surface adjacent to the minor flow orifice. On the pulmonic valve, deposition was seen around the entire circumference of both the inflow and the outflow crevices. No thrombi were seen on the diaphragm, the diaphragm-housing junction, or the housing of the left ventricle. Hypercoagulability of the patient's blood gradually developed during implant, reaching a maximum on the evening before the clinical embolus. Increasing resistance to heparin was also demonstrated by in vitro testing.

Immunoelectrophoretic characteristics of four broad bean stain virus isolates.

Four isolates of broad bean stain virus (BBSV; Comovirus group), representing two serotypes in immunodiffusion tests, were compared by immunoelectrophoresis. Two electrophoretic forms (slow and rapid) migrating from the cathode to the anode were found in virus preparations purified from above-ground plant parts harvested 14 or 21 days after inoculation with any of the four isolates. In virus preparations kept for a year at 4 degrees C in the presence of sodium azide both electrophoretic forms were preserved, while in preparations without a preservative only the rapidly migrating form could be detected. In virus preparations from plant roots, only the rapidly migrating form was demonstrated.

Fungal endophthalmitis in narcotic abusers. Medical and surgical therapy in 10 patients.

The presentation and management of 10 cases of proven or presumptive fungal endophthalmitis in narcotic-drug abusers is described. Miconazole was found to be an effective agent in some patients when administered in a dosage of 2400 mg/day. Eight patients received treatment with a combination of miconazole and flucytosine (5-fluorocytosine). Regression of the infection with preservation of the eye was observed in each case. However, visual acuity in the affected eye improved only in four of the eight patients; it was unchanged in two and had deteriorated in the other two. In two patients, who received amphotericin B and flucytosine as initial treatment, control of the infection was achieved, but vision remained unchanged. Vitrectomy was performed in three patients to remove residual sites of infection. However, vision remained unchanged in two of these patients and worsened in the third. The selection of individual modalities of therapy and responses to treatment are discussed.

Serotypes of red clover necrotic mosaic virus. III. Immunoelectrophoresis under different conditions.

The effects of 4 buffers at different pH and of different molarities was tested on the immunoelectrophoresis of representatives of three serotypes and their natural mixtures of red clover necrotic mosaic virus (RCNMV). No substantial differences were observed with buffers at pH 7.2, 8.0 and 8.6. The representatives of the three RCNMV serotypes showed different mobilities in 0.1 and 0.01 mol/l ionic buffers in the agarose gel from cathode to anode. In nonionic 0.01-0.1 mol/l N-2-hydroxyethylpiperazine-N'-ethanesulphonic acid (HEPES) buffer, the RCNMV isolates moved from the anode to the cathode. Isolate TpM48 (serotype B) moved the most rapidly, while isolates No. 6 (serotype C) and TpM34 (serotype A) showed lower mobilities distinct from one another. This made possible a reliable differentiation of serotype B from serotypes A and C as well as of serotype A from serotype C. Agarose gels in 0.001 mol/l buffers proved to be unsuitable for the immunoelectrophoresis of RCNMV.

Isoelectric points of red clover necrotic mosaic virus serotypes.

Isoelectric focusing in agarose gel was used to determine the isoelectric points of red clover necrotic mosaic virus serotypes. The isoelectric points of serotypes A, B and C were in the pH ranges from 5.03-5.06, from 4.82-4.84 and from 4.60-4.62, respectively.

Serological and electrophoretic typing of red clover mottle virus isolates.

Serological and immunoelectrophoretic properties of 22 RCMV isolates from Czechoslovakia were determined. Based on the differences among the isolates, two electrophoretic types were established: type I--isolates with one non-separable electrophoretic form, and type II--isolates with two electrophoretic forms. Depending on the mobility of the electrophoretic forms these types could be subdivided further into two subtypes each. In all selected isolates representing the individual electrophoretic types and subtypes two isoelectric points were determined at pH 5.2 and pH 5.6.
