A total of 38 preoperative computer-tomography findings were compared with the results obtained in central dimer-X-ventriculography. CT yields the exact localisation of every tumour. Determination of kind and size of the tumours was possible in about 30 per cent of the cases. No additional information was obtained about the infiltration tendency of the tumour with respect to the rhomboid fossa. The method appears to be insufficient for the appraisal of occlusion of the efferent passage of the cerebrospinal fluid which are not caused by tumours. In 360 f 38 cases, the direct puncture of the third ventricle could be carried out without any complications. In these cases, all tumours as well as the disturbances of the cerebrospinal fluid passage could be diagnosed. An exact tumour localisation, however, was less well possible than it was with CT. Only hemispheral tumours, tumours of the superior vermiform process, tumours of the 3rd ventricle and craniospinal processes could be differentiated. Excellent results were obtained in the diagnosis of stenoses in the region of the aqueduct or malformations. For the above mentioned reasons, we of course give preference to the computer tomography for the diagnosis of infratentorial processes, but we still consider the combination of the two methods to be indispensable because it increases the information value for the planning of the operation.

Among 5000 computer-tomographic examinations of the brain, there were only 82 tumours located in the 4th ventricle and spinal space. The method is of value for the indication of the localisation and extension of the tumours. The relation of tumours to the rhomboid fossa cannot always be clearly recognised. In spite of good indications in case of some tumours, this method cannot replace the histological method. It contributed only little to the early diagnosis in the series of examinations; the majority of the tumours had a volume of 30 ml. This is due to the fact that characteristic clinical symptoms occur late.
When taking the clinical and EEG findings into consideration, computer-tomographic control provides the possibility to treat localised inflammatory cerebral affections by conservative methods or to heal cerebral abscesses exclusively by systematic puncture and antibiotic treatment. Descriptions of cases including patients with seven brain abscesses treated with conservative means and two patients in each of whom a brain abscess was puncture are presented. In all cases, complete healing was achieved.
Case report. A 59 year old woman was thought to suffer from a neoplasm of the hypophysis. This "tumour" was simulated by a giant aneurysm of the infraorbital segment of the left carotid artery. For technical reasons the left carotid artery had not been demonstrated in the angiogram. For final clarification CT as well as bilateral angiography of the carotid arteries will be helpful.
The effect of vinblastine (VLB) and vincristine (VCR) on the survival and proliferation of HeLa cells has been studied during continuous and after 3-hr incubations using cell counting and time lapse cinematography. VLB and VCR are accumulated in the cells during drug exposure as shown with 3H-labeled drugs. The intracellular drug concentration after a 3-hr incubation with therapeutic doses (VLB, 0.1 micrograms/ml, or VCR, 0.03 micrograms/ml) and thorough drug removal is 150 to 500 times higher than that in the incubation medium. Both drugs are released from the cells upon restoration to alkaloid-free medium reaching concentrations that are lethal to the cells during continuous incubation. There is, however, a difference in the release of the two drugs. VLB is quickly and readily released from the cells while the release of VCR occurs slowly, since VCR is tenaciously retained by the cells. Both drugs are released predominantly from living cells.
We reviewed a group of 30 patients with Hashimoto's thyroiditis who underwent thyroidectomies for suspicion of cancer. A prominent nodule was present in all of the patients; compression symptoms were present in 13 and pain in the thyroid in six. The nodule was cold on scan in 21 patients. Lack of responsiveness to suppressive therapy or nodule enlargement occurred in 18 patients. In addition to the nodule, operative findings included adherence to surrounding tissues in seven patients, lymphadenopathy in seven, increased vascularity in four, and extension to distant cervical structures in two. Pathologic findings in the nodule were similar to those in the rest of the gland; they consisted of lymphocytic infiltration, fibrosis, and variable follicular size. Examination of lymph nodes demonstrated lymphoid hyperplasia. We emphasize the similarities between chronic thyroiditis and carcinoma of the thyroid; the occasional coexistence of these two conditions stresses the need for newer methods in the differential diagnosis.
Patients with Graves' disease were noted to have thickening of the skin on the extensor surfaces of their forearms. Skin biopsy specimens were obtained from nine consecutive patients with Graves' disease treated with sodium iodide I 131, from three patients with other thyroid disorders, and from one patient with acromegaly. Skin specimens from one patient with scleredema and one patient with scleromyxedema were used as controls for the histologic stains. Hematoxylin-eosin-stained sections of forearm skin from eight of nine patients with Graves' disease showed prominent round-cell (probably lymphocytic) infiltration around dermal capillaries and distortion of collagen fibers. In five of the patients with Graves' disease, staining with Mowry's colloidal iron disclosed dense deposits of mucin in the papillary dermis, with a distribution similar to that reported for pretibial myxedema. Mucopolysaccharide deposition in the skin on the extensor surface of the forearms may be the clinical counterpart of pretibial myxedema.

The current relative importance of nyeloscintigraphy is determined on the basis of more than 1,000 examinations over a period of ten years. Owing to its high sensivity the method is suited to early diagnosis and diagnosis by elimination for growing and displacing spinal processes and affections of the cerebrospinal fluid passage. In the case of pathological disturbances, the lack of specificness in the findings, especially in the cervical region, indicates further systematic contrast medium examinations.
Follow-up examinations of 14 patients on whom longitudinal frontal myelotomy had been performed because of extreme leg spasms, showed consistently good results as far as spasm removal was concerned. Effective rehabilitation is possible only if the basic condition is stationary, if the operation is made at an early stage and followed by a comprehensive rehabilitation programme. For patients with an advancing condition the operation can alleviate the subjective discomfort felt, and it can make care easier.
The present study is a report on 9 premature infants treated with aminophylline for relief of apnea. With serum theophylline levels of 2 to 10 microgram/ml, all infants experienced significant decrease of apneic episodes in association with increased wakefulness and increased amounts of active (REM) sleep. These effects may occur independently, but it is possible that the alteration of sleep states may be partially responsible for the decrease in apneic episodes in these infants.
During a stimulus train, the diastolic membrane potential of rat atria exhibits a depolarization phase followed by a slower repolarization phase which has been attributed to the activation of an electrogenic sodium pump (ATPase Na+, K+). This pump seems to be all the more active as stimulation frequency is higher. The parallel evolution of the sodium pump inhibition and a positive inotropic effect in response to ouabain perfusion, suggests that the enzymatic inhibition is directly involved in the development of the cardiotonic effect of digitalis.

The effects of ouabain 10(-6) M on rat and guinea pig hearts have been studied at 18 degrees C, in order to reduce almost fully both the Na+, K+-dependent ATPase activity and the ouabain induced inhibition of this enzyme. In isolated guinea pig hearts the positive inotropic response to ouabain obtained at 32 degrees C disappeared at 18 degrees C. On the contrary, the contractile strength of rat hearts was slightly reduced by ouabain and in the same manner at both temperatures. Current and voltage clamp experiments carried out at 18 degrees C in ventricular fibres revealed that ouabain 10(-6) M decreased both the action potential overshoot and the fast sodium current in rat and guinea pig, by reduction of the membrane sodium conductance. Ouabain did not change the calcium current in guinea pig preparations, whereas in rat heart muscle this current was reduced. The effects of ouabain on both the action potential plateau and outward repolarizing current indicated some inconsistencies from preparation to preparation and cannot therefore be considered as significant. The persistence of the ouabain induced alterations of g Na (in rat and guinea pig) and calcium current (in rat) at 18 degrees C supports the hypothesis of two ouabain cell receptors in heart muscle.
When comparing myeloscintigraphy with positive myelography (Dimer X) in the diagnosis of the lumbar prolapse of intervertebral disks, identical findings were obtained in 17 out of 24 cases. Of these, both methods showed pathological findings in 12 cases and normal conditions in 5 cases. Only in 5 pathological myeloscintigrams normal Dimer X myelograms were obtained and in 2 myeloscintigrams judged as normal the positive myelograms were pathologic. With a good correlation of the two methods in typical cases the Dimer X myelogram, however, offers the advantage of the possibility of examining in several planes under constant reference to the vertebral column. Owing to the good tolerance, myeloscintigraphy should be used in screening diagnoses and in cases where also thoracic parts have to be included in the examination.
