[Transverse syndrome in aneurysmal bone cyst].

Among the patients of the Hospital for Neurosurgery in Leipzig treated in the period from 1952 to 1986 66 per cent of 570 patients operated on because of spinal cord tumours showed a benign and 34 per cent a malignant nature of the tumours. The rate of primary bone tumours amounted to 6.5 per cent and that of secondary bone tumours was 5.8 per cent. The preoperative diagnostical and the differential-diagnostical as well as therapeutical problems were dealt with on the basis of the only case of an aneurysmal bone cyst.

[Effect of the position and size of the postoperative brain defect on psychological outcome].

Ascribing the origin of neurological and neuropsychological syndromes to certain cerebral areas is a well-known knowledge that has also been introduced in clinical practice. In spite of general acknowledgement of a multifactorial manner of the development of these psychic phenomena there are, however, with regard to the effects of the location and the size of the cerebral parenchymal defects, some diverging opinions with regard to these psychopathological phenomena. On the basis of clinical neuropsychiatric and neuropsychological examinations of 204 patients after extirpation of a cerebral tumour, the effects of the volume of the defects and its location as well as the cortical and subcortical atrophy on the psychic properties of performance and affective-emotional ranges were ascertained.

Effect of low nucleotide concentrations on abortive elongation catalysed by wheat-germ RNA polymerase II.

A kinetic study of the effect of elongating nucleotide concentration on the reactions of abortive elongation catalysed by wheat-germ RNA polymerase II on a poly[d(A-T)] template suggests that the shift from abortive to productive elongation may involve the participation of at least two nucleotides, according to a mechanism very similar to that reported for Escherichia coli RNA polymerase. Experiments performed with non-complementary nucleotides with respect to the DNA template, and with substrate derivatives, allow an analysis of the substrate specificity during these reactions. Similar experiments performed with poly[d(A-A-T)].poly[d(T-T-A)] as template provide a starting point for a better understanding of the effect of DNA sequence on the rates of abortive and productive elongation catalysed by the plant enzyme.

[Long-term behavior of ventricular fluid pressure in automatically controlled infusion therapy with sorbitol and relation to clinical and paraclinical parameters].

104 patients of our clinic suffering from intra-cranial space-occupying cerebral processes were treated in the postoperative phase with a cerebral-pressure dependent automatic system for the control of the sorbitol infusion. On the one hand we compared the resulting long-term pressure behaviour of the VFP (ventricular fluid pressure) with clinical and paraclinical parameters and, on the other, with the consumed sorbitol dose. In this procedure advantages of the automatically dehydrating cerebral oedema therapy could be proved. Moreover, it was possible to find clear relations between the pre-operative general changes in the EEG, the degree of the cerebral oedema in the CT, pre-operatively diagnosed choked papillas and the osmotherapeutical expenditure in the postoperative phase.

Effect of salts on abortive and productive elongation catalysed by wheat germ RNA polymerase II.

Modification of the ionic conditions in reaction assays containing wheat germ RNA polymerase II and poly(dAT) as template markedly alters the catalytic properties of the transcription complexes. These effects have been studied by measuring the rate of abortive initiation and the extent of productive RNA synthesis. Using combinations of metal ions or various salts, a marked inhibition of abortive initiation was always associated with an increased length of RNA chains. These results are discussed in terms of modulation of the stability of transcription complexes induced by salts or divalent cations. The behavior exhibited by wheat germ RNA polymerase II is also discussed in comparison with previously reported results for procaryotic and eucaryotic RNA polymerases.

[Diagnostic significance of computerized tomography in cerebral manifestations caused by Whipple's disease].

A woman, who died at the age of sixty, showed different clinical symptoms for more than 3 years. The disease was characterized especially by neurologic and psychical symptoms and kachexia. By autopsy a cerebral manifestation of Whipple's disease was found. The difficulties in diagnosing the main central nervous disturbances were discussed. Besides the detection of SPC-cells in the CSF and in bioptical obtained brain tissue the diagnostical importance of CT-findings increases more and more. The hitherto known and in the special literature published changes of cranial CT in Whipple's disease were reported. In the present case the cranial CT was normal.

[Antagonistic effects of acetylcholine and caffeine on the contractility of the auricular myocardium of rats].

In the rat atrium, the contraction rebound occurring after ACh treatment could not be related to an increase in action potential duration and disappeared in the presence of atropine or caffeine. Contractures elicited by caffeine (10 and 20 mM) were inhibited by ACh. This inhibition was suppressed by atropine. The hypothesis that ACh could increase the sarcoplasmic reticulum Ca diastolic reuptake is discussed.

[Neuroradiologic findings in tumors of ventricles I-III].

In the period from 1953-1983, apart from the computer tomography the following neurodiagnostic methods were employed for the diagnosis of ventricular tumours: native radiography of the skull, PEG, ventriculography, positive ventriculography, angiography and cerebral scintigraphy. This study shows that a clear change in the importance of the various methods has taken place in the above mentioned period of time. The X-ray pictures of the skull taken without a contrast medium have become less important in the last few years although they showed direct and indirect signs of the tumours in almost all ventricular tumours. The former paramount importance of the presentation of the tumours with negative or positive contrast media has decreased considerably. From 1980, for example, practically no pneumo-encephalograms and no air ventriculography have been carried out. The unreliable filling in PEG and the considerable stress for the patients, especially in classic ventriculography, and thus the relatively high rate of wrong diagnoses are the reasons for this change. Only the direct positive representation of the third ventricle, e.g. in case of pinealoma, are still attached the same importance as computer tomography. Carotid angiography which shows almost every ventricular tumour in our patients and can yield information about the vascular supply, has maintained its importance in the neuroradiological scheme of diagnostics.

Poly(dAT) dependent trinucleotide synthesis catalysed by wheat germ RNA polymerase II. Effects of nucleotide substrates and cordycepin triphosphate.

Kinetics of condensation of ribonucleotides to dinucleotides, leading to trinucleotide products formation, have been studied using wheat germ RNA polymerase II and poly(dAT). Assay conditions can be selected under which both ApUpA and UpApU are formed in catalytic amounts. The kinetic parameters associated with these reactions indicate that the rate of trinucleotide formation might be affected by DNA sequence, as reported for E.coli RNA polymerase. Kinetics of disappearance of ApUpA and UpApU were studied under experimental conditions allowing poly(rAU) synthesis. The results can be interpreted as if after formation of a phosphodiester bond, a slow isomerisation step of the ternary transcription complex could occur. During this step, transcription complexes could dissociate with a finite probability, releasing trinucleotides in an abortive pathway. The above results are discussed in the view that, under these experimental conditions, wheat germ RNA polymerase II catalyses poly(rAU) synthesis, as if it is a non-processive enzyme. Cordycepin triphosphate can be condensed to a dinucleotide primer, yielding ApUpA. However the ATP analogue cannot be incorporated into longer products than a trinucleotide. On the other hand 3'-dATP behaves as a very potent inhibitor of translocation, with an inhibition constant of 0.15 microM, a value which is two orders of magnitude smaller than the Km value corresponding to ATP utilization in poly(rAU) synthesis. Simple models are proposed which allow a comparison with E.coli RNA polymerase, for which the results are well documented.

[Computer tomography of tumors of the lateral and 3d ventricle].

In the diagnosis of tumours of the supratentorial ventricles, CT leaves some questions unanswered. This is true of the differential-diagnostic demarkation and the determination of the initial site of the tumour. Frequently, ventriculography cannot be renounced for an additional clearing-up of the last mentioned question.

[Current status of intrathecal antifibrinolytic therapy with p-aminomethylbenzoic acid (PAMBA) in subarachnoid hemorrhages].

The blood-brain barrier is barely affected by slight or moderate subarachnoidal bleeding so that the p-aminomethylbenzoic acid (PAMBA) administered systemically for antifibrinolytic therapy does not attain the continuous level necessary in the cerebrospinal fluid. Thus the antifibrinolytic agent should be applied intrathecally because this would totally inhibit fibrinolytic activity in cerebrospinal fluid containing blood and during the wound healing of ruptured aneurysmas. The improvement in wound healing reduces the occurrence of rebleeding, especially during the first 10 days, and improves conditions for successful surgical intervention.

Controlled comparison of plasma and serum for cystic fibrosis protein.

Investigators who have previously reported attempts to utilize the isoelectric focusing (IEF) method to detect the cystic fibrosis protein (CFP) in blood samples from CF homozygotes and heterozygotes, and normal healthy controls have restricted their evaluations to serum samples. The requirement of using serum and the stringent conditions laid down for the collection, processing, and storage of serum samples have been impediments, however, to the successful use of the IEF method for CFP detection. We now report results obtained from controlled ("blind") comparative analyses of matched serum and plasma samples from CF homozygotes and heterozygotes, and normal healthy controls which indicate that the CFP is present in plasma and that heparinized plasma in particular may be a better fluid than serum for use in future studies on the CFP.

Cranial computertomography in children with acute lymphatic leukemia (ALL) and non Hodgkin-lymphoma (NHL) after different methods of CNS-treatment.

42 children with acute lymphatic leukemia (ALL) or non-Hodgkin lymphoma (NHL) were subjected to cranial computer tomography (CT). 4 groups were formed according to CNS therapy. Group 1: 18 children in complete first remission after CNS prophylaxis with intrathecal 198 Au-colloid and methotrexate were examined between 3 1/2 and 7 1/3 years after beginning of therapy. 15 patients had normal computer tomograms, only 3 children had slight anomalies. The quantitative assessment of the computer tomograms yielded normal results for all 18 children. Group 2: 8 children were examined before CNS prophylaxis. 1 child had a connatal septum pellucidum cyst, 4 children had no anomalies and 3 children slight anomalies. The deviations from normal in groups 1 and 2 should be physiological variations. Group 3: 6 children were examined between 10 months and 8 1/4 years after termination of prophylactic CNS-irradiation (18-22.5 Gy) in combination with intrathecal methotrexate. Normal findings were obtained for 2 patients only. The other children had distinct cortical, subcortical or cerebellar atrophies and calcification of stem ganglia. Neurological complications had temporarily appeared in one child after skull irradiation. Group 4: A CT was made of 10 children during or after meningosis leukemia. The children who had received 198 Au-colloid for CNS prophylaxis yielded no pathological CT results. Distinct cortical, subcortical and cerebellar atrophies or calcification of stem ganglia were found in children after one or two CNS irradiations. These CT investigations confirm the results published by other authors, i.e. that owing to an irradiation--cytostatic therapy of the children's brains CNS lesions can be found in the CT. Their prognosis can only be determined by longterm observations. CNS prophylaxis by means of intrathecal 198 Au-colloid and methotrexate does not lead to any pathological CT anomalies.

Acanthosis nigricans in a patient with streak gonads.

Acanthosis nigricans (ACN) is associated with ovarian disorders or abnormalities of carbohydrate metabolism. We saw a 21-year-old woman who had primary amenorrhea and ACN. Results of endocrine studies showed anovulation and low serum estradiol levels with increased gonadotropin concentrations. Laparoscopic examination disclosed bilateral streak gonads; the ovaries were completely replaced by fibrous tissue. Cultures of peripheral lymphocytes, skin fibroblasts, and the right and left ovaries demonstrated normal female karyotype 46XX in all mitoses studied. An autoimmune disorder was excluded by the absence of antithyroid and antiadrenal antibodies. Specific antibodies against ovarian proteins were also absent. A glucose tolerance test demonstrated fasting hyperinsulinemia and exaggerated serum insulin response to the glucose load. The observations in this patient exclude a primary pathogenic role for the ovary in the production of ACN; they also lend further support to a connection between the cutaneous disorder and insulin resistance.
