Primary chondroid chordoma arising from the base of the temporal bone. A 10-year post-operative follow-up.

A 36-year-old male with a primary chondroid chordoma is presented. This tumour arose from the base of the temporal bone and extended to the mastoid cavity. It involved the facial nerve and was adherent to the internal jugular vein and internal carotid artery. The tumour was excised and the patient has been carefully followed up for 10 years. He has shown no evidence of local recurrence, intracranial extension of the residual tumour and distant metastasis.

Treatment of sudden deafness: carbon dioxide and oxygen inhalation and steroids.

The results of treating 86 patients with sudden deafness are reviewed in this paper. They were treated with intravenous ATP-2Na, Vitamin B1, B6, B12 and C, stellate ganglion block, peroral steroids, cyclandelate and Kallidinogenase. Fifty-one of 86 patients were additionally treated with carbon dioxide and oxygen inhalation. The first 35 patients (no gas inhalation group) and the latter 51 patients (gas inhalation group) were compared with each other concerning hearing improvement, and recovery rate associated with age of the patients, untreated period and effect of steroids. There was no statistical difference between the two groups with regard to these parameters.

Exercise-induced change of nasal resistance in asthmatic children.

Exercise-induced change of nasal resistance and forced expiratory volume in 1 second (FEV1.0) were studied in 30 asthmatic children and seven normal children. Exercise-induced asthma (EIA) was diagnosed in 19 (63%) of the 30 asthmatic patients. Unilateral complete nasal blockage after exercise (exercise-induced nasal obstruction [EINO]) was found in nine (30%) of the 30 asthmatic patients. A marked decrease in nasal resistance took place immediately or 4 minutes after exercise in all cases. EIA is most severe 5 minutes after exercise, and EINO took place 14 or 19 minutes after exercise. The seven normal children had neither EIA nor EINO. The pathophysiologic relationship between EIA and EINO is discussed.

A light and electron microscopic study of lamellated nerve endings found in the rat cheek mucosa.

Lamellated nerve endings in the cheek mucosa of fifteen adult Sprague-Dawley rats were studied with light and electron microscopes. Serial sections revealed that the terminal axon of the lamellated nerve endings is in the central portion, and that the inner core is made up of two stacks of lamellae. These are characterized by many corpuscles located within a single papilla, which shows structural specializations at several levels. Electron microscopy showed that the corpuscles are circular in shape and that the lamellar sheets possess a rich rough endoplasmic reticulum, mitochondria and numerous glycogen-like granules. Numerous bundles of collagen fibers and an amorphous substance are located in the interlamellar spaces. The axon terminal is characterized by the presence of neurofilaments, neurotubules and mitochondria. The capsule consists of several laminae formed by cytoplasmic extensions of perineural cells presenting many caveolae, mitochondria, rough endoplasmic reticulum profiles and microfilaments.

Electron microscope study of the Grandry and Herbst corpuscles in the palatine mucosa, gingival mucosa and beak skin of the duck.

Grandry and Herbst corpuscles of the palatine mucosa, gingival mucosa and beak skin were studied with the electron microscope. Typical Grandry corpuscles are surrounded by thick bundles of collagen fibers and composed of a terminal axon sandwiched between Grandry cells. Occasionally, two or three nerve endings are found within one corpuscle. The Grandry cell contains numerous electron-dense granules similar to those of a Merkel cell. The cell extends protrusions at the poles and the opposing faces. Desmosome-like attachments are noted between interdigitated protrusions, and between the axon and Grandry cell. Herbst corpuscles are composed of an outer capsule, inner core and central nerve ending. The outer capsule consists of 15 to 20 concentric lamellae, while the inner core possesses 60 to 70 sheets of cytoplasmic extensions. Under the scanning electron microscope, the corpuscles appeared as an elongated oval form surrounded by dense fibrous connective tissue, and each lamella of the outer capsule was composed of a dense network of thick and thin fibrils. These were seen under the transmission electron microscope as thick fibrils about 50 nm in diameter and peripheral thin fibrils about 10 nm in diameter. In some portions, a periodic increase of cytoplasmic density about 20 nm in width located in neighboring sheets was noted. The terminal axons of both corpuscles contain numerous neurofilaments, slender mitochondria, neurotubules, smooth endoplasmic reticulum and vesicles. Both corpuscles are limited by several flat cells which are arranged in parallel and probably correspond to the perineural cells of the peripheral nerve. Tight junctions were observed between these cells.

[Effects of noise letters upon selective identification of letters].

Eighteen undergraduate students participated in two experiments which were designed to investigate the effects of noise letters upon selective identification of target letters. The reaction time of pressing buttons to the target letter was measured under various combinations of accompanying noise letters located 1 degree 48' apart from a target in visual angle, and added noise letters inserted between the target and accompanying noise letters. The results provided evidence in favor of the hypothesis which attributes the interfering effects of noise letters not to the processing level, but to the response level. Moreover, the results suggested that the noise letter adjacent to a target plays an important part in this interference, though the noise letters can be processed even in the position as much as 1 degree 48' apart from a target, and that it is especially important what relations the adjacent noise letter has with the response of the target.

Angioarchitecture of thyroid carcinoma.

Since thyroid carcinoma is a tumor originating in an organ with a rich vascular supply, its biological characteristics seem to depend upon tumor vessels. From this point of view, we performed thyroid angiography clinically and examined the structure of the vessel wall histologically. The thyroid angiography demonstrated abnormal neoplastic vascularization, tumor stains and the presence of an arterio-venous fistula within the tumor. According to the pharmaco-thyroid angiography, the vessels in the cancerous tissue, unlike normal vessels, lacked a humoral response. Histochemical fluorescence study revealed no specific fluorescence indicative of the presence of catecholamines in tumor vessels, suggesting that the vessel is embryonic in character. The angioarchitectonic picture of the tumor vessels was polymorphous consisting chiefly of a tree-shaped ramification type and a peculiar saccular or fusiform dilatation type. The punctate or linear type seen in the normal tissue was only a minor component.

[Serum thyrotropin binding inhibitory immunoglobulin (TBII) activity before and after subtotal thyroidectomy in Grave's disease].

Serum level of TBII activity was measured in 81 patients with Graves' disease by the Smith's radioreceptor assay method and the activity more than +15% was designated as "positive". All the patients had been under medical treatment with antithyroid drugs and 71 of them underwent subtotal thyroidectomy. The negative TBII activity was seen in 4 (22%) of 18 patients in whom the duration of antithyroid drug treatment was less than 2 years and in 6 (60%) of 10 patients whose medical therapy was for more than 2 years. TBII activity was not correlated with serum MCHA-, TGHA-titers and circulating lymphocyte counts, but correlated well with the degree of lymphoid cell infiltration in the resected thyroid tissue. All the 7 patients was negative had never relapsed, but 2 of them remained at the euthyroid state 6 to 24 months after operation, 3 hypothyroid and 2 at a borderline between the normal and hypothyroid. The postoperative states of the 13 patients who showed positive TBII activity before operation were euthyroid in 7, hypothyroid in 2, borderline hypothyroid in 4 and relapse in one. A gradual decline of TBII activity following surgery was generally associated with postoperative euthyroidism. It is concluded that determination of TBII activity is of help to estimate the thyroid functional state in Graves' disease.

[Nuclear magnetic resonance investigations of DMBA-induced rat breast cancers--the relationship between tumor doubling times and water proton spin-lattice relaxation times].

Our studies of human neoplastic tissues had revealed that the T1 values of many malignant tissues had a tendency to be longer than those of normal ones. But the T1 values did not always reflect the grade of malignancy on account of the variety of their histologies. Therefore, by using DMBA-induced rat breast cancer, we investigated the relation between the degree of malignancy and the T1 values, and the following results were obtained. 1) The growth rate of the cancers had a correlation with their T1 values, and 2) the water content of the cancers correlated with their T1 values, but 3) there was no correlation between their growth rate and their water content.

[Studies on penetration of antibiotics to gallbladder tissue and bile, its surgical significance. Mainly with cefmenoxime].

Basic and clinical studies in 37 patients with biliary tract disease on comparison between cefmenoxime (CMX) and cefotiam (CTM) were studied and the following results were obtained. In vitro antibacterial activities of CMX and CTM against 25 strains (15 organisms) isolated from bile of patients with biliary tract disease were stronger than that of cefazolin (CEZ). In cholecystectomized patients, CMX (2 g) or CTM (2 g) was injected intravenously, followed by determination of concentration in bile and gallbladder tissue about 2 hours after administration. In CMX administration, the mean concentration in gallbladder bile was 812.1 micrograms/ml, and the mean concentration in duct bile was 1,050.6 micrograms/ml, and the mean concentration in gallbladder tissue was 100.7 micrograms/g. In CTM administration, the mean values were, 1,092.5 micrograms/ml, 1,287.8 micrograms/ml, 28.5 micrograms/g, respectively. The concentration of CMX and CTM were almost similarly. The bile concentration of CMX (i.v.) was compared with CTM (i.v.) by cross-over method in cases of T-tube drainage. The peak bile concentrations of CMX and CTM were as high as 172.4 micrograms/ml and 182.2 micrograms/ml, respectively, 1 approximately 2 hours after 2 g intravenous administrations. Furthermore, the concentration of them were highly gained, 16.1 micrograms/ml of CMX and 33.8 micrograms/ml of CTM, even at 5 approximately 6 hours. In choledochostomized patients, CMX (4 g/day) was injected intravenously, followed by determination of concentration in intraperitoneal exudate. The mean concentration of CMX was 15.3 micrograms/ml on the first day after the operation, and 6.0 micrograms/ml even on the third day after the operation. Those results suggest that the high antibacterial activity of CMX against organism in bile and, the high penetration of CMX to bile, gallbladder tissue and intraperitoneal exudate will promise its important role in treatment of biliary tract infections.
