[A study on the penetration of cefmenoxime into bile and gallbladder tissues and the pathological changes of inflamed tissues].

The penetration of cefmenoxime (CMX) into bile and gallbladder tissues was studied. A dose of 2 g CMX was given intravenously to each of patients with gallstones. Concentrations of CMX in the gallbladder tissues were 120.6 +/- 39.8 micrograms/g in normal tissues about 2 hours after the administration and 128.3 +/- 52.3 micrograms/g in inflamed tissues. The penetration of CMX into gallbladder tissue tended to decrease in proportion to the severity of inflammation, but the difference between the 2 groups, normal and inflamed tissues, were not statistically significant. The penetration of CMX into gallbladder tissue was presumed to increase in proportion to increase in the serum concentration of CMX. The result indicated that the achievement of high concentrations of CMX in the serum is important in the treatment of biliary tract diseases.

[Burkitt's lymphoma of the bilateral breasts presenting during lactation].

A 34-year-old postpartum woman with Burkitt's lymphoma is reported. Lymphoma involved the bilateral breasts chiefly, as well as the brain, ileocecal region and supraclavicular lymph nodes. Fourteen days after bilateral mastectomy, the patient suddenly went into a coma due to rapid progress of brain involvement. However, the coma improved the day after VEPA chemotherapy and radiation were undertaken, and the tumor-involved brain decreased significantly in size following chemoradiotherapy. Chemoradiotherapy produced the almost complete disappearance of the remaining tumor. The patient is now being treated by a maintenance dose of Endoxan and is doing well. Burkitt's lymphoma of the breast in the present case had the clinical characteristics of bilateral involvement with rapid growth arising during lactation and pregnancy.

[Clinical investigation of familial clustering of cancer].

Five cancer-prone families, i.e., a colorectal cancer family, a gastric cancer family, two breast cancer families and a cancer family without organ specificity are reported, and familial clustering of cancer was investigated clinically. It was strongly suggested that familial clustering of cancer was related to genetic factors. The clinical characteristics of cancer-prone families were a familial tendency to cancer, early age of onset, an extraordinary frequency of multiple primary malignant neoplasms and an autosomal dominant mode of gentic transmission. The clinical significance of noting familial clustering of cancer is to extract groups at high risk for cancer, to detect early cancerous lesions and to make early therapeutic decisions.

Hepatoblastoma producing both alpha-fetoprotein and human chorionic gonadotropin. Clinicopathologic analysis of four cases and a review of the literature.

A clinicopathologic study was done of four cases of hepatoblastoma with precocious puberty, together with an analysis of 21 such cases reported in the literature. All four patients were males, the age ranging from 11 to 35 months. Three of the four patients died within 12 months after operation, but the fourth is living. Histologically, all patients had a hepatoblastoma with the coexistence of both fetal and embryonal type cells, although predominantly fetal in two and predominantly embryonal in the other two. Tumor giant cells were rarely encountered in all these cases. Both alpha-fetoprotein (AFP) and human chorionic gonadotropin (hCG) in the serum or urine increased in our four cases and in five other cases reported in the literature. Though the serum AFP level paralleled the severity of clinical symptoms, the serum or urine hCG did not necessarily correspond to the clinical course. It is likely that these poorly prognostic virilizing hepatoblastomas secrete two different tumor markers, AFP and hCG, from different cells, and that these functioning tumor cells may not always exist concurrently in the recurrent or metastatic tumor.

Menkes' kinky hair disease: clinical and experimental study.

A pedigree of Menkes' kinky hair disease (MKHD) is reported. One patient of this family who underwent copper treatment was followed for three years with fundus examinations and ERG measurement. The blood copper level remained normal after six months of age, when intravenous treatment was switched from cupric acetate to cupric sulfate. Optic nerve atrophy and decrease in amplitude of the ERG were observed at three years of age. In an experiment using mouse models of MKHD (macular mouse mutant, Moml), we compared the affected mice that received copper treatment with normal mice. However, there was no difference between them in ERG responses, number of ganglion cells, or thickness of retinal layers. These results support the possibility of prolonged survival and maintained vision in the patients of MKHD by earlier normalization of the copper level.

Intramuscular myxoma of the neck.

Intramuscular myxomas are extremely rare tumors in the neck. In reviewing the world literature, we found a total of five cases of intramuscular myxoma that occurred in the head and neck region. The purpose of this study is to report an unusual case of intramuscular myxomas that were found bilaterally in the digastric muscles of a girl who presented with a painless palpable mass in the neck.

Effect of ophthalmic administration of S-596 (Arotinolol) on intraocular pressure and haemodynamics in health volunteers: comparison with timolol.

The effects on intraocular pressure (IOP) and haemodynamics of two beta-blockers, arotinolol and timolol, administered topically to the eye, were studied in 6 healthy volunteers in a cross-over trial. 0.5% timolol or 0.5% arotinolol ophthalmic solutions drop was instilled in both eyes of the volunteers at an interval of 48 hours. Timolol lowered IOP by about 31.9% 1 hour after administration and the effect continued until 3 h, whilst arotinolol lowered it significantly 2 h after instillation and the same maximum effect as that of timolol was obtained after 3 h. Arotinolol was detected in blood in all subject and timolol in blood in one subject, although it was found in all subjects in urine. Both drugs lowered heart rate at rest and attenuated the increase in the double product (products of blood pressure and heart rate) at exercise. The effect of timolol on the double products was larger. Thus, arotinolol and timolol decreased IOP to similar extent, although the maximal effect of arotinolol was delayed. Arotinolol as well as timolol affected haemodynamics through absorption into circulation, but the former had less effect on haemodynamics during exercise.

Ulcerative colitis complicated by idiopathic central serous chorioretinopathy with bullous retinal detachment.

A 44-year-old woman, hospitalized with severe diarrhea due to ulcerative colitis, was found to have a rare ocular complication, idiopathic central serous chorioretinopathy with bullous retinal detachment. The ocular symptom started with the flare-up of the ulcerative colitis. Steroid therapy was not effective on the ocular disorder, while her ulcerative colitis became dormant. Relationship between the two disorders is discussed.

Blow-out fracture of the orbit in 4 children.

4 cases of blow-out fractures of the orbit in children are presented. Four 6- to 12-year-old boys were successfully operated on via an orbital and transsinus approach. As material for restoration of the broken orbital floor, Silastic was used in 3 patients.
