[Follicular carcinoma in an autonomously functioning thyroid nodule].

The patient was a 66-year-old man who complained of goiter. A diagnosis of functioning thyroid tumor was made because of high uptake of 131I after T3 suppression test and no response to thyrotropin releasing hormone. The tumor was removed and histologically proven to be follicular carcinoma. Twelve cases of thyroid carcinoma in autonomously functioning thyroid nodules have been reported, and the relation between carcinoma and an autonomously functioning nodule is reviewed from the literature.

Effects of retinal detachment surgery on standing potential.

We studied the effects of retinal detachment surgery on the standing potential of the human eye using the electrooculographic technique. Before and two weeks after surgery, we measured the relative base value (rBV) which is the base value of the affected eye divided by the base value of the fellow eye. The rBV distributed around 1.0 before surgery and decreased below 1.0 two weeks after surgery. The change in the rBV through surgery represented by the value R is defined as preoperative rBV minus postoperative rBV divided by preoperative rBV; this was closely correlated with the area of the scleral buckling. The decrease in the rBV was thought to represent the reduction in the standing potential of the operated eye, probably due to circulatory disturbance of the choroid.

Congenital stationary night blindness with myopia: a clinico-pathologic study.

The left eye of a 77 year old male patient was enucleated because of absolute glaucoma. This eye showed subnormal electroretinogram (ERG) and oscillatory potentials preoperatively, but the other eye showed Schubert-Bornschein type ERG and monophasic dark adaptation curve. Light and electron microscopic studies of the left eye showed a normal arrangement of discs of rod outer segments, normal synaptic ends of the photoreceptors, and complete loss of ganglion cells. From the subnormal ERG in the left eye we assumed it was originally Schubert-Bornschein type ERG but inverted to subnormal type ERG following the loss of inhibitory mechanisms. Thus we propose that the cause of night blindness in congenital stationary cases with Schubert-Bornschein type ERG may be related to the mechanisms inhibitory to cells of the bipolar layer.

The influence of eye movements and tactile information on postural sway in patients with peripheral vestibular lesions.

The statokinesigram of the 16 benign paroxysmal positional vertigo (BPPV) patients and the 28 vestibular deficits (VDS) patients were analyzed in comparison with the 20 normal subjects (NS). Velocity of the track (LNG/T), root mean square (RMS), and standard deviation area (SD-Area) were used as parameters. Fourteen different tests were done. When the result for each subject group were analyzed there were interesting differences between the compensated VDS and BPPV patients and between the untreated period and the compensated stage in each patient group. Comparing the data between NS and BPPV at the compensated stage, the latter group was unstable while watching LED with OKN on the tactile information plate (S-G plate). The significant differences were for RMS and SD-Area. The data of the NS and the untreated VDS show that the latter group was more unstable while watching LED with OKN than in the dark on the S-G plate. According to the data of the NS and the VDS at the compensated stage, the latter group was more unstable during OKN or watching LED which was almost the same as in the dark on both plate conditions. And it was more unstable while watching LED than during OKN on the S-G plate. The significant differences were for velocity of body sway (LNG/T). The neural region for compensation in BPPV patients might be on the visuo-oculomotor pathway through the peripheral retina. One of the most important regions for compensation in VDS patients might be in the rombencephalic reticular formation.

Increased resistance to the anticellular effect of interferon in an ultraviolet light-resistant human cell line, UVr-1.

Interferon (alpha, beta and gamma) susceptibility was tested in a human cell line, UVr-1, a u.v. light-resistant variant of RSa cells; the latter have high sensitivity to both u.v. lethality and the cell proliferation inhibition (anticellular) effect of human interferon (HuIFN) preparations. UVr-1 cells were less sensitive than the parental RSa cells to the inhibitory effects of HuIFN preparations, as measured by cell proliferation and the incorporation of [3H]deoxythymidine and [3H]deoxyadenosine into acid-insoluble cellular material. Nevertheless, UVr-1 cells exposed to HuIFN showed almost the same enhanced levels of antiviral activity and pppA(2'p5'A)n synthetase activity as similarly treated RSa cells. Further, UVr-1 cells had much the same binding capacity for 125I-labelled HuIFN-alpha A. Thus, it seems likely that the variant has an increased resistance to the anticellular effect but not to the antiviral effect of HuIFN preparations. UVr-1 cells showed no significant difference from RSa cells in u.v.-induced DNA repair synthesis. However, when a comparison was made between the susceptibility of normal fibroblasts and fibroblasts from patients with Cockayne's syndrome, characterized by an altered u.v. sensitivity but no alteration of DNA repair replication synthesis, the Cockayne's syndrome fibroblasts, CCK-3 and CCK-4, were more susceptible to HuIFN-beta as judged by cell proliferation and deoxythymidine incorporation tests.

Interactions of endotoxin with cortisol and acute phase proteins in septic shock neonates.

CRP, alpha 1-acid glycoprotein and haptoglobin were studied in 13 septic shock neonates. Endotoxin was recovered from eight infants. Serum cortisol concentration from infants with endotoxemia (917 +/- 596 ng/ml) was significantly higher than that from infants without endotoxemia (398 +/- 239 ng/ml). Serum cortisol correlated well with immature neutrophil counts defined as the unit "band/neutrophil". Increased cortisol level and immature neutrophil counts preceded the elevation of CRP, alpha 1-acid glycoprotein and haptoglobin in four extremely premature neonates. We conclude that positive interactions between endotoxin, cortisol and acute phase protein synthesis are present in the initial period of infection, and delayed acute phase protein synthesis is suspected in extremely premature neonates.

Chromosomal aneuploidy in sarcoid granuloma cells.

Chromosomal examination and measurement of DNA content were performed in cultured sarcoid granuloma cells obtained from lymph node specimens of 11 patients with sarcoidosis. Chromosomal studies revealed that most of the cells were aneuploid; the chromosome number ranged from the near-haploid number to the near-diploid number, and the distribution of chromosome number was fairly wide. The DNA content of individual cells was measured by Feulgen microfluorospectrophotometry. The distribution of DNA content of the granuloma cells was fairly wide and did not show any recognizable peak. Flow cytofluorometry of DNA content of the cells also revealed wide frequency distribution of the DNA content in sarcoid granuloma cells. The majority of the sarcoid granuloma cells were aneuploid or hypodiploid in the short-term primary culture, suggesting that hypodiploidy may exist in granuloma cells of patients with sarcoidosis.

Cholinesterase activity of lamellated sensory corpuscles in the rat lip.

Non-specific cholinesterase (ChE) activity was demonstrated in lamellated sensory corpuscles of the rat lip by light and electron microscopy using Karnovsky and Root's method. ChE activity was present in the interlamellar spaces between neighbouring lamellae as well as in the periaxonal space between axon terminals and their adjacent lamellae. Reaction products of ChE activity were also deposited in some caveolae of the lamellar cell plasma membrane, and in the cisternae of the rough endoplasmic reticulum as well as in the nuclear envelope of lamellar cell bodies. No definite reaction products were detected within the axon terminals. These findings show that the lamellated corpuscles in the rat lip, like other mechanoreceptors, have an intense ChE activity which is mainly associated with lamellar cells. It can be said that ChE histochemistry is useful to detect mechanoreceptors. The functional significance of ChE in mechanoreceptors is discussed.

Correlative scanning and transmission electron microscopic study on the ependymal surface of Cebus apella.

Topographical variations of the ependymal surface of the whole brain ventricular system of Cebus apella, a wild Brazilian species, were studied at the scanning electron microscope after CO2 critical point drying and carbon-gold coating. Certain selected areas were also studied at the transmission electron microscope, following standard technical procedures. We observed distinct patterns in the distribution of cilia, microvilli, other cell projections, and the so called supraependymal structures. Based on the morphological findings, functional considerations are made.

[The cancer family syndrome--a case report].

A family diagnosed as cancer family syndrome because endometrial cancer was found in a member of this colorectal cancer-prone family was reported. The fact that uterine (endometrial) cancer was found in a family member during the serial follow-up of this family shows that there is no more than a difference of time between familial colorectal cancer and cancer family syndrome. The clinical significance of the familial cancer aggregation would be an important clue in the early detection of another new cancer in the family.

Stability of the mandible after sagittal ramus osteotomy for correction of prognathism.

The change in the position of the mandible after sagittal ramus osteotomy was evaluated on lateral cephalograms and in dental casts in 44 patients with mandibular prognathism. The postoperative relapse of the mandible at four landmarks was less than 1 mm on cephalograms and 1.1 mm at the first molar on the side with the larger posterior movement. The tendency to relapse was greater in cases in which large posterior and/or lateral movements of the mandible occurred at surgery. The importance of proper postoperative care and avoidance of lateral shift of the mandible to minimize relapse is stressed.
