Neurological complications of Grönblad-Strandberg syndrome.

Neurological complications resulting from pronounced cerebrovascular changes were observed in a 59-year-old female suffering from Grönblad-Strandberg syndrome. In addition to typical skin changes of the pseudoxanthoma elasticum and ocular alterations (exudative macular degeneration and angioid streaks) there were an unusual number of disturbed endocrinological functions. The ophthalmological findings revealed characteristic breaks in Bruch's membrane, proliferation of chorioidal vessels into the subretinal space, hyperplasia of retinal pigment epithelium and disciform macular degeneration. Postmortem examination of the brain showed multiple cerebral infarcts due to recurrent disturbances in cerebral blood supply and particularly pronounced changes in the elastica interna of many cerebral arteries. Although the observed neurological signs in our patient were not characteristic of Grönblad-Strandberg syndrome, the histopathological findings led to the assumption that the same basic mechanisms result in alterations of elastic fibres and collagen, mainly in the ocular-cerebral system.

[Clinical and histopathological findings in an unknown oculocerebral abnormality syndrome].

2 brothers of a genetically healthy family were born with multiple identical malformations and died at the age of 11 and 15 years. Clinically they presented hydrocephalus with convulsions, spastic dysplegia and blindness. Ophthalmologic investigations revealed one macrophthalmic eye with retinal malformations and deeply pigmented, partly occluded choroid, abnormal scleral pigmentation and optic atrophy. The other small eye showed meso- and ectodermal dysgenesis of the anterior segment and total retinal detachment as well as optic dysplasia. The brain was remarkably abnormal as well.

The influence of topical beta-blockers on the breakup time.

In a double-blind masked study with 100 young and healthy subjects we compared the influence of the beta-blocking agents timolol and metipranol on breakup time and their tolerance. While metipranolol provoked a slight burning sensation, timolol was well tolerated. Both beta-blockers, however, led to an equally enormous reduction in BUT in short-term observations.

[On the validity of 32P-test: mitotic activity].

32P-uptake over different tumor areas of 13 melanoma containing eyes was measured and samples of these areas histologically investigated, now mainly concerning the rate of mitosis. We found interindividually as well as intraindividually positive correlations between 32P-uptake, epitheloid-cell-content and mitoses.

Role of the beta-adrenergic receptor-binding agent 'timolol' in the treatment of open-angle glaucoma.

A follow-up study was performed on 54 eyes with chronic open-angle glaucoma treated with timolol maleate. It was carried out as an open clinical trial with a selected group of eyes. During 2 years of application the effect of timolol under normal conditions as well as under stress tests (darkness test, water-, coffee- or tea-drinking test, early morning measurements) was observed. Until now 40% of patients were regulated with 0.25% timolol twice daily. Another 50% obtained good pressure values under 0.5% timolol therapy and the rest with 0.5% timolol and 2% pilocarpine twice daily. Early morning measurement was shown to be the most important indicator of intraocular pressure regulation by beta-blocking agents. While timolol is usually very effective in reducing the intraocular pressure during daytime, several patients showed unattended high morning pressure values. Up to now, no severe side-effects of timolol therapy have been observed.

[Epibulbar osteomas (author's transl)].

Epibulbar osteomas are benign tumors which are usually present at birth. Up to now 39 cases have been described in the literature. In the last three years 4 further cases have been found in the Freiburg, Hamburg, Marburg, and Munich University Eye Clinics. Epibulbar osteomas probably occur more often than is generally believed. The clinical and histological findings in respect of these tumors are discussed.

[Timolol: long-term results, tolerance test and early-morning measurements (author's transl)].

In a study lasting 16 months, Timolol was tested on 57 eyes of a group of selected patients with open-angle glaucoma. Patients whose IOP could not be stabilized with the 0.25% solution were treated with 0.5% drops twice a day. In five separate cases, where regulation was inadequate, pilocarpine 2% twice a day was prescribed in addition. At the end of 16 months' treatment pressure had been lowered to a statistically highly significant degree in 66% of the patients. While the degree of regulation in the remainder was not statistically sifnificant, most of them were adequately stabilized. Further, following stabilization with Timolol, 42% of the patients were subjected to tolerance tests under constant conditions (darkness test, water test, coffee and tea tolerance) and their 6.00 a.m.values were measured. While good pressure levels were recorded in the tolerance tests, the early-morning IOP rose to critical threshold levels in some patients. However, during the entire period of observation visual fields and optic disk findings remained unaltered in all cases. Only a few cases were subjective and/or objective side-effects seen. Ten percent of those patients who had been treated previously required new glasses (+0,5 to +1 D over the previous correction).

[Conjunctival myxomas (author's transl)].

Myxomas are rare mesenchymal tumors appear everywhere in the body, but occur very seldom in orbital or subconjunctival regions. Clinical and histopathological demonstration of two cases of epibulbar myxoma and discussion of possible pathogenetic mechanisms.

[Aggressive eyelid basalioma with erythroderma ichthyosiforme congenita sicca].

Report on an agressive eyelid basalioma which led, in a patient with Erythrodermia ichthyosiformis congenita sicca, to exenteration of the oribt. Presentation of clinical and histopathological findings and discussion about the histogenic character of both diseases considering also present literature on this subject.
