[Treatment of trichiasis with a diode laser].

BACKGROUND: Various methods have been used in the treatment of trichiasis: epilation, excision, cryosurgery, thermocaustic needle, and argon laser. None of these is satisfactory. Based on the principle of selective photothermolysis for destroying specifically the hair follicle, we used a diode laser and evaluated its efficacy. MATERIAL AND METHODS: Nineteen eyes with 301 itching lashes were treated in three sessions at intervals of 1 week. In each session the single hair follicle received laser energy of 20 J/cm2 at a frequency of 806 nm for 15 ms. We used a bare fiber of 0.8 mm at a distance of 2 mm from the follicle. RESULTS: After 4 months two-thirds of the hairs were destroyed. The regrown lashes, however, are lighter and softer than the original ones. This treatment is less painful than all others. The laser is small, easy to use, and inexpensive. CONCLUSIONS: The diode laser is a useful tool for the treatment of trichiasis. The laser parameters, however, should be ameliorated.

Conjunctival malignant melanoma in children.

The first case concerns a 14-year-old girl with a brown conjunctival mass temporally in her right eye which was excised and diagnosed as active conjunctival melanotic lesion. Five years later, a malignant melanoma of the right lacrimal sac and orbit led to general metastatic spread. The second case demonstrates a conjunctival brown mass temporally in the left eye of a 3-year-old boy which enlarged over 4 years. The excision revealed a melanotic lesion which was of uncertain histological dignity. The third case occurred in a 4-year-old boy with a conjunctival glassy tumor temporally in his right eye which after excision led to controversial discussions concerning benign nevus or malignant melanoma.

Stress reactivity of intraocular pressure after relaxation training in open-angle glaucoma patients.

The present study was based on the hypothesis that stress may contribute to increased intraocular pressure (IOP) in open-angle glaucoma patients. It is investigated whether IOP reactivity to a mental stressor test (MST) can be influenced by relaxation training. Twenty three patients with open-angle glaucoma were randomly assigned either to a training group (TG) or to a waiting-list control group (CG). Prior to as well as after the completion of the training all patients were exposed to the MST. IOP and heart rate as well as self-ratings of psychological strain were assessed three times: (1) at baseline, (2) after exposition to the stressor, and (3) after a 10-min relaxation phase. Results provide evidence that the MST is a valid procedure to induce psychophysiological activation and that elevated IOP levels in open-angle glaucoma patients might be provoked by stressing situations. However, participation in the relaxation training did not influence IOP stress reactivity.

Effects of self-relaxation methods and visual imagery on IOP in patients with open-angle glaucoma.

In this study, the effects of a training in relaxation and visual imagery on the intra-ocular pressure (IOP) of patients with primary open-angle glaucoma were investigated. Twenty-three patients, aged 24-69 years, were assigned either to a training group or to a waiting-list control group. The intervention included a basic programme (BP) of standard autogenic relaxation exercises and an advanced programme (AP) in which special exercises in ocular relaxation and imagination of aqueous humour drainage were conducted. IOP was measured before and after each training session. Twenty-four-hour IOP profiles were ascertained, and the water drinking test to provoke maximum IOP levels was performed during clinical assessments prior to the training, between the BP and the AP and after termination of the training. Results indicate only slight short-term changes of IOP levels immediately after each training session. However, during the course of the BP as well as of the AP a relevant decrease in IOP could be measured. Twenty-four-hour IOP profiles as well as the water drinking test also showed significant reductions of IOP during time. Medication could be reduced for 56% of the initially treated patients. The findings suggest that relaxation and visual imagery techniques can be beneficial in reducing elevated IOP levels in patients with open-angle glaucoma though the conclusiveness of the data is limited due to methodological shortcomings.

Panophthalmitis with acute scleral necrosis after brachytherapy of a malignant melanoma of the ciliary body.

A 52-year-old man who presented with a large ciliochoroidal malignant melanoma was treated by application of a 106Ru plaque. At 8 days after removal of the plaque, severe panophthalmitis with rapidly progressive scleral necrosis at the site of the tumor made enucleation inevitable because of impending perforation of the globe. Microscopic examination revealed an intense inflammatory response and almost complete necrosis of the tumor and overlying sclera. Possible mechanisms for the occurrence of this rare complication are discussed.

[The dry eye and diabetes mellitus].

Diabetic patients seem to have problems with dry eye symptoms. Therefore, 92 patients with diabetes types I and II and aged from 7 to 69 years were compared with a group of normal healthy controls comparable in number, age and sex. A general ophthalmological check-up was performed. The main points of comparison were subjective complaints, objective findings on conjunctiva and cornea, break-up time (BUT), basal secretion test, impression cytology of the conjunctiva, and grade of diabetic retinopathy. The results show that 52.8% of all diabetic subjects complained of dry eye symptoms, as against 9.3% of the controls. A BUT value lower than 10 s was found in 94.2% of the diabetics and in only 5.8% of the controls. Basal secretion test lower than 5 mm was observed in 26% of the diabetics and in 16% of the normal controls. Pathologic conjunctival epithelium (grade III-V after Tseng) was found in 86% of the diabetic patients and in 6.7% of the healthy subjects. Among the type II diabetic patients, 70% had proven dry eye syndrome, while 57% with type I diabetes suffered from this. A correlation was found between the HBA1c values and the presence of dry eye syndrome. The higher the HBA1c values, the higher the rate of dry eye syndrome. The study thus supports the impression that diabetic patients have an elevated incidence of dry eye syndrome. Impression cytology was found to give the most distinctive and discriminating results. Close monitoring of diabetic patients and good blood sugar regulation is important for the prevention of dry eye syndrome and retinopathy.

Rare choroidal tumour simulating a malignant melanoma.

A 15-year-old boy had been treated for a central chorioretinitis of his right eye. The other eye had always been normal. Two years later he presented a relapse with deterioration of central visual acuity. All clinical tests, including fluorescein angiography, suggested an inflammatory process this time again. Thereafter, the patient did not show up for 6 years. Then he came to hospital because of strong pains in his right eye. Now, a blind and inflamed eye was found with a huge tumour from the optic nerve extending to the temporal periphery. All clinical investigations, amongst others ultrasonography, computerized tomography and magnetic resonance tomography, suggested a choroidal melanoma with extrabulbar extension. An enucleation with partial exenteration was performed. A demonstration and discussion of the clinical findings and the rare and complicated histological diagnosis of a very unusual ectopic meningioma-like lesion are given.

[The dry eye in kidney transplant patients].

Fifty-three patients with severe renal disease and/or kidney transplants underwent full ophthalmological examinations, including tear breakup time, Schirmer's test, and impression cytology of the conjunctivae. Although only some 50% of the patients complained of typical dry-eye symptoms, pathologic changes of the outer eye were found in almost 70%. Tear break-up times and Schirmer's test values were greatly reduced. Morphological changes of the conjunctiva were found in almost all the patients; in 50% of these cases the changes were moderate to severe.

[Effect of commercial beta blockers and their combination with artificial tears on the duration of tear film stability].

The influences of 8 different beta-blockers on the tear film stability-measured on the breakup time (BUT) of normal and healthy volunteers-was compared. All beta-blockers led to BUT reduction, however, the lowest influence was observed with levobunolol. The duration of this effect was compared with that of levobunolol, timolol and metipranolol. While the minor BUT alterations induced by levobunolol recovered after 30 min and were completely restored after 1 h, those of the two other beta-blockers lasted for over 2 h. The combination of timolol 0.5% with different artificial tears containing mainly polyvinyl alcohol (PVA) 1.4%, methylcellulose and polyacrylic acid initially led also to BUT reduction. The best results were obtained with PVA 1.4%.

[Orbital lipogranuloma].

Orbital lipogranulomas are rare inflammatory pseudotumors. Clinical and histopathological findings of 4 own cases are demonstrated and discussed.

Complications by liquid plastics in ophthalmic surgery. Histopathologic study.

Retrospective morphological studies of ocular tissues after treatment with liquid plastics reveal many complications induced by the material. Cyanoacrylates for punctal occlusion glide into the lacrimal sac and lead to obstructive dacryocystitis and even to reactive xanthogranuloma. The use of this glue in combination with a hard contact lens as 'artificial epithelium' provokes a remarkable stromal and endothelial cell loss of the cornea. Intraocular silicone oil sometimes induces a massive epiretinal macrophage activity with storage of the material or reactive fibrosis when it gets into the subretinal space. Liquid alloplasts for intraocular lens surgery of the future are tested in experimental studies but are still not satisfactory.

[Immediate effect of a topical beta-blocker on breakup time].

The comparison of different beta-blocking agents and their influences on the breakup time (BUT) reveals that levobunolol reduces the BUT in the same way as physiologic saline solutions, while other beta-blockers lead to an enormous reduction of BUT.
