Histogenesis and tissue reconstruction of mixed mesodermal tumor.

The clonal cell lines designated as HIRS-BMS and HIRS-BMA were established from HIRS-BM (multipotent primitive cells which differentiated into rhabdomyosarcoma and adenocarcinoma cells) by a single cell plating method. The HIRS-BMS was a rhabdomyosarcoma cell line composed of elongated fibrous cells which contained myoglobin. The HIRS-BMA was an adenocarcinoma cell line composed of round cells. The tissue reconstruction of these cell lines was studied in vitro (rotation culture system) and in vivo (transplantation into hamster cheek pouch). The HIRS-BMS cells produced rhabdomyosarcoma, both in vitro and in vivo, and the HIRS-BMA produced poorly differentiated adenocarcinoma, in vitro and in vivo. The mixture of HIRS-BMS and HIRS-BMA produced a mixed mesodermal tumor resembling the original tumor. These results support the theory of a combined tumor as a cause of mixed mesodermal tumor.

Histologic factors influencing prognosis of adenoid cystic carcinoma of the head and neck.

Histologic specimens from 61 cases of adenoid cystic carcinoma of the head and neck were reviewed in order to determine prognostic factors of this peculiar neoplasm. After the tumor was reviewed and classified according to several criteria, follow-up information, which was available in 54 cases, was analyzed and compared with each histological subgroup. As for the histologic pattern, the prognosis of adenoid cystic carcinoma with a solid pattern was worse than that without a solid pattern in respect of 5- and 10-year survival rates. However, there was no difference in 15-year survival between the two groups. Tumors with severe cell atypia showed more malignant behavior than those with moderate or mild atypia (p less than 0.001). Increased mitotic activity of more than two cells in mitosis per high power field was also found to be a sign of poor prognosis (p less than 0.001). Presence of perineural invasion was not correlated with the final outcome of the disease. Cell atypia and mitotic activity as well as the histologic pattern should be evaluated for assessing the degree of malignancy of adenoid cystic carcinoma.

[Malignant mucosal melanomas of the head and neck--collective review from six cancer hospitals].

Forty-nine patients with mucosal malignant melanoma of the head and neck treated at six cancer hospitals were reviewed. Of the 49 melanomas, 25 were located in the oral cavity and 24 in the nasal or paranasal cavity. Thirty of them were classified as stage I, 14 as stage II, and five as stage III. The initial treatment modality included surgery, radiotherapy, chemotherapy, cryotherapy and immunotherapy in varying combinations. The five-year survival rate was 17.4% for oral melanoma and 19.1% for nasal and paranasal melanoma.

[Head and neck cancer: choice of treatment in relation to clinical and pathological findings].

There are several organs in the head and neck region, and cancers arising in this region show a variety of histological findings. Therefore, the first choice of treatment for head and neck cancer should be decided according to sites of origin, clinical staging and pathological findings. Furthermore, the head and neck region plays very important roles in speaking, eating, and esthetic functions. The principles of treatment for head and neck cancer are as follows. As regards carcinoma of the larynx and oral cavity, radiotherapy is usually adopted when the carcinoma is small and limited, and surgical treatment is recommended in cases of rather advanced carcinoma. In the treatment of carcinoma of the nasopharynx, radiotherapy is undoubtedly the first choice of treatment and chemotherapy is combined in advanced cases. In the case of carcinoma of the methopharynx, surgical treatment is advisable for well differentiated squamous cell carcinoma and radiotherapy for poorly differentiated squamous cell carcinoma. For carcinoma of the hypopharynx, surgical treatment is advisable except for T1 cases of the pyriform sinus. Combination of radiotherapy, arterial chemotherapy and limited surgery is generally accepted as the method of treatment for maxillary sinus carcinoma.

The effect of a prostaglandin I2 analogue (OP-41483) on renal function in burned rabbits.

It is known that the number of platelets decreases after burn injury. However, the mechanism of such a decrease and its influence on systemic organs have not been fully elucidated. In this study, we evaluated the efficacy of OP-41483 (a derivative of the prostaglandin I2 (PGI2) in burned rabbits with special reference to its association with renal function. Full thickness skin loss burns, covering 35 per cent of the total body surface area, were experimentally produced on the back of rabbits. Among the following groups of rabbits (six rabbits each), the time courses of renal function and platelet function were compared; Group I (no treatment), Group II (fluid therapy only), Group III (treated with OP-41483 + fluid therapy; IIIa: 50 ng/kg/min, IIIb: 75 ng/kg/min, IIIc: 100 ng/kg/min). The results show that in Group I, all rabbits died after 8 h. Renal function tests in this group showed a decrease in creatinine clearance and increases in FENa and CH2O. Such changes were improved in Groups IIIb, IIIa and II (in order of the degree of improvement). Group IIIc showed no improvement. As for the platelets the decrease in platelets was the smallest in Group IIIb, where platelets showed no decrease until 8 h after injury and the fibrinogen concentration increased. No improvement was found in Group IIIc by our tests, presumably owing to an overdose of OP-41483. The optimal dose of OP-41483 for improving the renal function of burned rabbits was 75 ng/kg/min in our study.

Establishment and characterization of a new clonal strain derived from human benign gastric teratoma.

Various kinds of cell-types such as squamous epithelial cells, columnar epithelial cells, sarcomatous cells and fibroblast-like cells proliferated in vitro during the initial period of the primary culture from the human benign gastric tridermal teratoma. The fibroblast-like cells designated HOGT clonal strain were established from a single cell of primary cultures. The HOGT clonal strain grew well and 70 serial passages were successively carried out within 17 months. Although the HOGT cells are karyologically normal, when transplanted into the subcutis of nude mice they produced a benign tridermal teratoma. The results obtained from cell culture and heterotransplantation of teratoma strain enabled us to hypothesize that the graft (tridermal teratoma) in mice may arise from a pluripotent stem cell.

Radial forearm flap in reconstruction following surgery for head and neck cancers.

The forearm flap developed in China provided a most useful addition to the field of head and neck reconstruction. Because of its thin and pliable components, long and large caliber pedicle vessels and reliable vascularity, etc., we prefer to use the microvascular transfer of this flap for reconstruction of oral, oropharyngeal, and hypopharyngeal regions following cancer ablation and recognize further evidence of the versatility of this flap. In this paper, we describe the techniques of forearm flap transfer and the results of 37 cases experienced between March, 1983 and October, 1984.

Correlation between prognosis and degree of lymph node involvement in carcinoma of the head and neck.

The purpose of this report is to discover the correlation between the prognosis and the degree of lymph node involvement in carcinoma of the head and neck. Three hundred and thirty-eight cases of head and neck carcinoma who had undergone neck dissection at the National Cancer Center Hospital from 1962 through 1979 were reviewed and analyzed. The sites of the primary lesions were the oral cavity, larynx, and hypopharynx. Radical whole neck dissection had been performed in 177 cases, partial neck dissection in 97 cases, and bilateral neck dissection in 60 cases. The cervical lymph node metastases from oral cavity carcinoma were mostly confined to L1 and rare in L4 (L-classification by UICC). The prognosis of L4 cases was extremely poor, and thus oral carcinoma cases could be spared posterolateral dissection. The control rate of neck lymph nodes by neck dissection and the prognosis of the patients were influenced by the existence and number of histologically positive nodes. The cervical recurrence rates were 28% of all cases, 11% of histologically negative cases, and 39% of histologically positive cases. Recurrences in the dissected part of the neck were observed in 10% of all cases. The five-year survival rates were 75% of the cases without histologically positive nodes, 35% with histologically positive nodes, 59% with single node metastasis, and 26% with multiple node metastasis.

Pharyngoesophageal reconstruction using a fabricated forearm free flap.

A new microsurgical alternative in reconstruction of the pharynx and cervical esophagus is reported. A trapezoidal forearm flap is fabricated into an inverted skin tube and placed in the pharyngoesophageal defect. Although microvascular anastomoses are required to revascularize the transferred forearm flap, the long and large nutrient vessels of the flap make anastomoses easy and reliable. None of our 12 patients demonstrated any necrosis of the transferred flap. This one-stage, less invasive operation for pharyngoesophageal reconstruction greatly benefits older persons, who are the more likely to be involved with pharyngoesophageal carcinomas.

Repair of stenosed tracheostoma.

A technique to repair the stenosed tracheostoma following total laryngectomy is described along with a brief discussion of some of the factors causing tracheostomal stenosis. Technical details are presented, and the importance of an interruption of the line of circular scar contracture is emphasized.

Nasopharyngeal carcinoma: immunohistochemical study for keratin, secretory component and leukocyte common antigen.

Immunohistochemical studies using antisera to keratin and secretory component (SC) and monoclonal antibody against leukocyte common antigen (LCA) were performed on 92 biopsy and autopsy specimens taken from 65 patients with nasopharyngeal carcinoma. Five biopsy specimens from malignant lymphoma of the nasopharynx and tonsil, and 20 biopsy specimens of nasopharyngeal epithelium were also included in the study. Keratin was positively stained in all squamous cell carcinomas and nonkeratinizing carcinomas, and 38 of 46 undifferentiated carcinomas (82.6%), but in no malignant lymphomas. LCA was intensely stained in all malignant lymphoma, but in no nasopharyngeal carcinomas. SC was positively stained in two of the 46 undifferentiated carcinomas (4.3%). Nasopharyngeal carcinoma is a definite malignant epithelial neoplasm and can be distinguished from malignant lymphoma by immunostaining for keratin and LCA. Some undifferentiated carcinoma may show cellular differentiation toward ciliated epithelium.

[Present status and characteristics of the terminal care of dying patients in head and neck cancer].

Sixty-two head and neck cancer patients who died in 1981 and 1982 were analyzed. At the terminal stage, most of the patients could see or feel their tumors by themselves. Seventy-one percent of them were laryngectomized or tracheotomized, so they could not speak, and its was frequently difficult to know what they wanted to say or ask. These 2 facts made the clinical care of these patients difficult. More than half of the families of these patients did not want them to live longer only to be tortured by their disease.

[Cases of head and neck carcinoma detected by sputum cytology--lung cancer screening and head and neck cancer].

Seven cases of head and neck carcinoma are presented. They were first noticed by sputum cytology, which was ordered to check lung cancer because of hemosputum (three cases), or in a general check-up of asymptomatic individuals. None of them had lung cancer, and three were found to have piriform sinus carcinoma, two mesopharynx carcinoma, and two vocal cord carcinoma. All seven cases were histologically diagnosed as squamous cell carcinoma. Their clinical stages were as follows; T 1 N 0, three cases; T 1 N 1, one case; T 2 N 0, two cases; T 2 N 1, one case (UICC 1978). These cases remind us that positive sputum cytology is not only a sign of lung cancer, but also of other upper aerodigestive tract malignancies. Sputum cytology may be useful for checking asymptomatic head and neck cancer patients, though it is still too costly and time-consuming to be applied to mass screening.
