[Heterotransplantability of human anaplastic thyroid carcinoma line (HOTHC) in nude mice, and biological properties of the heterograft].

The heterotransplantability of HOTHC line (human anaplastic thyroid carcinoma) into the subcutis of BALB/c nude mice and the biological properties of grafts were discussed. (1) The HOTHC line showed high transplantability, and 1 x 10(4) cells produced anaplastic carcinoma (giant cell type) containing the colloid-like substance. (2) The grafted tumors grew rapidly and the mice were dead within 2 months after transplantation. (3) The number of leukocytes (neutrophils) of mouse peripheral blood increased as the tumor size increased, and the leukocyte count returned to a normal value after removal of the tumor. (4) The conditioned media of HOTHC line formed colonies of granulocytes (neutrophils) on soft agar. These phenomena revealed that HOTHC is a granulocyte-colony stimulating factor-producing line. (5) The conditioned media of HOTHC line showed promoting-effect on neovascularization on the chorioallantoic membrane.

[Establishment and characterization of a human malignant schwannoma cell line (HKMS)].

The malignant schwannoma cell line (HKMS) was established from the subcutaneous tumor of Axilla region of a 48-year-old Japanese woman. The HKMS line has the following biological properties. 1. The HKMS cells were spindle in shape and showed neoplastic and pleomorphic features. The monolayer sheet of HKMS cells showed the resemble cell-arrangement with that of the original tumor tissue. 2. The cells showed a stable growth and the serial passages were successively carried out 150 times within 3 years. Their population doubling time is about 40 hours. 3. The chromosome number varied widely, and the modal number was stable at the 78-80. The marker chromosomes were present. 4. The cells were transplanted into the subcutis of nude mice and produced the malignant schwannoma.

[Childhood Ki-1 lymphoma complicated with multiple bone destruction].

A 4-year-old girl was admitted because of fever, swelling of left chest wall and left axillary lymphadenopathy. Chest XP revealed left pleural effusion. Ga-scintigram showed multiple accumulation in skull, left ribs and iliac bone. A diagnosis of childhood Ki-1 lymphoma was made from the pathological findings of tumor in the skull. Immunopathological study revealed that the neoplastic cells were CD8 positive (suppressor phenotype).

[Establishment and characterization of a human malignant fibrous histiocytoma line serially transplanted in nude mice].

Serial heterotransplantation of human malignant fibrous histiocytoma (MFH) derived from tibia was attempted in BALB/c nu/nu mice, and HKMFH-nu transplantable tumor line was established. This line had the following biological properties. (1) Eighteen serial passages were carried out in 41 months. (2) Morphological changes of the grafts occurred in nude mice with serial passages: During the first 6 passages, histiological picture was consistent with the common type of MFH similar to that of the original tumor, then after the 7th passage, the myxoid type coexisted with the common type, and finally the myxoid type occupied the entire grafts to form large cysts. (3) The common type grafts grew more rapidly than the myxoid type grafts. (4) Granulocytosis (neutrophilia) was observed in mice bearing the common type tumor, but not in mice bearing the myxoid type tumor.

Myogenic expression in esophageal polypoid tumors.

Four cases of esophageal polypoid tumors composed of squamous cell carcinoma and spindle cell sarcomatous components were investigated. Squamous cell carcinoma was consistently present in the base of the polypoid lesions in all four cases and was also intermingled with spindle-shaped sarcomatous cells in two cases. Metastases in the lymph nodes were observed in two cases: one was squamous cell carcinoma with a sarcomatous component and the other consisted of a pure sarcomatous component. All tumors involved at least the muscularis mucosae. In the sarcomatous region, the tumor was composed of highly anaplastic cells with or without forming interlacing fascicles. Pleomorphism was marked and bizarre giant cell forms were frequent. Mitoses were frequently present. Immunohistochemical study revealed that the anaplastic cells in the sarcomatous component in all cases were immunoreactive to desmin, muscle actin, vimentin, and alpha 1-antichymotrypsin, but were negative for cytokeratin, even in the metastatic tumors of the lymph nodes. The immunohistochemical results favor myogenic differentiation of the anaplastic cells, and these tumors were considered to be true carcinosarcomas composed of squamous cell carcinoma and leiomyosarcoma.

Establishment and characterization of a human ovarian anaplastic carcinoma cell line.

Ovarian carcinoma cell line HTBOA was established from a human ovarian anaplastic carcinoma. It has been proliferating stably since the beginning of culture 3 years ago after 91 passages. The cells were spindle like, oval, and polygonal in shape, and the nuclear chromatin was coarse in grain with many nucleoli, showing neoplasticity and pleomorphism. The cells proliferated rapidly, and the population doubling time was 24-28 hr. The chromosomes showed a wide distribution of aneuploidy, the mode was in the hypotriploid range, and many marker chromosomes were observed. Heterotransplantation was easy, and subcutaneous transplantation of 1 X 10(5) cells in nude mice forms a tumor that is histologically similar to the original anaplastic carcinoma. The most noteworthy characteristic of the cell line was that it caused granulocytosis in nude mice with carcinomas, and formed the colonies of granulocytes in an in vitro culture system. The cell line was considered to be producing a granulocyte colony-stimulating factor.

Tumor angiogenic activity of gynecologic tumor cell lines on the chorioallantoic membrane.

Tumor angiogenic activity (TAA) from tumor angiogenesis factor (TAF), produced by 24 cell lines of various kinds of gynecologic tumors, was assayed onto chorioallantoic membranes (CAMs) of chick embryos. Methylcellulose (1%) pellets containing 1 x 10(7) cells were placed on 8-day-old postfertilized CAMs, and the grade of neovascularization was assayed 3 days after inoculation. Neovascularization occurred prominently in such cell lines, as HTBOA (poorly differentiated ovarian carcinoma), HUOCA-II (poorly differentiated clear cell adenocarcinoma), HWUA (poorly differentiated endometrial adenocarcinoma), and in HKUS (uterine cervical small cell carcinoma); however, neovascularization did not occur in SNK (uterine leiomyosarcoma line). The cell lines which secreted TAF showed high heterotransplantability in the nude mice and produced rapidly growing tumors which were rich in blood vessels. However, the SKN line which did not secret TAF was not transplantable. These results suggested that there was a close relationship among TAA, transplantability, and tumor growth rate.

[Quality of life in treated head and neck cancer patients].

Sixty-Seven patients with head and neck cancer have been studied with respect to their quality of life after their treatment. The Grogono Woodgate index was used to evaluate their quality of life, the quality of life of patients treated for laryngeal cancer was excellent, even if they underwent total laryngectomy. On the contrary, however, patients who had hypopharyngeal cancer and had undergone a pharyngolaryngoesophagectomy had the lowest indices. As for patients with an oral cancer, their indices were high if the cancer was being controlled by radiotherapy.

[Phase II study of carboplatin in head and neck cancer].

To evaluate the clinical efficacy and safety of carboplatin for head and neck cancer, a phase II study was conducted in 21 institutions. Eighty-eight patients were entered into this trial, of which 70 were evaluable. Carboplatin was administered intravenously over 0.5-1 hr at the dose of 400 mg/m2 in good risk cases and 300 mg/m2 in poor risk cases. One patient achieved complete response(CR) and 13 achieved partial response(PR) with an overall response rate of 20%. In terms of response by histology, the response rate was 22% in cases of squamous cell carcinoma. The response rate was 26% in previously untreated patients and 16% in previously treated patients. One CR and 2 PR were achieved in 14 patients with prior cisplatin treatment. Toxicity was observed in 47% of the 70 evaluable patients. Nausea and vomiting were the most common toxicity observed (36%) followed by anorexia in 27%. Concerning hematologic toxicity, thrombocytopenia was noted in 32% and leukopenia in 29%. Elevation of BUN and creatinine was observed in only 2 cases and 1 case, respectively. It was concluded that carboplatin suggested activity similar to cisplatin, and also could be safely used at the outpatient level.

Oral and pharyngeal reconstruction using the free forearm flap.

Recent progress in myocutaneous and free flaps has facilitated the primary closure of various complex defects resulting from ablative surgery of head and neck cancers. The musculocutaneous flap (such as the pectoralis major flap or the latissimus dorsi flap) has in particular proved its greater versatility. The forearm free flap for intraoral defects can provide thin and pliable skin and has great potential in the reconstruction of oropharyngeal defects. Between November 1982 and August 1985, we achieved 74 free forearm flap transfers for patients with head and neck cancers. Herein we present several clinical applications of the free forearm flap and discuss its advantages and disadvantages, especially hand morbidity after flap elevation.

Heterotransplantation of mixed mesodermal tumor cells in nude mouse--histology of metastatic foci.

The mixed mesodermal tumor has an admixture of carcinoma and sarcoma. The HIRS-BM tumor cell line, derived from the mixed mesodermal tumor, was transplanted into the peritoneal cavity of BALB/c nude mice, and the metastatic foci were examined histologically and karyologically. The HIRS-BM cells produced mixed mesodermal tumors (admixture of adenocarcinoma and rhabdomyosarcoma) resembling the original tumor. The normal interstitial tissue was not present on the boundary of the sarcoma and carcinoma. These results support the combination theory as the cause of mixed mesodermal tumor.

Primary esophageal reconstruction after resection of the cancer in the hypopharynx or cervical esophagus: comparison of free forearm skin tube flap, free jejunal transplantation and pull-through esophagectomy.

Sixty-six patients with carcinoma of the hypopharynx or cervical esophagus received esophageal extraction and primary reconstruction. The methods of reconstruction were free forearm skin tube grafting, free jejunal transplantation and immediate pharyngo-gastrostomy after pull-through esophagectomy. Considering the postoperative complications of each method, free forearm skin tube transplantation is the safest, but it can only be used for a short defect of the esophagus. With pull-through esophagectomy, we are able to extract the whole esophagus and have no fear of leaving any remaining cancer in the thoracic esophagus, but the operative burden and the rate at which complications occur are higher. Free jejunal transplantation can be used for a short or long defect of the cervical esophagus with small operative burden. Among three methods of reconstruction, free jejunal transplantation is recommended for primary reconstruction of the cervical esophagus.

[Alteration in coagulation and fibrinolysis after burn injury and significance of anticoagulation therapy using heparin and antithrombin III concentrate].

Burn injury causes dynamic alterations in the coagulation and fibrinolysis, and so-called DIC often occurs in burned patients. In this study the clinical significance of heparin therapy combined with antithrombin III concentrate in animal experiments and clinical experiences were discussed. The changes in blood coagulation, fibrinolysis and kidney function and the effect of anticoagulation therapy using heparin were investigated in rabbits with third degree burn covering 35% of the total body surface area. The animals were subjected to determinations for various kidney function tests, blood coagulation and fibrinolysis tests, blood viscosity and hematocrit value before induction of the burn and after 8 and 24 hours respectively. Thirty rabbits were divided into a non-therapy group, an intravenous infusion group, a heparin group, an antithrombin III group, and an antithrombin III plus heparin group and the results were compared among them. Oliguria and a disturbance of kidney function were noted even at hour 8 after burn in the non-therapy group. In the intravenous infusion group urine volume was maintained well although the early stage of non-oliguric renal insufficiency was noted. The changes noted in the intravenous infusion group were prevented almost completely in the heparin group at hour 8, but FENa and CH2O were elevated at hour 24 probably because antithrombin III activity was depressed markedly. In the antithrombin III group and the antithrombin III plus heparin group, however, creatinine clearance was moderately elevated while FENa and CH2O remained unchanged as compared with the values before the burn. The antithrombin III plus heparin group showed slightly better results than the antithrombin III group in Ucr/Pcr ratio, creatinine clearance and CH2O. The results of the present study indicate that it is extremely effective to initiate appropriate fluid infusion therapy immediately after a burn and administer antithrombin III concentrate in combination with or without heparin for the prevention of acute renal insufficiency in patient with a severe burn. The effects of antithrombin III concentrate when used clinically were also discussed.

Reconstruction of the oral commissure.

A technical refinement for oral commissure reconstruction is presented. The oral commissure has an important role in oral sphincteric function. We use a combination of local skin flaps for soft tissue reconstruction and a pedicled tongue flap for intraoral lining, with satisfactory results. A case involving a large mucosal ulcer around the oral commissure, in which the excisional defect was repaired by this method, is presented.

The effect of drugs which affect the arachidonate cascade in burned rabbits.

A full skin thickness burn covering 35 per cent of the total body surface area was inflected on the back of rabbits. The animals were divided into seven groups and the effectiveness of OP-41483, aspirin (ASA), OKY-046 and CV-3988 was studied. All rabbits died between 8 and 24 h after injury in the no therapy group. Renal function tests in this group showed a decrease in Cler and an increase in FENa and CH2O. Such changes were improved slightly with infusion therapy. On the contrary, with OP-41483, OKY-046 and ASA, distinct improvements were observed (in order of extent of improvement), but with CV-3988 the improvements were rather small. As a result these drugs may also be effective in preventing organ failure after burn injury, but as such changes presumably occur immediately after the burn it is considered that a fast-acting agent such as OP-41483 is the most effective form of treatment.
