Hysteroscope for CO2 laser application.

Endoscopic laser surgery is becoming more common with the development of suitable instruments. A hysteroscope designed for use with a Sharplan CO2 laser is described in detail. The method of application, results of preliminary experience, and possible indications are discussed.

The CO2 laser in pediatric surgery.

The CO2 laser has found its place in surgery and is now an accepted modality in many fields. Its application in pediatric surgery is well based and it is our opinion that its use in this field should be more widespread.

A solitary subcutaneous nodule as a presenting sign of metastatic malignant melanoma.

Two patients were referred because of an innocent-appearing solitary subcutaneous nodule; excision revealed metastasis of malignant melanoma, but comprehensive investigation failed to disclose the primary tumor. In neither patient was there subsequent evidence of local recurrence but both died of disseminated disease--the first 2 years and the second 10 months after the excision.

New instrumentation and technique for laparoscopic carbon dioxide laser operations: a preliminary report.

Laparoscopic surgery, the accepted procedure for several indications, still has some limitations, mainly due to the risk of uncontrollable bleeding and damage to the surrounding tissues that may result from the procedure. With the advent of the surgical carbon dioxide laser apparatus and its advantage in producing a bloodless incision, the authors have modified their approach toward laparoscopic surgery. A variety of special straight laser channels for introduction into the abdominal cavity have been developed; use of these channels requires special care to protect surrounding tissues from damage during the procedure. A Sharplan laser delivery system comprising an articulated arm was used, and the operative area visualized and controlled by single- and double-puncture laparoscope systems. The techniques were first applied in rabbits and, after several improvements, used in humans for various indications. The laser laparoscope can be particularly useful in the treatment of endometriosis and lysis of pelvic adhesions.

Early surgical correction in the cerebral palsied hand.

The 5 year follow-up of 20 children suffering from cerebral palsy who had undergone early surgical release procedures for deformities of the hand has indicated certain positive advantages for this method of treatment. In all cases there was an improved cosmetic appearance of the hand and improved function to a greater or lesser degree, as compared with the preoperative state. Night splints have been retained, as it was found that deformities tend to recur if splintage is discarded completely. None of the patients has developed fixed contractures, and to date improvement in function has been sufficient to obviate the need for further surgical procedures.

Effect of exercise on nasal mucous velocity and nasal airflow resistance in normal subjects.

Nasal mucous velocity and nasal airflow resistance were measured in nine healthy subjects before, during 5 min, and 1 h after submaximal exercise of 20 min with a cycle ergometer set in such a way that heart rate ranged from 125 to 135 beats/min. Nasal mucous velocity rose from a base line of 7.6-12.7 mm/min during exercise and returned to the base-line value 5 and 60 min after exercise. The mean expiratory nasal airflow resistance at a flow of 0.4 l/s decreased from a base line of 1.6-0.6 cmH2O . (l/s)-1 during exercise and returned to the baseline value 5 and 60 min after exercise.
