Reduction mammaplasty: nipple-areola survival on a single breast quadrant.

Nipple viability was retained in 165 breasts undergoing reduction mammaplasty without dermal pedicles--even when 3 quadrants of the breast were excised. This technique adds versatility in shaping the breast and permits resection of the hypertrophied portions of the breast, rather than predetermined segments. It allows one to translocate and reposition the nipple-areola without tension or kinking of the breast parenchyma. It gives the patient esthetically pleasing breasts.

Bone growth alterations resulting from application of CO2 laser beam to the epiphyseal growth plates. An experimental study in rabbits.

The CO2 laser beam was used to create defects 3 mm in depth in the distal growth plate of one femur in a group of 59 rabbits. In most of the 32 rabbits in which the defect was created on the lateral aspect of the epiphyseal plate the procedure resulted in the subsequent development of valgus deformity and shortening of the femur. In most of the 27 rabbits in which defects were produced both medially and laterally to the epiphyseal plate there was a subsequent marked shortening of the bones. Histological studies revealed that the laser-induced defects of the growth cartilage led to epiphysiodesis and premature disappearance of the epiphyseal growth plates.

Long-term follow-up of clefts of the secondary palate repaired by von Langenbeck's method.

In analysing our small series with caution we tend to support the findings of Lindsay (1971) and Blocksma et al. (1975). Thus the combination of no facial growth deformity and a reasonable rate of velopharyngeal competence has encouraged us to continue using this simple and safe procedure for the treatment of cleft palates with intact lip and alveolus.

Results of the Wilke operation to stop drooling in cerebral palsy.

Bilateral submaxillary gland excision and posterior transplantation of the elongated parotid ducts (as advocated by Wilkie) has proved to be an excellent procedure in the surgical treatment of drooling. I report a series of 20 patients who had excellent results from this, with amelioration of the drooling and minimal postoperative complications.

The use of the CO2 laser for partial nephrectomy: an experimental study.

The CO2- laser knife was used for partial nephrectomy in an experimental study of 20 mongrel cats. Of these, 16 survived, two died of peritonitis, and two died of unrelated causes. The inherent characteristics of precision and hemostasis of this modality, already found useful in controlling complications in other fields of surgery, proved to be a safe and effective method for partial nephrectomy as shown by angiography and histopathologic examinations of the operated kidneys at 0, 7, 14, 21, and 90 days after surgery. Complications such as hemorrhage or urinary fistula were nonexistent.

Current laser surgery.

With the development of a suitable apparatus sufficient experience has been accumulated in clinical surgery to enable one to reach the conclusion that the CO2 laser has an application in all branches of surgery without exception, whereas it has a decided advantage over other modalities in (1) All operations where the anticipated blood loss is significant. (2) Operations performed on or through infected tissue. (3) Extirpation of malignant tumors. (4) Cavitational surgery involving the use of an endoscope or microscope.
