[Reoperation after open mitral commissurotomy for mitral stenosis].

Between 1975 and 1993, 16 of 95 patients who received open mitral commissurotomy for mitral stenosis required reoperation for recurrent mitral lesions with a mean duration of 11 years after the initial operation at Kawasaki Medical School Hospital. The mitral lesions necessitating reoperation involved restenosis in eight, stenoinsufficiency in six and regurgitation in two. In 13 patients, mitral commissure was well separated, and the mitral restenosis and regurgitation were caused by progressions of valvular and subvalvular lesions. Significant tricuspid valve regurgitation was also seen in nine patients, and in seven out of eight patients who were in NYHA functional class III or IV, tricuspid regurgitation of grade 3 was observed. The combined tricuspid regurgitation aggravated the patient's symptoms and became a major risk factor of the reoperation after open mitral commissurotomy.

[Analysis of the gene encoding human PC2, a prohormone processing enzyme].

Prohormone Convertase 2 (PC2) is a specific endoprotease responsible for the processing of proinsulin to insulin. PC2 is expressed in pancreatic islets, pituitary and brain but is very low or absent in most other tissues, such as liver, spleen and kidney. To evaluate the regulated expression of the human PC2 gene we have analyzed its structure and characterized its promoter. The gene spans > 130 kilobase pairs and consists of 12 exons. Comparison with the structure of the gene encoding human furin, revealed a high degree of conservation of exon-intron junctions. The hPC2 gene was localized to chromosome 20, band p11.2. The promoter region of the PC2 gene is very G + C rich and contains six potential Sp1 binding sites but no TATA or CAAT box. Analysis of the level of chrolamphenicol acetyltransferase activity with several deletion mutants identified the region from -1100 to -539 from the translation start site as essential for the PC2 promoter activity.

Polyclonal B cell activation, endotoxin tolerance, and limulus tests of endotoxin preparations of some periodontopathogens.

Potencies of polyclonal B-cell activation in C3H/HeN mice of Actinobacillus actinomycetemcomitans, Fusobacterium nucleatum, and Porphyromonas gingivalis endotoxins were 0.36, 0.13 and 0.04, taking Salmonella abortusequi as 1.0. F. nucleatum and P. gingivalis endotoxins showed positive reactions in C3H/HeJ mice. Most activities in C3H/HeN other than that of F. nucleatum were suppressed by polymyxin B. In C3H/HeJ mice, similar inhibitions were only 60% for P. gingivalis and hardly observed with F. nucleatum. The resistances to polymyxin B could be due to protein in the endotoxins. A promoting effect of T cells added to B cells was observed only in the activity of F. nucleatum endotoxin in C3H/HeJ mice; there was no influence in other groups. Test endotoxins had nearly the same ability to produce colony stimulating factor as did references and could not produce the factor in tolerant mice. The clinical significance of tolerance is discussed. Regression lines of endotoxin doses and limulus activities of test endotoxins and Salmonella were parallel, either in specific or non-specific tests. The lines of two test groups were also parallel; values obtained by two tests were very close. These data indicate that the test endotoxins did not contain (1-3)-beta-D-glucan and elicited qualitatively similar limulus reactions to that of the reference, despite their different chemical natures. In conclusion, these test preparations had an endotoxicity similar to that of the reference and contribute to produce periodontitis through polyclonal B cell activation.

[Clinical study of neuroleptic malignant syndrome in demented patients--examination of our case and reported cases].

Recently, some cases of neuroleptic malignant syndrome (NMS) in demented patients have been reported, because the demented patients, who were administered neuroleptics due to their abnormal behaviors, have increased. We also experienced an 85-year-old male demented patient with NMS. His NMS was induced by haloperidol and disappeared by quitting the neuroleptic and a water supply. The post-NMS relapse of his abnormal behavior was ameliorated by a kind of herbal medicine "Ohren-Gedoku-Toh". We attempted to grasp the clinical characteristics of NMS in dementia by investigating the present case and reported cases. As the result, the following characteristics are suggested, (1) the NMS in dementia was seen more in vascular dementias than in Alzheimer's diseases, (2) in all the cases, the drug to be responsible to NMS was haloperidol, (3) a walking difficulty appeared at even the pre-stage or early stage of NMS, (4) the administration of dantrolene and/or bromocriptine was necessary to treat the patients with a high temperature. Moreover, examining our case, Levenson's criteria for NMS which regards a high serum CPK may be clinically more useful in the diagnosis of the NMS in dementia, because an early diagnosis and treatment can be done according to the criteria.

[Changes in the intraoperative hormonal homeostasis following infusion of lactated Ringer's solution with high potassium concentration].

Changes in the concentrations of serum electrolytes and those of water and electrolyte regulating hormones were studied in mongrel dogs undergoing a sham operation (gastrectomy) under general anesthesia. Six dogs (L-group) were infused regular lactated Ringer's solution containing 1% glucose, and other six dogs (KL-group) were infused lactated Ringer's solution containing 1% glucose and potassium 10 mEq.l-1. The serum potassium concentration decreased significantly during the surgery in the L-group, but it remained unchanged in the KL-group. Blood sugar level increased during and after the surgery in the L-group. On the other hand, the blood sugar level changed in a similar way as that of the L-group during the surgery, but it decreased after the surgery in the KL-group. NEFA level also decreased after the surgery in the KL-group. Aldosterone secretion was increased after the surgery in the L-group. On the other hand, the aldosterone secretion increased during the surgery in the KL-group. The differences of the aldosterone secretion between the groups may be due to the direct influence of potassium on the adrenal cortex. Serum concentration of ADH increased in both groups during the surgery. In the L-group, it remained later, but it recovered toward the initial level after the surgery in the KL-group. The above data indicate that no peculiar change occurred in hormonal homeostasis of subjects receiving operations under general anesthesia following the infusion of lactated Ringer's solution with high potassium concentration.

[Detection of antinuclear antibodies and their significance as disease markers].

The improvement of the quality control in the detection of fluorescent antinuclear antibodies (FANA) have decreased the differences among institutions. However, the positive lower or upper reference limit and the criteria for the determination vary among laboratories. By examining 725 sera from children and 227 sera from healthy adults, we proposed the titers of 160-320 for children and 80-160 for adults as the positive lower limits. The higher positive incidence in the children's sera might be due to some abnormality in the ANA production or the higher sensitivity of the reagents used. The clinical significances of the anti-U1RNP antibody and anticentromere antibody (ACA) in ANA have been described. The anti-U1RNP antibody, present in only MCTD and related disorders, might play some role in the development of Raynaud's phenomenon and symptoms in a case of neonatal lupus erythematosus. ACA was detected in sera not only of CREST syndrome patients but also of PBC patients. The functions and molecular structures of nuclear antigens have recently been extensively investigated. However, the pathogenetic significance of most of the ANA and the mechanism of their production have not been clarified yet.

Effects of an anti-platelet drug (dilazep) in IgA nephropathy: comparison of clinical effects with renal biopsy findings.

To investigate renal biopsy findings arising in response to treatment with an anti-platelet drug in IgA nephropathy, 46 patients were treated with dilazep dihydrochloride (Dilazep), and a retrospective comparison was performed between the clinical effects and renal biopsy findings. After 6 months of treatment, 18 patients (39%) were judged to be improved if their proteinuria was ameliorated by a 25% or greater decrease with improved or persistent renal function. The group of improved patients exhibited mean decreased levels of urinary proteins in the range from 1.9 to 0.8 g/day after treatment (p < 0.01). By contrast, the unimproved group showed increased urinary proteins in the range from 1.2 to 2.0 g/day (p < 0.05). The improved group showed histological findings with fewer glomeruli exhibiting sclerosis and/or cellular crescents, with a lesser increase in mesangial matrix and with smaller tubulo-interstitial lesions than the unimproved group. By immunofluorescence, the improved group was found to have smaller amounts of glomerular IgA and IgG deposits. These findings suggest that an anti-proteinuric effect of Dilazep administration can be expected in patients with IgA nephropathy with relatively mild glomerulo-sclerotic lesions.

Correlations of C1q- and C3d-bearing circulating immune complexes with immunopathological disease activity in lupus nephritis patients.

The serum levels of circulating immune complexes (CIC) measured by three different types of enzyme immunoassay (EIA) using monoclonal anti-C1q and antibodies and C1q as solid phase reagents were compared with clinical disease activity and immunohistological glomerular lesions in 29 SLE patients. Three types of CIC measured by these assays (anti-C1q CIC, anti-C3d CIC and C1q SP CIC) showed significantly higher levels in patients than in controls and were significantly associated with the clinical and serological disease activities. Anti-C1q CIC showed good correlation not only with mesangial IgG depositions (P < 0.01), but also with that of C1q (P < 0.05). C1q SP CIC also showed a weak correlation with mesangial C1q deposition (P < 0.05). Serum levels of anti-C3d CIC increased with the degree of mesangial IgG and complement depositions. Analysis of the clinical course of a patient with active SLE revealed a more rapid decrease of anti-C1q CIC and anti-C3d CIC along with the improvement of disease activity, including the mesangial lesion, than that of C1q SP CIC. According to these results, the CIC detected with assays using monoclonal antibodies against complement fragments, especially the anti-C1q assay, is likely to provide specific information regarding the clinical, serological and immunohistological disease activity in lupus nephritis.

[Usefulness of Dornier MPL9000 lithotriptor with new optional fluoroscopic guiding system (electrode X 155: depth 15.5cm) in treatment of urinary tract stone].

The Dornier MPL9000 lithotriptor was originally equipped with an ultrasound guiding system. Recently an optional fluoroscopic guiding system was developed to overcome the known disadvantages of the ultrasound system and to exploit the merits of both systems by using them in combination. Herein, we examined the effectiveness and safety of this new optional system. From December 1990 to February 1991, we treated 41 patients with upper urinary tract stones with MPL9000 using the optional fluoroscopic system. Treatments were judged as effective if the patients were stone-free or the residual fragments were smaller than 4mm in diameter. The overall effectiveness rates at one month after the treatment were 54.5% for renal stones and 76.9% for ureter stones respectively. At three months after the treatment the effectiveness rate for renal stones was 83.3%, and that for ureter stones 95.5%. The changes in the laboratory data after the treatment were alight and caused no clinical problems. Complications during and after the treatment were also minor and tolerable. MPL9000 was sufficiently effective and safe in the treatment of upper urinary tract stones when it was used the new optional fluoroscopic system instead of the original ultrasonic guiding system.

A preliminary investigation of a method of detecting temporomandibular joint sounds.

This study established a method of detecting temporomandibular joint sounds based on signal-to-noise ratios. After comparing the temporomandibular joint signals obtained from three different sites over the skin, the articular eminence was found to be the best site for detecting temporomandibular joint sounds; this site provided the highest mean amplitude in the time domain waveform. However, using an electret condenser microphone at the intra-auditory meatus provided a broader, 20-decibel signal-to-noise bandwidth, which resulted in minimized artifacts. This method may be useful for recording temporomandibular joint sounds in the differential diagnosis of various temporomandibular joint conditions using spectral analysis.

Biliary excretion of cholephilic organic dyes in glutathione-depleted rats.

Sprague-Dawley rats were treated with a combination of buthionine sulfoximine (2.5 mmol/kg body wt) and diethyl maleate (6.2 mmol/kg). After 4 h, the hepatic glutathione was depleted to 0.02 +/- 0.01 mumol/g liver (mean +/- SD, n = 6) (4.73 +/- 0.29 mumol/g in controls (n = 6), p < 0.005). The bile flow rate was lower in treated animals than in controls (39.0 +/- 11.2 vs. 73.7 +/- 13.7 microliters.kg-1.min-1, p < 0.005). Bile concentrations of bile acids, phospholipids, and cholesterol were not changed compared to controls, while glutathione was virtually absent from the bile. A bolus of indocyanine green (6.5 mumol/kg), rose bengal (6.5 mumol/kg), or sulfobromophthalein-glutathione conjugate (20 mumol/kg) was injected i.v. and the biliary excretion was monitored. In glutathione-depleted rats, the excretion of indocyanine green was delayed, and the cumulative excretion in 90 min was 36 +/- 8% (n = 6) of the injected dose (79 +/- 18% in controls (n = 7), p < 0.005). The bile concentration of indocyanine green was lower in the glutathione-depleted rats than in controls, while the other dyes increased. The plasma disappearance curve of indocyanine green was not significantly altered by the treatment, whereas the hepatic retention of indocyanine green 90 min after the injection was significantly increased (58 +/- 12% of the injected dose vs. 8 +/- 2% in controls, p < 0.005). The results indicate that depletion of hepatic glutathione inhibits biliary excretion of indocyanine green.

Does LDL-apheresis in steroid-resistant nephrotic syndrome affect prognosis?

Low-density lipoprotein apheresis (LDL-A) was performed for nine episodes of steroid-resistant nephrotic syndrome in eight patients. The clinical and immunohistological findings were analysed retrospectively. Six of the patients had focal glomerular sclerosis (FGS), one had minimal-change nephrotic syndrome (MCNS), and one had membranous nephropathy (MN) with FGS. The LDL-A treatment, carried out 2-13 times (mean 7.33 +/- 4.05) for one nephrotic episode, at average intervals of 3-16 days (mean: 8.5 +/- 5.1 days) and combined with steroid pulse therapy and the administration of an antihyperlipidaemic agent in some cases, led to rapid amelioration of hyperlipidaemia. In six nephrotic episodes (5 patients) more than 50% reduction of proteinuria occurred (less than 3.5 g/day) (response-group). A significant elevation of serum albumin (more than 3.0 g/dl) was obtained in five of these episodes. The other three patients were resistant (resistant-group). The number of and intervals between LDL-A treatments in the response group (5 +/- 2.8 times and 5.8 +/- 4.1 days) were significantly less than those in the resistant-group (12.0 +/- 0.8 times and 13.2 +/- 1.8 days) (P < 0.05). After LDL-A, significant reductions were observed in the serum total cholesterol (T-CHO), phospholipid (PL), triglyceride (TG), free-CHO (F-CHO), and beta-lipoprotein (beta-lipo) (P < 0.05). HDL-cholesterol (HDL-C) increased somewhat after LDL-A. Further, Ccr was elevated in all nephrotic episodes, except in one patient who manifested renal failure after 6 months.(ABSTRACT TRUNCATED AT 250 WORDS)

[Method of DNA extraction and mecA, femA and PBP2' in multi-drug resistant bacterial strains].

Recently, it has become more important to establish rapid and reliable methods for identifying bacterial species and their drug resistances because of the high incidence of nosocomial infection caused by multi-drug resistant bacteria. At first, several enzymes were tested to evaluate the efficacy of DNA extraction. N-acetylmuramidase, lysozyme and achromopeptidase were most effective to extract DNA from E. faecalis, E. faecium and S. aureus, respectively. After achromopeptidase extraction, the mecA gene was amplified by polymerase chain reaction (PCR) in clinically isolated MRSA (95 strains), MSSA (66), MRSE (methicillin-resistant S. epidermidis, 37), MSSE (methicillin-sensitive S. epidermidis, 1), S haemolyticus(5), S.hominis(1) and others (Enterococcus, Pseudomonas, and Streptococcus, total 45). PCR products were analyzed by agarose gel electrophoresis. The positive rates were 95% (MRSA), 1.5% (MSSA), and 97% (MRSE). The mecA gene was also positive in 2/2 of methicillin-resistant S.haemolyticus and 2/3 of methicillin-sensitive S.haemolyticus and 1/1 of methicillin-resistant S.hominis. The mecA gene was not detected in 45 non-Staphylococcal strains. MecA and femA gene by PCR and PBP 2' by IRMA were further detected in newly isolated MRSA (20 strains), MSSA(20), MRSE(14), MSSE(1) and S.simulans(2). Complete correlation between MPIPC susceptibility and mecA were found. The femA gene was positive in 39/40 of S. aureus, and 0/14 of S.epidermidis, 2/3 of S.simulans. PBP2' were positive in 20/20 of MRSA, 0/20 of MSSA, 14/14 of MRSE, 0/1 of MSSE, 1/1 of methicillin-resistant S.simulans, and 0/2 of methicillin-sensitive S.simulans. In conclusion, diagnoses of MRSA by simultaneous detection of mecA and femA gene as well as PBP2' are similarly useful because of their specificity and rapidity.

Characterization of radiation-induced products of thymidine 3'-monophosphate and thymidylyl (3'-->5') thymidine by high-performance liquid chromatography and laser-desorption Fourier-transform mass spectrometry.

High-performance liquid chromatography (HPLC) and laser-desorption Fourier-transform mass spectrometry (LD FTMS) have been applied for direct measurements of radiation-induced products of nucleic acid constituents containing thymidine. Laser desorption FTMS could be used for the direct detection (neither hydrolyzed nor derivatized) of X ray-induced decomposition products of aqueous thymidine monophosphate. After these initial experiments, a variety of hydrogenated and hydroxylated thymine standards were acquired and examined by FTMS to assist in the identification of unknown radiation-induced decomposition products of thymine-containing nucleotides and dinucleotides. To extend these studies to dinucleotides, the radiation-induced products generated by the gamma radiolysis of thymidylyl (3'-->5') thymidine (TpT) were isolated by reverse-phase HPLC and identified by LD FTMS. Thymine and thymidine 3'-monophosphate were observed as the major products in this case. Several of the minor products of the HPLC profile were pooled in a single fraction and characterized simultaneously by LD FTMS. The resulting mass spectra indicated the presence of hydroxy-5,6-dihydrothymidine monophosphate, 5,6-dihydrothymidine monophosphate and thymidine monophosphate, thymine glycol, hydroxy-5,6-dihydrothymine, 5-hydroxy-methyluracil and 5,6-dihydrothymine. The combination of HPLC purification and LD FTMS structural characterization provides a useful tool for the direct measurement of radiation-induced products of nucleotides and dinucleotides.

[Histological evaluation of the liver and response to interferon in chronic hepatitis C].

Liver histology was compared in patients with chronic hepatitis C to clarify the difference between responders and non-responders to interferon therapy. We have treated 50 patients with chronic hepatitis C by natural IFN-alpha 6.0 MU/day for 2 weeks, daily, followed by intermittent therapy for 3-6 months. The overall effect of IFN was a complete response (CR) in 16 (32%), transient response (TR) in 14 (28%), and no response (NR) in 20 (40%). According to the pre-treatment liver histology, CR had a lower total numerical scores than TR and NR. Histological difference between CR and TR appeared to be the degree of portal inflammation and piecemeal necrosis, though NR showed an increase in portal fibrosis compared of CR and also TR. Response to IFN may depend on the degree of liver fibrosis rather than other histological factors. To assess the degree of liver fibrosis is thought to be important for the prediction of the efficacy of IFN therapy.

Identification of human DAN gene, mapping to the putative neuroblastoma tumor suppressor locus.

The expression of DAN gene (previously designated as N03 gene) is significantly reduced in a variety of transformed rat fibroblasts, including v-src- (SR-3Y1), SV40- and v-mos-transformed 3Y1 cells, compared with that in parental 3Y1 cells. Recently, DAN gene has been shown to possess a tumor suppressive activity when it is overexpressed in SR-3Y1 cells (Ozaki & Sakiyama, 1994). To assess the involvement of DAN gene with human neoplasms, we have isolated human DAN counterpart from a normal lung cDNA library by using rat DAN cDNA as a probe, and determined its chromosomal location. Human DAN gene mapped to chromosome 1p36.11-p36.13, which is well known to show highly significant linkage with the genesis and/or progression of human neuroblastoma. Southern blot analysis on tumor DNA from 26 patients with neuroblastoma has detected three patients showing genomic rearrangement or deletion within or closely linked to the DAN gene locus. Collectively, we propose that human DAN gene is a possible candidate for a tumor suppressor gene of human neuroblastoma.

Divergent expression of EtA and EtB receptors in response to cyclosporine in mesangial cells.

We examined the effects of cyclosporine (Cy) on preproendothelin-1 (prepro Et-1) and both Et receptors, A-type (EtA) and B-type (EtB), in rat glomerular mesangial cells. Cy at 10(-5) mol/L, the concentration relevant to tissue levels in vivo, increased mRNA for prepro Et-1 after both 1 and 3 hours to 183 +/- 14% (n = 8) and 147 +/- 12% (n = 9) of levels seen in control cells. A similar pattern was observed at a lower dose of Cy (10(-6) mol/L, the concentration relevant to plasma levels in vivo): at 1 hour, expression increased to 146 +/- 15% (n = 5); at 3 hours, expression was 159 +/- 25% (n = 8). Concomitant with these changes in expression of prepro Et-1 mRNA, the Et-1 protein nearly doubled at 3 hours. Both EtA and EtB mRNA were expressed in unstimulated mesangial cells and were affected differently by Cy. Little change was observed in the EtA mRNA. In contrast, EtB mRNA increased with Cy (10(-5) mol/L) up to 129 +/- 8% (n = 6) at 1 hour and 265 +/- 62% (n = 3) at 3 hours. A similar effect was seen with the lower dose of Cy: EtB expression increased to 129 +/- 9% (n = 6) of control value at 1 hour and 253 +/- 74% (n = 4) at 3 hours. These results indicate that Cy enhances expression of mRNA for prepro Et-1 and production of mature Et-1 by glomerular mesangial cells. Further, Cy has differential effects on EtA and EtB, affecting EtA minimally while markedly increasing the expression of EtB. These results constitute the first demonstration that Et receptor subtypes are modulated in a pathophysiological setting. The divergent modulation of EtA and EtB raises the possibility that there are differences in contribution of each of the receptor subtypes to pathophysiological mechanisms of Cy toxicity.

Concentration- and pH-dependent ammonia yields in the reaction of hydrated electrons with glycylglycine.

Competition between recombination of the radicals formed in the gamma radiolysis of water (eaq-, OH, H) and the reaction of eaq- with glycylglycine is investigated in glycylglycine solutions of various concentrations and pH. First, the concentration dependence of the ammonia yields is measured in the glycylglycine solutions of between 0.5 mM and 1.0 M. The G values of ammonia appear to reach a plateau of 2.47 +/- 0.05 molecules/100 eV at the low concentrations (1-5 mM) and increase to 3.81 +/- 0.07 molecules/100 eV at the high concentration (1 M). Secondly, the pH dependence of the ammonia yields is measured at low (1 and 5 mM) and high (0.1 and 0.2 M) concentrations of glyclyglycine. For concentrated glyclyglycine solutions, the ammonia yields increase with increasing pH, as expected based on comparison of the probabilities of the competitive reactions. However, the G values of ammonia reach a maximum at pH 9.0 and then decrease as the pH increases for dilute solutions. The lower G values at higher pH for dilute glycylglycine solutions may result from increased interspur recombination since the rate constant for the reaction of eaq- with glycylglycine is decreased and, as a consequence, the scavenging capacity of glycylglycine for eaq- decreases approximately 4.7-fold from low to high pH.
