Simultaneous measurement of plasma 18-oxocortisol and 18-hydroxycortisol levels in normal man.

Plasma 18-oxocortisol (18-oxoF) and 18-hydroxycortisol (18-OH-F) were measured in 47 healthy subjects. Plasma 18-oxoF and 18-OH-F in the early morning were 0.827 +/- 0.04 nmol/l and 3.29 +/- 0.175 nmol/l, respectively. The plasma levels of both steroids correlated with each other and with cortisol, but not with aldosterone. Postural stimulation with or without furosemide administration increased 18-oxoF, 18-OH-F, aldosterone and plasma renin activity (PRA). Two hours after 2 mg of oral dexamethasone administration or after an overnight 2 mg of dexamethasone suppression cortisol, 18-oxoF and 18-OH-F decreased. Cortisol, aldosterone, 18-oxo-F and 18-OH-F increased after the intravenous administration of 250 micrograms of 1-24 ACTH. Changes in plasma 18-oxo-F and 18-OH-F levels correlated with PRA change during the posture studies and correlated with the change of ACTH during the dexamethasone studies. The ratios of post-/pre-test values of the postural stimulation and dexamethasone suppression in 18-oxoF and 18-OH-F were lower than that of aldosterone. Plasma 18-oxoF and 18-OH-F are more dependent on ACTH than on the renin-angiotensin system. The ratio of 18-OH-F/18-oxoF, which is between 4 and 5, remains constant during the various stimulation or suppression maneuvers.

The role of macrophage colony stimulating factor in the peritoneal fluid in infertile patients with endometriosis.

Macrophage colony stimulating factor (M-CSF) concentrations in the peritoneal fluid of 44 infertile patients were measured. The concentration of M-CSF was significantly higher in the patients with endometriosis than in those without endometriosis. There was no significant difference between the M-CSF concentration and stage of endometriosis classified by revised American Fertility Society (rAFS) classification in the 44 patients. However, in primary infertility patients with endometriosis there was a significant correlation between peritoneal M-CSF levels and rAFS points of endometriosis. We speculate that there is some relationship between the increased levels of M-CSF in the peritoneal cavity and the primary infertility associated with endometriosis especially progression of endometriosis.

Syalil-SSEA1(SLX) levels in supernatant of cultured human lung carcinoma cell lines.

SLX levels in the culture supernatant of the following 50 cell lines were measured by RIA: pulmonary carcinoma cell lines derived from 46 patients, cell lines of other human cancers derived from 4 patients, and 2 passaged human fibroblast cells as control. Of the 46 pulmonary carcinoma cell lines, 17 (37%) were SLX positive. When the SLX-positive rate was analyzed in relation to the histological type of pulmonary carcinoma, the positive rate was 71% (10/14) for adenocarcinoma, 27% (3/11) for squamous cell carcinoma, 33% (2/6) for large cell carcinoma, 0% (0/11) for small cell carcinoma and 50% (2/4) for adenosquamous cell carcinoma. Analysis of the relationship between tumor cell proliferation and SLX level in 20 patients revealed that the SLX level in the supernatant of SLX-producing cell lines becomes higher in proportion to the increase in the number of these cells. The SLX-positive rate did not differ significantly among different stages of pulmonary carcinoma at the time of tissue collection. There was no significant correlation between SLX production and prognosis. SLX production by each cell line was not correlated with the doubling time of the same cell line in vitro or in vivo (in nude mice). SLX production also showed no correlation with the duration of tumor cell passage.

Asialoglycoprotein receptor function of hepatocytes in patients with autoimmune hepatitis.

To assess whether the hepatocyte asialoglycoprotein receptors are affected in patients with autoimmune hepatitis, the function of the hepatocyte asialoglycoprotein receptor was investigated in patients with autoimmune hepatitis or chronic hepatitis C. A new radionuclide liver imaging technique, Technetium-99m diethylenetriamine-pentaacetic acid-galactosyl human serum albumin, was applied to evaluate the function of the receptor. A Receptor Index (LHL 15) was calculated by dividing the radioactivity of the liver region-of-interest (ROI) by that of the liver plus heart ROI 15 min after radiolabeled ligand injection. Serum albumin, prothrombin time, hepaplastin test and plasma retention rate of indocyanine green at 15 min were not significantly different between patients in the autoimmune hepatitis group and chronic hepatitis C group. In addition, the Receptor Index of patients with autoimmune hepatitis, in whom the asialoglycoprotein receptor is a candidate target antigen for autoimmune response, was similar to that of patients with chronic hepatitis C. These results indicate that the hepatocyte receptor for asialoglycoproteins is not affected in patients with autoimmune hepatitis.

Inhibitory effect of atrial natriuretic peptide on accelerated endothelin secretion from cultured human endothelial cells.

This study investigated the effect of atrial natriuretic peptide (ANP) on endothelin (ET) secretion from cultured human endothelial cells. Confluent umbilical venous endothelial cells were incubated with experimental agents in multi-well plates, and the level of immunoreactive ET in the medium was measured by radioimmunoassay. There was no significant effect of ANP (10(-8), 10(-7) and 10(-6) M) on ET secretion after a 3- or 6-hour incubation. However, with 24-hour incubation, ANP significantly inhibited ET secretion from cultured human endothelial cells (control, 139.0 +/- 7.2 fmol/well; 10(-7) M, 89.4 +/- 4.7 fmol/well; 10(-7) M, 79.4 +/- 8.2 fmol/well; 10(-6) M, 71.0 +/- 10.1 fmol/well, P < 0.01). Furthermore, the addition of 8-bromo-cyclic GMP to the medium inhibited ET secretion (control, 147.2 +/- 2.9 fmol/well; 10(-5) M, 140.9 +/- 2.3 fmol/well; 10(-4) M, 143.0 +/- 1.0 fmol/well; 10(-3) M, 96.6 +/- 6.3 fmol/well, P < 0.01). These findings demonstrate that ANP inhibits accelerated ET secretion from cultured human endothelial cells, probably due to augmentation of intracellular cyclic GMP levels by ANP-activated guanylate cyclase.

Nitric oxide may participate in V2 vasopressin-receptor-mediated renal vasodilation.

Using pentobarbital-anesthetized euvolemic dogs, we investigated whether vasopressin V2-receptor stimulation induced renal vasodilation and whether nitric oxide (NO) had a role in the process. Intrarenal infusion of arginine-vasopressin (AVP) resulted in renal vasoconstriction with pressor response. After preadministration of a V1-receptor antagonist, however, intrarenal infusion of AVP caused an increase in renal blood flow (RBF) without pressor action, indicating renal vasodilation. This renal vasodilation was not observed when we infused AVP after-simultaneous pretreatment with V1- and V2-receptor antagonists. Even in the absence of the V2-receptor antagonist, this renal vasodilation was attenuated by intrarenal infusion of L-NG-nitroarginine (L-NNA). Concomitant infusion of L-arginine prevented the inhibitory effect of L-NNA on renal vasodilation induced by intrarenal infusion of AVP in the presence of the V1-antagonist. These data indicate that the renal vasodilation caused by intrarenal infusion of AVP in the presence of a V1-antagonist was mediated by V2-receptor stimulation, and the inhibitory effect of L-NNA on V2-receptor-mediated renal vasodilation was attributed to its inhibitory effect on NO synthesis, suggesting that NO may participate in V2-receptor-mediated renal vasodilation.

[Influence of anti-androgen therapy for prostatic hypertrophy on lipid metabolism].

Antiandrogen therapy has an important role in the treatment of patients with benign prostatic hypertrophy who lack indication for surgery. Herein, the effects on lipid metabolism of administration of antiandrogen agents for benign prostatic hypertrophy are reported. Eighty patients with benign prostatic hypertrophy were each treated with the antiandrogen agents, chlormadinone acetate, allylestrenol, gestonolone caproate and oxendolone for 12 months. The levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL), alpha-lipoprotein, apoprotein, and maronediardehyde (MDA) were measured every 4 weeks after initiation of antiandrogen treatment. In the chlormadinone acetate group, the TG level was significantly decreased between 3 and 6 months after treatment (p < 0.05). In the oxendolone group, the alpha-lipoprotein level was also elevated between 3 and 6 months and between 6 to 12 months after treatment (p < 0.05). The MDA level was also significantly elevated 6 and 12 months after treatment. However, the levels of the other lipids were within the normal range. In conclusion, the changes in the levels of plasma lipoprotein, apoprotein and MDA resulting from antiandrogen therapy were unlikely to be a cause of ischemic coronary disease.

[Study for relapse cases of chronic hepatitis C treated with interferon (IFN) and approach to more successful re-treatment of IFN].

It is well known that relapse of hepatitis develops in 30 approximately 40% cases of chronic hepatitis C after treatment of IFN. We assessed the background of those cases and investigated a scheme providing more successful re-treatment of IFN for those relapse cases. Cases developing complete response to re-treatment showed as follows. (1) During first IFN therapy, ALT sustained within normal. (2) Serum HCV-RNA levels before re-treatment demonstrated significantly lower than the levels of first therapy. (3) IFN dose of re-treatment was greater than first therapy. (4) Terms of initial daily IFN administration tended to be more longer (4W) than first therapy (2W).

[Long-term interferon (IFN), neominophagen C (SNMC) combination therapy of active liver cirrhosis type C].

Possible effects of IFN and SNMC combination therapy for active liver cirrhosis were described. 28 patients with active liver cirrhosis were treated with natural IFN alpha 3-6 MU TIW for 3-17 months (mean, 8 +/- 6). 8 of 28 patients received combination therapy of SNMC (40 ml iv, TIW). 8 (29%) of 28 patients sustained normal ALT levels with no relapse (complete response) which involved 6 patients with persistent lack of serum HCV RNA sequences after IFN therapy. In those 8 patients, 4 had been received combination of SNMC. There were 4 (14%) patients with relative response. Patients with genotype III or IV had significantly higher response to IFN therapy than the patients with genotype II. In active liver cirrhosis, determination of HCV genotype and HCV RNA levels thought to be significant for the indication of IFN therapy, and that combination of SNMC may provide beneficial effect to IFN therapy for active liver cirrhosis.

[A new liver support system composed of functional human cells and a radial-flow bioreactor].

An artificial liver will be useful for the treatment of acute hepatic failure and a bridge of liver transplantation. The current reports suggest that the hybrid type of artificial liver composed of functional human liver cells and a bioreactor is practical for clinical use. In the present study, we succeeded high density culture on a large-scale of human functional hepatoma (JHH-7) using a newly developed radial flow packed-bed bioreactor. Since the shear stress of this bioreactor is lower than the other type, high density culture without cell damage is possible. JHH-7 cells produced large amounts of human albumin and other liver specific proteins, and then have the function of ammonia metabolism in the system. This study suggests that a radial flow bioreactor will be developed as a new type of artificial liver.

[A case-report of pacemaker malfunction caused by dry pocket after replacement of pulse generator].

Pacemaker malfunction was attributed to the increasing in impedance to current flow caused by a pocket of air separating the anodal contact plate of a unipolar generator from the overlying connective tissue (so-called dry pocket). Lack of capture was noted just after replacement of pulse generator. The malfunction was permanently corrected by bediside infusion of saline to pacemaker pocket.

[A case of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia associated with chronic nephritis].

A 48-year-old female who had general fatigue was admitted to our hospital. She had swelling of the axillary, inguinal, and paraaortic lymph nodes and mediastinal lesions. Laboratory examinations showed anemia, polyclonal hyperimmunoglobulinemia with IgG 5570 mg/dl, renal dysfunction and interstitial changes of the lungs. Microscopic findings of hematoxylin-eosin staining in biopsy specimens of the left inguinal and axillary lymph nodes revealed increased levels of infiltration of mature plasma cells without evidence of malignancy. Immunoperoxidase staining showed intracytoplasmic polyclonal immunoglobulin. These findings were identical to those of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia (IPL) described by Mori et al. (1980). The specimens also showed evidence of chronic nephritis with infiltration of lymph cells and a slight invasion of plasma cells. Accordingly this case was diagnosed as IPL with renal involvement, which is associated with chronic nephritis. Recently, five cases of IPL with renal dysfunction have been reported. In particular, two cases of IPL with renal dysfunction, which included our case, revealed an increased level of IL6. These findings suggest that the occurrence of renal involvement with IPL may be related to changes in IL6, which is an important factor in the pathogenesis of IPL.

[Modified low-density lipoprotein].

Oxidative modification of low-density lipoprotein (LDL) has been implicated in the pathogenesis of coronary artery disease (CAD). Macrophages take up oxidized LDL via scavenger receptors, which is not regulated by cellular cholesterol contents, and oxidized LDL stimulates cholesterol esterification and this results in cellular cholesterol accumulation and foam cell formation. Several papers have reported a positive correlation between the severity of coronary atherosclerosis and the oxidative susceptibility of LDL. Small, dense LDL, LDL rich in polyunsaturated fatty acids (PUFA) or LDL poor in antioxidants (vitamin E, beta-carotene, CoQ10) should be more susceptible to lipid peroxidation and possibly more atherogenic. Therefore, to prevent CAD, it is important not only to reduce LDL cholesterol levels but also to suppress the oxidative modification of LDL.

[Recent advances and problems in peripheral arterial reconstruction].

Recent advances in the field of peripheral arterial reconstruction have been remarkable. These advances are mainly derived from the development of diagnostic technology such as Doppler ultrasound analysis, further refinement of surgical technique including surgical instruments and materials, and naturally improvement of patient care in the pre-, peri- and postoperative period. During the past 14 years we have performed peripheral arterial reconstruction in 1,200 cases (1,660 limbs) mainly with occlusive lesion due to arteriosclerosis obliterans. In our hand, aorto-iliac occlusive lesion has been routinely bypassed with Dacron prostheses and the secondary patency rate at eleven years reached 96.5%. The main problems encountered in this region were anastomotic aneurysm in the femoral artery (3.9%) and graft infection (1.2%). In the infrainguinal region there are still problems to be solved, although the primary (secondary) 5 year patency rates of reversed vein graft as well as in situ vein graft including femoro-popliteal below knee and femoro-crural bypasses have exceeded 70% (85%). First, there is no graft material comparable to the autogenous vein (AVG). Secondly, many of the AVG should suffer occlusive pathological changes from the environment in the long term following surgery. At the present time, we have no effective method to solve these problems, therefore we should follow up the patients heedfully to maintain the graft patency as well as to improve their quality of life and prognosis.

[Cardiac operations in nine patients aged 80 years or older].

Nine patients aged 80 years or older underwent cardiac operations with cardiopulmonary bypass. Seven patients underwent coronary artery bypass surgery, one modified Bentall's operation and another patch closure of ventricular septal perforation. One patient died soon after operation (11 POD) and operative mortality was 11%. One died 7 months after operation because of rupture of dissecting thoracic aortic aneurysm. Seven patients are still alive within physical status of New York Heart Association Classes I or II. We conclude that cardiac operations should not be excluded in octogenarians because of their chronological age alone.

[Re-operation long after the Rastelli's procedures].

Secondary reconstructive operations were performed in six patients, who received Rastelli's procedure in their childhood. Six patients consisted of three tetralogy of Fallot with pulmonary atresia, two double outlet right ventricle, and one truncus arteriosus. The reoperations were undergone five to eleven years after the first operation. One patient received a replacement of a old valved conduit with a new larger one because of relative stenosis. The other five patients received reconstructive operations in which an old valved conduit was removed and a new conduit was constructed using the conduit bed as the posterior wall and a patch of xenograft pericardium. All operations performed successfully with no hospital or late cardiac death.
