Adenine nucleotides and glycolytic metabolites were examined during hypothermic storage of porcine livers by different preservation methods. The parameters ATP, ADP, AMP, G6P, F6P, FDP, GAP, 2PG, PEP, Pyr and Lact were investigated. The results indicate a dependence of adenine nucleotides but no dependence of glycolytic metabolites on preservation method. Storage by plasma protein fraction or Collins C2 solution causes a better result in TAN concentration after 6 h than albumin and Ringer storage or Ringer trickle perfusion. Conditioning the organ for a better outcome of preservation aimed at increasing the glycolytic rate is of questionable value.

68 patients suffering from complications of chronic pancreatitis or tumours of the periampullary region respectively underwent partial duodeno-pancreatectomy (modified Whipple procedure) with or without obstruction of the remaining duct of the pancreas. These patients were followed-up and examined for glucose tolerance, serum insulin and C peptide levels until 2 years after surgery. There were no significant differences in endocrine function between patients with or without pancreatic duct obliteration by acrylate glueing.
From 1967 to 1982, 109 thymectomies were performed mainly in patients with moderate severity of the disease. The age of patients averaged 30,3 years. Operative lethality amounted to 2.7%. Another 4 patients died of the basic disease 6 months after the operation. During the subsequent course 70% of the patients showed a longlasting improvement after thymectomy, with complete recovery occurring in 23% and a significant amelioration in 40% of thymectomized patients. In another 32% there was a slight improvement of complaints. The best results could be observed in myasthenic patients with thymic hyperplasia whereas only a slight remission occurred after removing thymomas. 5 to 10 years after thymectomy the improvement rates remained approximately at the same level. Consequently, thymectomy can be regarded as a successful therapy in myasthenia gravis pseudoparalytica.
Hemodynamic parameters of livers--especially resistance of the sinusoids and of functional portosystemic shunts--are investigated using an electro-physical model. During liver perfusion the inflow conduit is compressed irregularly. The dynamic changes of pressure and flow allow a calculation of the volume passing the sinusoids and the functional shunts.
Perfusion of sinusoids and of functional portosystemic shunts was investigated before and after PPF-, Collins- and Ringer storage and in Ringer-trickle-perfusion of the liver using a physical-mathematical method. The increase of the sinusoidal resistance after PPF-storage is less than after preservation by electrolyte solutions. Accordingly the amount of volume passing the sinusoids is higher after PPF-storage. This fact is of great importance for haemodynamic integrity after transplantation.
Currently the rate of complications in segmental pancreas transplantation is very high. Reasons for this come from technical failures and from the site of immunology. To prove the cause of technical complications, several methods were used for the allogeneic segmental pancreas transplantation in diabetic dogs. To influence the exocrine pancreas secretion in grafts, we applied the intraductal injection of Ethibloc and Neoprene and the intraperitoneal drainage. By all these approaches it was possible to ameliorate an experimental diabetes in the recipients. Clearly better results were achieved in the Ethibloc-injected and open-duct grafts. The longest function time was about 6 months. Main complications, especially in the Neoprene-injected group, were venous thrombosis, pancreatitis and graft rejection.
Liver transplantation is indicated in infaust liver damages only. The most important complications are rejection, disturbances in the bile tract, problems with preservation and general infections. In the surgical clinic of the Charité Berlin 17 liver transplantations have been practiced. 5 of those patients are living now between 2 months and 3.3 years. A new technique of bile duct reconstruction was tested and is clinically used now. Diagnosis of rejection and of liver preservation are subjects of our own scientific work.
The physical and dynamic condition of the liver-gall was examined. Flow-condition as the basis for intraoperational gallducts, already applied method of operation, was analysed. Our results show, the dynamic condition of liver-gall included in the law of Hagen-Poisseuille in the gall-flowing, do not have structured-viscosity and that the liver-gall is approximately identical to the Newton-fluid. A new method of examining the intraoperative flow and pressure measurement is suggested.
Eight hundred and four high school students were interviewed about selected health-related behaviors in an effort to determine the interrelationships of these characteristics. We found the age of onset and present use of cigarettes by the adolescents to be positively correlated to their use of hashish and having sexual intercourse. The use of hashish and having sexual intercourse were also positively related with each other and the use of hashish to drinking alcohol.
In 1851 Theodor Billroth came to Berlin to continue and complete his study of medicine. In 1852 he graduated and became Dr. med. His surgical training began in 1853 in the Ziegelstrasse--Clinic of the Berlin-University--under the guidance of Prof. Bernhard v. Langenbeck. In 1860 Billroth was appointed to full professorship and head of the Surgical Department of the Kantonsspital Zurich (Switzerland). After a stay of altogether 9 years he left Berlin.

The central function of the liver as place of the formation of the most plasmatic coagulation factors in expressed by coagulation disturbances in severe liver diseases. Apart for intoxications, vitamin-K-deficiency and relatively rare congenital dysproteinoses the synthesis of the coagulation factors is apparently a relatively stable and rapidly recoverable function of the hepatocyte. On the other hand, however, the pathogenetic dominance of the accelerated balance in the sense of a disseminated intravasal coagulation and a secondarily increased fibrinolysis is emphasized for the severe disturbances of haemostasis in hepatopathies. In all kinds of shock, in tumours, inflammations, sepsis and intoxications as well as in portal hypertension of any reason the activation of the coagulation system up to the consumption coagulopathy develops. In the liver transplantation in addition to the operation shock deterioratingly comes the intensive blood contact with the often heavily damaged graft cells, the effectors of the immune system (rejection), the temporary disturbance of the clearance function of the liver RES and the basic disease of the recipient which in most cases disposes to the disseminated intravasal coagulation. The reduction of thrombocytes and coagulation factors extensively transgresses the changes which are to be expected by a weakness of the synthesis during and immediately after the anhepatic phase. For the therapy the demand of rapid removal of the coagulation-activating moments (minimum times of ischaemia, careful anastomoses) and of an adequate substitution with simultaneous prevention of the disseminated intravasal coagulation by heparinisation.

The transplantation of the endocrine pancreas has been investigated intensively as one way to optimize the therapy of diabetes mellitus. Starting with the free pancreas transplantation 1966 in diabetics, more recently research has been focused on transplantation of pancreatic islets or fetal pancreatic slices and the segmental pancreas transplantation. These forms of transplantation of the endocrine induced diabetes in various animals. Whereas the transplantation of a dispersed pancreatic preparation (islets or fetal pieces) is not connected with any acute or chronic alternation for the patient, the complication rate of the segmental pancreatic transplantation has to be reduced for a broader clinical application. In the case of islet transplantation the research efforts were concentrated on the improvement of tissue preparation and prolongation of the tissue survival.
