[Surgical treatment of benign liver tumors].

We reviewed the records of 105 patients treated for benign hepatic disease at the Department of Surgery of the Charité since 1979. Treatment has to be tailored to the different pathological processes. Modern imaging techniques are of utmost importance for diagnosis and follow up of all patients regardless if they underwent operative therapy or were treated conservatively. Indications and contraindications in the treatment of benign liver tumours are developed on the basis of 44 liver resections with special reference to adenomas and hemangiomas. In all cases a hemihepatectomy seemed justified inspite of the benign disease process. Postoperative complications were mostly related to the surgical incision and caused by abnormalities of protein synthesis and coagulation, frequently observed after major hepatic surgery. Close monitoring and immediate correction of all clinical and biochemical parameters are essential in the postoperative period.

[Therapy of portal hypertension in childhood].

From 1980 to 1984 fortyeight children with portal hypertension were treated, 37 out of these had a prehepatic bloc. In 19 children a sclerotherapy was performed. Seven times a bleeding recurrency occurred and an esophageal stenosis was seen in 2 cases. The distal splenorenal shunt (Warren) was performed 16 times. All children survived, one pneumonia and one slight pancreatitis were observed post-operatively. Four times a shunt obstruction was found at follow-up examinations. Bleeding recurrencies did not appear.

Alteration of the ultrastructure of the pig liver after a continuous perfusion of six hours in vitro.

Livers of mini LEWE pigs were perfused continuously for 6 h on a novel perfusion system through the portal vein (laminar flow from 200 to 300 ml/min, pressure 270 Pa) and through the A. hepatica (pulsating flow from 100 to 200 ml/min, pressure: 10.6/5.5 kPa). The following perfusion media were used for determining the best conservation conditions at 10 to 12 degrees C: Pig blood/Krebs-Henseleit bicarbonate buffer 1:1; Pig blood/Ringer lactate solution 1:1; Pig plasma; Collins C 2 perfusion solution. After perfusion with Collins C 2 solution the hepatocytes show swelling and dilation of the granular endoplasmic reticulum; after perfusion with the other solutions the changes of the hepatocytes are negligible. The alterations of the sinusoidal wall cells depend on the perfusion pressure and flow and the composition of the solution. After continuous perfusion the sinusoidal walls are characterized by vesicles of endothelial cells, dilatation of the Disse space and fragmentation of hepatic microvilli.

[Diagnosis and surgical therapy of organic hyperinsulinism].

The diagnosis of an insulin producing tumour can be confirmed by a minimum of biochemical investigations. Its preoperative localisation is more difficult. Sonogram, Computertomogram, selective angiography and percutaneous transhepatic collecting of blood samples for insulin analysis from the portal system were preoperative measured to localize the tumours in 32 of 37 patients of our series. In 2 patients intraoperative tumour localisation by measurement of incorporated p32 proved to be effective. In B-cell-carinomas pancreas resection is the adequate therapy. With regard to the therapeutic effects a high risk is involved in the 'blind' left or right sited resection of non-localized tumours.

[The LeVeen peritoneovenous shunt--an alternative in therapy-resistant ascites].

6 patients with therapy resistant ascites underwent implantation of a Le-Veen shunt. The risk of infection in this procedure is low, the surgical technique can easily be handled. In all 6 patients a significant reduction of the ascites could be demonstrated. Postoperatively, a right ventricular insufficiency could be observed in every case and, in addition, there were imbalances of the water and electrolyte metabolism Disseminated intravascular coagulation could not be traced.

[Auxiliary liver transplantation in an animal experiment--a novel surgical model].

At the problem of the auxiliary liver transplantation has been worked for about 30 years. Extensive series of tests with animals with a novel transplantation model justify these promises well of a successful clinical use. Laboratory chemical, histologic, angiographic and nuclearmedical examinations prove the fitness of this method. Basic supposition for a success is the preservation of physiological conditions at the donor liver with sufficient perfusion of the recipient liver.

[Clinical liver transplantation in East Germany].

In the GDR 28 liver transplantations have been performed up to now. The 1 year survival rate amounts to 30%. At present 8 patients are alive between 3 months and 5 years. There were no biliary complications since the biliary reconstruction was performed by using a special 'mucosa suture'-technique. Ischaemic graft failure was a main cause for early postoperative mortality. A better immunosuppressive treatment may help to improve the results of liver transplantation in future.

[Significance of antithrombin III in clinical liver transplantations].

The activity of AT III was measured by chromogenous substrates in 18 of 28 patients before, between and after liver transplantation. In half of all cases basic pathological activities could be observed. During transplantation, AT III is on an average decreased to 43% of the initial level. Examples are demonstrated for important AT III deficiencies after liver transplantation in man. Long-time examinations have shown AT III to be in the normal range or higher in 1 to 60 months after transplantation when the graft function is correct.

[Nuclear medicine diagnosis in auxiliary liver transplantation in the minipig].

Experiences in the blood-flow measurement (133Xe washing out method) and function test (131J-bromsulphthalein, 99mTc-IDA) were reported after auxiliary liver transplantation in miniature swine. Normal values for the global blood-flow [70 (35-144) ml/100 g . min] and the bromsulphthalein half-time (6,5 +/- 2,4 min) were defined pre-operatively. Selective blood-flow measurements were carried out after transplantation invasively. Only insufficient experiences could be received about the liver-cholescintigraphy in consequence of graft insufficiency.

[The new Charité building and the University Surgical Clinic].

The new multi-storey hospital of the Humboldt-University in Berlin has a capacity of 1050 beds, encompasses a surgical clinic with 312 beds, 11 theatres and its own unit for intensive care. All kinds of modern surgery can be performed in its 7 specialised wards.

Antisperm antibodies in infertile and homosexual men: relationship to serologic and clinical findings.

The incidence and significance of antisperm antibodies in different groups of men were evaluated by a modified enzyme-linked immunosorbent assay. In serum, 4.0% of dermatologic patients (n = 223), 9.6% of andrologic patients (n = 178), and 28.6% of homosexual men (n = 42) were positive for IgG and/or IgM antibodies. In seminal fluids, 7.3% of the andrologic patients had IgA (and IgG) antibodies to spermatozoa. Only 1 of 29 positive men had antibodies both in serum and in seminal fluid. No correlation between antisperm antibodies and IgG/IgM concentrations was found in serum, whereas in seminal plasma men with antisperm antibodies showed higher IgG/IgA concentrations than men without (IgA, 3.2 versus 1.7 mg/dl; IgG, 9.8 versus 6.3 mg/dl). It is concluded that there is a high incidence of antisperm antibodies among homosexual men, probably because of contact of spermatozoa with the immune system by passive anal intercourse. There is little correlation between antisperm antibodies in serum and seminal plasma of infertile men because of a lack of relevant antibody transfer from the serum and the formation of local antibodies in seminal plasma. Antisperm antibodies in seminal fluid are associated with elevated local IgG and IgA concentrations.

[Value of histopathologic findings following liver transplantation based on biopsy course studies].

A total of forty-four biopsies from nine liver transplantation patients, who had developed complications postoperatively, were studied histologically and, in part, electron-microscopically. Morphologic signs of acute rejection were seen in three patients, while there were indications of chronic rejection in one patient. In combined lesions, such as drug-induced hepatosis, cholangitis, hepatitis, and rejections, the interpretation of biopsy findings may be difficult, and special entities cannot in every case be separated unequivocally. Changes of serum bilirubin and of serum enzyme activity do not run entirely parallel to the microscopically detectable structural lesions. For diagnosis of complications after liver transplantation both the morphologic and the clinical findings must be considered.

Relationship between insulin secretion and pancreas morphology in subjects with chronic pancreatitis.

In order to investigate whether a relationship exists between in vivo insulin secretion and islet mass, 8 patients suffering from severe chronic relapsing pancreatitis were studied before and after pancreatectomy by glucose-glucagon-test (per os 1.75 g glucose; i.v. glucagon 0.01 mg/kg b.w.) and by intravenous glucose-tolerance-test (iGTT) (i.v. glucose 0.33 g/kg b.w.). Postoperative in vitro assessments of pancreatic insulin and alpha-amylase content were performed, and morphometric studies were carried out. Patients were characterized by reduced c-peptide secretion when compared with healthy subjects. The c-peptide response to the glucose-glucagon-test correlated well with the morphometrically estimated exocrine and islet tissue mass (P less than 0.05) and with the content of insulin and amylase in the tissue. The findings suggest that in subjects suffering from severe chronic relapsing pancreatitis the maximal insulin response might represent a parameter for the patient's islet mass.

[Analysis of the results of 96 CT-guided fine needle biopsies of liver masses].

A prospective study of 96 CT-guided fine needle biopsies in 93 patients showed 47 malignant and 46 benign lesions in the liver. Overall accuracy was 97%, with a sensitivity of 92% as specificity of 96%. At least four punctures per biopsy were necessary for a reliable diagnosis. In order to save time, fine needle biopsy is recommended to be carried out immediately after the initial CT.
