[Systemically administered regional tumor therapy. Regional hemi-body chemotherapy of metastatic malignant melanoma--an experimental therapy concept].

Since systemic application of a high-dose chemotherapy is limited by the extent of intolerable toxicity and overall response rates so far are rather poor, the systemic mode of chemotherapy for metastatic melanoma appears to be of only limited benefit. On the other hand, results from isolated limb perfusion for satellitosis and in-transit metastasis suggest distinct dose-response correlations with tumoricidal properties of appropriate antineoplastic agents. This experience prompted the idea to pilot the anti-tumor action of a high-dose regimen confined to one hemibody compartment for targeted tumor therapy. After having standardized the surgical procedure this goal appeared to be achievable by expanding the perfused area and by simultaneously detoxifying toxic drug levels within the non-perfused compartment by venous filtration. Two initial causal experiences revealed impressive tumor regressions and are therefore reported on preceding subsequent evaluation within a controlled clinical trial being designed at different solid tumors.

[Valproate dosage].

Starting from Valproat dosage recommendations and serum levels and patient risk from acute hepatic failure, the results of our therapeutic service are set forth. The question comes up for discussion as to whether the proposal of Mattson et al. of a mean therapeutic blood level of between 40 and 45 micrograms per ml should be aimed at.

[The course of blood glucose and electrolyte status in parenteral nutrition with a complete solution following intestinal resection].

In 15 patients after resection of a part of the large intestine the efficacy of a complete solution for total parenteral nutrition was investigated. Glucose concentrations were 20 mg/dl higher than normal from the operation up to the fifth postoperative day. Electrolyte concentrations were normal during the investigation period. There was only a decrease in the total protein concentration to a lower normal range after the operation till the fifth postoperative day. No systemic or special side effects appeared after the infusion.

Inhibition of proteases during extracorporeal extremity perfusion experimental and clinical results.

In an experimental and clinical study a significant reduction of the postoperative edema after extracorporeal extremity perfusion under addition of Aprotinin was observed. Together with reduction of edema usually expected complications like pain and peripheral nerve damage could be reduced significantly. Preliminary, yet unpublished data on cytostatic infusion of the pancreas also indicate that intraarterial Aprotinin when given simultaneously may prevent pancreatitis. It is mandatory Aprotinin to be given early enough in sufficiently high dose and there seems to be an advantage when the arterial route is used.

[Intra-individual comparison of the long term effect of Non-Ovlon on caffeine pharmacokinetics].

The effect of the oral contraceptive, Non-Ovlon, on the pharmacokinetics of the model substance, caffeine, was studied in an intraindividual comparison in the course of seven months, i.e. four and two weeks before as well as 0.5, one, three and six months after the women had begun to take the contraceptive. The study commenced in twelve women of which, after seven months, only five were involved. The measuring compartment chosen was saliva so as to minimize strain on the volunteers by this noninvasive approach. Non-Ovlon delayed the caffeine elimination of 4.0 +/- 1.7 hr to 9.0 +/- 4.5 hr and 11.0 +/- 10.4 hr at the end of the first and third intake cycle, respectively. Markedly successive decreases in clearance and an increase in AUC levels were seen in addition to longer half-lives. Interindividual variations were considerable, a finding that might be relevant to the therapy by other, similarly interacting, drugs. There were no differences observed between the initial phase and the median phase of normal menstrual cycle. In one smoker, the caffeine elimination was highly accelerated compared to the average, throughout the period of investigation.

[Peculiarities of theophylline therapy in childhood].

Profound knowledge of the pharmacokinetics of theophylline is a prerequisite to successful therapy. In children the clearance of theophylline is considerably greater than in adults. The required higher dosage in relation to body weight results in more pronounced oscillations of drug levels in serum. Investigations in 280 children in the age of 1 to 18 years with bronchial asthma, treated with theophylline compounds, confirm the necessity of individual dosage and surveillance of the drug level in serum. Instead of the determination of the drug level in serum, its measurement in saliva is sufficiently reliable and less molesting. Checks of drug level are also a valuable educational measure to improve patients' compliance with drug intake. The effectivity of asthma therapy in children could be decisively improved by an adequate formulation of a slow release preparation of theophylline.

[Pharmacokinetics of anticonvulsants in the perinatal period].

We have examined the concentration of a range of anticonvulsant drugs in serum, amniotic fluid and breast milk in 23 pregnant patients. Drug determinations were made during the last 3 months of pregnancy, at birth and during the first week postpartum. In the cord serum the range of concentrations, expressed as a percentage of the maternal serum concentration was: primidone (67-131%), phenobarbitone (80-108%), carbamazepine (60-76%), phenytoin (95-162%), ethosuximide (104%), and valproic acid (125-147%). In the amniotic fluid the corresponding ranges were: primidone (62-230%), phenobarbitone (28-59%), carbamazepine (18-45%), phenytoin (18-60%), ethosuximide (111%) and valproic acid (5-11%). In breast milk the corresponding percentages were: primidone (44-228%), phenobarbitone (20-50%), carbamazepine (32-80%), phenytoin (46-53%), ethosuximide (92%) and valproic acid (2-8%). Anticonvulsant drugs can be given in relatively high doses during pregnancy. Pregnant patients and neonates are patients with therapeutic problems. For this group of patients is recommendable a therapeutic drug monitoring.

[Thermologic studies in inflammatory rheumatic diseases--general presentation and personal results in rheumatoid arthritis].

The registration of the temperature of the body surface gains an increasing significance among others for the diagnosis of inflammatory rheumatic diseases. A criterion of the inflammation of the joint, the hyperthermy as well as the disturbances of the microcirculation may be made evident by means of the thermological methods. Own results in healthy persons and patients with rheumatoid arthritis as well as evidence of the literature confirm the possibilities of the measurements of temperature for the rheumatology. On account of the great inter- and intra-individual variability of the skin temperature an orientation at the temperatures of normal values of certain measuring regions is not reasonable. When the results of the temperature measurement are estimated, therefore, a comparison with temperature patterns of healthy persons must be done. Using dynamic measuring methods the evidence of thermological investigations may essentially be enlarged. The evaluation of the results of the measurement is increasingly performed with the help of computers.
