The constant B2E linked scan is shown to be an excellent tool for the profiling of biogenic amines in wine samples. The fragment ion at m/z 169 of the dansylated amines is used for the precursor ion search in combination with direct sample evaporation. Besides the profile a semi-quantification may be achieved when using suitable amines as internal standards with standard deviations of 12-17%. More accurate results with standard deviations of 3-5% are obtained when using deuterated analogues as internal standards. Both methods are applied to the analysis of biogenic amines in synthetic mixtures and actual wine samples.
The communications of literature regarding osteosynthesis in glenoid fractures of the scapula are reviewed, and six own cases are presented. The results are better than expected even in case of comminuted fractures. The indication for surgical reconstruction should therefore be adopted in dislocated fractures of the shoulder joint surface. Although the adequate operation technique is still under discussion, there seems to be a trend towards screwing and a narrow DC plate.
Antiepileptics can impair the performance of subjects in both acute and subacute doses. The effects of phenobarbital and its derivatives and benzodiazepines are stronger in this respect than that of phenytoin, ethosuccimides and valproic acid. Carbamazepine, in contrast, has rather a stimulatory action. Impaired cognition, psychomotor performance and emotions must also be expected under therapeutic conditions. Attention is drawn to various factors that may influence the results of tests, including the dose and blood concentration, interval between application and test, initial performance of the probands or patients, levels of vigilance and personality structure. Lack of compliance is particularly important since adaptation and readaptation processes that can be dangerous take place in these patients. The careful exploration of side-effects on the patient may have a decisive influence on practicability and cost if the relationship between patient and doctor is good.
The care of pregnant epileptics calls for a high measure of personal attention and material expenditure. Above all, it means close cooperation between the epileptic specialist, the obstetrician, and the clinical pharmacologist. The need for this collaboration, and its effectiveness, are demonstrated by means of practical examples, with special consideration of blood level.

Reduction of [Mg2+]o induced spontaneous epileptiform activity consisting of 40-100-ms bursts of population spikes in hippocampal slices. This activity disappeared from area CA1 when the connections to area CA3 were cut, but persisted in isolated minislices of area CA3. Spontaneous activity was also observed in the dentate gyrus, provided that the connections to the subiculum and entorhinal cortex (EC) were intact. In the parasubiculum and EC longer lasting epileptiform events were observed which resembled seizure-like behaviour. The epileptiform activity was completely suppressed by 2-aminophosphonovalerate (30 microM) suggesting that N-methyl-D-aspartate receptors for excitatory amino acid transmitters participate in the generation of this activity. These findings show that the EC possesses properties which permit the generation of seizure-like activity in contrast to the hippocampus where the activity resembled recurrent interictal events.

Determinations of antiepileptic drug blood level for therapy monitoring have been carried out in combined obstetric-neurologic prenatal care of epileptic pregnant women. In our opinion this blood level oriented dosage necessary in high risk patients for seeing in time if and in which scale serum level may drop during pregnancy, to change dosage or eventually to change drug in cases with increasing frequency or severity of convulsions. This method allows especially to control intake or to diagnose a pharmacophobia during gestational period.

In an investigation is proved that already after a treatment with the histamine-H2-receptor blocker Cimetidine (Altramet) lasting 14 days the elimination of Antipyrine is significantly delayed. In 14 male patients suffering from peptic ulcer the half-life period of Antipyrine (t50) was 8.6 +/- 0.6 h, whereas in the second test after 14 days it was 13 +/- 0.8 h. The systemic clearance (Cltot) was estimated with 78 +/- 11.3 ml X min-1 before treatment and with 40.8 +/- 3.6 ml X min-1 after treatment. It is pointed out that under a therapy with Cimetidine preparations and simultaneous application of medicaments which were eliminated by the so-called phase-I-reaction a hepatic interaction may occur.

With a newly developed photoplethysmographical measuring device it is possible to record the conjunctival pulse of the human eye. The device consists of the self-constructed photoplethysmographical amplifier unit, the sucking electrode from silicone hard-rubber with photo-detector and luminescence diode, and a 3-channel recorder (3-NEK). Electrocardiogram and the conjunctival pulse are recorded simultaneously. Parameters that are derived from the curves allow to judge on the circulation in the tissue region investigated. Investigations were carried out in 70 test persons without clinical or other signs of pathological conditions regarding eye or vessels (classified into sex and age groups up to 40 years and over). 133 recordings with six consecutive curve waves were examined. Additionally, the influence of vasoactive drugs on the conjunctival pulse curves was investigated. The topical application of Oculoguttae Tolazolini 2% and Oculoguttae Epinephrini 2% showed no influence compared with the reference curves. Contrary to this, differences were proved after application of Nitrangin liquidum systemically in all parameters tested. The photoplethysmography of the conjunctiva bulbi is a noninvasive running-on method with good reproducibility without troubles for the patient, with an easy performance and only a small time demand. The results qualified the method to test the influence of systemically applied vasoactive drugs.
Experience from more than 380 operations resulted in the standardisation of isolated hyperthermic perfusion with cytostatic drugs for the treatment of malignancies of the extremities. A side-effect of this procedure is postoperative oedema in varying degrees of severity. The influence of prophylactic intraoperative administration of aprotinin (Trasylol) was investigated under experimental and clinical conditions. In these preliminary studies aprotinin distinctly reduced maximum extent and duration of the postoperative oedema. An investigation concerning interaction of aprotinin with the cytotoxic activity of the drugs used in perfusion showed only minor inhibition which does not limit clinical usefulness.
