[Theses on the evaluation of arterial circulatory disorders of the extremities].

In an expert opinion of disturbance of the arterial blood supply concerning invalidity the vascular disease must be proved with sufficient ascertainment as well as the functional capacity remained must be estimated. Every expert opinion should contain as far as possible concrete data already in the angiologic-diagnostic minimum programme. In problematic cases supplementations by a special angiological diagnostics are necessary. The criteria of evaluation of arterial obstructive diseases for an expert opinion concerning invalidity are also summarized in theses. Also the relations between disturbances of the peripheral blood supply and effects of injury are discussed. The paper concludes with references to the acknowledgement of a disability card in arterial obstructive diseases and to the motor-car driving ability of patients with vascular diseases.

[Step-wise diagnosis of primary and secondary lymphedema].

The diagnosis of the lymphatic oedema is possible by means of non-invasive methods. The degree of severity of the lymphatic oedema can already be assessed within the basic diagnostics (anamnesis, clinical findings). The recognition of secondary lymphatic oedemas is important, particularly of those due to tumours of pelvic organs. Indications and contraindications of other diagnostic methods (plethysmography, dye test, computer tomography, sonography, lymphography) in diseases of the lymphatic vessels are presented.

[Development of collateral circulation in arterial occlusions in advanced age].

Plethysmographic course controls in 15 male patients at the age between 70 and 76.5 years showed that the maximum of the collateral blood supply in arteriosclerosis in the femoral region is reached 1-2 years at the latest after vascular occlusion. The peak-flow-values measured confirm the possibility of efficient collaterals also at older age. When there is a bad early compensation of the artery occlusions, we intended to test in time, whether or not a percutaneous transluminal angioplasty or a local thrombolysis are possible. Furthermore, in 100 male patients with arteriosclerotic disturbances of the blood supply of the legs the plethysmographic measurement of the reactive blood supply showed that the internationally usual haemodynamic subdivision into degrees of severity also concerns the age group between 60 and 75 years. With progressing Fontaine-stage a decrease of the reactive blood supply (first-flow, peak-flow) and an increase of the peak-flow-time were found.

PALA, vindesine, and cisplatin combination chemotherapy in advanced malignant melanoma. A pilot study.

Twenty-two patients with advanced malignant melanoma were entered in a pilot study receiving combination chemotherapy with PALA, vindesine, and cisplatin (PVP). Treatment consisted of PALA 3000 mg/m2 IV on days 1 and 2, vindesine 3 mg/m2 IV on days 1 and 8, cisplatin 30 mg/m2 IV on days 1 through 5, with treatment cycles repeated on day 21 every 3 weeks. Of 22 patients, 3 had non-visceral disease confined to iuxtaregional tumor growth (Stage III), and 19 had disseminated and/or visceral disease (Stage IV). The male/female sex distribution was 13/9; median age was 45 years. All 22 patients had measurable disease; 21 were evaluable for response and toxicity. Five patients (24%) had a complete response (CR) with a median duration of 5 months, and four patients (19%) had a partial response (PR) with a median duration of 3 months. Seven patients showed disease stabilization (33%) with a median duration of 2 months. Progressive disease was seen in five patients (24%) and was commonly due to widespread visceral disease. CR could be found predominantly in non-visceral disease, whereas PR could be observed in visceral disease also. Survival time from the onset of PVP chemotherapy cannot be estimated finally, since some of the responses are continuing at the present time, and 7 of 21 patients are still alive. Currently, median survival time for responders is 8 months, and for nonresponders, 5 months. Toxicity of PVP chemotherapy is mild to moderate and allows cytotoxic drug administration on an outpatient basis. PVP chemotherapy appears to have significant activity against malignant melanoma and may therefore be an alternative regimen in the management of advanced disease. However, despite the relative high remission rate especially in non-visceral disease, response duration remains disappointingly low.

[Analysis of eclampsia cases of the years 1957-1982 at the University Gynecologic Clinic in Leipzig].

Of the total of 104 873 patients who have been delivered between 1957 and 1982, 99 women (0.094 per cent) were diagnosed as having eclampsia. Maternal lethality was 4.0 per cent, and perinatal lethality was 20.8 per cent. Delivery was spontaneous in 34.3 per cent, and caesarean section, vacuum extraction, and forceps delivery were performed in 45.5 per cent, 8.1 per cent and 12.1 per cent respectively. The incidence of eclampsia decreased generally. The prognosis for both mother and fetus could be improved by modern methods of intensive care in close connection with termination of pregnancy.

[Trace elements and lipid parameters in arteriosclerotic circulatory disorders by polyene fatty acids].

In 106 patients with peripheral arteriosclerotic obturating disease low values of serum magnesium as well as an increase of the relation of vasoaggressive to vasoprotective lipoproteins could be established. Under a polyene fatty acid diet (40 ml sunflower oil or linseed oil a day) the serum copper values significantly decreased, whereas of the indices of arteriosclerosis only in the linseed oil group of apolipoprotein-B/A-quotient, in the sunflower oil group the triglyceride level decreased statistically significantly. Therefore the favourable influences of physical conditioning as well as of pharmacotherapy on the disturbed lipoprotein metabolism of patients with vascular diseases should additionally be used.

[Detection of exposure in welders by determining manganese contents of biological materials].

The contents of manganese in the biological materials urine, hair and faeces were measured atom-absorption-spectral-photometrically and compared in non-exposed and exposed welders. Significant differences between the two groups in the manganese-urine-excretions and manganese content of the hair were the result. For the industrial medical control of manganese exposed welders for the prevention of professional diseases the routine determination of manganese in the urine is recommended as exposition test.

[Experiences with high-dose cisplatin therapy in metastasized malignant melanoma].

14 patients with advanced malignant melanoma were treated in 36 therapy cycles with cisplatin. 8 patients had been pretreated with dacarbazine and 6 had received additional BCG immunotherapy. 4 patients had been irradiated after surgical removal of lymph node metastases. All patients showed significant tumor progression. 4 patients were treated showing ultimately disseminated melanoma with widespread visceral involvement. 5 patients with lymph node metastases had been operated radically and were treated postoperatively. Cisplatin was administered as a 24-h high-dose therapy (200 mg or 120-200 mg/m2) under forced mannitol diuresis, treatment cycles were repeated monthly. Of all the patients, 1 showed complete remission of supraclavicular metastases lasting for 6 months until now. 1 patient showed an initial minor response with subsequent stabilization without appearance of additional metastases for 1 year. 2 patients who had received cisplatin postoperatively showed no reappearance of tumor growth for 8 months up until now. 11 patients showed no change or progression of disease, 6 of them had received only one therapy cycle. Under clinical conditions, side effects of cisplatin treatment can be managed satisfactorily, no irreversible kidney damage could be observed under forced diuresis. As far as the above-mentioned results are concerned, antineoplastic activity of cisplatin as to advanced malignant melanoma must be considered to be of limited benefit using cisplatin as a single-agent treatment. Improvement of results might be obtained using cisplatin in a combination therapy together with other antineoplastic agents, which at the present time are being investigated in several prospective trials.

[Allogenic lung transplantations following pretreatment of recipients with donor-specific splenic antigens].

Lung transplantations were carried out in 20 not immunologically selected mongrel dogs which were pretreated with donor specific spleno-antigen mixture. The average survival time was double the size (12.3 days) as in a control group of untreated dogs after allogenic lung transplantation. A number of the receivers got Cyclophosphamid in addition to the spleno-antigen mixture. In this group the average survival time amounted to 17 days. A chronic course of the rejection was not to be achieved with this method. The autopsy showed thoracic infectious complications in nearly all receivers. A great number of central pulmonary vein thromboses in the transplant was discovered in this series in contrast to other lung transplantation series. Circumstances refer to a connection with hyperacute rejection courses. The receivers of this series survived the destruction of the transplant longer on an average as such in other experimental series.
