[Serum magnesium and heart arrhythmias in patients with pelvic-leg arteriosclerosis].

In 122 male patients with arteriosclerosis of pelvis and legs up to 5 resting electrocardiograms were registered in the course of 4 years. 5.2% of these electrocardiograms showed disturbances of rhythm. In these cases no significant differences in the serum magnesium level could be ascertained between patients with and without disturbances of rhythm.

[Initial experiences with the effect of chronic intermittent electromyostimulation in patients with hyperlipoproteinemias and arterial circulatory disorders].

Metabolic and clinical effects of an original electromyostimulation are explained which induces muscle contractions by changes of the field intensity in the region of microseconds in forms of delivered impulse groups. On account of the good local compatibility due to optimal irritation parameters the muscle stimulator was suitable for a long-term application. Examinations on 23 patients with hyperlipoproteinaemia and mixed HLP (n = 15) and primary hypercholesterolaemia (n = 8) after a 2 times 2 hours/day mutual stimulation of the femoral musculature in a frequency of 2 s-1 lasting four weeks yielded a significant decrease of the triglyceride and cholesterol concentrations as well as an increase of the HDL-cholesterol in mixed HLP-forms. Examinations not yet finished refer to an increase of the extrahepatic activity of the postheparin-lipoprotein lipase and to the concentration of the HDL2-cholesterol. An influence of the hypercholesterolaemia IIa could not be made evident. An electrostimulation performed for 3 months in 8 patients with obliterating arteriosclerosis resulted in a more sufficient collateral blood supply of the legs. The long-term use of the device was unobjectionable also under ambulatory conditions. On the basis of additionally gathered experiences further indications of an electrostimulation were cited. Possible variations of the electrostimulation irritation parameters might make accessible further fields for the preventive and curative medicine.

[Animal experiment studies of morphologic changes in the trachea and lung in high frequency ventilation].

Prolonged high frequency ventilation (24 hrs) in dogs leads to morphological changes in the trachea as well as in the bronchi. Erosive changes of tracheal epithelium were observed whereas small bronchi and bronchioli exhibited luminal obstructions by mucus plugs, caused by hypersecretion of goblet cells. Using histochemical methods degenerative changes of bronchiolar epithelium were observed.

[Cardiovascular diseases in exposure to the saltpeter acid ester ethylene glycol dinitrate].

By means of the establishment of constituents of explosives on the basis of the measuring technique the exposition intensity to ethylene glycol dinitrate in the mining industry could be recognized. By comprehensive clinical and paraclinical differential-diagnostic examinations as a consequence of this haemodynamically greatly effective exposition manifest disturbances of the cardiac rhythm were objectified in 8 workers and acknowledged as occupational disease Nr. 27.

[New aspects in the therapy of thrombosis and prevention of recurrence in pregnancy].

Therapy of phlebothrombosis during pregnancy general is carried out with heparin, in which case the initial hospital treatment may be continued later on with subcutaneous applications of heparin in the out-patient department. Regular clinical and apparative controls are performed during pregnancy within. Low dose heparin is injected up to the optimal dicumarol blood level (beginning with the 2nd day post partum) immediately pre-, intra- and postpartelly. - Coagulation analyses in 11 patients with former phlebothrombosis during pregnancy revealed no antithrombin III-deficiency, but hints to insufficient fibrinolysis in four cases.

[Rheologic studies in intrauterine retardation and pregnancy-induced hypertension].

Hemorheological investigations were done in 23 pregnant women with intrauterine growth retardation, 23 patients with pregnancy induced hypertension and 21 healthy pregnant women, of whom 10 regularly smoked. The rigidity of red cells was disturbed and the hematocrit was elevated by smoking. The same disturbances could be observed in women with intrauterine growth retardation. In addition, the erythrocyte aggregation was significantly elevated in patients with hypotrophic newborns at term. Pregnancy induced hypertension was associated with disturbances in all rheological parameters. Placental perfusion was significantly prolonged in all pathologic pregnancies.

[Diabetic angiopathy--retrospect and prospect].

Max Bürger intensively dealt with the problems of angiopathia diabetica which determines the prognosis of the diabetic. His successful action in diabetogenic gangrene must from the modern point of view possibly supplemented by methods opening the vascular lumenor by haemorheological therapy. In contrast to this many questions of the prophylaxis and early therapy of arteriosclerosis in diabetes mellitus still await their only interdisciplinary possible clarification. In practice hopes are connected with the new possibilities of the management of metabolism in diabetes type I and the improvement of the patient's compliance.

[Results of a long-term study of arteriosclerotic circulatory disorders with polyene fatty acid therapy].

The antiatherogenic oil diet as one factor of therapy causes the hope of favourable effects in the complex primary prophylaxis. By means of this alone, however, a regression of a clinically significant arteriosclerosis cannot be achieved within 3 years, through biochemically unequivocal "antiatherogenic" effects are to be proved. Linseed-oil, fish and fish oil seem to be particularly suited to cause these effects desired.

[Lipid peroxides and antioxidants in the blood of patients with arteriosclerotic circulatory disorders].

Malondialdehyde, vitamin E, glutathione peroxidase and selenium were analysed in the blood of 96 male patients with arteriosclerotic obstructive disease in the region of the leg. Of these 32 patients additionally had manifestations of a coronary or cerebral arteriosclerosis. In the parameters mentioned no significant differences were found between patients with general arteriosclerosis and isolated arteriosclerosis of the leg. Patients with arteriosclerosis and hyperlipoproteinaemia revealed higher values of malondialdehyde than patients without hyperlipoproteinaemia. No correlations were found between malondialdehyde and total cholesterol, triglycerides, vitamin E, glutathione peroxidase as well as selenium.

[Long-term anticoagulation of the post-thrombotic syndrome].

Observations of the course in 20 patients who underwent a long-term anticoagulation due to postthrombotic syndromes resulted in a clinical improvement according to the degrees of severity of the GDR section phlebology as well as in an increase of the plethysmographically measurable venous regurgitation. Thus the late sequelae of the postthrombotic syndrome may possibly reduced by long-term anticoagulation. In one third of the patients despite the anticoagulant therapy a decreased antithrombin-III-value was found.

[Changes in the peripheral lymph nodes seen by ultrasonics].

Sonographic examinations of 1 416 patients performed between 1980 and 1983 with particular reference to the inguinal, axillary and neck regions have shown that it is quite possible to effect sonographic differentiation between enlarged lymph nodes with cicatricial indurations after non-specific inflammation on the one hand, and primary lymphomas or metastases of lymph nodes on the other. In many cases, it is also possible to differentiate between the enlargement of a total lymph node (acute inflammatory, primary malignant) and metastases in a lymph node. Positive findings in the soft parts were seen in 278 patients; of these, 175 were confirmed histologically and cytologically (via puncture with sonographic control). One finding was revealed as false positive. In 102 patients the sonographic finding was confirmed by means of lymphographies (n = 12), computed tomography (n = 27) or via the subsequent clinical course, proving the fact that it is also possible to detect sonographically the ability of lymphomas to respond to treatment with antibiotics, cytostatics, and radiation; the effects can be recognised and interpreted via changes of the echo patterns.

[Hemostaseologic studies of maternal blood in intrauterine retardation and threatened premature labor].

Changes in coagulation of maternal blood in the case of severe gestosis, which indicated a relation to disturbed uteroplacental blood flow, gave rise to study other risk pregnancies and to do specific coagulation analyses. The following parameters were determined in 102 women showing a distinct tendency for premature labor, 20 women patients with suspicion of intrauterine growth retardation, and 18 normal pregnant women: Number of thrombocytes in whole venous blood, fibrinogen, plasminogen, reptilase time, thrombin time. Compared to the control group, a statistically significant increase in the number of thrombocytes was found only in the cohort of women showing signs of imminent premature labor during the 34th to 36th weeks of pregnancy. Both cohorts of patients showed markedly higher levels of plasminogen than the control group over the entire period of gestation studied in this investigation. All other parameters of coagulation showed no significant changes of pathological pregnancies in comparison with normal gravidas.
