[Renal angiomyolipoma: report of two cases (including a case with spontaneous rupture)].

Two cases of renal angiomyolipoma with tuberous sclerosis are reported. The first was a 42-year-old housewife with complaints of left flank pain, palpable mass at the same region and in a preshock state. Ultrasonogram of the left kidney demonstrated a large echogenic mass with fluid. Preoperative diagnosis was renal angiomyolipoma with spontaneous rupture. Emergency nephrectomy and lymphadenectomy were performed. The histopathological diagnosis was renal angiomyolipoma with nodal involvements which reflects the multicentricity of the disorder. The second was a 44-year-old housewife with dull right flank pain. CT scan showed a mass in the upper part of the kidney which had an attenuation value of -60 HU. Adrenal tumor could not be distinguished from renal tumor by selective renal angiogram. Nephrectomy was performed and histopathological diagnosis was renal angiomyolipoma. Both cases remain well. We reviewed 32 cases of renal angiomyolipoma with spontaneous rupture in the Japanese literature with respect to age, affected side, presence or absence of tuberous sclerosis, preoperative diagnosis and treatment. Previous diagnoses were done with plain film, excretory urogram and angiogram. Preoperative diagnosis of angiomyolipoma was difficult or impossible, and operative procedure of choice was nephrectomy. Now CT and ultrasonogram are the specific tools in the diagnosis of angiomyolipoma. Partial nephrectomy, enucleation or observation is selected because renal angiomyolipoma is a benign tumor.

[Tuberculosis of the penis: report of a case].

Tuberculosis of the penis has become a rare disease since antituberculosis drugs have come to be widely used. It may be primary or secondary to coexisting tuberculosis elsewhere in the body. In the case of gross destruction of the glans penis and urethra, it is not always easy to differentiate tuberculosis from carcinoma of the penis. A 87-year-old man was admitted with painful ulceration on the glans penis and dysuria. Physical examination strongly suggested carcinoma of the penis other than infectious diseases. In addition, there was no evidence of a tuberculous lesion in other urogenital organs or lungs. Therefore, partial amputation of the penis was performed after admission to the hospital. Histopathological findings, however, revealed typical tuberculosis with Langhans' type of giant cells and no malignancy.

[Clinical value of testicular lymphangiography in diagnosis of retroperitoneal metastases].

Testicular lymphangiography was performed before retroperitoneal lymph node dissection in 20 patients with testicular tumor. The clinical value of testicular lymphangiography in the diagnosis of retroperitoneal metastases was evaluated retrospectively in comparison with the findings obtained by retroperitoneal lymph node dissection. In 12 patients who had no metastasis in the primary lymph nodes of the testis, testicular lymphangiography showed the lymph vessels to be diverged into 2 to 6 vessels (mean: 3.5) at the level between L2 and L4, and 4 to 10 lymph nodes (mean: 6.2) at the level between L1 and L4 were filled with contrast medium. On the other hand, in 8 patients who had metastases in the primary lymph nodes, several abnormal findings were observed in both lymph vessels and nodes, i.e., discontinuity, extravasation of contrast medium, dilatation, displacement and reflux to the distal side in the lymph vessels, and decrease in number (less than 2), non-visualization, filling defect, displacement and contrastfilling in the contralateral side in lymph nodes. Three to 5 of these abnormal findings were usually found in each case. The extravasation of contrast medium was not a finding specific to cases with lymph node metastases, because it was also found in a few cases without metastases. Testicular lymphangiography is a valuable method to detect primary lymph node metastases from testicular tumor. However, the combination of testicular and foot lymphangiography is imperative to demonstrate wide spread lymph node involvement in the retroperitoneum.

Potentiation of anticancer agents by new synthetic isoprenoids. I. Inhibition of the growth of cultured mammalian cells.

In this study, 9 new synthetic isoprenoids with 9-10 isoprene chains were tested. One-fifth of the D10 value (concentration of drug that reduced colony-formation ability to 10% that of control) for each isoprenoid was calculated from dose-response curves of Chinese hamster V79 cells or human HeLa cells in culture. That dose was combined with various anticancer agent [1-(4-amino-2-methylpyrimidin-5-yl)methyl-3-(2-chloroethyl)-3-n itrosurea HCl, bleomycin A2 (BLM), cisplatin, doxorubicin (Dx), 5-fluorouracil, mitomycin, and 6-mercaptopurine] to test whether isoprenoids can potentiate the cytotoxic effect of anticancer agents against cultured mammalian cells. Among the 9 isoprenoids tested, decaprenoic acid, dacaprenylamine.HCl, and N-(p-methylbenzyl)decaprenylamine.HCl significantly potentiated BLM and Dx. Some isoprenoids increased intracellular levels of anticancer agents. Both enhanced uptake and reduced efflux of Dx were observed.

Presynaptic alpha 2- and dopamine-receptor-mediated inhibitory mechanisms and dopamine nerve terminals in the rat hypothalamus.

Slices of rat hypothalamus were superfused and endogenous release of dopamine was measured. The potassium (20 mM)-evoked release in the presence of tetrodotoxin was Ca2+-dependent. The evoked release was reduced by adrenaline 10(-7) M, with a potent alpha 2-agonist action, and was reduced by a dopamine receptor agonist, apomorphine 10(-7) M, in the presence of yohimbine 10(-6) M. These inhibitory effects were antagonized by an alpha 2-antagonist, yohimbine 10(-6) M, and a dopamine receptor antagonist, (-)-sulpiride 10(-7) M, respectively. The electrically evoked release of DA was enhanced by (-)-sulpiride but not by the (+)-isomer. Taking into consideration that yohimbine at a relatively high concentration (10(-6) M) increases the electrically evoked DA release, it is suggested that DA release in the rat hypothalamus may be mediated not only via autoreceptors but also in part via presynaptic alpha 2-receptors on the DA nerve terminals.

Presynaptic mediation by alpha 2-, beta 1- and beta 2-adrenoceptors of endogenous noradrenaline and dopamine release from slices of rat hypothalamus.

Using high performance liquid chromatography with an electro-chemical detector, we studied effects of different compounds on the impulse-evoked release of endogenous noradrenaline (NA) and dopamine (DA) release from slices of the rat hypothalamus. Adrenaline (10(-7) M), with a potent alpha-agonistic action decreased both NA and DA release, and these effects were blocked by pretreatment with yohimbine (10(-7 M). The alpha 2-antagonist, yohimbine alone (10(-8) - 10(-6) M) concentration-dependently increased these releases, while alpha 1-antagonist, prazosin showed weak increase on NA but not DA release at 10(-6) M. Isoproterenol (10(-10) - 10(-8) M) concentration-dependently increased these releases and the effects were antagonized by pretreatment with a non-selective beta-antagonist, 1-propranolol, a beta 1-antagonist, atenolol or a beta 2-antagonist, butoxamine. 1-Propranolol (3 X 10(-7) M) alone, but not the d-isomer inhibited the releases. Thus, in the rat hypothalamus, the release of NA and DA may be mediated via presynaptic alpha 2-, beta 1- and beta 2-adrenoceptors.

Immunocytochemical identification of thyrotropin (TSH)-producing cells in pituitary glands of several species of teleosts with antiserum to human TSH beta subunit.

A rabbit antiserum raised against human thyrotropin (TSH) beta subunit (anti-human beta-TSH) was used to identify TSH cells in the pituitaries of five species of teleosts by means of the unlabeled antibody peroxidase-antiperoxidase (PAP) technique. The anti-human beta-TSH reacted only with polygonal basophils located in the rostral pars distalis (RPD) in the pituitaries of sexually immature eel (Anguilla japonica). In the pituitaries of medaka (Oryzias latipes) and white-edged rockfish (Sebastes taczanowskii), this serum reacted specifically with only one cell-type located in the antero-dorsal proximal pars distalis (PPD) and antero-ventral PPD, respectively. In the pituitaries of two species of salmonids, sexually immature rainbow trout (Salmo gairdneri) and sexually mature chum salmon (Oncorhynchus keta), the anti-human beta-TSH bound to the polygonal basophils in the ventral PPD adjacent to the RPD. Thus, the present immunocytochemical study identified polygonal basophils located in the RPD or PPD as TSH cells in five species of teleosts.

17 alpha,20 beta-dihydroxy-4-pregnen-3-one: plasma levels during sexual maturation and in vitro production by the testes of amago salmon (Oncorhynchus rhodurus) and rainbow trout (Salmo gairdneri).

The present study investigated seasonal changes in plasma 17 alpha,20 beta-dihydroxy-4-pregnen-3-one (17 alpha,20 beta-diOHprog) levels in precociously mature and fully grown mature male amago salmon (Oncorhynchus rhodurus) and in vitro 17 alpha,20 beta-diOHprog production by the testes of amago salmon and rainbow trout (Salmo gairdneri) in response to chum salmon gonadotropin. Plasma 17 alpha,20 beta-diOHprog levels were very low from June to September, and rapidly increased in October at the beginning of the spawning season in both precociously mature and fully grown mature male amago salmon. High levels of 17 alpha,20 beta-diOHprog were maintained during the period of active spermiation in precociously mature males, and quickly declined in mid-November. Incubation of testicular fragments from spermiating rainbow trout with chum salmon gonadotropin or 17 alpha-hydroxyprogesterone resulted in a highly significant increase in 17 alpha,20 beta-diOHprog levels in the incubation medium. The chum salmon gonadotropin was also effective in stimulating 17 alpha,20 beta-diOHprog production by testicular fragments obtained from amago salmon after the breeding season. These findings are discussed in relation to the possible involvement of 17 alpha,20 beta-diOHprog in spermiation of salmonids.

Estradiol-17 beta and 17 alpha, 20 beta-dihydroxy-4-pregnen-3-one production by isolated ovarian follicles of amago salmon (Oncorhynchus rhodurus) in response to mammalian pituitary and placental hormones and salmon gonadotropin.

The effects of mammalian pituitary and placental hormones and dibutyryl cyclic AMP (cAMP) on the production of estradiol-17 beta and 17 alpha, 20 beta-dihydroxy-4-pregnen-3-one (17 alpha, 20 beta-diOHprog) by amago salmon (Oncorhynchus rhodurus) ovarian follicles in vitro were compared with partially purified chum salmon (Oncorhynchus keta) gonadotropin (SGA). Estradiol-17 beta production was examined using midvitellogenic ovarian follicles; SGA was the most effective hormone tested; at a dose of 1 microgram/ml it stimulated the production of approximately 4 ng/ml estradiol-17 beta. Ovine LH (o-LH) was about 0.5% as effective as SGA. Taking into account the relative purity of the preparations used, ovine FSH (o-FSH) was almost as effective as o-LH. Both HCG and bovine TSH (b-TSH) stimulated estradiol-17 beta production at high doses. Porcine ACTH (p-ACTH), ovine GH (o-GH), and ovine PRL (o-PRL) induced very modest increases in estradiol-17 beta; cAMP was highly effective. 17 alpha, 20 beta-DiOHprog production was examined using full-grown immature oocytes which were capable of maturing in vitro in response to 1 microgram/ml SGA. o-LH was about 0.5% as effective as SGA. o-FSH, b-TSH, and HCG appeared to be less effective in stimulating 17 alpha, 20 beta-diOHprog production, compared to estradiol-17 beta. o-GH and o-PRL induced marginal increases in production and p-ACTH had a very modest inverse dose-response effect.(ABSTRACT TRUNCATED AT 250 WORDS)
