[Total cavopulmonary shunt operation for single atrium and single ventricle].

A 17-year-old woman with single atrium, single ventricle, common atrioventricular valve and hemiazygous continuation underwent a total cavopulmonary shunt operation successfully. She had been cyanotic and with a heart murmur since two months of age. She was first seen at our hospital at three years of age. Catheterization, performed at the same time, had revealed pulmonary stenosis and transposition of the great arteries. At 17 years of age, catheterization was again performed, and a clinical diagnosis of single atrium, single ventricle, common atrioventricular valve and hemiazygous continuation was made. After the diagnosis was confirmed, a total cavopulmonary after ligation of the main pulmonary artery and shunt operation was performed division of the left superior vena cava, the distal end of the left superior vena cava was anastomosed to the confluent pulmonary artery. Two hours after return to ICU, extubation was done. The cardiac index was 4.36 L/min/m2 with central venous pressure of 145 mmH2O. The arterial oxygen saturation was 88.4%. After the surgery, this woman is doing well, with decreased CTR and improved exercise tolerance. Total cavopulmonary shunt can produce a marked improvement in the condition of patients with some type of complex cyanotic congenital heart disease.

[Serum adenosine deaminase and its isozyme activity in leukemia and MDS].

We studied the activity of serum adenosine deaminase (ADA) and its isozyme in 36 leukemic patients (16 ANLL, 11 ALL, and 9 CML) and 8 MDS. Isozyme was measured by erythro-9-(2-hydroxy-3-nonyl) adenine (EHNA) inhibitory assay. This assay was simple and reliable. The appearance rate of abnormally high ADA value were 81.24% for ANLL, 100% for ALL, 77.8% for CML and 37.5% for MDS. The ADA level became high when MDS turned into overt leukemia. In isozyme pattern, there was a clear difference between ANLL and ALL. The isozyme I/II ratio was significantly higher (p less than 0.001) in ALL than ANLL. Lymphoblastic crisis of CML also had a high isozyme I/II ratio. There was a correlation between isozyme I and absolute number of peripheral blasts in ALL (r = 0.768). When observed time sequentially, ADA and isozyme changed correlatively with the number of blasts counts. Serum ADA and its isozyme are useful parameters both for leukemic diagnosis and treatment.

[Continuous epidural infusion of bupivacaine and morphine for postoperative pain relief].

Forty-five patients admitted to the intensive care unit following thoracic or abdominal surgery received continuous epidural infusion of bupivacaine and morphine for 48 hours. During the first 10 hours, the patients received 0.25% bupivacaine solution with 0.005% morphine at the rate of 4 ml.h-1, and bupivacaine concentration was decreased to 0.125% with the same morphine concentration. The mean infusion rate of bupivacaine during 48 hours was 0.12 +/- 0.03 (SD) mg.kg-1.h-1 and that of morphine was 4.0 +/- 1.0 micrograms.kg-1.h-1. Thirty-one patients (69%) complained no pain on deep breathing at 24 hours and 33 patients (74%) required no other type of analgesics during this study. The mean plasma bupivacaine concentration was 0.6 +/- 0.3 microgram.ml-1 at 48 hours. Hypotension defined as systolic arterial pressure below 90 mmHg and itching were observed in 15 patients (33%), but no other severe side effects were noted. Continuous epidural infusion of bupivacaine and morphine mixture for 48 hours postoperatively provided effective pain relief with a low incidence of side effects.

[Evaluation of MRI for the staging of esophageal cancer].

The usefulness of MRI (magnetic resonance imaging) was evaluated in the diagnosis of the mediastinal invasion of esophageal cancer. MR examination was performed in 30 subjects without esophageal diseases and 23 patients with esophageal cancer. Normal esophagi and cancers were well visualized using T1-weighted image. In the axial section, every normal esophagus was contact to the aorta in the angle of less than 30 degree of the aortic circle. When the contact angle was less than 30 degrees, the stage of the cancer was below a1. When the angle was 30 degrees or greater, the stages were ranged from a0 to a3. In the oblique section orthogonal to the contact plain between the cancer and the aorta, the depth of the aortic invasion could be estimated. The sagittal scan was also helpful in the diagnosis of the invasion to trachea, left main bronchus and left atrium.

[Computed tomography for the diagnosis of thrombosed aortic dissection with extravasation].

Four cases of thrombosed aortic dissection with extravasation demonstrated by CT were presented. In CT diagnosis for this type of dissection, cautions should be employed not only in an inhomogenous density area in the mediastinum and pleural cavity but also in the presence of deviation of intimal calcification and relatively high density area of crescent shape in aortic wall on plain CT.

[Transarterial hepatic embolization of metastatic liver cancers with reference to 17 cases of survival for more than 2 years].

The evaluation of TAE was studied for 73 patients with metastatic liver cancers but without metastasis in other organs, who were treated with initial TAE more than 2 years ago. Seventeen patients survived for more than 2 years. The primary lesions consisted of colon cancer in 9 patients, the Langerhans islet cell tumor of the pancreas in 4, gastric cancers in 2, breast cancer in 1 and leiomyosarcoma of the colon in 1. The overall 2-year survival rate calculated by the direct method was 23.3% and mean survival period was 15.8 months. The prognosis was good in H1 and V3. Lp-TAE produced better effects but no statistically significant difference was found between the 2-year survivals by Lp-TAE and by GS-TAE.

[Biliary endoprosthesis using an expandable metallic stent].

Expandable metallic stents were inserted to treat malignant obstructive jaundice in 4 patients and no migration was noted. In 2 patients, all stents expanded sufficiently enough to remove the external drainage and remained patent. A relatively small caliber (7 Fr) was needed for the introduction of the stent, and when the stent was released, it acquired a much larger diameter. The expandable metallic stent could be used for the effective treatment of obstructive jaundice.

[Evaluation of a biliary endoprosthesis using expandable metallic stents].

A biliary endoprosthesis constructed of Gianturco self-expanding metallic stents was placed in twenty-nine patients with obstructive jaundice, 27 malignant and 2 benign. All endoprostheses were placed successfully. External drainage catheters were removed in twenty-seven patients. At the follow-up, ranging from two to 59 weeks, the 30 day mortality rate was 7.4%. Twenty-eight patients had obvious clinical improvement with relief of jaundice. Seven patients experienced recurrent jaundice. In two of these patients, the obstruction was relieved by additional metallic stents. The expandable metallic biliary endoprosthesis is suggested as an effective treatment for biliary obstruction.

[Treatment with expandable metallic stent for arterial occlusive disease].

Gianturco expandable metallic stents were used in 4 cases with ASO of the iliac artery after PTA for the purpose of the prevention of restenosis. The insertion of stent was succeeded in all 4 cases, and the improvement of clinical symptoms and elevation of ankle pressure index were observed. Follow-up over 3 to 8 months revealed no occlusion and migration of stent. Expandable metallic stent was suggested to be contributable to the treatment of arterial occlusive disease.

A complete deletion mutant of the Escherichia coli dnaKdnaJ operon.

Southern hydridization analyses of genomic DNAs from various dnaJ mutants of Escherichia coli showed that mutant K7052, which has well characterized dnaK706 and dnaJ705 double mutantions, is a deletion mutant. The deletion is about 8.0 kb long and encompasses the whole of the dnaKdnaJ operon.

Marked reduction of subcutaneous tumor growth by intraperitoneal administration of recombinant human interleukin 2 with a cell accumulator, proteose-peptone, in mice.

The growth of 3-methylcholanthrene-induced fibrosarcomas, Meth 1 and Meth A, was strongly suppressed by a combination of recombinant human interleukin 2 (rIL-2) with proteose-peptone (PP) administered i.p. to syngeneic mice. When 1 ml of 10% PP was injected i.p. on Day 6 followed by rIL-2 (50 micrograms) administered i.p. on Days 7 and 8 after the s.c. inoculation of tumor cells into female BALB/c mice, the tumors regressed. A similar result was also obtained when 12.5 micrograms of rIL-2 were injected on Days 7, 8, and 9 after s.c. inoculation of Meth 1 cells. The treatment with an anti-asialo-GM1 antibody had no effect on the regression of the Meth 1 tumor induced by the combination. However, the combined treatment with rIL-2 and PP did not suppress the growth of the Meth 1 tumor in adult thymectomized, irradiated, and fetal liver cell-reconstituted BALB/c mice. Therefore, this suggests that the T-cells might be the principal effectors of this antitumor system. The cytolytic activity of splenocytes and peritoneal exudate cells from Meth 1 tumor-bearing mice against Meth 1 cells was significantly augmented by the combined treatment. This peritoneal exudate cell also showed cytolytic activity against other target cells such as Meth A, antigenically distinct from Meth 1, YAC-1, a leukemic cell line sensitive to natural killer cells, and EL-4, a lymphoma cell line resistant to natural killer cells. The cytolytic activity of these effectors was reduced by the treatment with anti-thy1.2 antibody plus complement. The adherent cells in this peritoneal cavity had only a small cytolytic activity on Meth 1 and Meth A targets. The mechanism of antitumor immunity by rIL-2 in combination with PP and the therapeutic availability of this lymphokine are discussed.

Isolation and characterization of herC, a mutation of Escherichia coli affecting maintenance of ColE1.

Two modes of ColE1 DNA replication are known, one dependent on RNase H, and the other RNase H independent. The cer114 mutant of the ColE1 replicon is defective in both modes and carries a single base pair alteration 95 bp upstream of the replication origin. An Escherichia coli mutant which restored maintenance of the cer114 replicon was isolated. This host suppressor mutant is defective in RNase H and carries a herC mutation located at 62 min of the E. coli chromosome. The herC mutation is recessive to its wild-type allele and supports maintenance of the mutant replicon in the absence of RNase H. The herC mutation alone conferred cold-sensitive growth, suggesting that the herC gene product is essential for cell growth. The 1832 bp E. coli DNA fragment, containing the wild-type allele of the herC mutation, was cloned and an open reading frame for the HerC protein was determined.

Transcatheter arterial embolization using iodized oil (lipiodol) mixed with an anticancer drug for the treatment of hepatocellular carcinoma.

The therapeutic results of Lp-TAE (transcatheter arterial embolization with Gelfoam particles preceded by the infusion of a mixture of lipiodol and an anticancer drug via the proper hepatic artery) were evaluated in hepatocellular carcinomas (523 non-resected and 24 resected cases). Excellent therapeutic effects were confirmed not only for the main tumor but also for the daughter nodules by a histological examination of the liver tissues resected after Lp-TAE. The cumulative 1-year, 2-year and 3-year survival rates in the 523 non-resected cases were 60.4%, 42.9% and 28.0% respectively. These survival rates were all higher than those achieved by Gelfoam TAE. The above results suggest the usefulness of Lp-TAE in the treatment of hepatocellular carcinoma.

Inflammatory fibrous histiocytoma presenting leukemoid reaction.

A case of inflammatory fibrous histiocytoma presenting leukemoid reaction is reported. A tumor had developed at the right scapular region 10 years ago and recurred twice with fever and leukocytosis, and finally with leukemoid reaction. Besides the histopathological features typical of malignant fibrous histiocytoma, this tumor showed prominent and diffuse infiltration of neutrophilic leukocytes. Hematologic examination of the bone marrow aspirates disclosed myeloid hyperplasia. Bacteriological examination revealed no pathogenic microorganisms in the tumor tissue. Chemotactic activity of the neutrophilic leukocytes was demonstrated in the extract obtained from the tumor, and subcutaneous injection of the extract into rats induced myeloid hyperplasia in the bone marrow and neutrophil infiltration in the liver and spleen of the animals. These results suggest the possibility of the case as a colony-stimulating factor-producing tumor.

Quantitative computed tomographic evaluation of femoral bone mineral content in renal osteodystrophy compared with radial photon absorptiometry.

The computed tomography (CT) numbers of cortical bone at the level of 20 cm (CT20) and of spongiosa in the lateral condyle at the level of 2 cm (CT02) from the distal end of the femur were obtained by a quantitative CT method and compared with the bone mineral density of mostly cortical bone within the radius (BMD) by photon absorptiometry. The study included 47 patients with chronic renal failure not dialyzed or induced to regular hemodialysis within 4 weeks of the study (group 1), 28 patients on regular hemodialysis for more than one month (group 2), and ten healthy volunteers (group 3). The measures of bone mineral content (BMC), namely CT20, CT02, and BMD, were compared in terms of their abilities to distinguish members in the various groups. For group 1 and group 3, the greatest variation in BMC was in the difference in CT02, which was primarily a measurement of the BMC of spongiosa. For groups 1 and 2, the greatest variation was in the difference in BMD, which was primarily a measurement of the BMC of cortex. The reproducibility of CT02 was estimated as almost equal to the difference in CT02 values at intervals of 10 months' duration of hemodialysis. The results indicated that CT02 was a useful measurement for evaluating the progress in the early stage of the renal osteodystrophy, and it is recommended that the bone mineral measurement with this QCT method should be performed once or twice a year.
