An immunohistochemical study of cytokeratin and vimentin in benign and malignant thyroid lesions.

Intermediate filaments in benign and malignant thyroid lesions were immunohistochemically studied using polyclonal and monoclonal anti-cytokeratin (CK), and monoclonal anti-vimentin antibodies. Antigenicity of CK and vimentin was almost completely destroyed during formalin fixation in normal thyroid and all thyroid lesions except for some cases of papillary and squamous cell carcinoma, although the latter showed negative immunostaining with anti-vimentin antibody. In sections fixed with Carnoy's fixative, most cases of papillary carcinoma showed an intense reaction product for polyclonal anti-CK, monoclonal anti-CK-7, CK-19 and anti-vimentin antibodies. The reaction product for anti-CK antibodies was located mainly in the apical cytoplasm and that for anti-vimentin antibody in the basal cytoplasm. However antigenicity was still destroyed by the fixative in many specimens of normal thyroid, benign thyroid lesions and follicular carcinoma. In frozen sections, all specimens showed preserved antigenicity for both antigens with an intense reaction product in papillary carcinoma, but this was weaker in normal thyroid, benign thyroid lesions and follicular carcinoma. Therefore, follicular cells under normal and pathological conditions contain intermediate filaments of CK and vimentin in their cytoplasm and co-expression of the antigens is significantly increased in papillary carcinoma.

Right upper lobe versus right middle lobe: differentiation with thin-section, high-resolution CT.

A new method was developed to differentiate the upper lobe from the middle lobe of the right lung with thin-section, high-resolution computed tomography. In the upper lobe, a medial subsegmental bronchus of an anterior segmental bronchus is always located lateral to the corresponding artery. In the middle lobe, a medial subsegmental bronchus of a lateral segmental bronchus and a superior and inferior subsegmental bronchus of a medial segmental bronchus are always located medial to the corresponding artery. In other words, the anatomic relationship between the subsegmental bronchi and the corresponding pulmonary arteries in the upper lobe is opposite to that in the middle lobe. One hundred seventeen cases, including 54 cases of lobar volume loss, were reviewed with this method, and in each case it was possible to differentiate the upper lobe from the middle lobe without the contiguous section analysis.

Measurement in vivo of masticatory mucosal thickness with 20 MHz B-mode ultrasonic diagnostic equipment.

Analysis of the movement of the denture foundation under function is very important for construction of a denture. For this analysis, the thickness of the masticatory mucosa gives the required quantitative information. The thickness measurements were taken from 21 oral sites in 100 edentulous patients, by means of 20 MHz B-mode ultrasonic equipment. The average thickness of the masticatory mucosa ranged from 1.92 mm to 2.38 mm at the upper edentulous ridge and from 1.45 mm to 1.58 mm at the lower edentulous ridge. The center of the palate had the thinnest mucosa and the lateral area of the palate had the thickest among all the measuring points. Contrary to what is often believed, a decrease in mucosal thickness with age was not observed by one-way analysis of variance. Furthermore, as the degree of ridge reduction increased, a decrease in the mucosal thickness was found by one-way analysis of variance at only two of the 14 measuring sites on the ridges. At four of the 21 measuring sites, male patients had significantly thicker mucosa than did females, and the correlation coefficients of the thickness of mucosa between right and left sides in the palate were higher than those in residual ridges.

[A case of gall bladder cancer with high level alpha-fetoprotein].

A case of primary gall bladder tumor with high level AFP were demonstrated, which showed hepatocellular carcinoma like pattern with trabecular proliferation histologically, positive AFP stain of cancer cells by a peroxidase conjugated antibody method immunohistochemically. This case was no metastasis with liver and lymph nodes.

[Manipulation of gallium restorative materials. 1. Control of wetting action of mixture by mulling with ethanol].

The wetting action of gallium alloy is a handicap when handling the material. Ethanol mulling after mixing was assayed in an attempt to control the wetting action of such mixtures. A mixture of gallium and Ag-Sn-Cu spherical alloy particles was mulled with a small piece of an ethanol-soaked sponge for 10 seconds following mixing by an amalgamator. The effect of this mulling on the adherence of the mixture to the capsules, physical properties and microstructures was determined. Mulling with ethanol effectively controlled the wetting action of the mixture, and reduced adherence to the capsules without affecting physical properties. A decreased reaction phase around the alloy particles was observed in the microstructure of the ethanol-mulled specimen.

[Study on the disinfection of alginate impressions. 2. Dimensional stability and surface roughness after immersing or spraying treatment with povidone-iodine solution].

The dimensional changes of four alginate impression materials and the surface roughness of stone casts following the treatment with 0.07% Povidone-Iodine solution were evaluated. The impressions were treated by either immersing or spraying. The dimensional change was measured by an electronic micrometer with a non-pressure direct method for 60 minutes. The surface roughness of stone casts obtained from the impressions following optimal treatment was also measured. Two products were dimensionally stable for 60 minutes after spraying with the solution, while the other two were stable for 30 minutes in the solution. No effect of the treatment was observed on the surface roughness of stone casts.

[Boron neutron capture therapy using 10B entrapped anti-CEA immunoliposome].

A new murine monoclonal antibody (2C-8) was prepared by immunizing mice ip with CEA producing human pancreatic cancer cell line, AsPC-1.SDS-PAGE and Western blot analysis showed that 2C-8 monoclonal antibody recognized CEA and NCA. This anti-CEA monoclonal antibody was conjugated with large multilamellar liposomes incorporated 10B compound (Cs2 10B12H11SH). This immunoliposomes applicated to boron neutron capture therapy. AsPC-1 cells were incubated with the 10B-Lip-MoAb(CEA) for 8 hours. After the irradiation with thermal neutron (1 x 10(11)-1 x 10(13) n/cm2), boronated AsPC-1 cells were showed decreasing uptake of 3H-TdR compared with control group. The numbers of 10B atoms in liposomes bound to an antibody were in proportion to the dose of 10B compounds added and maximum number of 10B atoms was approximatory 1.2 x 10(4)/Ab. These data indicated that the immunoliposomes could deliver highly amount of 10B atoms to the tumor cells and exert cytotoxic effect by thermal neutron. BNCT with immunoliposome may be useful to the non resectable malignant tumors in clinical application.

[The usefulness of motion artifact suppression technic (MAST) in the MRI diagnosis of liver tumors].

The usefulness of motion artifact suppression technique (MAST) in the MRI diagnosis of liver tumors was assessed. Spin-echo with MAST was superior in lesion detection and contrast-to-noise ratio to conventional spin-echo. Its usefulness was verified. As the portal vein is delineated frequently as a high signal area, the portal structure up to the third branches was distinctly visualized. It was found helpful for the segmental diagnosis of the liver. However, careful attention is needed in the diagnosis of portal vein thrombosis.

[Specific cytoplasmic crystalline inclusions in multiple myeloma with amyloidosis].

It is widely known that multiple myeloma is sometimes followed by amyloidosis. It is also not particularly rare that inclusions exist in myeloma cells. However, there has been no previous report of a case of myeloma with both inclusions and amyloidosis. A 60-year-old female initially complained of a tendency to bleed, which was caused by fibrinolysis. Amyloid deposition in bone marrow stroma and the gastric submucosa was recognized, in addition to crystalline inclusion in the cytoplasm of myeloma cells. An immunoelectron microscopic study demonstrated the amyloid fibrils and the crystals to react positively to anti-lambda serum. No crystals were found in macrophages, and no relationship was recognized between lysosomes and crystalline inclusions in the cytoplasm of myeloma cells. This case had disturbed transportation or secretion of the lambda type L chain and it was considered that crystals derived from the lambda type L chain were formed in the cytoplasm of myeloma cells.
