Hair trace elements in cerebellar degeneration: low copper levels in late cortical cerebellar atrophy.

The levels of ten elements in the hair of 7 patients with olivopontocerebellar atrophy (OPCA), 10 patients with late cortical cerebellar atrophy (LCCA) and 1489 healthy persons were determined by inductively coupled plasma emission spectrometry. The phosphorus, zinc, selenium, calcium, magnesium, iron, manganese, aluminium and mercury concentrations did not show any statistically significant difference among the three groups. The copper concentration was significantly lower in the patients with LCCA than in the patients with OPCA, and in the healthy controls. The possibility that LCCA is associated with a mild degree of copper deficiency is considered.

Left-right asymmetry of the temporal and parietal regions in children: based on the medullary pattern of cerebral white matter.

Lateralization of the posterior temporal lobe and inferior parietal lobule in childhood was studied on CT of 276 cases and compared with those in adulthood of 371 cases. CT images were analyzed based on the morphological features of medullary branches of the cerebral white matter. In children below 5 years of age, the left planum temporale was already more posteriorly located than the right in 56%, while the right was more so in 21%. The asymmetry of the middle temporal gyrus was similar to those of the planum temporale. The folding of the inferior parietal lobule was extensive and intricate on the left side in 63% and on the right side in 8% of children, almost identical to those in the adult. The visualization rates of the intraparietal sulcus were lower compared to those of the adult, but the higher visualization of the left side was a common tendency regardless of age. These alterations in left-right asymmetry seemed to be a morphologic substratum indicating the processes of localization of auditory function to those association areas.

Effects of mexiletine on delayed after-depolarization and triggered activity.

The effects of mexiletine on delayed afterdepolarization (DAD) and triggered activity (TA) were studied in the rabbit ventricular muscle using standard microelectrode techniques. First, three kinds of perfusate (modified Tyrode's solution, K(+)-free solution, and 1 mM K+ + 5.4 mM Ca2+ solution) were used to see whether DAD and TA could be induced by rapid stimulations (cycle lengths of 1000, 600, and 300 ms with trains of 10 and 20 stimuli). The inducibility of DAD and TA in modified Tyrode's solution, in K(+)-free solution, and in low K+ + high Ca2+ solution was 0%, 8%, and 83%, respectively. The last value was significantly higher than the former values. DAD and TA were induced only by stimulation at the shortest cycle length of 300 ms, and the inducibility was significantly higher with trains of 20 than with those of 10 stimuli. When DAD and/or TA were induced, the effects of mexiletine (5 mg/l) were tested. Mexiletine totally suppressed DAD and TA in 94% of the preparations within 20 min after its addition. Abolition of DAD and TA was associated with a failure of the 1:1 response to the stimuli in 53% of the preparations. The drug tended to prolong the coupling interval of DAD and TA, and significantly reduced the DAD amplitude. Possible mechanisms of action are: (1) lowered intracellular Ca2+ concentration either via the blocking of the fast Na+ current or the reduction of the number of action potentials; and (2) a decrease in the transient inward current due to blockage of the Ca2+ current.

Continuous epidural infusion for postoperative pain relief: a comparison of three regimens.

We evaluated the postoperative pain relief and side-effects of continuous epidural infusion of three analgesic regimens following major thoracic and/or abdominal surgery. One hundred and twenty patients were randomly divided into three treatment groups: (1) 0.25% or 0.5% bupivacaine at a rate of 3-7 ml.hr(-1), (2) 0.01% morphine at a rate of 1-2 ml.hr(-1), (3) a combination of 0.125% or 0.25% bupivacaine and 0.0025% or 0.005% morphine at a rate of 2-4 ml.hr(-1). The study continued for the first 48 postoperative hours. The effect of pain relief was evaluated by assessment of the further requirement for parenteral analgesics. Sixty-four percent of the patients given bupivacaine, 56% of the patients given morphine and 80% of the patients given the combination required no supplemental analgesics. Continuous epidural infusion of bupivacaine was associated with hypotension (21%) and with numbness and weakness of hands or legs (18%). Continuous epidural infusion of morphine was associated with pruritus (18%) and with peristaltic depression (12%). The combination regimen was associated with pruritus (17%) and with drowsiness (14%). We conclude that the combination of bupivacaine and morphine significantly provides superior analgesia with less deleterious complications compared with either bupivacaine or morphine alone.

Determination of non-metallic elements by capacitively coupled helium microwave plasma atomic emission spectrometry with capillary gas chromatography.

A capacitively coupled microwave helium plasma with a tubular tantalum electrode was evaluated as an element selective detector for gas chromatography (GC). The end of a 10-m bonded fused capillary column was directly inserted into the tubular electrode without any switching system. A heated copper tube was used to house the part of the GC column that protruded from the oven. The optimisation of operating parameters, line selection, background emission and horizontal and vertical observation position is described. Analytical figures of merit including sensitivity, reproducibility, signal to background ratio, selectivity, dynamic range and limit of detection (LOD), were evaluated for carbon, hydrogen, chlorine and bromine emission. Limits of detection in the low ng range (20 pmol) were obtained for halogenated compounds using carbon emission, whereas LODs in the 0.1 micrograms range (2 nmol) were obtained using chlorine or bromine emission lines.

Combination treatment with irritant and recombinant interleukin 2 in the peritoneal cavity for evoking effective anti-tumor activity: generation of lymphokine-activated killer cells and tumor-specific killer cells.

Meth A sarcoma, growing in the subcutaneous tissue of syngeneic BALB/c mice, regressed completely after an intraperitoneal (ip) injection of proteose peptone (PP) (on day 6) followed by 2 ip administrations (on days 7 and 8) of human recombinant interleukin-2 (IL-2, 25 micrograms/day), whereas one such treatment alone had little effect on the tumor growth. While this combination treatment was effective in anti-asialo GM1 antibody-treated mice, no such effect was noted in T cell-depleted ATXFL (thymectomized, irradiated and fetal liver cell-reconstituted) mice. These results show that T cells are mainly responsible for this antitumor effect. Treatment with a combination of PP and IL-2, but not with either PP or IL-2 alone, resulted in a marked increase in the T cell population in the peritoneal cavity after the treatment. At an early stage after the combination treatment, both peritoneal exudate cells (PEC) and spleen cells exhibited killing activity with a promiscuous specificity. However, at a later stage, 7 days after the treatment, Meth A-specific killer activity was observed in both PEC and the spleen. Meth A rechallenge was rejected by the mice in which the tumor had regressed, but the antigenically different Meth 1 was accepted by them. A similar result was obtained in Winn's neutralization test. These results suggest that this combination treatment, which is effective in the generation of lymphokine-activated killer cells in the peritoneal cavity, finally resulted in the induction of tumor-specific killer cells in the periphery. These results clearly show the anti-tumor efficacy of combination treatment with PP and rIL-2.

Expandable metallic biliary endoprostheses: preliminary clinical evaluation.

A biliary endoprosthesis constructed of self-expanding metallic "Z" stents was placed in 23 patients with obstructive jaundice. The biliary obstruction was due to a malignant neoplasm in 21 patients and a postoperative biliary stricture in two patients. The lesions affected the intrahepatic biliary ducts in 13 patients. Twelve patients had undergone radiation therapy before stent placement. The endoprostheses consisted of 196 expandable metallic biliary stents placed singly (n = 10) or in tandem (n = 186). As many as 18 stents were used to relieve an obstruction in one patient. A transhepatic approach was employed in all patients except one in whom stents were placed through a T-tube tract. Within 1 week after placement, all stents expanded to at least 90% of their original diameter. Three misplaced, two deformed, and two dislodged stents caused no obvious clinical problems. At follow-up, which ranged from 2 to 59 weeks, five patients experienced recurrent jaundice. Two patients with recurrent jaundice due to obstruction of the bile duct containing the stent were treated with external catheter drainage. The expandable biliary endoprosthesis is suggested as an effective treatment for benign and malignant biliary obstruction.

Iliac artery stenosis and occlusion: preliminary results of treatment with Gianturco expandable metallic stents.

Ten patients with atherosclerotic stenosis or occlusion of the iliac artery were treated with Gianturco expandable metallic stents. In the five cases of stenosis, only balloon dilation was performed prior to placement of stents. The five patients with occluded arteries were given intraarterial infusions of urokinase before balloon dilation and stent placement. Clinical symptoms improved in all patients, and no technical failures or complications occurred. Doppler ankle-brachial index studies were performed in nine cases, and in all nine cases the indexes improved after stent placement. During follow-up of 2-18 months (mean, 10.3 months), all arteries remained patent. Follow-up angiograms showed slight intimal thickening and no restenosis. Long-term follow-up and more clinical experience will be necessary to evaluate the efficacy of this stent. However, preliminary results suggest that the Gianturco expandable metallic stent is of value in the treatment of arterial occlusive disease.

A case of Sweet's syndrome with early gastric cancer.

The case of a 49-year-old man with Sweet's syndrome (acute febrile neutrophilic dermatosis) is described, who had coincidently gastric cancer. The therapeutic effectiveness of colchicine is also presented as one treatment for Sweet's syndrome, because rapid regression of the lesions was noticed.

A case of pheochromocytoma producing parathyroid hormone-related protein and presenting with hypercalcemia.

A 54-yr-old man with a left adrenal pheochromocytoma showed mild hypercalcemia and elevated nephrogenous cAMP. Serum levels of PTH and 1,25-dihydroxyvitamin D3 were not elevated. Postoperatively, serum calcium and nephrogenous cAMP declined to normal ranges. Pathologically, the tumor was a benign pheochromocytoma. The clinical findings resembled those of humoral hypercalcemia of malignancy (HHM), and PTH-related protein (PTHrP) immunoreactivity was detected in the tumor extract at a concentration of 80.7 pmol/g wet wt, which is high compared to levels in malignant tumors causing HHM. Production of PTHrP was further confirmed by the demonstration of PTHrP mRNA with Northern blot hybridization analysis. Gel filtration of the extract revealed the presence of at least two different molecules with both immunological and biological activities. One of the peaks appeared close to PTHrP-(1-34), and the other between cytochrome-c and BSA. The latter showed a higher bioactivity to immunoreactivity ratio. These data indicate the multiplicity of PTHrP molecules in pheochromocytoma and support the idea that PTHrP produced by pheochromocytoma causes hypercalcemia in a similar fashion as HHM.

[The effect of donor specific blood transfusion (DST) on graft survival--intermittent coverage of cyclophosphamide].

Donor specific blood transfusion (DST), given prior to living related kidney transplantation has resulted in significant improvement in graft survival. This improvement, however, has been accomplished with a high rate of adverse sensitization against the donor. In an attempt to reduce the incidence of sensitization, we have employed DST with intermittent coverage of cyclophosphamide. A comparative study was done between 2 methods of DST with or without the coverage of immunosuppressant for prospective kidney transplant recipients from living related donors. In addition, the beneficial effect of DST on graft survival was evaluated in our recent series of living related transplantation using cyclosporine A (CsA) as postoperative immunosuppression. Twenty-nine prospective kidney transplant patients received 200 ml of fresh whole blood 3 times at 2 week intervals from HLA one-haploidentical living related donors. The first 13 patients received DST alone, while the remaining 16 were given cyclophosphamide (CPM 1.5 mg/kg/day) for 3 days prior to each DST. In patients with CPM coverage, 6.3% (1 of 16) developed positive T-warm antibody against donor and 15% of patients (2 of 13) with DST alone developed it. Like-wise 19% (3 of 16) of the former and 38% (5 of 13) of the latter became positive B-warm crossmatch. The difference in sensitization rates between these 2 groups was not statistically significant. Nineteen patients receiving DST were compared to 21 non-DST patients in incidence of acute rejection, graft function and graft survival with the same immunosuppressive regimen, such as CsA, prednisolone, and mizoribine.(ABSTRACT TRUNCATED AT 250 WORDS)

[Establishment and characterization of human pancreatic adenocarcinoma cell line SOJ producing carcinoembryonic antigen and carbohydrate antigen 19-9].

A new tumor cell line derived from a human pancreatic exocrine adenocarcinoma was established in tissue culture and was transplantable in a nude mouse. In tissue culture, the neoplastic cells grew as epithelial-like, mucin-producing cells with a population doubling time of 50-70 hrs. Chromosomes ranged from 63 to 186 with a modal number of 77. Subcutaneous injection of 1 x 10(6) cultured neoplastic cells into nude mice resulted in tumor formation histologically closely resembling the original neoplasm. Ultrastructurally, the cell line showed characteristic ductal epithelium. Immunohistochemically, carcinoembryonic antigen (CEA). Carbohydrate Antigen 19-9 (CA19-9) and DU-PAN-2 antigen were demonstrated in the original tumor, the culture cells and the transplanted tumor. The cells secreted CEA (48.7 ng/1 x 10(5) cells/24 hrs) and CA19-9 (325 U/1 x 10(5) cells/24 hrs) in spent medium as well as sera of the nude mouse. This cell line has been passaged 30 times in vitro and maintained for more than one year. These characteristics will make the cell line SOJ a valuable tool in studying various aspects of biology of human pancreatic cancer.

[A behavioral dental science research concerning prevention of dental fear. A report on the surveys of dental fear in four countries Japan, Indonesia, Brazil and Argentina].

We conducted a behavioral scientific research from the point of view of dental fear prevention. This time, the writers carried out the surveys with the goal of understanding and making clear the realities concerning dental anxiety in the four countries: Japan, Indonesia, Brazil and Argentina. The subjects were 728 dental students from the four countries, and the surveys consisted of 25 multiple choice questions which were divided into three major areas. The first area consisted of preliminary questions such as chief complaints and treatment experiences, the second area dealt with the reactions and physical responses on receiving dental treatment and the third area was on the different levels of anxiety shown during treatment expressed through feelings and reactions. We concluded as follows: 1. In Japan, 77% of the subjects reported having at least a little fear and this percentage was higher than those of other countries (45-65%). But in all countries, 4-7% of subjects reported having extreme dental fear. 2. In Japan and Brazil, women reported the higher fear than men. In Indonesia and Argentina, these results were the opposite. 3. According to canonical correlation analysis, a significant correlation between the level of physical reactions and the degree of dental anxiety was found in all countries. 4. In the high fear group, dental anxiety increased by psychological stress before treatment and the expressions of physical reactions were stronger than in the low fear group.

[The analysis of superselective catheterization technique; transcatheter segmental arterial embolization using lipiodol for hepatocellular carcinoma].

The practical limitations of catheters used for infusion were studied by their types in 47 patients with hepatocellular carcinoma who were treated by Segmental Lp-TAE. For catheterization into the arterial branches of S5 and S8, a coaxial infusion catheter was helpful. However, a coaxial infusion catheter was seldom used for catheterization into the arterial branches of S6 and S7. For infusion into vessels having diameters of less than 1.9 mm at the tip end of the catheter, positive use of a coaxial infusion catheter is strongly recommendable. A Tracker-18 catheter was especially helpful for catheterization into the vessels of S5.

[Behavior of anti-cancer drug of transcatheter hepatic segmental arterial chemoembolization using lipiodol mixed with an anti-cancer drug followed by gelatin sponge assessed by Tc-99m pertechnetate].

The behavior of an anti-cancer drug consisting of transcatheter hepatic segmental arterial chemoembolization using Lipiodol mixed with an anti-cancer drug followed by gelatin sponge (segmental Lp-TAE: Seg-Lp-TAE) was assessed in comparison to that of the non-segmental Lp-TAE to whole liver (Lp-TAE) by means of Tc-99m pertechnetate (RI). Injection of Lipiodol mixed with Adriamycin and 740 mBq of RI followed by gelatin sponge was carried out from segmental or subsegmental hepatic artery in 12 cases of hepatocellular carcinoma and from proper or right hepatic artery in 11 cases (9 with hepatocellular carcinoma and 2 with metastatic liver tumor). The distribution and retention of RI were studied from the time activity curves of ROIs in tumor and non-tumor area, and scintigrams at 4 and 24 hours after injection. The integral value ratio of tumor to non-tumor was significantly higher in Seg-Lp-TAE, indicating that es the distribution of RI after Seg-Lp-TAE is selective in the segments where tumor exists. In Seg-Lp-TAE, the T1/2 was higher and the count ratio of tumor to non-tumor was significantly higher at 4 hours and even higher at 24 hours after injection. These results suggest that an anti-cancer drug reinforces the effect on tumor with little adverse effect on normal liver in Seg-Lp-TAE, which is more advantageous to the treatment of hepatocellular carcinoma.

Possibility for induction of convulsion by fleroxacin and its disposition in the central nervous system in animals.

Fleroxacin and its metabolites were examined for their possibility of the induction of convulsion in mice, inhibitory activity on GABA receptor binding, and disposition in the central nervous system of mice, rats and dogs. No convulsion was evoked in mice at high oral combination doses of fleroxacin and fenbufen. Brain-to-serum concentration ratios of fleroxacin after oral dose were 0.13 in mice, 0.19 in rats and 0.28 in dogs. Rats showed no accumulation of fleroxacin in brain and similar elimination from brain to that from serum after the oral dose. Dogs exhibited the similar distribution of fleroxacin into various brain regions. The inhibitory effect of fleroxacin on GABA receptor binding was relatively weak and slightly potentiated in the presence of 4-biphenylacetic acid. Demethyl fleroxacin showed more potent inhibitory activity on GABA receptor binding and potentiation with 4-biphenylacetic acid. The combination of intravenous demethyl fleroxacin and oral fenbufen showed no convulsion in mice. Fleroxacin N-oxide showed only slight inhibitory activity on GABA receptor binding, being not influenced with 4-biphenylacetic acid.

[Ruptured congenital aneurysm of the sinus of valsalva in the aged].

Ruptured aneurysm of the sinus of Valsalva is a rare cardiac lesion. A ruptured aneurysm of the sinus of Valsalva in the right ventricle of a 64-year-old man was successfully repaired. The patient was admitted to the hospital with high fever and chest oppression. Diagnosis was made by two dimensional echocardiography, cardiac catheterization, and cardiac angiography. An aortotomy, main pulmonary arteriotomy, and right ventriculotomy were performed. There was no VSD, and the aneurysm originated from the right coronary sinus, rupturing into the right ventricle inlet portion. The ruptured aneurysm of the sinus of Valsalva was closed with a Dacron patch from inside the aorta. He is doing well after surgery. There was no heart murmur. CTR decreased and pulmonary blood flow fell to a normal value. As far as we know, this patient is the second oldest patient in Japan with surgical repair.
