Percutaneous hot saline injection therapy for hepatic tumors: an alternative to percutaneous ethanol injection therapy.

PURPOSE: To evaluate the usefulness of percutaneous hot saline injection therapy (PSIT) in the treatment of hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Twenty patients with HCC (23 nodules) underwent PSIT. Mean tumor diameter was < or = 30 mm. PSIT was performed by injecting hot saline into the HCC. The therapeutic effect of PSIT was evaluated with computed tomography, angiography, measurement of alpha-fetoprotein levels, and histopathologic examination of needle biopsy and resected specimens. RESULTS: No complications were encountered, and a therapeutic effect was obtained in all cases. When the injected material cools, it becomes physiologic saline making it possible to inject relatively large volumes in a single session. CONCLUSION: PSIT has promise as a form of curative local therapy for small HCCs and is expected to exert a safe and favorable therapeutic effect on large HCCs as well.

A case of 11q-syndrome associated with abnormalities of the retinal vessels.

A case of 11q- syndrome associated with abnormalities of the retinal vessels similar to those of familial exudative vitreoretinopathy was followed from 6 months to 3 years of age. The patient was a female infant born at 39 weeks of gestation and weighing 2,360 g without oxygen therapy. A chromosomal analysis showed a 46,XX,del(11)(q23.3). An ocular examination revealed exotropia and a positive kappa angle of the left eye with psychomotor retardation. An abnormal pattern of the retinal vessels and an avascular retina was observed in the periphery of both eyes. Therefore we concluded that the abnormalities of the retinal vessels may be related to the deletion of the long arm of chromosome 11.

Production and epitope specificity of monoclonal antibody against mouse peptidylarginine deiminase type II.

Peptidylarginine deiminase catalyzes the conversion of arginyl residues in proteins to citrullyl residues in the presence of Ca2+. We described the preparation of monoclonal antibody (subclass type IgG1) specific to mouse peptidylarginine deiminase type II. The antibody had no effect on the enzyme activity and its specific epitope was localized in the eight-residue segment at the amino-terminal portion of the enzyme.

Comparative analysis on the physique and batting records of the players in the National Summer High School Baseball Tournaments before and after the adoption of metal bats.

To clarify the influence of the introduction of metal bats on the physique and batting records of the players in the National Summer High School Baseball Tournaments, a comparative analysis was conducted between height, body weight, body-weight ratio (body weight/height), batting average and home run average of the best four teams' players (n = 493) and those of the other teams' players (n = 4,590) in three periods: the period of the use of wooden bats, that of the use of both wooden and metal bats and that of the use of metal bats. In the period of metal bat use, the mean values of physique of the best four teams' players were significantly larger (P < 0.05) and their average number of home runs was significantly higher than those of the other teams' players (P < 0.01). The only significant differences between the two groups in those indices for each time period were for height in the wood and metal/wood periods. This study demonstrated that the differences between the home run average and physique, including height, body weight and body-weight ratio of the best four teams and the rest of the teams were the greatest after the use of metal bats. These findings suggest that the importance of the home run average increased and was associated with large-size of physique after the use of metal bats in the National Summer High School Baseball Tournaments.

Correlation between the layer of an intimal tear and the progression of aortic dissection.

Thoracic aortas isolated from dogs were evaluated to determine the correlation between the depth of an intimal tear and the progression of aortic dissection. Thoracic aortas obtained from 99 adult mongrel dogs were used. An intimal tear (aortic pocket) was created on the aortic wall and the aorta was connected in series to a closed circuit (150 mmHg/100 mmHg and 60 beats/min). The progression of the dissection was most notable in the aortic pocket prepared in the first one-third of the external media (87.5%). There was no correlation however between the width of the pocket and the progression of dissection. Examinations of samples from surgical patients have shown that dissection usually progresses at the same site. This has been attributed to the diseased media. Our results demonstrated that dissection occurred almost always at the same site without any medial changes suggesting that whether dissection develops or not likely depends upon whether the intimal tear reaches the first one-third of the external media or not.

An experimental hemodynamic study of the pelvic collateral circulation.

Iliac arteries were occluded in adult mongrel dogs to investigate pelvic hemodynamics. When the unilateral common iliac artery was occluded, the blood flow making a "stopover" within the pelvis was found to be significantly less than that of anatomical hemodynamics even under a resting condition. The blood flow decreased more significantly under exercise loading than under a resting condition, which demonstrates the presence of the "steal" phenomenon. This only occurs in the collateral circulation in the pelvis formed by two arterial systems which are related in a series. In deciding the appropriacy of reconstruction for the internal iliac artery in patients with aorto-iliac occlusive disease, this "steal" phenomenon should be kept in mind. In most cases, ischemic symptoms in pelvic organs may be due to a simple decrease of the blood flow supplied to the pelvis, or due to the "steal" phenomenon. If the pelvic region is in the state of ischemia owing to the "steal" phenomenon, reconstruction of the blood vessels flowing into the pelvis is not required.

Acquired idiopathic pure red cell aplasia in a hemodialyzed patient with inactive systemic lupus erythematosus.

A male patient suffered chronic renal failure due to lupus nephritis and was undergoing hemodialysis. Six years after beginning hemodialysis, anemia developed, which improved by erythropoietin. Unresponsiveness to erythropoietin gradually appeared, and with a suspicion of pure red cell aplasia, he was treated with a high-dose corticosteroid but the unresponsiveness did not improve. Neither his serum nor lymphocytes inhibited erythropoiesis of either normal bone marrow stem cells or his own in vitro. These observations suggest an impaired hematopoietic microenvironment in his bone marrow.

Chrymutasins: novel-aglycone antitumor antibiotics from a mutant of Streptomyces chartreusis. I. Taxonomy, mutation, fermentation, isolation and biological activities.

Three novel antibiotics, named chrymutasins A, B and C, were isolated from the fermentation products of a mutant strain obtained by NTG (N-methyl-N'-nitro-N-nitrosoguanidine) treatment. The mutant strain produced the chrymutasins, which differed in the aglycone moiety from chartreusin, and related compounds. The production of these compounds needed a characteristically long fermentation period. The antitumor activity of chrymutasin A is better in vivo than that of chartreusin, the cytotoxic activity against cell lines in vitro is equivalent, and the antimicrobial spectrum is narrower.

Chrymutasins: novel-aglycone antitumor antibiotics from a mutant of Streptomyces chartreusis. II. Characterization and structural elucidation.

Chrymutasins A, B and C are glycosidic antibiotics produced by a mutant of the chartreusin producer-organism Streptomyces chartreusis. We report here the structure elucidation of these compounds. The sugar moieties involved were determined by comparison with the related chartreusins. The structure of the aglycone, the same in all three compounds, was elucidated by NMR, incorporation studies of labeled compounds and synthesis of derivatives. The chrymutasin aglycone differs from that of chartreusin by a single carbon and an amino group.

[Role of neutrophils in warm renal ischemia and reperfusion injury in the rat].

This study attempts to determine whether the circulating neutrophils play a role in mediating injury resulting from renal ischemia, using rats treated with cyclophosphamide (CY) and/or granulocyte colony-stimulating factor (G-CSF). The neutrophil counts immediately before 60-minute ischemia decreased by 89% in the CY-treated rats in comparison with the untreated animals (p < 0.01). In the rats given G-CSF with CY, the neutrophil counts significantly increased over that of the CY-treated rats (p < 0.01). The serum creatinine level 24 hours after reperfusion amounted to 4.42 +/- 0.37 mg/dl in the control rats and 2.63 +/- 0.29 mg/dl in the CY-treated rats (p < 0.01). In 19 rats, 10 controls and the 9 CY-treated, the neutrophil counts immediately before ischemia correlated well with the serum creatinine levels 24 hours after reperfusion (r = 0.597; p < 0.01). Our results appear to indicate that neutrophils play an important role in warm ischemia and reperfusion injury of the kidney.

[Sevoflurane anesthesia in a patient with multiple sclerosis].

Multiple sclerosis (MS) is a disorder of the nervous system in which myelin breakdown is a prominent feature. We report a 65-yr-old woman scheduled for femoral neck prosthetic replacement under sevoflurane anesthesia, who had been suffering from a progressive type of MS for 27 years. Previously she had a history of exacerbation of symptoms after caudal block done for sciatic neuralgia. Stress such as surgery, anesthesia, emotional distress and variable body temperature can exacerbate the disease. Spinal and epidural block are relatively contraindicated in MS. Thiopental was reported to worsen MS. But there is no evidence that any of the volatile anesthetic agents has a deleterious effect on MS. Sevoflurane can be used to induce anesthesia quickly. To avoid thiopental, we chose slow induction with sevoflurane (GOS). Anesthesia was maintained with GOS. It seems that intraoperative hypotension which resists to ephedrine administration is partly due to autonomic abnormality. Yet, there is no report of sevoflurane anesthesia in MS patient. We consider that in this case sevoflurane is a safe anesthetic because of its less effect on postopreative neurological symptoms than that of epidural block.

[A case report of innominate and carotid artery aneurysms causing cerebral embolism].

Brachiocephalic aneurysms are relatively rare in Japan. A case with tandem aneurysms of innominate and right carotid artery was experienced and its successful surgical treatment was reported. A 54-year-old female patient had an attack of cerebral infarction with left hemiparesis. Computed tomography and angiogram revealed aneurysms of innominate and right carotid artery causing cerebral embolisms. Aneurysmectomy and replacement with Dacron prosthesis was performed. A temporary shunt was utilized between ascending aorta and common carotid artery during clamping carotid artery. She had no problems after operation, and her postoperative course was uneventful.
