Familial occurrence of oral cancer: report of cases.

Four cases of familial occurrence of oral cancer are described and the postulated causes and predisposing factors of oral cancer are reviewed. None of these factors could satisfactorily explain the unusual familial occurrence observed in the cases presented.

Mandibular ameloblastoma associated with salivary gland tumor.

2 cases of mandibular ameloblastoma associated with salivary gland tumor are presented. Case no. 1, a 32-year-old male was diagnosed as mandibular ameloblastoma and acinic cell tumor of the submandibular gland, both of which were found on the left side. Case no. 2, a 62-year-old male was diagnosed as recurrent ameloblastoma of the mandible and adenolymphoma adjacent to a lower pole of the parotid gland.

Cervical lymph node metastasis in carcinoma of the tongue. Correlation between clinical and histopathological findings and metastasis.

The factors relating to lymph node metastasis occurring in association with carcinoma of the tongue have been investigated clinically and histopathologically by many authors. Nevertheless, accuracy in predicting lymph node metastasis has not yet been attained. The present study describes the results of analysis performed in an attempt to predict lymph node metastasis of carcinoma of the tongue. Ninety-eight patients with squamous cell carcinoma of the tongue who were treated at our institution were investigated. Among the 98 patients, lymph node metastases were histologically proven after neck dissection in 40 patients. For statistical analysis in the present study, the quantification theory Type II of Hayashi (1954) was employed for discriminatory analysis. According to this discriminatory analysis, the correlations between the clinical (age, tumour size, N-category) and histopathological (degree of keratinization, mitoses, structure, mode of invasion) findings and metastasis were assessed on the basis of the partial correlation coefficients; these coefficients for each finding were not greatly different from each other and 74 out of 98 patients (76%) could be differentiated between patients with metastasis and without metastasis accurately. The prediction of lymph node metastasis by such discriminatory analysis had significant validity.

[Effects of high-dose medroxyprogesterone acetate in advanced breast cancer, with special reference to prior endocrine therapy and hormone receptors].

Forty-six patients with advanced breast cancer were treated orally with high-dose medroxyprogesterone acetate (MPA), and a 28% (2 CRs, 11 PRs) response rate was obtained. The patient groups that showed a favorable response to MPA were as follows: patients in a postmenopausal state, with soft tissue or bone metastases, with slow-growing tumors, without prior therapy or with good response to prior endocrine therapy, and with positive estrogen (ER)- and/or progesterone (PgR) receptors. Six of 10 patients who had responded to prior endocrine therapy responded to MPA, while only 2 of 23 who had not responded to prior endocrine therapy showed a good response to MPA. There was a significant difference in response to MPA between patients with positive ER or PgR and those with negative ER or PgR tumors. The ER detected in primary or initially relapsed tumors correlated well with the effects of MPA given as a 2nd-line endocrine therapy for advanced breast cancer. The possible roles of MPA in the treatment of advanced breast cancer were discussed.

[A case of spontaneous superficial temporal artery aneurysm].

A rare case of a 14-year-old boy with a spontaneous (true) aneurysm of the superficial temporal artery is presented. The mass in the right parietal region was pulsatile and gradually enlarged in size for the last several months without any notable history of head trauma. The mass was successfully removed by surgery and has proved histopathologically to be a true aneurysm. While almost all cases of the superficial temporal artery aneurysm reported are traumatic ones, the true aneurysm of this artery which is verified by angiography is extremely rare. The pathology, etiology and treatment of this true aneurysm of the superficial temporal artery were discussed.

[Pulmonary tuberculosis in patients with oral cancer].

During an 11 year period, from 1976 to 1986, 33 (10.1%) out of 328 patients with oral cancer treated at our clinic had either a scarred pulmonary tuberculosis (TB) or active TB. Four of these 33 patients developed active TB. In two of these cases TB occurred before the therapy of oral cancer commenced and in the other two, after the operation for oral cancer. In one of the 4 cases, a recrudescence of the TB was suspected. Five cases that were suspected of having TB on analysis of their chest X-rays were administered prophylactic anti-tuberculous chemotherapy, after which no recrudescence or onset of TB.

Hemorrhagic cyst of the mandible associated with a retained root apex of the lower third molar.

An unusual case of hemorrhagic cyst of the mandible, associated with a retained root of the lower right third molar, is reported. The cyst was revealed by routine radiologic examination as part of treatment of the lower right second molar. Radiologically, a faintly outlined cavity with radiopaque mass was found in the third molar region. A final diagnosis of this condition was made after surgical intervention and histologic examination. A nonepithelial lining cavity was found, and the radiopaque mass was identified as a root apex.

[Hormone-dependency of human breast cancer in in vitro clonogenic assay].

The in vitro clonogenic assay has been used to assess the biological effects of estradiol-17 beta (E 2) and an antiestrogen, tamoxifen (TAM) on human breast cancer, and the results were compared with the estrogen receptor status (ER). The sera used were treated with dextran-coated charcoal to avoid the modification effects of the endogenous hormones. There was a significant difference between ER (+) and ER (-) breast cancers in the incidence of cases in which the plating efficiency of cancers increased more than 50% over those of untreated control by adding 10(-8) M of E 2 (40.2% vs. 21.3%). TAM, 10(-6) M revealed a less than 50% colony formation of the control in 30.5% of ER (+) cancers, whereas only 13.0% of ER (-) cancers showed a response to TAM.

Effect of retinoic acid on in vitro proliferation activity and glycosaminoglycan synthesis of mesenchymal cells from palatal shelves of mouse fetuses.

To clarify the mechanism by which retinoid causes cleft palate, we investigated the effect of retinoic acid (RA) on proliferation activity and glycosaminoglycan (GAG) synthesis in mouse fetuses palatal mesenchymal (MFPM) cells. MFPM cells were incubated for 1-11 days with various concentrations of RA to examine its effect on growth rate. Also, confluent cultures were incubated with [3H]glucosamine or [35S]sulfate in the presence of various concentrations of RA to investigate the effect of RA on GAG synthesis. RA remarkably inhibited the growth of MFPM cells in a dose-dependent manner. RA also inhibited the synthesis of GAGs, with sulfated GAGs being more severely affected than hyaluronic acid. These data suggest that the inhibition of proliferation activity and GAG synthesis of palatal mesenchymal cells might be involved in the induction of cleft palate by retinoic acid.

[Clinicopathological study of 9 cases presenting axillary masses only].

A clinicopathological study of 9 cases presenting axillary mass alone was reported. Histological diagnosis revealed 3 occult breast cancers, 2 apocrine gland adenocarcinomas of the skin, a carcinoma and a fibroadenoma in the axillary accessory breast, a neurogenic tumor in the axilla, and a benign lymphadenopathy. There were no clinical characteristics helpful in preoperative differential diagnosis. As the axilla includes the main lymphatic routes from the breast, the axillary tail of the breast and accessory breast, breast-associated tumors should be kept in mind in the management of patients with axillary mass as a sole clinical feature.

Posterior lingual mandibular bone depression.

An unusual case of posterior lingual mandibular bone depression in a 39-year-old Japanese man is reported. The bilocular type of this condition was composed of 2 large ovoid radiolucent areas which were adjacent to each other. The diagnosis of this condition was made by sialography and tomography. The present case was compared with other unusual cases reported in the Japanese literature.

Congenital parotid gland fistula.

A rare case of congenital parotid gland fistula is reported. A 4-year-old boy presented with a fistula opening at the facial surface of the skin posterior to the left commissure of the lips. This fistula originated from an accessory parotid gland with a duct entirely distinct from the original Stensen's duct. Because of the salivary outflow from the fistula opening, the opening was translocated to the oral cavity using Delore's method.
