[Right-to-left shunt in the 1st month of life].

Pre- and post-ductal paO2-values of 116 newborn babies were measured under hyperoxic conditions (FiO2 = 1.0). Total and ductal right-to-left shunts were calculated according to Berggren (1942) and Gersony et al. (1972), respectively. Total shunt volumes amount to 17.4 +/- 5.9 (9--28)% of the aortic minute output in healthy term newborn babies, to 25.0 +/- 5.3 (19--36)% in healthy pre-term newborns and to 13.5 +/- 4.7 (4--19)% on the first day of life in healthy newborn babies from 4. to 28. day of life emphasising the significance of the decline of the arterial oxygen content during the neonatal adaptation period. This is still more evident in patients with disturbed adaptation such as asphyctic mature (28.0 +/- 7.5 (16--45)%) or hypoxic pre-term newborn babies (31.4 +/- 12 0 (12--56)%). As to be expected, the largest total shunt (up to 80% of aortic minute output) is found in newborn babies suffering from severe cyanotic cardiopathies, especially transposition of the great arteries. The incidence of a detectable ductal right-to-left shunt is low both in healthy and asphyctic infants.

The "Internal Leak" as a possible cause in the pathogenesis of peptic ulcer.

In cat gastric mucosa stimulated with histamine and damaged by direct application of acetylsalicylic acid, the fluorescent dye acridine orange may be found outside the parietal cell. Normally it is distributed throughout, or bound to the limiting membrane of, the vesicles in the parietal cell. The special properties of this cationic dye in an acid environment, support the hypothesis that a so-called "internal leak" may possibly play an important role in the pathogenesis of peptic ulcer.

[Amyloidosis of the intestinal tract (author's transl)].

Amyloidosis may afflict different organs of the body and thus cause a variety of clinical symptoms. Amyloidosis of the gastro-intestinal tract can mimic several gastric and intestinal disorders, as for instance disturbances of motility, vascular processes and tumors. The amyloid infiltrates the tissue affected, replaces it and thus cause insufficient organ function. The disease usually progresses slowly, in some cases however, acutely life threatening situations may occur. Biopsy of the rectal mucosa will allow to establish the diagnosis; generalized amyloidosis was present in about 80% of the cases analysed. A medical management has to be done.

Eosinophilic cellulitis. (Well's syndrome) in a child.

Granulomatous dermatitis with eosinophilia (Well's syndrome) occurred in an 11-year-old boy. To our knowledge, eosinophilic cellulitis in a child in whom atrophic alopecia of the affect scalp later develops during the regression phase has not previously been reported. No notable effect of steroid therapy was observed. The cause of eosinophilic cellulitis is still unknown, but the presence of disseminated fibrinoid necrosis, vasculitis, eosinophilia, and a positive antinuclear factor test result might indicate an autoimmune or allergic disease.

Treatment of dyskeratosis follicularis Darier with the retinoic acid derivative Ro 10-9359 (Tigason).

The results of treatment of 29 patients with Darier's disease with the aromatic retinoid Ro 10-9359 are reported. Therapeutic tests for a period of more than 18 months seem to justify its use in Darier's disease. With a modest dose it is possible to keep the symptoms at an acceptable level, i. e., 50% clearing or more without too troublesome side effects. Because of the side effects the dosage must be fixed individually. It is pointed out that with the development of Ro 10-9359, an efficient remedy for Darier's disease has become available for the first time.

[Antithrombotic prophylaxis of the complications of arteriosclerosis obliterans].

The clinical efficacy of antithrombolytic drugs in different combinations was studied on a large contingent of patients with cerebral, coronary atherosclerosis and obliterating atherosclerosis of the vessels. It was shown that regular administration of acetylsalicylic acid with coumarin derivatives prolongs life of patients with cardiovascular diseases and decreases the number of vascular catastrophes.

Extracellular recording of localized electrical activity in denervated frog slow muscle fibres.

Pyriformis muscles of Rana temporaria were denervated by cutting the sciatic nerve in the pelvis. Slow muscle fibres were depolarized with intracellular current pulses, and the electrical activity was recorded simultaneously with intracellular and extracellular recording electrodes. When the extracellular electrode was moved along the fibre surface, outward and inward currents of variable amplitude were recorded. Inward currents coincided with the fast rising phase of the intracellularly recorded action potential; up to four inward current peaks could be detected in single fibres investigated over 3.4--8 mm of their length. The distance between inward current peaks was generally 1--2 mm, but greater distances were also observed. Composite action potentials could be shown to be due to inward currents arising in separate areas of the slow fibre membrane. It is concluded that after denervation Na-channels are incorporated into spatially limited areas of the membrane of slow muscle fibres.

[Immunization against Capillaria hepatica: the effects of primary infections, x-irradiated stages, non-embryonated eggs, and soluble egg extracts (author's transl)].

The influence of primary infections with embryonated infective eggs or with X-irradiated infective eggs, and of non-embryonated eggs, and egg homogenate extracts on challenge infections with Capillaria hepatica was investigated. The worm reproductivity was significantly suppressed in a sublethal challenge infection given 11 days after a primary infection of Mastomys natalensis with 50, 150, 400, and 800 eggs per animal. The administration of 600 X-irradiated (2.2 Krd) embryonated eggs 36 days before challenge as well as an intraperitoneal injection of non-embryonated eggs 12, 10, 8, 6, 4, and 2 days before challenge (simulating the egg production of a normal infection) also reduced significantly the egg production of a weak (50 eggs/ animal) infection. No effect was observed on a moderate challenge (300 eggs/animal). The effect was not markedly enhanced by the repeated administration of X-irradiated eggs or by the combination of X-irradiated infective eggs and non-embryonated eggs. Immunization of mice with soluble egg extracts resulted in significant reduction of egg production determined 60 days after challenge. Two hundred and thirty eggs of C. hepatica/g body weight proved to be a lethal infection dose for M. natalensis. The animals died between 20 and 35 days after infection. After single infections with 50, 150, 400, or 800 eggs per animal the mortality of Mastomys challenged 36 or 52 days later was reduced to 0--30%. Using X-irradiated embryonated eggs for immunization only repeated administration led to protection in 70 to 80% of the animals. About 40% of the animals could be protected by the intraperitoneal injection of non-embryonated eggs. If death occurred it was delayed. The combination of X-irradiated stages and eggs did not enhance the protection.

[Follow-up results of Dega's acetabuloplasty (author's transl)].

From 1966 to 1974 231 acetabuloplasties were performed in 193 children. Dega's technique was not consequently observed in these operations. The follow-up was made in 166 children with 202 acetabuloplasties. 65 per cent of the operated hips were found within the normal range. The clinical examination revealed excellent and good results in 82 per cent. There was a relatively great number of failures due to technical problems, mainly in the first time. The results obtained allow the conclusion that Dega's acetabuloplasty is a valuable method in the operative treatment of the congenital hip in early childhood provided that it is performed with a good operative technique. Because of the possible failures we prefer now Pemberton's pericapsular osteotomy of the ilium.

Petechial reaction following patch testing with cobalt.

A total of 132 patch tested patients reacted with petechial reactions to cobalt chloride 1% in petrolatum; 23 were retested with various concentrations of cobalt. In about 60% of those retested the petechial reaction could be reproduced. Histopathological examination showed slight perivascular lymphocytic infiltration, swollen endothelium and extravasation of erythrocytes but no signs of vasculitis. It is suggested that in predisposed patients the petechial reaction following patch testing could be the result of primary irritation.

The fine structure of colonic submucosal nerves in patients with chronic laxative abuse.

Ultrastructural studies of colonic biopsies from patients with a history of long-term laxative abuse, predominantly with stimulant cathartics such as anthraquinone derivatives or bisacodyl, indicated that submucosal nerve fibres may be severely damaged in relation to dosage and time of addiction. The main pathological features found were ballooning of axons, reduction of nerve-specific cell organelles, lysosomal activity, and increase of melanin-loaded macrophages. Morphometric analysis revealed a significant increase (P less than 0.01) in axonal area, with simultaneous reduction of neurotubules. Nerve endings showed a significant decrease of neurosecretory granules when compared with those of normals. It is concluded that these alterations might be a morphological correlation to the clinically evident disturbance of gut motility in patients with chronic laxative abuse, because the intact enteric plexus system is a prerequisite for coordination of normal gut peristalsis.

Combination chemotherapy in the tumour stage of mycosis fungoides with cyclophosphamide, vincristine, vp-16, adriamycin and prednisolone (cop, chop, cavop): a report from the Scandinavian mycosis fungoides study group.

Combination chemotherapy with cyclophosphamide, vincristine, VP-16, adriamycin and prednisolone was given in 9 cases of mycosis fungoides in the tumor stage; 3 of these patients received COP, one received CHOP, and 5 CAVOP. Complete remission was obtained in one case and partial remission in 5, the response rate thus being 6/9. It was impossible, however, to maintain remission. The treatment was reduced or withdrawn owing to toxic effects in 4 cases. These forms of combination chemotherapy are beneficial in non-Hodgkin lymphomas but do not seem to be of value in the tumour stage of mycosis fungoides.
