[The corrective phase of periodontal therapy].

After pointing to the hygienizing phase which must in any case precede, the author deals with the medicinal local and general treatment, the occlusal function diagnosis, the function analysis, and the fundamentals of the grinding therapy. Furthermore, the temporary splinting, the treatment of secondary malpositions and problems as well as methods of periodontal surgery are mentioned.

Excretion of pterins in phenylketonuria and phenylketonuria variants.

Total urinary biopterin (B), neopterin (Ne) and monapterin (M) were measured in 25 healthy newborns, children and adults, in 49 patients with phenylketonuria (PKU) assumed to be deficient in phenylalanine-4-hydroxylase (PH), in 7 patients with dihydrobiopterin synthetase (DHBS) deficiency and in 4 patients with dihydropteridine reductase (DHPR) deficiency. Excretion of Ne based on creatinine (Ne/C) was 6.6 times higher in healthy newborns than in adults, suggesting a slow maturation of DHBS activity. Newborns excreted more Ne than B and adults more B than Ne (32 and 72% B of the sum of B + Ne, respectively). In all cases, excretion of M was 4-15% of that of Ne. PH deficient patients excreted more B and Ne than healthy controls and again, newborns more than older children. In individual patients, excretion of pterins correlated with phenylalanine (Phe) concentration in plasma; plasma Phe of different patients did not correlate well with excretion of pterins. In PKU variants with deficiency of tetrahydrobiopterin (BH4), extreme pterin patterns were observed: in DHBS- and DHPR-deficient patients, less than 3.5 and more than 81% B were found, respectively. All 30 samples from these patients investigated could be distinguished from those of PH-deficient patients and controls by a two-dimensional plot of % B versus B/C. Thus it seems likely that PKU variants due to BH4 deficiency could be detected early and differentiated by measurement of urinary B, Ne and C. This was exemplified already in one case. - In urine of patients with DHBS deficiency, high concentrations of 3'-hydroxysepiapterin were found in addition to Ne.

[Sound signals in the diagnosis of internal knee disorders].

In the light of own examinations the problem of the diagnosis of internal knee derangements by utilizing sound emissions from the knee-joint is presented. By means of a ECG recorder with a phono device acoustic emissions from 92 joints have been recorded under the restrictions of such a device. A following comparison with the intra-operative findings gave first hints at connections.

Cyclophosphamide-resistant Yoshida ascites tumor cells and their cross resistance to some alkylating agents.

A cyclophosphamide-resistant subline of a Yoshida ascites tumor was developed by giving increasing doses of the drug after transplantation. The effect of several alkylating agents on the cell proliferation of both the sensitive and resistance cell line was compared establishind dose response curves and D50 values. The developed subline revealed a 260 fold resistance to cyclophosphamide. It was completely cross-resistant to hydroperoxycyclophosphamide, whereas for triaziquone, N-oxide-mustard, and N-methyl-mustard only a partial cross resistance existed. These results give further evidence that cyclophosphamide and hydropeoxycyclophosphamide have the same mechanism of action. Regarding the other alkylating agents the results demonstrate differences concerning either the molecular mode of action or protecting effects. A decreased activation or uptake of substance is probably not the base for resistance.

[Circulating immune complexes of patients with malignant lymphomas (author's transl)].

Using the 125J-Clq deviation method according to Sobel, the polyethylenglycol precipitation test of Nydegger, and a Sepharose 6B column chromatography in a modification of MacLennan's method, circulating immune complexes (CIC) have been detected in 20--27% of patients with Hodgkin's lymphoma, in 8% of patients with non-Hodgkin lymphoma of low, and in 20--40% with high malignancy. CIC appeared in advanced stages of the diseases and correlated well with the activity of the disease. CIC disappeared in remissions and reappeared during reactivation of the disease. CIC were only found in patients with general symptoms, i.e. B-symptoms. The pathogenetic role of CIC in the initiation of B-symptoms and in tumor spread is still not fully explained. The antibody part of the CIC is IgG, the antigenic component has not yet been characterized.

[Operative treatment of vesico-vaginal fistulae in the University Hospital, Lomé (Togo, West Africa) (author's transl)].

This publication draws attention to the importance of knowing about the existence of vesico-vaginal fistulae with or without disturbance of normal sphincteric control of the bladder. The repair procedure and avoidance of urinary incontinence after successful anatomical closure of the fistulae depends on the preoperative assessment of sphincteric function. A classification is presented in schematic form of the various types of vesico-vaginal fistulae with the appropriate surgical method of choice, according to our experience in this field, to obtain optimum functional results. In particular, our own operative procedure for substitution of absent sphincteric bladder control is detailed and discussed. Finally, attention is drawn to the balloon catheter test and the possibility of obtaining improvement in the surgical management of vesico-vaginal fistulae by a combination and modification of established operative procedures.

[Long-term course of aortic arch syndrome caused by arteriosclerosis].

The course of the extracranially conditioned cerebrovascular disease is characterized by a worse prognosis in comparison with the average population of the GDR (11 years older) and by increased cardiovascular complication rate (every fifth patient suffered a myocardial infarction, every fourth patient suffered an apoplexy). Younger age of manifestation and increasing number of concomitant diseases form the basis of a course which is altogether endangered by complications, which, however, compared with peripheral and coronary arteriosclerosis, is more favourable. Operative reconstruction of the path of a vessel leads to an enduring improvement of the picture of the complaint. The total prognosis corresponds to the so-called spontaneous course. Under permanent treatment with anticoagulants a retardation of the general progressing of arteriosclerosis and improvement of the survival rates is to be proved.
