Experimental Trypanosoma (Nannomonas) congolense infection in Microtus montanus.

Seventy-two voles (Microtus montanus) were infected with a strain of Trypanosoma congolense of low virulence in mice to study their susceptibility and value as a tool for testing chemotherapeutic agents. The infection in Microtus was highly virulent. Parasitaemia was low to moderate. Significant spleen and body weight changes were observed. Blood capillaries of the heart, brain, kidneys and lungs were markedly dilated and numerous parasites were present in the dilatations, particularly in the heart. There was a generalized serous inflammation, often associated with histiocytic interstitial lesions (except in the brain), particularly in the heart. Treatment of voles with diamidines on cumulative mortality day 60% gave a complete cure.

A randomized trial on the D-homosteroid domoprednate (Ro 12-7024) in the treatment of dermatoses.

39 patients with bilateral dermatoses, mainly psoriasis vulgaris and atopic dermatitis, were treated with 0.1% Ro 12-7024 ointment and 0.1% betamethasone valerate ointment according to a double-blind, right-left, randomized design. Treatment lasted up to 4 weeks. 5 patients did not complete the trial. Assessment of efficacy, expressed as degree of healing in percent of treated skin area and according to an overall assessment of efficacy made by both physician and patient only revealed marginal, mostly statistically insignificant differences, with a trend in favor of betamethasone. Patients' preferences for one of the two treatments favored betamethasone valerate in 17 cases and Ro 12-7024 in 5 cases; 13 cases were ties (p less than 0.001). With the exception of one case of bilateral erythema and itching no side effects were reported. The efficacy of the D-homosteroid Ro 12-7024 is evidently of the same order as that of the group III steroid betamethasone valerate, and the tolerance of the two ointments is good and equal.

Therapy and monitoring of hypersiderosis in chronic renal insufficiency.

In RDT hemosiderosis appears to be an inevitable complication only in the small number of patients in need of frequent transfusions. To prevent clinical consequences (e.g. cardiomyopathy) known from polytransfused patients without renal disease, transplantation should be considered in RDT patients in need of frequent transfusions. Iron substitution - preferably oral - to replace dialysis-related iron loss does not cause clinically significant hemosiderosis provided iron stores are monitored adequately. A sufficient method of controlling iron stores in RDT patients under iron substitution or regular transfusion therapy is a twice annual determination of serum ferritin concentration. The treatment of choice for hemosiderosis in nontransfused RDT patients is discontinuation of iron substitution. When polytransfused RDT patients with severe hemosiderosis cannot be transplanted and submitted consecutively to phlebotomy, DFO treatment is indicated. Quantitative data regarding optimal dosage and application of DFO in RDT patients are not yet available. Constant infusion of DFO during hemodialysis may be superior to bolus application.

Meclosorb, a new topical antibiotic agent in the treatment of acne vulgaris: a double-blind clinical study.

The clinical effect on acne vulgaris of topical treatment with meclocycline sulfosalicylate and systemic treatment with peroral tetracycline (500 mg daily) was compared in a double-blind study of 60 patients treated for 8 weeks. The reducing effect of Meclosorb cream and tetracycline tablets on the number of closed comedones, pustules, papules and cysts was marked and not significantly different. The effect of Meclosorb on open comedones was weak and of slow onset. No side effects were registered. Topical treatment with Meclosorb is an effective and safe alternative to systemic tetracycline treatment of acne vulgaris.

Capacity of different cell types to stimulate cytotoxic T lymphocyte precursor cells in the presence of interleukin 2.

Plastic-adherent cells enriched for dendritic cells (AC) were found to be among the most potent stimulator cells for the activation of cytotoxic T lymphocytes (CTL) in vitro in the presence of interleukin 2 (IL 2) and a constant second set of allogeneic stimulator cells. Concanavalin A-activated nylon wool-nonadherent spleen cells ( CNWT ), concanavalin A-activated unfractionated spleen cells ( Cspl ), and some variants of the ESb T lymphoma line were equally effective as stimulator cells, however, and provoked a substantial cytotoxic response at concentrations of 10(4) cells per culture or less. In contrast, nonactivated nylon wool-nonadherent spleen cells ( NWT ) or unfractionated spleen cells (Spl) and cells of the P815 mastocytoma, the Meth A fibrosarcoma, and the T cell lymphomas Ly 5178 Eb and ESb did not stimulate cytotoxic responses at these cell concentrations. The strong stimulatory potential of the Cspl preparation was reduced by treatment with anti-Thy-1 antibody plus complement, whereas the stimulatory activity of the AC preparation was resistant to this treatment. All cell types tested expressed class I major histocompatibility antigens. Nonactivated NWT cells, in contrast to the CNWT preparation, showed no detectable staining with anti-I-E or anti-I-A antibodies and also a slightly weaker staining with class I antisera. Experiments with the tumor cell lines revealed, however, that there was no strict correlation between stimulatory potential and density of class I alloantigens or the expression of I-E determinants. Experiments on primary cytotoxic responses in vivo gave similar results. Experiments in cultures with a single set of stimulator cells and I region-compatible responder cells indicated that AC and Cspl or CNWT also have a markedly stronger capacity than NWT to induce IL 2-dependent DNA synthesis.

[Arndt-Gottron scleromyxedema and associated phenomena].

Scleromyxedema is an uncommon fibromucinous connective tissue disease characterized by accumulation of mucinous material in the dermis. A monoclonal paraprotein is regularly identified. A review of the literature (57 cases) shows the exceptional association of scleromyxedema with multiple myeloma (8.7%) and macroglobulinemia Waldenström (3.5%). A man with scleromyxedema, IgG lambda paraproteinemia, and sclerodactylia--as a special sign of scleromyxedema--is reported. Melphalan is the drug of choice in serious cases, but not effective in sclerodactylia.

[Lipophilic naphthols and 1,4-naphthoquinones as inhibitors of prostaglandin synthesis. 6. Study of 1,4-naphthoquinones].

A number of plumgagin homologues (2-alkyl-1,4-naphthoquinones) and their 3-methyl derivatives are synthesized to enhance the inhibition of the prostaglandin synthetase (PGS)-activity by the natural compound plumbagin and to minimize its toxicity. The inhibition of the enzyme activity by the new naphthoquinones is only weak but their naphthol precursors are strong PGS-inhibitors. The mechanism of this enzyme interaction by the lipophilic naphthol derivatives is discussed in context with their singlet oxygen (1O2) reactive properties.

Idiopathic de Toni-Debré-Fanconi syndrome with absence of proximal tubular brush border.

In a girl with idiopathic de Toni-Debré-Fanconi syndrome associated with psychomotor retardation, severe renal tubular dysfunction was observed from the first day of life. At the age of 21/2 and 4 years the glomerular filtration rate (GFR) was only 60 ml/min/1.73 m2. No tubular transport of glucose, phosphate, paraaminohippurate and amino acids could be demonstrated. The tubular handling of uric acid, potassium and calcium, was also disturbed. Renal net acid excretion was zero at a plasma bicarbonate level of 14 mmol/l. Urinary osmolality ranged between 88 and 680 mosmol/kg. During hypotonic saline diuresis GFR decreased further; a GFR of 19 ml/min/1.73 m2 was accompanied by a fractional distal sodium delivery of 96.5% and a fractional free water clearance of 73%. In a renal biopsy specimen the proximal tubular cells showed variations in height with dedifferentiation and a widespread absence of brush border on electron microscopy. This formerly undescribed tubulopathy offers a unique chance to investigate glomerulo-tubular balance, adaptive mechanisms of distal tubular transport and renal metabolism under conditions where an apparently unchanged ultrafiltrate is offered by the proximal tubule to the loop of Henle and to a primarily intact distal tubule.

[Initiation of treatment following screening for phenylketonuria].

The newborn screening for PKU is widely established in the F.R.G since 1969. Apart from the quality of dietary control, the age at starting therapy seems to be of high importance for the normal development of these patients. Therefore, the steps from first recording of an elevated Phe level until the beginning of treatment are listed and both, the optimal and the usual time procedure --that is without errors or mishaps--are described. The data of the PKU-Collaborative Study serve to exemplify these courses of events. Reasons for delay are discussed und suggestions for its avoidance are made.

Treatment modalities of psoriatics over a 6-year period (1975-1981).

For an evaluation of the various treatments given to psoriatics, a total of 243 outpatients, all seen in 1975, were followed during a 6-year period. 18.1% underwent only one sort of treatment, 44.9% two sorts and 18.1% three sorts. The rest underwent four or more treatment modalities. Topical steroids were the most commonly used form of therapy given to 88.5%. Grenz-rays were given in small amounts to 73.3%, most often as a supplement to topical steroids. Methotrexate was given to 12.3%, PUVA to 13.6%, etretinate to 3.3% and UVB to 1.2%. 26.3% were treated as inpatients during the observation period, and finally 7.0% were treated at the Dead Sea.

[Results of radiotherapy in recurrent colorectal tumors].

65 patients are presented who have been irradiated at the Radiologic University Hospital of M unster for recurrences of colorectal carcinomas primarily operated upon. In 54 cases the recurrence was located in the small pelvis, in 6 cases within the anastomotic region. The other patients showed cutaneous manifestations or an infestation of adjacent intestinal segments. Apart from the volume occupied by the tumor, the symptoms were above all marked pains. In 62% (40 patients), these pains could be controlled. 30% (19 patients) reported a considerable subjective improvement and 32% (21 patients) a marked diminution of pains. In 20% (13 patients) no modification was achieved, and 12 patients reported a deterioration of troubles. A prolonged survival time after irradiation of the recurrence was observed in those patients irradiated with a focal dose of more than 40 Gy.

Short-term modulation of glycogen metabolism, glycolysis and gluconeogenesis by physiological oxygen concentrations in hepatocyte cultures.

The influence of different oxygen concentrations (0% to 20%, v/v) on the main pathways of carbohydrate metabolism was studied in rat hepatocyte cultures. Cells resembling the periportal or the perivenous cell type were obtained after 48 h culture under different hormonal conditions; they are referred to as 'periportal' or 'perivenous', respectively. Using radiochemical techniques the metabolic rates of the two cell types were measured between 48 h and 50 h under different oxygen tensions. Standard physiological substrates of 5 mM glucose, 2 mM lactate and endogenous glycogen were used. In cells incubated under 4% O2, mimicking hepatovenous oxygen levels, compared to cells assayed under 13% O2, mimicking arterial levels, glycogen degradation to free glucose and to lactate was faster while glycogen synthesis was slower; glycolysis, from glucose to lactate, was faster and gluconeogenesis, from lactate to glucose, was only slightly slower. Under anoxic conditions glycogen breakdown was maximal and glycogen synthesis minimal; gluconeogenesis was also minimal, but glycolysis was not maximal, it reached its peak rate at 4% O2. CO2-formation increased up to 6% and then stayed essentially constant at higher O2 tensions. Net glycogen metabolism: in 'perivenous' cells net glycogen synthesis was observed above, net glycogen degradation below 4% O2. In 'periportal' cells, which had a very low glycogen content, net glycogen metabolism was very small. Net glucose metabolism: in 'perivenous' cells net glucose formation was only seen under anoxic conditions. Net glucose utilization was observed at about the same rate under all physiological O2 tensions. In 'periportal' cells net glucose formation increased clearly up to 6% O2 and then remained almost constant. Net lactate metabolism: in 'perivenous' cells net lactate formation occurred below, and net lactate utilization above, 6% O2. In 'periportal' cells a pronounced net lactate utilization was observed under all physiological O2 tensions. Net flow between glucose-6-phosphate and pyruvate was observed in the glycolytic direction in 'perivenous' and in the gluconeogenic direction in 'periportal' cells except under anoxic conditions. When O2 tensions were lowered, the percentage of 'futile cycling' was decreased in the 'perivenous', glycolytic hepatocytes; conversely, it was increased in the 'periportal', gluconeogenic hepatocytes. It is concluded that physiological oxygen concentrations modulate hepatic carbohydrate metabolism and that they contribute further to the proposed metabolic differences between periportal and perivenous cells in vivo.
