Fusidic acid plus betamethasone in infected or potentially infected eczema.

Eighty-one patients with atopic dermatitis and contact dermatitis received a 1-week treatment with 0.1% betamethasone alone on one side and 0.1% betamethasone plus 2% fusidic acid on the other side. Clinically, the combination was marginally superior and in patient preference a significant difference in favour of the combination was recorded. Bacteriologically, the combination eliminated 67% of bacteria originally present in the skin lesions compared with 51% with steroid alone.

Suppression of cytotoxic T lymphocyte activation by L-ornithine.

The effect of L-ornithine on several types of immune reactions was analyzed. L-ornithine was found to suppress the activation of cytotoxic T lymphocytes (CTL) in vivo and in vitro. This suppressive effect was not observed with the structural analogues D-ornithine, L-lysine, or putrescine or with the amino acids L-histidine or L-alanine. The concentration of 9 X 10(-3) M L-ornithine was found to mediate a practically complete suppression of the cytotoxic response in vitro if applied on day 0 or day 1 of the culture, but a comparably weak suppression if applied on day 3. The same concentration of L-ornithine had no effect on the production of the lymphokines interleukin 2 (IL 2) and gamma-interferon (IFN-gamma). This concentration of ornithine had also no substantial effect on several types of proliferative responses, including the allogeneic mixed lymphocyte reaction, the concanavalin A-activated IL 2-dependent proliferation of thymocytes, and IL 2-dependent proliferation of the T cell clone W-2. These observations suggest that L-ornithine inhibits selectively the differentiation of CTL effector cells. By the criteria tested, the immunosuppressive effect of L-ornithine is more selective than that of cyclosporine A, which was previously found to suppress not only the activation of cytotoxic activity but also proliferative responses and the production of the lymphokines IL 2 and IFN-gamma.

[Behavior of phenoloxidase-containing cells in experimental granuloma].

Using the histochemical method for phenoloxidase (PO) in albino rats, the development of carrageenan granuloma was investigated. It is shown that especially those cells containing the methanol resistent PO activity are the cells which were be found during the formation of the granuloma. These findings support our earlier suggestion that POC are involved in the defence reactions.

Desferrioxamine treatment for aluminium and iron overload in uraemic patients by haemodialysis or haemofiltration.

In an attempt to quantify iron and aluminium removal in patients following desferrioxamine therapy, a comparative study of haemodialysis and haemofiltration was performed in five patients. Haemofiltration was found to be the more efficient treatment especially if iron and aluminium clearances were related to urea clearances. The distribution volumes of chelated iron and aluminium at the low clearance rates obtained, behave as a single compartment which amounts to about 40 per cent of body weight.

[Pretherapeutic computer tomographic studies and follow-up of patients with beta-irradiated choroid melanomas].

CT examinations before and after radiotherapy were performed in 48 patients with choroidal melanomas who were submitted for a bulb-preserving irradiation with ruthenium-106. The authors wanted to investigate to which extent further, especially pretherapeutic informations are obtained by computed tomography, if performed in addition to well-known ophthalmologic examinations (ophthalmoscopy, fluorescence angiography, sonography). Furthermore the side reactions of therapy induced by the intervention or by the tumor should be visualized. The measurements of the tumor prominence were of special importance for the therapy planning, because they allowed to verify the response of radiotherapy by computed tomography. Our own patients had an average tumor regression of 1 to 2 mm within the first three months after the irradiation with ruthenium-106; after six months the prominence of the melanomas had decreased by 3 to 4 mm on an average. Interesting side effects, also for the ophthalmologist, were acute radiogenic retino-uveitises and parabulbar oedemas found in several patients who showed a marked regression within one or two weeks. Some late effects were scleral scars at the point of fixation of the ruthenium calotte.

[Follow-up studies and special cardiologic diagnosis in patients with suspected heart sarcoidosis].

During a retrospective analysis of 1236 patients with pulmonary sarcoidosis who showed electrocardiographic disturbances or cardiomegaly the existence of a possible heart sarcoidosis was suspected. It was tried to confirm this suspected diagnosis in subsequent examinations of these patients by support of modern non-invasive and invasive cardiological methods. Thereby the differential diagnosis to the coronary heart disease is especially difficult in elderly. This problem represents a permanent diagnostic challenge because of the therapeutic chance in case of heart sarcoidosis.

[Ultrastructure of diabetic autonomic neuropathy of the gastrointestinal tract].

Gastrointestinal motility disorders induced by diabetes mellitus are most commonly manifested in the colon, and are considered to be an expression of diabetic autonomic polyneuropathy. In this study, for the first time, the intrinsic nervous system of the bowel has been subjected to an ultrastructural and morphometric examination in rectal biopsy material obtained from diabetics with and without diabetic enteropathy. In patients with diabetic enteropathy, the nerves of the submucous plexus (Meissner's plexus) revealed significant swelling of the axons, as an expression of neural degeneration. Furthermore, in about a half of these patients, thickening of the basal membrane of Schwann's cells was also detected. Additional, although unspecific, changes found in the diabetics were a quantitative increase in the lysosomes, lipofuscin and glycogen in the Schwann cells. The ultrastructural findings represent evidence for the presence of an intrinsic autonomic polyneuropathy in diabetics with diabetic enteropathy. At the same time, they indicate that the diagnosis of diabetic enteropathy is no longer merely an "exclusion" diagnosis.

Cervicobrachial disorders in certain occupations, with special reference to compression in the thoracic outlet.

One hundred ninety-one workers in three different occupations were examined regarding presence of symptoms from the cervicobrachial region. Eighteen percent of the workers (27% of the female and 11% of the male workers) had symptoms of compression in the thoracic outlet. However, only 2% of all workers had pronounced symptoms of compression in the thoracic outlet. In addition, 27% of the workers (33% of the female and 23% of the male workers) had symptoms of other cervicobrachial disorders. Thus, 45% of the workers had symptoms from the cervicobrachial region. The workers with symptoms of compression in the thoracic outlet were significantly younger than the workers with other symptoms of cervicobrachial disorders, p less than 0.01. No significant correlation was noted between vascular compression and symptoms of compression in the thoracic outlet.

Multiple congenital anomalies/mental retardation (MCA/MR) syndrome due to interstitial deletion 1q.

A severely retarded male infant was found to have a previously undescribed multiple congenital anomalies/mental retardation (MCA/MR) syndrome including microdolichocephaly, prominence of metopic suture, coarse scalp hair, epicanthus, anteverted nostrils, micrognathia, posteriorly angulated malformed auricles, preaxial hexadactyly, clinodactyly, camptodactyly, hypospadias, cryptorchidism, inguinal hernias, agenesis of left kidney, and pyloric stenosis. This syndrome was due to an interstitial del(1)(q25.2q31.2) associated with a paracentric inv(1)(q31.2q44).

Acetylcholine-sensitivity and local regenerative activity in denervated frog slow muscle fibres.

The distribution of acetylcholine (ACh)-sensitive membrane areas was determined in 11-75 days denervated slow muscle fibres of Rana temporaria by iontophoretic application of acetylcholine. The fibres were also stimulated directly, and their electrical activity was recorded with an intra- and an extracellular electrode. During the first two weeks following denervation the size of the ACh-sensitive fibre surface was similar to that of normal slow fibres, but a spreading out of ACh-sensitivity occurred between the 13th and 20th day. The slow fibre membrane did not become homogeneously ACh-sensitive; even after long periods of denervation large local sensitivity gradients could be observed. Throughout the denervation period maximum values of ACh-sensitivity were in the same range as in normal slow fibres. Action potentials were fully developed when ACh-sensitivity started to spread out. Extracellularly recorded inward currents varied in amplitude along the fibre surface, and either one or two peaks were observed in individual fibres. The spatial relationship between inward current peaks and peaks of ACh-sensitivity was investigated in 12 fibres. Fifteen inward current peaks were located at distances of 30-640 microns from points of maximum ACh-sensitivity; only once did the centers of ACh-sensitivity and excitability coincide. It is concluded that Na channels are incorporated into nonjunctional membrane areas of denervated slow fibres; this process precedes the incorporation of ACh receptors by approximately one week.

Transcripts of human HLA gene fragments lacking the 5'-terminal region in transfected mouse cells.

Clones of mouse L cells transfected with a human HLA-B7 gene fragment lacking the 5' segment of exon 2 and all upstream sequences express HLA-specific transcripts of various lengths. These include species that correspond in size to full-length HLA-B7 mRNA. The level of these transcripts is increased in cells treated with interferon. It is probable that the full-length transcripts arise as a result of the linkage of the HLA-B7 gene fragments with DNA segments providing transcription initiation or polyadenylylation signals.
