[Sonography of malignant hemangioendothelioma of the liver].

Malignant haemangioendotheliomas of the liver are very rare mesenchymal tumours. Sonographically, they appear as solitary, although more frequently as multicentric, space-occupying growths with blurred and irregularly delineated outlines; the sonographic image shows both echo-dense structures and structures with a low amount of echoes, or which are even echo-free. Short-term sonographic follow-up examinations point to a tendency to rapid growth and varying echogenicity.

The muscarinic receptor of chick embryo cells: correlation between ligand binding and calcium mobilization.

In this report we characterize muscarinic cholinergic receptor on embryonic cells. We established dose-response curves by fluorometric measurement of Ca2+ mobilization in cell suspensions of whole chick embryos stage 23/24. Ca2+ mobilization was quantitated by standardization of chlorotetracycline (CTC) fluorescence changes after stimulation with muscarinic agonists. We determined ED50 values for the agonists acetylcholine and carbachol as 3.4 X 10(-6) and 2.7 X 10(-5) M, respectively. Pilocarpine and oxotremorine were found to act as reversible competitive antagonists with inhibition constants (Kl) of 5.0 X 10(-6) and 1.4 X 10(-6) M, respectively. Bethanechol, which induced only 23% of the maximal effect obtained by acetylcholine, was a partial agonist with an ED50 of 4.8 X 10(-4) M. Its antagonistic component is expressed by an inhibition constant of 1.9 X 10(-4) M. In parallel, binding studies were performed in a competition assay with [3H]-quinuclidinylbenzilate. For the agonists acetylcholine and carbachol, binding parameters were best fitted by a "two binding-sites model." Comparison with dose-response curves indicated that Ca2+ mobilization was triggered via the high-affinity binding site. The inhibition constants of antagonists derived from the shift of dose-response curves corresponded to the fitted KD values of the binding studies when a "one binding-site model" was applied. Combination of dose-response and binding data showed close proportionality between receptor occupancy and calcium mobilization. No spare receptors were present.

Vaginal discharge and Gardnerella vaginalis. Predisposing factors.

A total of 284 women in the reproductive years with vaginal discharge from which only Gardnerella vaginalis (GV) had been isolated, entered a questionnaire study on the possible causal relation between presence of GV and type of menstrual hygiene, type of contraception, number of deliveries, infertility and number of sexual partners. A control group consisted of 225 women without vaginal discharge. The study showed that patients with GV significantly more frequently used tampons for menstrual hygiene, sought medical assistance for vaginal discharge more often, and had more sexual partners. On the other hand, no difference was found between the two groups as to type of contraception, number of previous deliveries, and unsuccessful attempts of getting pregnant.

Diagnostic value of clinical examination, direct microscopy, and culture in the Gardnerella vaginalis syndrome.

In a general practice 467 women aged 15-49 years with vaginal discharge were consecutively examined in order to compare the diagnostic value in the Gardnerella vaginalis syndrome of the clinical examination, direct microscopy of the vaginal secretion, and culture. We found significant correlations between all relevant combinations of the diagnostic methods. The predictive value of a positive test for the clinical examination, the microscopy, and the clinical examination combined with microscopy, respectively, was 0.75, 0.89 and 0.90, whereas the predictive values of the negative test were 0.59, 0.72 and 0.61, respectively. Because even small numbers of Gardnerella vaginalis may yield growth on selective culture media, the clinical examination and the microscopy are often negative despite positive culture. To avoid overtreatment of a benign condition we therefore conclude that the presence of a characteristic clinical picture and positive microscopy constitute a safe basis for the diagnosis of the Gardnerella vaginalis syndrome, and that the diagnosis should be based on this combination or on the fulfilment of three of the following four criteria: 1) characteristic vaginal secretion; 2) vaginal pH greater than 4.5; 3) positive potassium hydroxide test or characteristic herring-brine smell, and 4) clue cells at microscopy. With the present methods culture for Gardnerella vaginalis should not routinely be performed, until the value of quantitated methods has been proved. Coexistence of other microorganisms rendered the diagnosis difficult. We recommend to treat the most predominant infection first.

Treatment of the Gardnerella vaginalis syndrome. A controlled, double-blind study comparing pivampicillin and metronidazole.

A double-blind, randomized, therapeutic study was carried out in 289 patients with vaginal discharge and growth of Gardnerella vaginalis (GV) and no growth of Neisseria gonorrhoeae, Trichomonas vaginalis or Candida. Treatment consisted of either pivampicillin (Pondocillin), 700 mg twice daily for seven days, or metronidazole, 500 mg twice daily for seven days. The effect was evaluated on the basis of the patient's statement and on the result of culture for GV immediately after end of treatment. Evaluated from the culture results metronidazole was significantly more effective than pivampicillin in eradicating GV. The efficacy of the two medications was 69% and 54%, respectively. The evaluations by the patients in the two groups, on the other hand, did not differ significantly, although there was a tendency to consider metronidazole as the most effective. Metronidazole must be considered the most potent drug today, and we recommend it as the medication of choice, provided that the diagnosis is confirmed by the clinical picture and microscopy and not only by culture.

African trypanosomiasis: treatment-induced invasion of brain and encephalitis.

Histological sections of the brain from Microtus montanus infected with Trypanosoma rhodesiense and treated after the initial infection period showed that trypanosomes promptly invade the brain tissue and induce a severe encephalitis within a very short time. These findings suggest that the brain invasion and encephalitis that occur after treatment in sleeping sickness must be considered an effect of the treatment. The results illustrate that the brain invasion is probably not hematogenic but rather due to a process of parasite migration from the subarachnoidal space over the Virchow-Robin spaces into the brain.

[Electron microscopic determination of phenoloxidase (EC 1.14.18.1) in eosinophilic granulocytes of the small intestine of white rats].

It is shown that the light microscopic permanent detectable phenoloxidase activity, using dihydroxyphenylalanine as substrate, of the small intestine of white rats is localized in the most cases in eosinophilic granulocytes. The enzyme has been found as well in the cytoplasma as in the granules. The enzyme proof triggered in the granules the so called reverse effect. The results allow to conclude new aspects for the cell mediated immunological defense reaction.

Time sequence of osteoinduction and osteostimulation elicited by biologic bone replacement materials.

The combinations of a biologic bone replacement material (BRM), bone gelatine (BG), a matrix extract, with one of the synthetic BRMs, either powderised beta-tricalcium-phosphate (TCP) or particulate hydroxyapatite (HA), as well as BG alone were implanted in muscle pouches and diaphyseal femoral drill-holes in rats. Implantation between one and 21 days. Evaluation by histomorphometry of PMMA embedded undecalcified specimen. In heterotopic implantation bone formation occurred earliest in the BG/TCP combination with highest values for volume density bone and osteoid after 21 days. In orthotopic implantation starting between days 9 and 13 the BG/HA combination stimulated intense woven bone formation with massive ingrowth of vital bone into the HA pores. The biodegradable TCP, by activating the monocyte-/macrophage-system for its degradation, obviously enhanced the already ongoing immunologic reaction to allogeneic BG, thus impeding bone formation and repair.

[Thallium myocardial scintigraphy, hemodynamics and left ventricular wall kinetics in patients with suspected myocardial sarcoidosis].

The cardiological examination of 22 patients (pts) with suspected heart sarcoidosis (histologically established lung sarcoidosis, heart rhythm disturbances, cardiomegaly) has revealed thallium scan defects in 20 pts at rest and during exercise. In 19 pts we found pathologic left heart wall motions established by laevocardiography by applying the half axis method. The number of pathologic half axis shortenings correlated with ejection fraction, but not with left ventricular enddiastolic pressure and left ventricular enddiastolic volume index. IHD could not be found in coronarography in any case. Myocardial biopsy did not show myocarditis. Cardiomyopathies, other specific heart muscle diseases or rheumatic myocarditis could not be excluded as causes of the results mentioned above. The follow-up examinations of the patients will give more detailed information on the etiology of the pathologic cardiac findings.

An epidemic of syphilis among homosexuals and bisexuals.

22 cases of syphilis were diagnosed in the last part of 1982 in the Department of Dermatology and Venereology. Out of them 21 were male cases and 16 were homo- or bisexuals. Most cases were in the secondary stage and Herxheimer reaction only developed in these. Serological titers to both lipoidal and treponemal antigens were extremely high. All bisexual males were positive in herpes and CMV complement fixation test. Two of the homosexuals appeared with iridocyclitis and motile treponemes were isolated from the anterior chamber in one case.
