Malignant tumours of the paranasal sinuses and nasal cavity. A series of 180 patients.

In a series of 180 malignant tumours in the paranasal sinuses and nasal cavity the largest groups comprised 90 epidermoid carcinomas, 25 adenocarcinomas, and 19 adenocystic carcinomas. The epidermoid carcinomas were classified according to various proposed methods. The planned treatment schedule for carcinoma was 57 to 60 Gy in 30 fractions over 42 days, followed 8 to 12 weeks later by radical surgery. In the group of 80 patients with epidermoid carcinomas treated with radical intention, the 5-year corrected survival was 42 per cent. The prognosis has been analysed against a number of parameters, and it is found that sex, tumour size, and histologic score are important but neither totally independent nor evenly distributed parameters.

Testicular germ cell tumours in Denmark 1976-1980. Pathology of 1058 consecutive cases.

In the first five-year period of the Danish Testicular Carcinoma Study (DATECA) 1058 consecutive testicular germ cell tumours were examined. Of these, 554 were seminomas comprising 515 of typical type, 26 anaplastic and 13 spermatocytic; 497 were non-seminomas comprising 145 pure tumours and 352 mixed tumours of various types. Among the various subtypes of non-seminomas embryonal carcinoma (EC) was recorded in 87 per cent, endodermal sinus tumour (yolk sac tumour; EST) in 22 per cent, teratoma (T) in 55 per cent and choriocarcinoma (CC) in 17 per cent. Only very few tumours were pure EST or pure CC. Five tumours were recorded as 'others or uncertain'. The tumours were graded with regard to various histologic features. Moderate and severe necrosis, bleeding, and a large number of mitoses were significantly more frequent in non-seminomas. The presence of tumour tissue at the resection margin was also more frequent in non-seminomas. Tumours with a largest diameter of less than 2.5 cm had already caused metastases in 16 per cent of the seminomas and 29 per cent of the non-seminomas. Increasing size of the tumours was associated with increasing frequency of metastatic disease but this association was not directly proportional. Distribution of the various histologic types according to the stage of disease varied. Thus, 78 per cent of the seminomas presented in stage I while 54 per cent of the non-seminomas had localized disease. Anaplastic seminomas were distributed similarly to the non-seminomas while all spermatocytic seminomas, with one exception, were recorded as stage I. Of non-seminomatous subtypes pure EC was associated with the highest frequency of stage III, followed by mixed tumours containing CC components. Although the present series is large the heterogeneity of germ cell tumours demands further investigation of larger numbers to confirm some of the findings.

The effect of antibiotics on mannose-resistant haemagglutination by K88- and K99-positive Escherichia coli strains.

Five E. coli strains carrying K99 antigen isolated from the intestines of calves which had succumbed to diarrhoea and six K88-positive strains isolated from fatal cases of diarrhoea in piglets were examined for their mannose-resistant haemagglutination (MRHA) capacity against pig erythrocytes. The bovine strains showed a geometric mean MRHA-titre of 1/18 and the porcine strains one of 1/45. Similar experiments were carried out after addition of the following antibiotics in doubling dilutions: ampicillin, chloramphenicol, colistin, dihydrostreptomycin, gentamicin, neomycin, polymyxin B and oxytetracycline. Colistin and polymyxin B had a marked concentration-dependent inhibitory effect on MRHA. Neomycin and gentamicin also inhibited MRHA but to a lesser degree. Chloramphenicol, dihydrostreptomycin and oxytetracycline showed no effect. With ampicillin, a trend was found for the ratio values to be inversely proportional to the concentration. This suggests that this antibiotic has an enhancing effect on the haemagglutination.

Relationship between the occurrence of coliphages and E. coli in Danish marine bathing areas.

During 2 bathing seasons, 1978 and 1979, water samples were collected with intervals of 2 weeks from 17 marine bathing areas situated on the coast of the island of Fyn. Quantitative examinations for coliphages and fecal coliforms were carried out. Coliphages were present in approximately 50% of the samples. 75% of the positive samples contained less than or equal to 10 coliphages per 100 ml. 6 samples were found with greater than 100 coliphages per 100 ml. A positive correlation between coliphage and fecal coliform concentrations was demonstrated. The ratios of fecal coliforms to coliphages varied only little between individual sampling stations. The mean ratio was 1.9 and 2.0 for the 2 sampling periods. The mean concentration of fecal coliforms was gradually increasing from 2.4/100 ml in May to 7.2/100 ml in September. Conversely the coliphage concentrations were quite uniform during the bathing seasons. As a result of those 2 trends, the fecal coliform/coliphages ratio increased during the seasons from 0.86 to 3.13. Finally, the potential value of coliphages as an indicator of pathogenic microorganisms in marine bathing areas is discussed.

Colistin inhibition of mannose-resistant haemagglutination by K88-positive and K99-positive escherichia coli strains. A preliminary report.

Two enteropathogenic E. coli strains isolated from a calf and a piglet succumbed to diarrhoea were studied. The bovine strain carried K99-antigen and the porcine strain was K88-positive. Both strains agglutinated pig erythrocytes in the presence of D-mannose. In the test a bacterial cell density of 3 x 10(9) per ml and doubling dilutions hereof were used. The haemagglutination titres were 16 and 128, respectively. When the bacteria were exposed to colistin before mixing with the red cells, haemagglutination was inhibited completely with 1.0 and 0.5 microgram/mg of colistin. At a colistin concentration of 0.25 microgram/ml (1/4-1/2 of the MIC's) the titres were lowered by a factor of 16-32.

The use of skin flaps in the treatment of malignant melanomas in the head and neck region.

The use of skin flaps in the treatment of malignant melanomas in the head and neck is compared to the application of free skin grafts. In 13 patients the wounds were closed directly, in 58 the defects were covered by free skin grafts, and in 80 patients the defects were covered by flap plasties. The distribution of tumours with various degrees of invasion was similar within the groups. Local recurrences or local skin metastases occurred in 18 patients. Eight were found in the skin graft group. In the group with direct closure only 1 patient had a recurrence. Nine were found in patients treated with skin flaps. The use of skin flaps is often a great advantage from an aesthetic point of view and this survey does not indicate that it is contraindicated for oncologic reasons.

Teratoma orbitae. Report of a case.

A case of orbital teratoma was noted at birth. The neoplasm with a normally developed eye at its apex protruded between distended eyelids from an enlarged but intact orbit. The tumour was successfully extirpated on the day after birth. Histological examination revealed components from all 3 germinal layers and no sign of malignancy.

Eosinophilic cellulitis. (Well's syndrome) in a child.

Granulomatous dermatitis with eosinophilia (Well's syndrome) occurred in an 11-year-old boy. To our knowledge, eosinophilic cellulitis in a child in whom atrophic alopecia of the affect scalp later develops during the regression phase has not previously been reported. No notable effect of steroid therapy was observed. The cause of eosinophilic cellulitis is still unknown, but the presence of disseminated fibrinoid necrosis, vasculitis, eosinophilia, and a positive antinuclear factor test result might indicate an autoimmune or allergic disease.

Petechial reaction following patch testing with cobalt.

A total of 132 patch tested patients reacted with petechial reactions to cobalt chloride 1% in petrolatum; 23 were retested with various concentrations of cobalt. In about 60% of those retested the petechial reaction could be reproduced. Histopathological examination showed slight perivascular lymphocytic infiltration, swollen endothelium and extravasation of erythrocytes but no signs of vasculitis. It is suggested that in predisposed patients the petechial reaction following patch testing could be the result of primary irritation.

Vasocative intestinal polypeptide (VIP)-producing ganglioneuroma in a child with chronic diarrhea.

In a 2 1/2-year-old girl with persistent watery diarrhea a retroperitoneal tumour and lymph node composed of benign ganglioneuroma cells were found. The histological picture was compatible with an original metastasizing neuroblastoma which has passed through a stage of late maturation. Electron microscopy of the ganglioneuroma showed cytological evidence of increased secretory activity. The tumour contained large amounts of vasoactive intestinal polypeptide (VIP) and preoperative plasma concentration of this peptide was increased. Postoperatively the VIP concentration was normalized, and the diarrhea ceased. Analysis of catecholamine metabolites in urine was normal. The findings suggest that the watery diarrhea was due to the benign VIP-producing neural crest tumour.
