[Work of breathing in spontaneous respiration with continuous positive airway pressure].

In postoperative respiratory failure continuous positive airway pressure (CPAP), besides improving pulmonary gas exchange, influences respiratory mechanics. Elastic and flowresistive components of respiratory work were measured in intensive care patients being weaned from the respirator. Measured values during breathing with a continuous-flow CPAP system with elastic reservoir were compared with those for O2 breathing via T-tube. Under CPAP total breathing work and resistance were reduced. Compliance, tidal volume and respiratory frequency did not change significantly. The reduction in respiratory work was primarily due to decreased nonelastic work associated with a decrease in resistance. Thus, on weaning from mechanical ventilation using a reservoir-CPAP system, the respiratory work was reduced in comparison to O2 breathing via T-tube.

Quantification of parasite development in the host-parasite system Biomphalaria glabrata and Schistosoma mansoni.

The visceral mass of Biomphalaria glabrata uninfected or infected with Schistosoma mansoni was serially sectioned. The amount of hepatopancreas tissue and of parasite tissue was quantified. In pool-infected snails the volume of the whole visceral mass increased very significantly until week 6 and then decreased. Due to the growing parasites the volume of the visceral mass in infected snails was at most times higher when compared with uninfected snails of the same dimensions. Two weeks p.i. there was a permanent and significant increase of parasite tissue until week 6. During this time the amount of hepatopancreas tissue still increased. From this time onwards till week 12 the proportion of parasite tissue remained rather constant, indicating a kind of regulation, whereas the hepatopancreas tissue decreased to about one-third of the volume found in uninfected snails of the same shell diameter. Compared with infections by a single miracidium there was a significant increase in parasite tissue after infection with two miracidia. Infections with more miracidia (5, 10, and 20) gave no significant further increase. This also demonstrates a kind of regulation. Mortality rate, growth rate and egg production were studied during an infection period of 12 weeks.

Systolic time intervals before and after haemodialysis.

We investigated changes of haemodynamics during haemodialysis in 107 patients by noninvasive measurement of systolic time intervals. Heart rate and preejection period index increased and systolic and diastolic blood pressure, left ventricular ejection time and isovolumic contraction time decreased. We found, that if the pre-dialysis ejection fraction (EF) was more than 55 per cent, post-dialysis EF decreased by about 10 per cent. However if pre-dialysis EF was less than 55 per cent, post-dialysis EF increased by five per cent. Regarding cardio-thoracic-ratio, Sokolow-Lyon-Index (SLI) and repolarisation disorders we found the most remarkable decrease in EF in patients with normal sized hearts with enlarged SLI and repolarisation disorders.

[Early and late results of internal small intestine splinting for ileus].

The continuous inside splinting of the small intestine was carried out in 82 children and adults, because of postoperative ileus, chronic recurrent ileus and diffuse peritonitis. Possibly the splinting had contributed in one case to the perforation of the paralytic small intestine with a fatal outcome. In another case there occurred a relapse of an ileus. All other patients have no complaints in connexion with the splinting operation. Control examinations take place. A "total folding" of the small intestine was not observed by the radioscopy.

[Surgical treatment of divertikulitis of the sigmoid colon (author's transl)].

32 patients suffering from diverticulitis of the sigmoid colon underwent resection with end-to-end-anastomosis. In 24 cases the operation was performed electively as a one step procedure, the rest of the patients were operated on by two or three steps. One patient with ileus and peritonitis came to death. 28 from 31 surviving patients live without trouble. Resection of the inflammatory part of intestine is recommended instead of permanent colostomy.

[Behavior of various serum enzymes following ligation of the superior mesenteric artery in the rat (preliminary report)].

In rats, shortly after ligation of superior mesenteric artery serum enzyme activities are found significantly altered. Those changes concern aspartate aminotransferase (GOT), alanine aminotransferase (GPT), lipase, alpha amylase, and isocitrate dehydrogenase as well as glutamate dehydrogenase. The causes are discussed. The authors emphasize that the assessment of serum enzymes possibly gives some help in diagnosing acute intestinal ischemias in early stages.

[The effect of pyloric stenosis on the development of stress ulcers in the rat].

The authors examined in rats the importance of pyloric stenosis for the development of spontaneous acute ulcers and stress ulcers in the stomach. Spontaneous superficial lesions arise only during the first 20 days. They apparently signify an intensified narrowing by postoperative swelling with mechanical stimulation of gastrin secretion in the antrum. They were not observed during later periods. Chronic ulcers did not develop. Stress ulcers caused by immobilization were more frequently seen in rats with pyloric narrowing than without. Vagotomy protected the rats with pyloric stenosis from stress ulcer, although not as certain as in control animals.

[Haemodynamic and respiratory changes after lung resection (author's transl)].

After lobectomy a slight increase of pulmonary arterial pressure can already be observed in the state of rest. The resistance of the pulmonary vessels reveals a marked dependance on the date of operation, it increases moderately on strain, but more significantly in the state of rest. At the same time cardiac output is decreased due to the exhaustion of the myocard after straining. Cardiac insufficiency developing postoperatively is due to the reduction of pulmonary hypertension. The reduction of VC and a slight increase of resistance do not cause the arterial PO2 pressure to be lowered under conditions of strain or rest.

[Mediastinoscopy and scar-keloid's in boeck's sarcoid (author's transl)].

The paper represents the results of the mediastinoscopy in 135 cases of Boeck's sarcoid. The frequent formation of keloid's in the scar of the mediastinotomy is emphasized. The histological examinations of the scar keloid's after mediastinoscopy are compared with histological examinations of scars after resections of the thyroid glands. The possible reasons of the differences are discussed.

[Mediastinoscopy in preoperative assessment of operability in patients with bronchial neoplasms (author's transl)].

The results and conclusions of the endoscopic exploration of mediastinum in 352 patients suffering from bronchogenic cancer are analysed. The importance of mediastinoscopy is emphasized especially in the assessment of operability. In the opinion of the authors the cancer is not operable, if metastases are found in the paratracheal and contralateral lymphnodes. Only in young men the extended resection should be done in individual cases if mediastinal lymphnode metastases are detected. Regarding these indications the rate of explorative thoracotomy could be reduced to 10%. The particular importance of preoperative examination of load capacity of the heart and circulatory system is stressed. Finally it is concluded, that an decisive improvement of the common prognosis of the bronchogenic cancer is only possible by an earlier detection and a more rapid introduction of therapy, but not by the extension of the surgical procedures. The enlargement of the surgical procedure should be decided individually. The great possibilities hencefollowing for the mass x-ray examinations are emphasized.

[Complications of mediastinoscopy and their effects on the choice of indication (author's transl)].

Complications of 735 mediastinoscopies are evaluated. In the small number of early complications the bleedings and unjuries of pleura dominate in the described group of patients. At no time a thoracotomy was necessary. The danger of bleeding from an aneurysma of the aorta is pointed out, which is rare but possible early complication. The inoculated metastases do not deteriorate the prognosis of the bronchogenic cancer, if the mediastinoscopy is strictly limited to the paratracheal and tracheobronchial lymphnodes. The observation of a metastasis in the collar cicatrix following three months after mediastinoscopy due to cancer of the oesophagus is described. The low number of observed complications in the opinion of the writers does not justify to limit the range of indications for mediastinoscopy, its diagnostic value is high.

[Role of the antrum in the etiology of stress ulcers in the rat].

In rats, antrectomy results in a decrease of ulcer rate by immobilization without statistic significance. Stress ulcer rate is also lowered by antral vagotomy, with statistically significant decrease of acid production. By antrectomy as well as by antral vagotomy just the acid production but not the circulatory disturbance in the mucosa of corpus is influenced, which is primarily responsible for the development of stress ulcers. This fact gives an explantation for the relatively high ulcer rate, compared with the conduction in rats after truncal vagotomy. Unclear remains whether antrectomy with consecutive total gastrin elimination is merely working by inhibiting the secretion, or if gastrin has a circulatory effect too.
