[Esophageal duplication in a former bronchogenic cyst. Case report and review of the literature].

We report a case of a cystic esophageal duplication with primary cardiac symptoms and bronchogenic cyst in an adult. Duplications of the esophagus are often operated on the diagnosis of mediastinal tumor. With new imaging techniques the diagnosis can be guessed before operation. Nuclear magnetic resonance and endoluminal ultrasound are two newer techniques which are valuable in assessing "mediastinal" tumors.

Evaluation of a radioimmunoassay for the measurement of amphibian parathyroid hormone.

A commercial mid-region to carboxyl-region parathyroid hormone (PTH) radioimmunoassay (RIA) for measurement of PTH concentration in serum of humans was evaluated for its ability to detect amphibian PTH. The recovery of known amounts of human PTH added to aliquots of pooled frog serum demonstrated that frog serum does not interfere with the RIA system. The mean intraassay coefficient of variation was 0.11. During the spring and summer removal of the parathyroid glands from frogs resulted in significant decreases in serum calcium levels which the RIA revealed were associated with significant decreases in PTH levels. In the winter the parathyroid glands of frogs showed histological changes associated with inactivity. Also, parathyroidectomy of winter frogs did not affect serum calcium concentrations, and PTH levels of experimental and control animals were below or near the level of detection.

[Appendicitis in early childhood].

Between 1982 and 1987 550 appendectomies in children under sixteen years of age were carried out in our surgical department. Only 21 patients were children under 4 years of age, accounting for 4% of all patients. The perforation rate however, was 44% in contrast to 12% in older children. One reason is the more difficult, und therefore often delayed, diagnosis. The mean duration of symptoms of appendicitis in young children was 3.4 days. In the special case of perforated appendicitis the mean duration of symptoms was 132 hours, as compared with 42 hours in children without perforation. Clinical examination is decisive for the diagnosis. Fever and vomiting are non-specific symptoms, but frequently present. Some 81% of the patients had leukocytosis over 15,000. If there are any doubts about the indication for appendectomy, x-ray examination should be carried out as a further useful diagnostic procedure.

[Ischemic optic neuropathy (apoplexia papillae) and ischemic micro-infarcts of the retinal nerve fiber layer in extreme internal carotid artery kinking].

The authors report on 2 patients with extreme kinking of the internal carotid artery and ipsilateral, recurrent ischemic optic neuropathy associated with localized microangiopathy of the retinal nerve fiber layer (cotton wool spots). In conjunction with the kinking phenomenon, the atheromatous plaques represent a potential cause of dangerous embolization causing ocular symptoms. The kinking phenomenon may not be diagnosed by Doppler sonography, including duplex B-scan evaluation, and digital subtraction angiography may be necessary. Extreme kinking of the internal carotid artery, which causes ocular embolisms, must be recognized and treated immediately by angioplasty to prevent damage to the eyes and brain.

[Effect of neurosurgical interventions on serum levels of the essential trace elements zinc and copper--a perioperative study].

The serum-concentrations of the trace-elements zinc and copper were assessed before and after neurosurgical interventions in 60 patients. On the day of surgery the mean values of zinc and copper showed a significant decrease, but had reached their preoperative level one week later. While a influence of dexamethasone on the serum zinc levels could not be deducted, the serum copper concentrations seem to be influenced by dexamethasone. These changes seem to be connected with the activation of the hypothalamus-pituitary-adrenal axis during surgical stress.

Alterations in hemolymph components in Biomphalaria glabrata during long-term infection with Schistosoma mansoni.

Total protein, hemoglobin, glucose, and urea concentrations in hemolymph of Biomphalaria glabrata were studied during a 12-week infection of the snails with Schistosoma mansoni. The results suggest an equilibrium between host and parasite metabolism, which is reached after about 6 weeks of infection. During the period studied, the protein spectrum of infected snails showed no difference in quality compared with that of noninfected snails; all fractions were present, and no additional fractions appeared. Only the lipoprotein fraction increased fourfold in the 1st week of infection, decreasing during the 2nd week to concentrations that were also found in noninfected individuals. No further significant alterations could be detected until week 12 postinfection (PI).

[Light microscopic studies of the cartilage in funnel chest. A new view of the pathogenesis].

In patients with a funnel chest, light-microscopic studies of their rib cartilage showed vessels in all slices of the cartilage. In all analysed stages of life there is a constant number of vessels per unit area; even in not deformed parts of the cartilage. Furthermore, the number of the chondrocytes strongly increases within the single chondrons with rising age in an extraordinary way.

[Pathogenesis of chest wall abnormalities--electron microscopy studies and trace element analysis of rib cartilage].

In 71 patients with chest deformity (Group B) and in 43 patients who underwent thoracotomy due to heart disease (Group BN), cartilage of the ribs was removed intraoperatively. Analyses of trace elements showed a highly significant decrease in zinc, coupled with a significant increase in magnesium and calcium in Group B. Electron microscope studies in the cartilage of patients with chest deformity revealed chondrocytes with a normal structure only in some parts, but many degenerative altered chondrocytes with vacuoles of fat and osmiophilic depositions, as well as atypical fibrils and so-called "long-spacing collagen".

[Oxidative degradation of linoleic acid methyl ester in suspensions of inorganic excipients. 1. Auto-oxidation in the presence of silicic acid products and aluminum oxide].

Linoleic acid methyl ester (LME) was selected as a model to study the autoxidation of unsaturated compounds at the surfaces of inorganic excipients. From IR spectra of LME-silica adsorbates it was concluded that the interaction between LME and the silica surface is mainly due to hydrogen bonds, established between the ester carbonyl and the silica silanol groups. The autoxidation was continuously monitored by measuring the oxygen consumption of LME under UV light exposure and isobaric conditions, and, in parallel, by determining the carbonyl and peroxide values of LME. The oxidative degradation of LME is enhanced by porous and colloidal silicas as well as by colloidal aluminium oxide. From the oxygen consumption and the ratio of intermediates (carbonyls, peroxides) formed during the degradation of LME it was concluded that the prooxidative effect of the inorganic excipients can be attributed to an accelerated reaction of the peroxides of LME.

[Wound healing following planned aseptic operations in relation to a tent hospital in the tropics].

A prospective study was conducted into wound healing disorders, following 274 operations from the beginning of planned aseptic surgery in "Karl Marx" Hospital of Managua. Wound healing disorders accounted for 4.7 per cent and wound infections for 1.8 per cent relative to the above total. Wound healing disorders accounted for 5.7 per cent and wound infections for 2.6 per cent relative to 192 cases of herniotomy. These data did not deviate from those of established hospitals, notwithstanding all critical appraisals of the method used in this study. Adiposity, advanced age, and inadequate hygiene are discussed as high-risk factors. The need is stressed for an adequate hygiene regime.
