Influence of irradiation and chemotherapy on the ovaries of children with abdominal tumours.

The ovaries of children with abdominal tumours were studied in 12 autopsy specimens. Ovaries from 25 children who died in accidents or after a short acute disease served as controls. All ovaries from normal children showed follicle growth, but follicle development was inhibited in 67% of the children with abdominal tumours. The effect of treatment with cytotoxic drugs and/or abdominal irradiation on ovarian morphology was investigated. Normal ovaries were found only in children who had received no chemotherapy or a short course. All patients who had been treated with radiation therapy either alone or in conjunction with chemotherapy had severely damaged ovaries: follicle growth was inhibited in all cases, and the number of small non-growing follicles was markedly reduced in most. It is argued that abdominal irradiation might impair follicle development as well as destroy small follicles.

[Modification of immunoglobulins during combined peroral and parenteral nutrition in colitis, ileitis and rectal carcinoma].

Reaction of immunoglobulins (IgA, IgG, IgM) during a four week period under condition with a special parenteral and peroral alimentation was observed in three groups of patients (rectum neoplasm, ulcerative colitis, Crohn's disease) during a period of two weeks all patients received 150 g aminoacids and 510 g carbohydrates by central venous catheter. In the following 14 days parenteral nutrition was limited and oral alimentation began. All patients showed a remarkable approvment of recreation. Globulins rose in between normal spectrum. The most marked changes with a significant uprising of IgG showed patients with rectum carcinoma treated cryosurgically.

Influence of ovarian surface epithelium and rete ovarii on follicle formation.

Early follicle development has been studied in mouse ovaries grafted to immunologically incompetent hosts. Early follicular formation and growth is apparently dependent on the presence of the rete ovarii: the surface epithelium does not seem to play a decisive role. This was shown in transplantation studies of fetal gonads which had been divided into parts with and without the rete ovarii. None of the rete-depleted ovaries exhibited normal follicular differentiation regardless of the presence or absence of the surface epithelium, whereas follicles developed normally in all the implants containing the rete system.

Enchondroma of the hand.

Thirty-three patients with enchondroma of the hand underwent operation for removal of the enchondroma and filling of the cavity with bone meal. Twenty-six patients had pathological fractures of the phalanges or metacarpals.

[Gallstone ileus (author's transl)].

Gallstone ileus may complicate biliary disease in 0.3 to 4.0%. The incidence of mechanical obstruction of the small intestine due to gallstones is between 0.1 and 7.7%. This being an illness of the elderly patient, the prognosis is most unfavorable. The mortality lies between 10 and 50%. Further lowering of the mortality rate is only possible by early surgery.

[The emptying process of the residual stomach after various methods of anastomoses (author's transl)].

Similar to the high pressure zone of the lower esophageal segment (LES) gastric emptying results from alternating intraluminal pressure in the antrum, pylorus and duodenum. Alterations of this antro-duodenal pressure zone depend on hormonal and nervous control mechanisms and are regulated by intraduodenal localized pH-, osmo- and fat-receptors. Experimental investigations in partially gastrectomized dogs emphasize the important role of the duodenum for gastric emptying. After resection of the lower half of the stomach and reconstruction of the orthograde passage between the gastric remnant and the duodenum by gastroduodenostomy (b i-anastomosis), a hormonally controlled gastric emptying comparable to that of the intact organ can be noticed.

[Early cancer of the stomach (author's transl)].

Early cancer of the stomach is defined as a carcinomatous degeneration restricted to gastric mucosa and/or submucosa. The size of extension as well as the mode of perhaps previously spread metastases are not taken into account. The addition early is correct as far as the pathologic-anatomical localized findings are concerned. The clinician however associates early besides the idea of timely recognition and therapy with a better prognosis, e.g. real recovery for the patient. Yet, so-called early gastric cancer does by no means imply absolute curing. Therefore it seems to be more pregnant to speak mucosal and/or submucosal cancer of the stomach.

Citrate anticoagulation and cell washing for intraoperative autotransfusion in the baboon.

Extracorporeal citrate was used for anticoagulation during autotransfusion of baboons. A cell-washing plasmaphoresis procedure was added in one group of animals in order to remove activated clotting materials. Both groups became hypocoagulable, but the cell-washed group had less evidence of disseminated intravascular coagulation as well as lower plasma hemoglobin levels. Citrate anticoagulation plus cell washing is a potential alternative to heparinization for autotransfusion.

[Leiomyoma of the esophagus (author's transl)].

Benign neoplasms of the esophagus are rare. According to Linder and Grözinger (1968) they come up to about 0,5-2% of all esophageal tumors. Among new growths of mesenchymal origin leiomyomas are dominant in 75-85%. In nearly the half of the concerned patients symptoms occur, indicating operative intervention. At the Surgical Department of the RWTH Aachen besides 58 patients with esophageal cancer only 4 patients suffering from benign tumors have been operated on between 1967 and 1975. Histologically three times the diagnosis of a leiomyoma could be established. In one female, a preoperatively simulated leiomyoma was verified as an intramural cyst of bronchogenic origin. Thoracotomy and enucleation or extirpation are the preferable methods of treatment. Postoperatively no essential complications were seen. A clinically silent fistula healed up spontaneously.

The normal development of the ovary in childhood.

The development of the ovary during childhood has been evaluated in 52 organs obtained from autopsy of children who died in accidents or after a brief acute disease. The ovaries at all ages showed follicle growth. 93% of the organs were actively growing ovaries containing healthy and degenerating follicles in progressive stages of development with and without follicle fluid. Three ovaries showed follicle growth up to pre-antral stages. None of the 52 ovaries were quiescent in which follicle development was not in progress. It is therefore argued that the normal ovary in childhood shows follicle growth at all ages. The number and size of antral follicles increases after the age of 6 years coinciding with the progressive increase in FSH and oestrogen output in childhood.

Follicular atresia in the infant human ovary.

The pattern of follicular atresia was studied in nine ovaries from children between the ages 3 months and 8 years. Atretic follicles were found among follicles at all stages of development. The percentage of follicles with signs of atresia became larger as the size of the follicles increased. Only 2% of small follicles (Type 3b) showed signs of atresia, while all follicles greater than 1 mm in diameter (Type 8) were atretic. In follicles of Type 5 and larger, four stages of atresia, which represent consecutive stages of a single atretic process, were defined. The beginning of atresia was characterized by the presence of pyknotic granulosa cells. As atresia progressed, the granulosa layer disappeared, the oocyte became necrotic, the follicle collapsed and the theca cells became hypertrophied. The oocyte can degenerate in several ways: it can be penetrated by cells, the nucleus can become pyknotic or it may complete meiotic prophase. It is suggested that the last event is only possible after the oocyte has reached its full size and has completed RNA synthesis.

Effect of stress on field dependence.

60 subjects were tested in the rod-and-frame test under flicker conditions (stress). As compared to scores in a control situation (no flicker), the rod-and-frame scores were large under stress and increased monotonically during the session. Futhermore, both intra- and interindividual variability of rod-and-frame performance changed under stress conditions in a consistent manner. The general results, which clearly point to a reliable influence of stress on field dependency, are discussed within the methodological framework of Witkin's theory of perception and personality.

[Experimental studies of the effects of gastrin, neostigmine and prostaglandin on the motility of the trunculary vagotomized stomach].

The working effect of Gastrin, neostigmine and prostaglandin in regard to the emptying of the stomach was tested on 31 dogs that had undergone a truncular vagotomy. Gastrin and neostigmine were proved to increase the motility at the stomach pylorus but was not able to bring about a coordinated emptying pattern. Prostaglandin shows different effects on the emptying reflex of the stomach. PGF 2 alpha increases the motility on the measuring marks, but at different time intervals. Because of this a coordinated motility is made possible.

[Dietetic and antibiotic preparation in surgery of the large intestine].

Besides patient's cardiac, pulmonary, renal and hepatic status, a correct operative technique and mechanical bowel cleansing the protein status is of central importance to prevent anastomotic leakage and wound infection in colon surgery. Using an oral elemental diet prior to surgery both sufficient caloric and protein intake and reduction of intestinal flora can be achieved. Wound infection and anastomotic leakage are reduced significantly without the side effects of antibiotic bowel preparation. If oral antibiotics are used preoperatively only a short term low dose regimen is effective for the control of infection after colon surgery which reduces the concentration of aerobes and anaerobes.
