Cryosurgery for hemangiomas of the body surface and oral cavity.

The indications for and limitations of cryosurgery for various types of hemangiomas on the body surface or in the intraoral region are reported based on our clinical experience. In our opinion, cryosurgery is not the method of choice in the management of hemangiomas but is useful under certain conditions, as with a localized cavernous under certain conditions, as with a localized cavernous hemangioma in the facial or intraoral region. It is also valuable for strawberry marks with bleeding or infection and may be acceptable for small port-wine stains; in the treatment of extensive port-wine stains, a resulting hypertrophic scar or depigmentation may present difficulties.

Ornithine transcarbamylase (OTC) in white blood cells.

A radiochemical assay method of ornithine transcarbamylase (OTC) was developed using labeled carbamyl phosphate as a substrate. The enzyme activities determined by this method in peripheral white blood cells from ten normal subjects were 1.32 +/- 0.95 nmoles/mg/hr and the apparent Km's, when assayed at pH 8.5, were 6.4 mM for ornithine and 0.6 mM for carbamyl phosphate. On the contrary, the apparent Km's of human liver OTC were 0.6 mM for ornithine and 0.12 mM for carbamyl phosphate. The average OTC activity of granulocytes was 1.0 nmoles/mg/hr, whereas that of mononuclear cells was 0.4 mmoles/mg/hr. Lymphoid cell lines were established from three normal subjects and an OTC-deficient infant. All these cell lines demonstrated no OTC activity. When arginine was removed from the medium and replaced by ornithine, the lymphoid cells were unable to grow in culture. On autoradiography, the lymphoid cells showed labelling at incubation in the presence of 14C-citrulline, but not with 14C-ornithine.

Free omental transfer to the lower limbs.

Utilization of a free omental transfer with a skin graft in two lower limbs for soft tissue and skin cover in difficult situations is reported. In 1 patient, it was used to cover open and damaged joints around the ankle, and in the other, for the reconstruction of a circumferential defect in a lower limb. The results were quite satisfactory.

Modified Limberg flap for lumbosacral meningomyelocele defects.

We have recently developed a technique for closing round or wide oval lumbosacral meningomyelocele defects by modifying the Limberg flap. Four cases of repair of lumbosacral meningomyelocele were treated, with good skin closure in 3. The indications for and advantages of our method are discussed.

Methods of vascular pedicle coverage.

Based on our clinical experience, various methods of donor or recipient vascular pedicle coverage for an arterialized or free flap are reported. The vascular pedicle may be placed in the subcutaneous space either by incising the skin along the full length of the vascular route or by passing it through a subcutaneous tunnel. It has been our experience that the vascular pedicle can also be covered by either a local flap or a free skin graft. On rare occasions, a narrow skin flap strip including the vascular pedicle can be lifted en bloc and rotated toward the base of the area transferred.

Unusual facial hemangioma.

An unusual hemangioma of the face, found in a 7-year-old girl in the masticator space and consisting of multiple lobes, is reported. Direct angiography was effective in clarifying the space occupied by the tumor. It was removed through a combined approach, using both temporal and oral incisions, with almost no bleeding.

Delayed cutaneous hypersensitivity in children with severe multiple handicaps treated with phenytoin.

Peripheral lymphocyte counts, T-cell and B-cell populations, DNCB skin reactions, Mantoux reactions, mitogenic transformation using PHA, and determination of serum phenytoin concentration were carried out in 45 children with severe handicaps. Twenty two of these patients were receiving phenytoin treatment. When compared with control subjects, the skin reactions were significantly depressed in the handicapped children, amongst whom those treated with phenytoin had more impaired reactions than those who were untreated. No significant relationship between the results of the skin reactions and the other parameters was found.

Penile disorders in two paraplegics.

We present two rare cases of paraplegics with remarkable sequelae of the penis due to long-term usage of a condom-catheter. One involved hypertrophy of the glans prepuce, and the other had severe constriction of the distal portion of the corpus penis with two fistulae.
