Correction of myopathic face associated with myotonic muscular dystrophy. A case report.

Two cases of myotonic muscular dystrophy manifested oro-facial disharmony and dento-labial dysfunction. Le Fort I osteotomy, sagittal osteotomy of the mandible, genioplasty and wedge excisions of the mucosa of the lower lip were performed on both patients. The desired results with regard to oro-facial appearance and dento-labial function were achieved in both patients after follow-up periods of six and twelve months.

Structural identification and synthesis of luciferin from the bioluminescent earthworm, Diplocardia longa.

For the first time, luciferin from a bioluminescent earthworm has been purified, identified, and synthesized. This luciferin from the North American species, Diplocardia longa, is a simple aldehyde compound, N-isovaleryl-3-aminopropanal, with an amide functional group. It is a clear, odorless oil at room temperature. It is nonvolatile and has no near-uv-visible absorption or fluorescence. Derivatives of this compound were made to facilitate its identification: the luciferin 2,4-dinitrophenylhydrazone (mp 174 degrees C), a yellow crystalline solid; and the luciferin alcohol, a clear oil. Synthesis of Diplocardia luciferin yielded an oil of identical spectroscopic (proton nuclear magnetic resonance (NMR), 13C NMR, mass, and ir), chemical (dinitrophenylhydrazone and alcohol derivatives, bioluminescence activity), and physical (thin-layer chromatography, volatility) properties to those of the purified native Diplocardia luciferin.

Involvement of cyclic GMP in the initial stage of hepatocytes proliferation.

A transient rise in cyclic guanosine 3' : 5' monophosphate (c-GMP) in the liver was observed in rats in vivo 10--20 min after partial hepatectomy. A similar increase in c-GMP in the liver was also found in rats in vivo 15 min after infusion of TGH solution (a mixture of triiodothyronine, glucagon, and heparin). In both cases, inductions of ornithine decarboxylase [EC 4.1.1.17] and tyrosine aminotransferase [EC 2.6.1.5] were found 4 hr after the beginning of the experiments. Later, 22 hr after the surgical intervention or hormone infusion, thymidine kinase [EC 2.7.1.21] was activated and liver slices were able to incorporate [3H]thymidine into DNA. These biochemical phenomena were observed commonly in regenerating liver as well as in the liver of rats infused with TGH solution. c-GMP, but not c-AMP, could induce ornithine decarboxylase and tyrosine aminotransferase in isolated, perfused liver.

Replantations and free flap transfers by microvascular surgery.

We have replanted 11 digits and two hands, with only one failure (a digit). Eight free groin flaps and one free dorsalis pedis flap transfer were performed, resulting in 5 complete survivals, two partial necroses, one superficial necrosis, and one failure. One hallux-to-thumb transfer was done, with a good result. We note the essential differences between doing replantations and free flap transfers.

Single administration of enterococcal preparation (FK-23) augments non-specific immune responses in healthy dogs.

Influence of a single administration of heat-killed Enterococcus faecalis FK-23 preparation (FK-23) on non-specific immune responses was evaluated in healthy dogs. The dogs received a single administration of FK-23 at a dose of 100 mg/kg perorally and thereafter a complete blood count (CBC), leukocyte differential count, phagocytic activity of peripheral blood neutrophils and a lymphocyte blast transformation (LBT) test were assessed. Although FK-23 had no effect on the CBC or leukocyte differential count, treatment of the drug caused a 1.4-fold increase in neutrophil phagocytosis compared to FK-23 non-treated healthy dogs. LBT activities of phytohemagglutinin (PHA) and pokeweed mitogen (PWM) were also activated to twice the control levels and that of concanavalin A (Con A) was increased to one and a half times by the FK-23 treatment. Thus, a single administration of FK-23 appears to augment host resistance through stimulation of the non-specific immune responses in vivo.

Restoration of portal vein flow by splenorenal shunt ligation and splenectomy after living-related liver transplantation.

We describe here a case of living-related liver transplantation with a spontaneous splenorenal shunt. On the third postoperative day, Doppler ultrasound follow-up revealed impairment of intrahepatic portal vein flow and reoperation was performed. Intraoperatively, simple compression of the collateral veins forming the splenorenal shunt resulted in an immediate change in the portal flow pattern, which suggested the absence of a portal thrombus. Splenectomy and ligature of the more dilated collateral veins were carried out and satisfactory portal flow was restored. Although the role of splenectomy was controversial, splenectomy provided an increased portal vein flow in the present case. The uneventful postoperative course validated our decision.
