[A case of multiple cerebral aneurysms caused by cardiac myxoma].

A case of neoplastic aneurysm caused by left atrial myxoma is reported. The patient was a 50-year-old woman who has been suffered from occasional vertigo and syncopal attack. She was admitted with a sudden loss of consciousness and cerebellar ataxia on December 1, 1983. Cerebral angiogram revealed multiple aneurysms in the periphery of both middle cerebral arteries and obstruction of the right superior cerebellar artery. Echocardiography displayed a cardiac myxoma in the left atrium. She suddenly died from recurrent cerebral embolism on January 7, 1984. The diagnosis was confirmed by the autopsy. The histopathological examination revealed that the wall of cerebral arteries were destroyed by the tumor cells of myxoma, and it caused cerebral aneurysms. We presented the detail of this case, and discussed about a mechanism and a treatment of neoplastic aneurysms.

A simple assay of solubilized adenosine receptors with nitrocellulose membrane filters.

A rapid and simple assay of solubilized adenosine receptors with nitrocellulose membrane filters is described. This assay was sensitive and reproducible when applied to adenosine receptors solubilized from rat brainstem membranes with 3-[(3-cholamidopropyl)dimethylammonio]-1-propanesulfonate. Appropriate values of dissociation constants for the solubilized adenosine receptors to their tritiated agonists were obtained by the membrane filter technique. This method should be applicable for the assay of a variety of solubilized receptors.

An autopsy case of miliary tuberculosis and review of autopsy cases in Japan.

Presented is an autopsy case of a 79 year-old male who died of miliary tuberculosis. The patient was admitted to the University Hospital with an unknown fever and suspicion of leukemia. He expired on the 13th day of hospitalization. Postmortem examination revealed generalized miliary tuberculosis. The diagnosis of tuberculosis was established only after autopsy. Difficulty in diagnosis of miliary tuberculosis is discussed. Also discussed is a probable association of acute interstitial nephritis with chemotherapy or miliary tuberculosis. Leukemoid reaction is thought to be associated with miliary tuberculosis. The present case reminds us that miliary tuberculosis should never be forgotten when treating a patient having a fever of unknown origin. Cases with tuberculosis were compiled from the Annual of Pathological Autopsy Cases in Japan during 1974 through 1981. There were 3,242 cases of tuberculosis (1.4%) out of 235,095 autopsies recorded in the Annual of Pathological Autopsy Cases in Japan during the above 8 years. Of 3,242 cases 618 were miliary tuberculosis. The latter figure indicates 19.0% of all tuberculosis and 0.26% of all autopsies. Although all age groups were not spared, the 7th and 8th decades were more affected than the others. Tuberculosis has markedly declined since the advent of antituberculous antibiotics. But, autopsy cases with tuberculosis have not recently decreased in number. Clearly, increased awareness of the continuing presence of tuberculosis is needed.

[A combination chemotherapy consisting of vincristine, etoposide and cyclophosphamide (VEC) for small cell carcinoma of the lung].

Twenty-nine patients with small cell carcinoma of the lung were treated with a combination therapy consisting of vincristine 1 mg/m2 i.v. day 1, etoposide 200 mg/body p.o day 1-5 and cyclophosphamide 500 mg/m2 i.v. day 1 (VEC) in 3 week interval. After 2 courses of VEC, four out of 10 patients with limited disease (LD) showed a complete response (CR), while 7 out of 19 patients with extensive disease (ED) obtained a CR. Six patients with LD and 6 patients with ED received subsequent radiotherapy to primary tumors and to regional lymph nodes, and 5 with LD and 3 with ED attained a CR. Overall complete response was observed in 9 patients (90%) in LD and 10 patients (53%) in ED. The median duration of survival was 17 months in complete responders with LD and that was 12 months in those with ED. Dose limiting toxicity of the VEC regimen was leukopenia which occurred in most patients and thrombocytopenia was less frequent. Non-hematologic toxicities containing alopecia, numbness, nausea and others were acceptable and well tolerated.

Role of calmodulin in neurotransmitter synthesis.

Tyrosine 3-monooxygenase (EC 1.14.16.2) and tryptophan 5-monooxygenase (EC 1.14.16.4) are generally believed to be the rate-limiting enzymes in the biosynthesis of the neurotransmitters, catecholamines and serotonin, respectively, and therefore the regulation of their activities is of particular importance. At least three calmodulin-dependent protein kinases differed in their molecular weights and substrate specificities, designated I, II, and III in the order of decreasing molecular weight, in rat brain cytosol. Among them, calmodulin-dependent protein kinase II with a molecular weight of about 540,000 appeared to occur only in the nervous tissues. Kinase II was found, on the one hand, to phosphorylate tyrosine 3-monooxygenase and tryptophan 5-monooxygenase, leading to the activation of these monooxygenases in the presence of activator protein and, on the other hand, to phosphorylate tubulin and microtubule-associated protein 2, which results in disassembly of the microtubules that had been assembled. These results suggest the possibility that both the secretion and biosynthesis of monoamine neurotransmitters stimulated by Ca2+ influx in the nervous system may be regulated by calmodulin-dependent protein kinase II via the phosphorylation of microtubule proteins and the phosphorylation of the monooxygenases that are the rate-limiting enzymes in the biosynthesis of the neurotransmitters.

[Clinical significance of new tumor marker CA 125 in gynecological cancer--particularly usefulness in diagnosis of ovarian cancer].

A concentration in sera of a new antigen CA125 related to cancer of the ovary was measured by radioimmunoassay which used a monoclonal antibody, and its usefulness as a tumor marker in the diagnosis of cancer of the ovary was investigated. The mean values for CA125 in sera of healthy controls (80 females) were 14.2 +/- 12.2 U/ml. Mean values for CA125 in sera of various patients were 29.0 +/- 39.6 for myoma uteri (30 cases), 26.7 +/- 30.1 for benign ovarian tumors (10 cases), 64.4 +/- 146.2 for cervical carcinoma (14 cases), and 31.5 +/- 19.8 for endometrial carcinoma (6 cases), whereas the mean values for ovarian cancers (23 cases) were as high as 311.3 +/- 250.4. On the other hand, positive rates for CA125, when the cut-off value was set to 65 U/ml, were 1.3% for healthy controls (1 out of 80 cases), 10.0% for myoma uteri (3 out of 30 cases), 10.0% for benign ovarian tumors (1 out of 10 cases), 14.3% for cervical carcinoma (2 out of 14 cases), and 16.7% for endometrial carcinoma (1 out of 6 cases), whereas the positive rate for ovarian cancers was 78.3% (18 out of 23 cases). Especially for serous cystadenocarcinoma, the positive rate was 100% (10 out of 10 cases). From the above, the measurement of CA125 in sera was considered to be significant in the diagnosis of ovarian cancer.

Solubilization and partial characterization of adenosine binding sites from rat brainstem.

Binding sites for adenosine were solubilized from rat brainstem membranes with either sodium cholate, sodium deoxycholate or 3-[(3-cholamidopropyl)dimethyl-ammonio]-1-propanesulfonate. About 30% of the binding activity were released by [3H]phenylisopropyladenosine (PIA) binding. Specific [3H]PIA binding to the solubilized fraction was saturable and was found to be a monophasic saturation profile. In contrast, [3H]PIA binding to the brainstem membranes exhibited a biphasic profile suggesting the presence of two binding sites. By gel filtration on a Sepharose CL-6B column, the adenosine binding site--detergent complex was estimated to have app. Mr 280000 and rs = 5.4 nm.
