A retrospective analysis was made of 3,280 diaphyseal femur fracture surgically treated in 73 medical centres of the GDR. The conclusion drawn from this quality check was that the high osteomyelitis rate of 5.6 per cent had been primarily attributable to shortcomings in osteosynthesis techniques but also to inadequate choice of methods. The following conclusions may be derived from the study reduction of bone infections: The osteomyelitis rate in the wake of Küntscher's nailing was as low as 2.2 per cent. This was the best result in the context of simple fractures with closed soft-part trauma. The advantages of covered intramedullary nailing were particularly felt in cases in which optimum protection against infection was impossible for the given surrounding conditions. Plate osteosynthesis has proved to be the most effective approach to open fractures, provided careful wound dressing, non-invasive surgical techniques, and generous indication in favour of autogenic spongiosa transplantation. Interlocked nailing is considered to be the optional technique of osteosynthesis to cope with comminuted fractures. External stabilisation is urgently indicated in cases of massive traumatisation of the femoral diaphysis in conjunction with severe open lesions of soft parts.

In chloralose-anaesthetized cats, the impulse activity of single afferent units conducting at less than 30 m s-1 and having receptive fields in the triceps surae muscle or the calcaneal tendon, was recorded from thin filaments of the dorsal roots L7 and S1. The receptive fields of the units were tested with a variety of graded natural stimuli (local pressure, stretch, contractions, temperature changes). In addition, the algesic agent bradykinin was injected into the receptive fields, but the sensitivity of the receptors to this substance was not used for classification purposes. Four types of receptors could be distinguished using the strongest response to innocuous natural stimulation as the criterion for characterizing a given ending: (a) nociceptors showing no response to innocuous forms of stimulation and requiring noxious (tissue-threatening) stimuli to be clearly activated; (b) low-threshold pressure-sensitive receptors responding to innocuous indentation of the tissue but being relatively insensitive to stretch and contractions; (c) contraction-sensitive receptors reaching high discharge frequencies during active contractions of moderate force and innocuous stretch, but being relatively insensitive to local pressure stimulation; (d) thermosensitive receptors responding strongly to small changes in temperature without reacting to innocuous mechanical stimulation. The possible involvement of the different receptor types in central nervous functions (nociception, mechanoreception, ergoreception, thermoregulation) is discussed.

Over a 9 year period, 55 infants underwent resection and end-to-end anastomosis for symptomatic coarctation of the thoracic aorta during their first 120 days of life (mean age 47 days; mean weight 3.7 kg). Forty-two had preductal coarctation and 13, postductal. Additional cardiac lesions were found in 48 patients. Ventricular septal defect, either isolated or associated with other malformations, was the most frequent finding (37 patients). Simultaneous banding of the pulmonary artery was performed in 14 infants because of nonrestrictive ventricular septal defects. The hospital mortality was 3.6% (two patients). There were no late deaths. All survivors have been reinvestigated, and 27 have been recatheterized. In the group as a whole, after an average follow-up of 4.5 years, the mean pressure gradient (arm/leg) was 7 mm Hg (range 0 to 45 mm Hg). In the recatheterized infants, the average systolic pressure gradient at the anastomotic site was 16 mm Hg (range 2 to 62 mm Hg), whereas the mean pressure gradient in this group was 7 mm Hg (range 0 to 33); only three of them had systolic pressure gradients of more than 20 mm Hg. One reoperation is scheduled. Our data suggest, that resection and end-to-end anastomosis for symptomatic coarctation in the first 3 months of life can be performed with very low operative mortality and excellent long-term results.

In a patient group of "underweight" persons, a total of 20 factors of the pregnant status, of the course of pregnancy and birth, as well as of the perinatal birth result, were tested by means of univariate analysis of variance and multivariate analysis of discriminance. Despite marked complexity of the influencing parameters and their interactions, it is shown that it will be approximately sufficient to pay attention to a few parameters to arrive at a comparable objectivation of overall clinical results. It is also evident that conclusions valid for direct obstetric and perinatal medical work cannot be arrived at from retrospective analysis alone.
The inducer PIND-AVI was used prophylactically and therapeutically to test the effects of paramunization in a SPF cat colony and in various small animal practices. Prophylactic paramunization significantly improved the breeding results in the SPF cat colony. Mortality declined from 44.6% to 3.4%. The corresponding placebo groups were also tested thereby demonstrating the significance of the results. In comparison, the treatment of acutely diseased cats (all of the same litter or the same colony) showed that paramunization can also be used as an additional measure to the conventional therapy of infectious diseases with different etiology. Furthermore, it is possible that this measure alone can limit the incidence of disease providing the dosage is high enough and treatment is long enough (at least 3-5 days). The course of disease is thereafter mild and complications can be hindered. Chronic and recurrent infections (e.g., dermatitis and gingivitis) can be controlled with repeated paramunization. Prophylactic paramunization decreases the danger of an acute infection and provides for the newborn's fast adaptation to a contaminated environment. It also reduces the consequences of an exogenous and endogenous immune suppression.
