[Nutrition and skin diseases in the horse].

Skin diseases of the horse can be produced or enhanced through nutrient deficiency, intestinal formation of detrimental substances, photosensitizing compounds and by intake of allergenes. An exact case history regarding feeding is useful for evaluation of every skin abnormality.

Adult abdominoscrotal hydrocele.

Abdominoscrotal hydrocele is very uncommon in both adults and children. A case is presented of an unusually large abdominoscrotal hydrocele in an adult associated with unilateral hydronephrosis owing to contiguous pressure. To our knowledge this is an unique case in which the preoperative diagnosis became apparent after abdominal ultrasound. An IVU and an abdominal CT study confirmed the sonographic diagnosis. An entity as such has never been reported nor has it been presented in recent literature.

[Reoperation following correction of partial defects of the endocardial cushion (indication, incidence, results)].

In the period between 1955 and 1982, 297 patients underwent surgical correction of a partial AV-canal. Closure of the ostium primum defect was performed either by direct suture or by patch (prosthetic material or pericardium). Only in cases with severe mitral incompetence was the cleft in the anterior leaflet of the mitral valve surgically treated. At an average of 6 years (range: 3 months - 22 years) after the initial procedure 21 patients (7.8 per cent) underwent reoperation. In 20 patients reoperation was necessary for hemodynamic reasons (recurrence of ASD: n = 8; severe AV-valve regurgitation: n = 3; or both: n = 9). One patient with moderate mitral valve incompetence suffered from severe "patch-hemolysis" due to direction of the blood-jet towards the prosthetic patch. Residual or recurrent atrial septal defects were closed by using a patch in cases with previous direct suture (39 patients - 8 reoperations) or by reinsertion or enlargement of the present patch (258 patients - 13 reoperations). AV-valve incompetence could be treated in all cases but two with reconstructive methods. In two patients implantation of a prosthetic valve was necessary. In the single case with "patch-hemolysis" the previous prosthetic patch was replaced by a pericardial one, together with a suture of the mitral cleft 3 months after operation. Mortality of reoperation was 14.5 per cent (3 early deaths). Major complications in the surviving patients did not occur, with one exception: one patient with postoperative total AV-block received a permanent pacemaker.

Nimodipine: synthesis and metabolic pathway.

Key step of the synthesis of the calcium antagonistic cerebral vasodilator (+/-) isopropyl-2-methoxyethyl 1,4-dihydro-2,6-dimethyl-4-(3-nitrophenyl)-3,5-pyridinedicarboxylate (Bay e 9736, nimodipine) (5) is the cyclizing Michael addition of 3 onto 4. A pharmacokinetic study with 14C-nimodipine in the rat revealed as major metabolites the dihydropyridines 6 and 8 as well as the pyridines 7, 9, 10, 11, 13 and 14. A potential metabolic pathway is discussed involving ether cleavage and oxidation to the pyridine form as primary biotransformation steps. Reference metabolites were synthesized using 1,4-dihydropyridines with appropriate functionalities as precursors.

An experimental study on bacterial colonization, nitrite and nitrosamine production in the operated stomach.

Intragastric bacterial growth and nitrite and nitrosamine formation were investigated in animal experiments after different surgical procedures of ulcer management with regard to carcinogenesis in the operated stomach. The operative procedures resulted in an alteration in the gastric flora with an increase and predominance of nitrate-reducing bacteria. There was an increase in the intragastric nitrite and nitrosamine concentration corresponding to the increase in nitrate-reducing bacteria. All three parameters showed the greatest increase after gastric resection, whereas there were no significant differences after vagotomy with pyloroplasty, compared with a control group. These findings in animal experiments are of etiopathological relevance to the known danger of cancer in the resected human stomach and should be taken into consideration when choosing a procedure for surgical ulcer therapy.

[Left ventricular basal fibrous myocardial aneurysm in a 3-week-old girl].

A congenital bulging of the myocardial wall is a rare finding; little is known about its etiology and clinical course. The case of a 3-week-old girl in whom rapidly progressive cardiac insufficiency occurred after a completely normal postnatal development is described. Selective ventricular angiography revealed ventricular malfunction and an akinetic area occupying 57% of the basal free left ventricular wall. The child died in acute cardiac failure. Autopsy disclosed a basal fibrous aneurysm which extended to 34% of the free ventricular wall. There was no indication of a coronary or inflammatory etiology, so a focal disorder of myocardial architecture was presumed. Congenital bulging of the ventricular wall can be divided into broad-based aneurysms and diverticula, the latter being connected with the ventricle by a small orificium. A further distinction can be made on the basis of localization and histology. A reclassification of the congenital bulging of the left ventricular wall presented in the literature showed that apical muscular diverticula are the most frequent, whereas only four cases of fibrous aneurysms were reported. Differences were found, with regard to the distribution of age and race, the clinical course, and the possible etiology.

Nitrendipine: identification and synthesis of main metabolites.

(+/-)-Ethylmethyl-1,4-dihydro-2,6-dimethyl-4-(3-nitrophenyl)-3,5-pyridinedicarboxylate (nitrendipine, Bay e 5009) 1, a calcium antagonistic 1,4-dihydropyridine derivative, is currently under development as an antihypertensive. A pharmacokinetic study with 14C-nitrendipine in the rat revealed as major metabolites the pyridine 2, 3, 4, 5, 6 and 9 using glc-, tlc- and gc/ms-techniques. A potential metabolic pathway is discussed involving oxidation to the pyridine form, saponification of the ester groups and hydroxylation of the methyl groups as general biotransformation steps. 1,4-Dihydropyridines bearing appropriate functionalities were precursors in the synthesis of the reference metabolites 2, 3, 4, 7, 8 and 10.

[The treatment of melanoma of the iris].

According to current concepts the treatment of malignant melanomas of the iris, uvea or choroid of the eye tends to be conservative. Removal of the eye is only indicated if the local complications cannot be controlled by medical means. The classification of malignant melanomas according to size may be a better indicator of prognosis and choice of treatment than cytological assessment alone.

[Animal experiments to assess the risk of cancer in the stomach after vagotomy (author's transl)].

To assess the possible risk of cancer in the operated stomach, the influence of truncal vagotomy combined with different operative drainage procedures (pyloroplasty, gastro-enterostomy without entero-anastomosis, Roux-en-Y gastro-enterostomy) was studied in animal experiments. The aim of the study was to determine whether, compared to controls an earlier and higher incidence of precancerous epithelial lesions or cancers in the stomach occurs in vagotomized animals after oral administration of carcinogens (MNNG). To consider conditions, that may predispose to in vivo formation of carcinogenic N-nitroso compounds, bacterial flora and intragastric nitrite concentration were measured and compared in the various operative groups. The severity of induced epithelial lesions and the quantity of bacterial growth and nitrite concentration in the gastric juice were influenced, apart from the reduction of acid output after vagotomy, mainly by prolonged gastric stasis or the degree of enterogastric reflux. The results suggest that there is probably no increased risk of cancer following vagotomy, if sufficient gastric clearance is guaranteed and enterogastric reflux is largely excluded.
