Ewing's sarcoma metastazing from the right pelvis via the inferior vena cava into the heart: diagnosis and successful surgical treatment.

A case of Ewing's sarcoma metastazing from the pelvis into the heart is presented. Fainting as a result of circulatory obstruction within the right heart was the leading symptom. Open-heart surgery was performed to excise the tumor from the right atrium and the inferior vena cava. Postoperative angiography showed the tumor coming up out of the right iliac vein and growing along the wall of the inferior vena cava.

Isolation and characterization of monoclonal antibodies against an attenuated vaccine strain of equine herpesvirus type 1 (EHV-1).

The production and differentiation of monoclonal antibodies (mabs) against the Rac-H strain of EHV-1 used as an attenuated live vaccine to prevent rhinopneumonitis and abortion is described. Seven different antigenic sites were detected by the 15 mabs produced. EHV-1 specific mabs as well as EHV-1 and -4 common mabs could be established, allowing easy typing of EHV isolates. One mab recognized the vaccine strain only. This reaction was used to investigate a possible involvement of the vaccine strain in cases of abortion. Common antigenic determinants with EHV-1,-3,-4 and BHV-1 could also be detected, indicating the presence of highly-conserved epitopes of alpha-herpesviruses.

Bradykinin-induced modulation of the response behaviour of different types of feline group III and IV muscle receptors.

1. In order to test the hypothesis that bradykinin has a sensitizing action on muscle receptors (e.g. during a myositis), the response properties of single group III and IV afferent units from the cat gastrocnemius-soleus muscle were compared before and after infiltration of their receptive fields with a bradykinin solution. According to their responses to graded natural stimuli (local pressure, stretch, contractions and temperature changes) the units were classified as (a) nociceptors, (b) low-threshold pressure-sensitive (LTP) receptors, (c) contraction-sensitive (CS) receptors and (d) thermosensitive receptors. 2. Bradykinin activated the majority of both the nociceptive and low-threshold (LTP, CS and thermosensitive) receptors but a sensitization was prominent only among the nociceptors. Most of the sensitized nociceptors showed increased responses to mechanical, but not to thermal, stimuli. The sensitization appeared to be quite specific in that the nociceptors were sensitized either towards local pressure stimulation or to active contractions, but never towards both forms of stimulation. 3. Both group III and group IV nociceptors were sensitized by bradykinin, the proportion of sensitized receptors being greater for group III units. 4. Some of the low-threshold receptors (particularly the CS units) showed a desensitization under the influence of bradykinin. 5. Although bradykinin (by lowering the mechanical thresholds of nociceptors into the innocuous range) could produce the symptom of allodynia, it was not capable of eliciting all the changes in receptor behaviour which are known to occur in inflamed tissues. For instance, no ongoing activity of longer duration and no substantial sensitization of low-threshold receptors have been observed in the present study.

Identification and procaryotic expression of the gene coding for the highly immunogenic 28-kilodalton structural phosphoprotein (pp28) of human cytomegalovirus.

Human cytomegalovirus contains a structural polypeptide that is 28 kilodaltons in apparent molecular size and is reactive in Western blot (immunoblot) analysis with the majority of human sera. The gene coding for this polypeptide was mapped on the genome of human cytomegalovirus strain AD169. A monoclonal antibody specific for the 28-kilodalton polypeptide was used to screen a cDNA library constructed from poly(A)+ RNA of human cytomegalovirus-infected cells in the procaryotic expression vector lambda gt11. Hybridization of cDNA with cosmid and plasmid clones mapped the gene to the HindIII R fragment. The gene was transcribed into a late 1.3-kilobase RNA. The nucleotide sequence of the coding region was determined. Parts of the 28-kilodalton polypeptide were expressed in Escherichia coli as hybrid proteins fused to beta-galactosidase. In Western blots these proteins were recognized by human sera. Antibodies raised against the hybrid proteins reacted specifically with the viral antigen in immunoprecipitations and Western blots. In vitro phosphorylation of HCMV virions and immunoprecipitation showed that the 28-kilodalton polypeptide was phosphorylated.

A computer-assisted device for the intraoperative CT-correlated localization of brain tumors.

In a series of 72 open brain tumor operations the 32P (radiophosphorus) test proved to be valuable for the removal of visually not detectable tumor residues. As during resection a topographical orientation in the depth of tumor cavities was nearly impossible, a computerized measuring device was developed which enables the surgeon to locate the tumor boundaries by simultaneous comparison with preoperative CT scans. For 32P-beta emission measurements a miniature semiconductor probe can be attached to the tip of the four-axis digitizing arm allowing the evaluation of radicality by topographic distinction between tumor ramifications and normal brain tissue. The device was tested during 3 craniotomies and the perspectives of computerized stereotaxy are discussed.

Regional myocardial free fatty acid extraction in normal and ischemic myocardium.

The rate constant for free fatty acid influx (k1) was studied in normal and ischemic myocardium. In 15 normal subjects and 30 patients with coronary artery disease, 201Tl and 15-(p-123I-iodophenyl)-pentadecanoic acid (IPPA) were administered during exercise under fasting conditions and at rest. In 10 patients, the study was repeated after percutaneous transluminal coronary angioplasty; in three patients, the study was repeated after infarction. The initial accumulation of IPPA, related to that of 201Tl (both background and crossover corrected), was used for determinations of the regional rate constant of IPPA influx into myocardial tissue (k1*). In normal subjects, no significant differences in k1* between major myocardial segments were found; the average value of k1* was 0.57 +/- 0.13/min (mean +/- SD) at rest and 0.42 +/- 0.06/min at exercise (average workload, 123 +/- 47 W). With increasing free fatty acid plasma concentration and perfusion, free fatty acid influx increased in a saturable fashion. The Michaelis-Menten constant (KM*) and the maximal velocity (Vmax*) for IPPA influx into myocardial tissue were estimated to be 470 nmol/g and 430 nmol/g.min, respectively. In ischemic areas, k1* was reduced to 57 +/- 18% of k1* value in nonaffected segments. The areas were larger than those showing reduced 201Tl uptake. Preinfarction and postinfarction studies showed that the size of 201Tl defects in postinfarction images corresponded with the size of the area with reduced k1* observed in preinfarction scintigrams. Revascularization led to an increase of 201Tl uptake and to normalization of k1*.

[Histologic diagnosis and ultrastructural cytology of epithelioid sarcoma].

Epithelioid sarcoma is a rare but distinctive soft tissue tumor showing characteristic histologic features associated with a typical course, but there is still some debate as to its histogenesis. Two cases of this unique tumor were analyzed using the electron microscope. Light microscopically the diagnostically relevant cell type is oval to polyhedral exhibiting a dense eosinophilic cytoplasm. In such epithelioid cells the cytoplasm may sometimes be completely filled with an inclusion-body like hyaline eosinophilic material. Ultrastructurally the most striking feature are abundant intermediate-type filaments in these eosinophilic cells. While none of the electron microscopic findings is specific, the accumulation of the intermediate-type filaments in conjunction with the other cellular features described in sufficiently consistent and distinctive and can thus be used for confirming the diagnosis. In formalin-fixed specimens the electron microscopical investigation may be helpful in everyday diagnosis. The suggestion from the literature that epithelioid sarcoma is related to the synovial sarcoma can not be supported by our ultrastructural findings.
