[Effect of celiprolol, fenoterol and aminophylline on respiratory tract obstruction during an asthma attack].

Selective beta 1-blocking drugs without bronchoconstrictor effect are desirable in patients with bronchial asthma and systemic hypertension or tachycardia. Therefore, 15 patients with bronchial asthma were given a single dose of 100 mg celiprolol (and 5 of them 300 mg) and airway obstruction (Raw, FEV1), pulse rate and blood pressure were documented for 3 hours. After 2 hours an additional infusion of aminophylline and after 3 hours an additional inhalation of fenoterol were given. Within 120 minutes after the intake of celiprolol no statistical changes in the measured data (Raw, FEV1, blood pressure and pulse rate) were found. The same was true after the infusion of aminophylline. Reversibility of airway obstruction by fenoterol was better after celiprolol plus aminophylline than without this premedication (p less than 0.05). It is concluded that the beta 1-antagonist celiprolol does not induce relevant airway obstruction in patients with bronchial asthma, and that an additional infusion of aminophylline enhances the bronchodilatory effect of the beta 2-agonist fenoterol (p less than 0.05).

Long-term study of the protective effect of ketotifen in children with allergic bronchial asthma. The value of a provocation test in assessment of treatment.

The effect of ketotifen on allergic symptoms and parameters of respiratory function were studied in 23 allergic children during a period of 6 months therapy. A reduction in nocturnal dyspnea and dyspnea during exercise was observed in 17 patients. Improvement of the FVC, PEF and FEV1 was detected in 14, 15 and 19 patients, respectively. Following a 4-day treatment with ketotifen, a further 15 patients with allergen-induced bronchospasm were challenged by inhalative provocation test, comparing the protective effects of ketotifen and of DSCG in a randomized double-blind crossover study. 7 of 15 patients showed an improved allergen tolerance to the immediate hypersensitivity with ketotifen and 5 patients with DSCG. A protective effect with respect to the delayed hypersensitivity reaction was observe in 2 of 8 patients with ketotifen and in 1 with DSCG.

Different therapies and factors influencing response to therapy in idiopathic diffuse fibrosing alveolitis.

The success of 3 different treatment regimens was compared in 15 patients with diffuse fibrosing alveolitis. Improvement or deterioration of lung function (vital capacity (VC) and PaO2 changes during exercise) were used as a control of therapy. The combination of prednisolone and azathioprine proved to be most effective: in 4 of 6 patients both VC and PaO2 improved. Combinations with D-penicillamine resulted in a high number of unwanted side effects: in 5 of 13 treated patients it had to be withdrawn. 6 patients were treated with prednisolone alone; only 1 of them improved. Patients with a short duration of symptoms prior to therapy and initially mild impairment of lung function showed a higher degree of improvement during therapy. The consequence for the management of lung fibrosis is to establish diagnosis and treatment as early as possible.

[Clinical features of chronic respiratory insufficiency].

The etiology and location of chronic respiratory insufficiency and functional differential diagnosis of this disease are briefly discussed, viz. --central hypoventilation, --neurological and muscular respiratory failure. --cardiac causes of respiratory failure (pulmo cardialis), --respiratory insufficiency due to anatomical changes of passive structures, viz.: --malformations of the thorax (fibrothorax), --airway and lung tissue diseases, --malformations and diseases of the pulmonary vascular bed. Respiratory insufficiency means abnormality of O2 and/or CO2 partial pressures for which there are no reliable clinical signs. The absence of O2 and CO2 abnormalities in the various tissues of the human body obviously does not exclude diseases of the respiratory organs.

[A new method for the measurement of regional lung ventilation].

The measurement of regional ventilation with radioactive gases (133Xe, 81Kr) is complicated and expensive. By means of a special device the authors have produced a 99mTc-aerosol with a particle size of less than 2.5 micrometer. If this aerosol is inhaled during a slow inspiratory vital capacity manoeuvre, the distribution of ventilation can also be estimated in patients with obstructive lung diseases. Comparative measurement of ventilation distribution with 133Xe and the 99mTc-aerosol in patients with severe airway obstruction demonstrates the reliability of the aerosol deposition pattern in assessing ventilated volume distribution in the lung 132 measurements in patients with clinical signs of lung emboli demonstrate the clinical applicability of the new device.

[Protective effect of ketotifen and disodium cromoglycate in bronchial challenge tests with allergens (author's transl)].

The protective effect of ketotifen and disodium cromoglycate (DSCG) was evaluated by a randomised double blind cross-over study in 15 children with allergic bronchial asthma. 13 children were allergic to house dust mite (Dermatophagoides pteronyssinus), two to grass pollen. After a positive bronchial challenge test ketotifen or DSCG was given for 4 days followed by bronchial challenge with increasing allergen concentrations. Seven patients showed better allergen tolerance after ketotifen, 5 after DSCG. Protective action against late reactions was found only in 2 children after ketotifen and one child after DSCG.

[A new method of combined inhalation and perfusion scintigraphy for the diagnosis of pulmonary embolism (author's transl)].

The combined inhalation and perfusion scintigraphy as a specific identification method of thromboembolism of the lung was tried out in 132 patients. A fine aerosol with selected droplet size was produced in a newly developed atomiser device and inhaled by taking a few deep breaths. Perfusion scintigraphy, which was performed immediately after inhalation scintigraphy, was effected with the same radioisotope, 99mTechnetiun. The disadvantages of inhalation scintigraphy, which are inherent in the method itself, were reduced to such an extent that it was possible to distinguish clearly between primary and secondary disturbances of pulmonary perfusion even in obstructive diseases of the respiratory tract, provided perfusion was disturbed in sufficiently large areas, i.e. of segment or lobe size.

Effect of theophylline on mucociliary clearance in man.

An aminophylline infusion of 6 mg/kg/body weight improves the mucociliary clearance rate in some normal subjects and infertile male. The theophylline response cannot be predicted from the blood serum level, the smoking habits, or the alcohol consumption of normal males and infertile males. The in vitro sperm-motility needs further investigation. 99mTc-tagged monodisperse erythrocytes inhaled during a slow inspiratory vital capacity with a diameter of 7.5 micrometer and a volume of 200 microliter are most suitable to get reproducable deposition patterns. The reproducability of the clearance measurements in the same subjects with this method is within the 5% range.
