Polyclonal anti-desmosine antiserum is specific to desmosine molecule: no cross-reaction to pyridinoline.

Inhibition ELISA assay was used to examine the cross-reaction of the polyclonal anti-desmosine antiserum produced in rabbit against molecules possessing a "pyridinium ring" as their core structure i.e. isodesmosine, pentasine, pyridinoline and 2'-deoxypyridinoline, highly purified with column chromatography, and structurally unrelated substances, i.e. cysteic acid, taurine and 2-aminopyridine (core structure of desmosine). No cross-reaction was observed to the pyridinoline, 2'-deoxypyridinoline possessing "pyridinium ring" and derived from collagen cross-links, structurally unrelated cysteic acid and taurine, nor core structure of 2-aminopyridine. The antiserum specifically recognized the molecules derived from the elastin cross-links. Using the ELISA assay system with antisera and amino acids analysis, 10 micrograms of desmosine were extracted from 1.0 mg of the hydrolysate of commercial elastin.

The effect of abstinence from smoking on urinary excretion of hydroxyproline.

The effect of abstinence from smoking on the urinary hydroxyproline to creatinine ratio (HOP ratio) was assessed in 49 smokers who participated in an anti-smoking course. Urine samples were collected at the beginning of the course and weekly during the subsequent 14 weeks in 1985. The subjects were divided into five groups depending on the number of cigarettes smoked daily before abstinence: 1-10, 11-20, 21-30, 31-40 and greater than 40 cigarettes. The urinary HOP ratio immediately after abstinence from smoking was proportional to the mean daily number of cigarettes smoked in the past. All groups showed decreasing HOP ratios with a longer period of abstinence. Half of the total decrease observed in the HOP ratios after 14 weeks was reached within 5 or 6 weeks. When using the Brinkman index to adjust for the number of smoking years, half of the maximum decrease in all groups was reached within 4 weeks. In an exponential decay model fitted to the data, the half-life time taken to reach the non-smokers' level was 9-10 weeks for all groups. The HOP level of smokers who smoked less than 41 cigarettes before cessation approached the HOP level of the control group after 61-74 weeks. The results suggested that the urinary HOP ratio was useful as a biochemical marker for short-term breakdown of lung collagen and lung elastin.

[A clinical survey of renal pelvic and ureteral tumors associated with bladder tumor].

A clinical survey was performed on 80 cases of renal pelvic and ureteral transitional cell carcinomas we treated between January, 1963 and December, 1986. The cases included 30 of renal pelvic tumors, 17 of ureteral tumors, 3 of renal pelvic and ureteral tumors, 7 of renal pelvic and ureteral and bladder tumors, 16 of ureteral and bladder tumors and 7 of renal pelvic or ureteral tumors after treatment for bladder tumors. There were 37 cases of bladder tumors: 7 cases with preceding bladder tumors, 23 cases of synchronous bladder tumors, and 13 cases of subsequent bladder tumors. The 5-year survival for all cases was 60.2%. The 5-year survival for 43 cases unrelated with bladder tumors was 80.5% and that for 37 cases of bladder tumor was 41.6%. Therefore, there was a significant difference between these 2 groups (p less than 0.005). The 5-year survival for 50 cases without synchronous bladder tumors at first diagnosis was significantly higher than that for 23 cases with synchronous bladder tumors (p less than 0.001). Subsequent bladder tumors occurred after 2 to 48 months (mean 10 months) of the initial treatment for renal pelvic and ureteral tumors. Six of the 7 cases of preceeding bladder tumors were superficial tumors of pTa and pT1 and 3 cases had vesicoureteral reflux.

Effects of trisodium nitrilotriacetate monohydrate, nitrilotriacetic acid and ammonium chloride on urinary bladder carcinogenesis in rats pretreated with N-bis(2-hydroxypropyl) nitrosamine.

The effects of trisodium nitrilotriacetate monohydrate (Na3 NTA.H2O) nitrilotriacetic acid (H3NTA) and ammonium chloride (NH4Cl) on two-stage urinary bladder carcinogenesis were examined. Carcinogenesis was initiated by administration of 0.2% N-bis (2-hydroxypropyl)-nitrosamine (DHPN) to male Wistar rats in the drinking water for 2 weeks, and then the animals were treated with basal diet containing Na3NTA.H2O, Na3NTA.H2O plus NH4Cl, H3NTA, H3NTA plus NH4Cl, or without these chemicals for 28 weeks. Na3NTA.H2O increased significantly the resultant incidence of neoplastic and preneoplastic lesions of the urinary bladder. Moreover, treatment with Na3NTA.H2O, without the initiation, itself induced papillary or nodular (PN)-hyperplasia. H3NTA produced only a slight increase in the incidence of preneoplastic urinary bladder lesions (PN-hyperplasia) in rats initiated by DHPN, and this was not statistically significant. Elevation of both pH and sodium ion concentration in the urine were correlated with promotion of tumor development. These data showed that Na3NTA.H2O was more effective than H3NTA with regard to promoting potential, and that changes in both urinary pH and concentration of sodium played important roles in enhancement of urinary bladder tumorigenesis by these chemicals.

Transcatheter arterial embolization of vesical artery in the treatment of invasive bladder cancer.

Transcatheter arterial embolization was performed in a total of 70 patients with invasive bladder cancer from October 1980 to December 1987. It was performed as a part of multidisciplinary treatment in patients with invasive bladder cancer. Microencapsulated mitomycin C, gelatin sponge and lipiodol (iodized oil) were used as the embolic material. As a result, reduction of tumor size was noted in 56.7%, but there was no embolic materials-related significant difference. Stage reduction was noted in 72.7% of patients where pathological stages were confirmed by operation. Hemostatic effects were noted in 76.5%. The symptoms of side effects included fever, leukocytosis, urinary frequency and pain, but none of them was severe. The results of the present study indicate that embolization of the vesical artery is useful in the treatment of bladder cancer.

A comparison of the prevalence of respiratory illnesses among nonsmoking mothers and their children in Japan and Hong Kong.

Previous epidemiologic studies have associated symptoms of chronic bronchitis and other respiratory diseases with the risk for lung cancer. To assess the possible precursor or premonitory role of these conditions for lung cancer among nonsmokers, a comparison of the prevalence rates of these conditions in 2 urban industrialized communities (Hong Kong and a Tokyo suburb) with a 300% difference in female lung cancer incidence rates was conducted. A community survey of 314 nonsmoking mothers and their children in Hong Kong, and 243 mothers and children in Japan showed that the prevalence of reported chronic cough and sputum symptoms was 10 or more times higher in Hong Kong than in Japan. The disparity in the rates of respiratory diseases/symptoms was most apparent in the comparison of children. Occupational exposure to dust or fumes and larger household sizes were found to be associated with higher levels of respiratory illnesses among the Hong Kong mothers. The much higher prevalence rates of respiratory symptoms among Hong Kong than among Japanese subjects correlated with each community's female lung cancer incidence rates of 27.1 versus 8.1/100,000, respectively.

[A clinical study of testicular tumors].

Thirty-one patients with testicular germ cell tumors were treated in our Department between January, 1979 and June, 1987. Of the 31 patients, 17 (54.8%) had seminoma and 14 (45.2%) had non-seminomatous germ cell tumor (NSGCT) histopathologically. Clinical stage of seminoma was stage I in 13 patients (76.5%) and stage II in 4 patients (23.5%). Clinical stage of NSGCT was stage I in 8 patients (57.2%), stage II in 1 patient (7.1%) and stage III in 5 patients (35.7%). The patients with stage II seminoma underwent high inguinal orchiectomy, radiation therapy, retroperitoneal lymphnode dissection (RPLND) and chemotherapy, and those with stage II and III NSGCT underwent high inguinal orchiectomy and chemotherapy. All patients with stage II seminoma are alive with no evidence of disease. Three patients with stage II and III NSGCT are alive with no evidence of disease, but 3 patients died of the disease. Survival of patients with stage I and II seminoma at 5 years was 100% Survival of stage I and II NSGCT and III NSGCT at 5 years was 100% and 40%, respectively. According to our experience, for patients with advanced seminoma, combination chemotherapy and RPLND as well as radiation therapy are effective. In addition, for patients with advanced NSCGT, adjuvant chemotherapy followed by RPLND is effective.

Effect of 1-hexylcarbamoyl-5-fluorouracil on development of rat urinary bladder tumor induced by N-butyl-N-(4-hydroxybutyl)nitrosamine.

The present study was conducted to examine the effect of 1-hexylcarbamoyl-5-fluorouracil (HCFU) on the development of urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) in rats. One hundred twenty male Fischer 344 rats were divided into 6 groups. Sixty rats (groups 1, 2 and 3) were given 0.05% BBN in drinking water for the initial 8 weeks of the experiment and another 60 rats (groups 4, 5 and 6) served as controls. HCFU mixed in the powder diet was administered at daily doses of 100 mg per kg of body weight. Group 2 received HCFU throughout and group 3 after BBN administration for 8 weeks. All animals were sacrificed at 20 weeks from the beginning of the experiment and examined histopathologically. In groups 1, 2 and 3, urinary bladder tumors developed in 13 of 18, 0 of 20, and 4 of 19, respectively. Incidences of tumors in the 2 groups treated with HCFU were significantly lower than that of group 1 treated with BBN alone. These results indicated that HCFU was effective in suppressing the incidence of bladder tumor.

Experience with an enzyme-linked immuno sorbent assay for the quantitation of urinary desmosine.

An enzyme-linked immuno sorbent assay has been set up for the quantitation of the elastin crosslink desmosine. With the assay desmosine could be detected in the range 0.01-10 ng (the amount is corresponding to 0.4-400 ng/ml) in standard solutions. One hundred percent crossreactivity with isodesmosine was found. The low titer rabbit antiserum used in this inhibition immunoassay was less suitable for the measurement of desmosine in urinary hydrolysates, because a number of unknown urinary substances interfered with the measurement. Interfering substances could not be removed completely with a column purification method. Future research is needed to determine the applicability of the higher titer anti-desmosine antiserum from a second rabbit.

Effects of single chemotherapeutic agents on development of urinary bladder tumor induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) in rats.

Chemotherapeutic agents were evaluated for effect on the development of urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) in male Wistar strain rats. Seven hundred and two rats were given 0.05% BBN in drinking water for 8 weeks. After BBN treatment, the animals were divided into 26 groups to follow regimens of single chemotherapy. All drugs were administered intraperitoneally except in one group that was treated orally. In our experimental series, 5-fluorouracil (5-FU), N-(2-tetrahydrofuryl)-5-fluorouracil (FT-207), carbazilquinone (CQ), vincristine (VCR) and cis-diamminedichloroplatinum (CDDP) were effective in inhibiting the incidence of bladder tumor, however, adriamycin (ADM), mitomycin C (MMC), neocarsinostatin (NCS), cyclophosphamide (CPM) and bleomycin (BLM) were not effective.

Determinants of indoor NO2-concentration and the personal exposures of children and their mothers in Japanese homes.

In February and in July 1986 the personal exposures to NO2 of 60 children and their mothers were measured and NO2-concentrations in their homes were measured. In the summer also the NO2-concentration in the kitchen was determined during the time that the mother was in the kitchen. The mothers were asked to keep a diary of the activities of her and her child during that day and of the time that the heating appliances, cooking range, etc. were used. In the winter indoor NO2-concentrations and personal exposures of mother and child were much higher than in the summer. In the winter in many cases the Japanese Ambient Air Quality Standard is exceeded. The elevated levels in the winter can be related to the use of NO2-producing heating appliances especially the gas- or oilstove without a flue. Appliance of a flue still causes the indoor NO2-concentrations to be elevated. The division of a study population into two exposure categories on the basis of presence of a non-vented gasstove is therefore inappropriate. An instantaneous gas fired water heater may also contribute to elevated indoor NO2-concentrations and personal exposures, although the present survey could not provide detailed information about the use of a flue and the location of the geyser. The time that a gasrange is being used may influence the indoor NO2-concentrations, but the results from this survey were not clear. Ventilation in the kitchen reduces the indoor NO2-concentrations, although this seems especially true in case the gas range is being used for a long time. In houses with smokers higher NO2-concentrations were found than in houses with non smokers. In the winter the non-vented gas stove is the most important indoor NO2-source. The present study was not able to demonstrate the effect of outdoor NO2-concentrations on the indoor air concentrations. A longer time span would be necessary. The housestructure, i.e. the construction of the house and the number of rooms influence the indoor NO2-concentrations and personal exposure. In the winter all indoor NO2-concentrations are highly intercorrelated and also highly correlated with the personal exposures of mother and child. Multiple regression models based on either measured or time-weighted NO2-concentrations explain about 85 percent of the variance in the personal exposures of child and mother in the winter. The exposures calculated from the time-weighted NO2-concentrations correlate very well with the measured personal exposures. This was not the case in the summer.(ABSTRACT TRUNCATED AT 400 WORDS)

Impact of smoking on the concentration and activity of alpha-1-antitrypsin in serum in relation to the urinary excretion of hydroxyproline.

The impact of active and passive smoking on the serum levels of alpha 1-AT, the trypsin inhibitory capacity (TIC), the trypsin inhibitory activity (TIA) and the urinary hydroxyproline to creatinine ratio (HOP-ratio) was studied. The subjects used in the study on active smoking were 167 healthy adult men and in the study on passive smoking 189 healthy primary school children. Serum levels of alpha 1-AT in active smokers were significantly higher than those in non-smokers. The TIC as well as the TIA in active smokers decreased with increasing number of cigarettes smoked. The urinary HOP-ratio increased significantly with increasing number of cigarettes smoked. On the other hand, in the case of passive smokers a significant difference was obtained only for the HOP-ratio. The correlations between all markers in active smokers were significant. Less significant correlations were found in the case of passive smokers. These results suggest that the urinary excretion of hydroxyproline can be considered as a marker for the imbalance between proteases and anti-proteases as a result of smoking.

[Behavior of urinary hydroxyproline and the effect of cigarette smoking in silicosis].

This study was conducted through regular pneumonoconiosis examination according to the law on 1,096 employees of medium and small-sized ceramic enterprises in Tokai district in 1981-82. Interview examination with BMRC questionnaire, X-ray examination and measurement of urinary hydroxyproline to creatinine ratio (HOP ratio) were carried out in order to elucidate the relationship between silicosis and urinary HOP ratio and to demonstrate the effect of smoking on pneumofibrosis. Grade of silicosis was classified into five types (0 to 4) based on the Japanese Classification of Radiographs of Pneumoconioses. In evaluating the behavior of urinary HOP ratio, when smoking factor is added in the early grade of pneumofibrosis (type 1 and type 2), collagen decomposition rate is rapidly repressed and fibroplastic conditions develop to the final grade as type 3 and 4, although smoking itself does not seem to induce pneumofibrosis. To exclude the effects of smoking, nonsmoking group was used for measurement of HOP ratio by grade. The HOP ratio in type 0 was lowest and HOP ratio increased in the order of type 1 and type 2. The turning point was found in type 2 and their HOP ratio decreased one after another. The turning point shifted from type 2 to type 3 in the case of non-smokers without any index symptoms by BMRC questionnaire and also shifted to type 1, in the case of non-smokers with them. Shifting of turning point suggests that index symptoms also promote fibroplastic activities.

Personal exposure to nitrogen dioxide and its relation to indoor and outdoor air quality.

An estimating equation of annual geometric average of personal NO2 exposure was proposed. The average could be calculated from three variables, that is, personal NO2 exposures in one winter and one summer month and ratio of winter period in a year. Ten housewives living in Suginami Ward, a residential area in Tokyo, were chosen as subjects of this study. They were asked to measure daily averages of the personal NO2 exposures with a filter badge. Measurements were performed on consecutive seven days in every month from January to December of 1982 for a total of 84 samples per person. Their activities, the amount of fuel consumption and other living conditions were recorded at the same time. Outdoor NO2 concentrations and maximum, minimum and average temperatures were also measured at the center of their living area. The personal NO2 exposure and its seasonal variation were found to depend mainly on the type of space heaters, frequency of ventilation fan usage in a kitchen, and the amount of fuel consumption.
