A comparative study on the health effects of smoking and indoor air pollution in summer and winter.

This study was performed by a comparison between summer and winter for the purpose of demonstration the actual conditions of health effects of indoor air pollution with special reference to NO2 and smoking, on the subjects composed of 820 school children and their 546 mothers in the two areas with different ambient NO2 concentrations. In either case, examination was carried out with standardized questionnaire test for respiratory symptoms, personal NO2 exposure measurement using the filter badge by Yanagisawa, and analysis of urinary hydroxyproline and creatinine in two areas with different ambient NO2 levels. Personal NO2 exposure level in winter season was 2-3 times higher than that in summer, particularly NO2 level among residents living in homes with non-vented stove for space heating was substantially higher from those of residents with vented stove. Wives with vented stove had a moderate exposure level in winter season by the contribution of NO2 originated from the kitchen and poor ventilation rate. Since the hydroxyproline to creatinine ratio (HOP-ratio) of children increased more, their household location were nearer to any heavy traffic roads in summer, health effects from automobile exhaust were suggested only in summer season. In summer season, personal NO2 exposure level were almost the same with the ambient NO2 concentrations over both areas. These results suggest that indoor air pollution in winter season may be separated from outdoor air pollution. It was a matter of course that hydroxyproline to creatinine ratio in winter season was higher than that in summer, in any group and in any area, but the range of variation of hydroxyproline to creatinine ratio was smaller by far than that of personal NO2 exposure level. Judging from urinary hydroxyproline to creatinine ratio, health effects of active smoking and passive smoking increased with increasing the number of smoked, dose-dependently in any season. According to stepwise multiple regression analysis, hydroxyproline to creatinine ratio had significant relation to either NO2 and active or passive smoking, but personal NO2 exposure level had no relation to cigarette smoking. NO2 and cigarette smoke were seemed to make hydroxyproline to creatinine ratio increase, independently each other.

[Effect of UFT on urinary bladder tumors in rats induced by N-butyl-N-(4-hydroxybutyl)nitrosamine].

The effect of combined tegafur and uracil (UFT) on the development of rat urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) was studied. Two hundred F344 male rats were divided into 10 groups. Groups 1 to 5 were given 0.05% BBN in drinking water for the initial 8 weeks of the experiment, and Groups 6 to 10 were the controls of the prior 5 groups treated with BBN. UFT in commercial diet was administered at daily doses of 100mg (30.9 mg as FT-207) per kg of body weight in Groups 2 and 4 and 200 mg (61.7 mg as FT-207) per kg of body weight in Groups 3 and 5. Groups 2 and 3 received UFT throughout the period of the experiment, and Groups 4 and 5 for 12 weeks after 8 week treatment with BBN. All animals were sacrificed at 20 weeks, and studied histopathologically. In Groups 1 to 5, urinary bladder tumors developed in 20 of 20, 13 of 20, 6 of 20, 14 of 20 and 6 of 20, respectively. Incidences of tumors in the 4 groups treated with UFT were significantly lower than that in Group 1 treated with BBN alone. This result shows that UFT inhibits the development of urinary bladder tumors in rats induced by BBN.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. I: On the effects of intravesical instillation of cis-diamminedichloroplatinum in normal beagle dogs].

Cis-Diamminedichloroplatinum (II) (DDP) is one of the most effective agents in the treatments of advanced bladder cancer, but little is known of its local and systemic toxicity when it is instilled intravesically. The objectives of this experiment were to study the local and systemic toxicity of intravesical DDP in order to know whether intravesical DDP is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six female Beagle dogs were used for the 48-hour retention study and 2 mongrel dogs were used for the 3-hour study. Plasma and bladder platinum concentration were measured following intravesical DDP, and also histopathological examination, urinalysis, complete blood count and blood chemistry were performed in order to know the toxicity of intravesical DDP. Plasma platinum concentration was not elevated significantly and no remarkable changes were observed in blood examinations after intravesical DDP. Bladder tissue platinum concentration was significantly elevated up to 16.96 micrograms/g . wet after 3 hours DDP retention and 4.02 +/- 2.8 micrograms/g . wet after 48 hours retention in dogs. At present, intravesical instillation of DDP seems to have minimum local toxicity and no systemic toxicity, but further study is necessary for long term topical instillation of DDP.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. II. On the effects of intravesical instillation of cis-diamminedichloroplatinum in bladder cancer patients].

This study was performed to determine whether intravesical Cis-Diamminedichloroplatinum (II) (DDP), one of the most effective agents in the treatments of advanced bladder cancer, is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six patients were instilled 10 mg/20 ml of DDP immediately after TUR-Bt, but no remarkable elevation of plasma platinum concentration was observed. In one patient planned to have total cystectomy, 20 mg/40 ml of DDP was instilled daily for 10 days. No remarkable changes in symptom and blood examination was found during the instillation periods. Transitional cell carcinoma of grade 2 was necrosed extensively, but minimum cellular injury was observed in normal mucosa. Tissue platinum concentration was 3.19 micrograms/g. wet in carcinoma and 0.28 microgram/g. wet in normal mucosa. Longterm prophylactic instillation of 20 mg/40 ml of DDP was performed in 16 patients starting immediately after TUR-Bt. As the side effects of DDP instillation, contracted bladder was observed in one patient and allergic reactions, such as flush of the skin and dyspnea, were observed in 4 cases. These allergic reactions were happened suddenly after 15 approximately 19 times of DDP instillation without any prelude. At present, intravesical instillation of DDP seems to cause minimal irritation of bladder and absorption through the bladder wall, but clinical trials of intravesical DDP should be taken under careful observation until the allergic reaction by intravesical DDP are clarified.

Relationship between housing environment and allergic symptoms of children using ATS-DLD questionnaires.

Using pupils in three areas with different environmental factors (Suginami-ku, Tokyo: Aikawa-machi, Kanagawa prefecture: and Shimizu-shi, Shizuoka prefecture) as subjects, respiratory symptoms and domestic environment were surveyed by meams of the American Thoratic Society-Division of Lung Disease (ATS-DLD) questionnaires together with measurement of total immunoglobulin E (IgE) by the radioimmunosorbent test (RIST) and specific mite and cedar IgE by the radioallergosorbent test (RAST), as well as the relationship between "allergic symptoms" as designated in the ATS-DLD questionnaire and housing structure. The following knowledge was obtained. According to the ATS-DLD standardized questionnaires for prevalence of respiratory symptoms and allergic symptoms, Aikawa showed a significantly low value of 8.5% (p less than 0.001) compared with 15.8% for Suginami and 15.7% for Shimizu, referring to allergic rhinitis only. The positive rate of 300 U/ml and over of total IgE was 55.5% in Suginami, 49.1% in Shimizu and 40.4% in Aikawa. Aikawa showed the lowest the positive rate of total IgE in allergic symptoms. The housing was classified into three types according to the original structure each of which had characteristic ventilation rates. The positive rates according to the housing structure showed no differences by areas for Type A (wooden house) with good ventilation, but for Types B (mortar frame house) and C (steel reinforced frame house) with airtight structures, the rates increased in the order of Aikawa less than Shimizu less than Suginami, and a significant difference was observed between Aikawa and Suginami (p less than 0.01). However, in the same area no difference was noted between Type A and Types B and C. No difference in the positive rate of allergic experiences by housing structure, was noted among areas and housing structures in the same area for the group not experiencing allergic symptoms (A. Inexp. Group), while no difference was noted for Type A but the rates were high in the order of 60.0% in Aikawa, 65.2% in Shimizu and 75.3% in Suginami for Types B and C in the group experiencing allergic symptoms (A. Exp. Group). Furthermore, in the same area when the Type A was compared with Types B and C, the latter showed a higher positive rate than the formar in all three areas, with a significant difference (p less than 0.05) noted in Suginami.

[A case of congenital mesoblastic nephroma growing with a polypoid pattern in the renal pelvis and ureter].

Congenital mesoblastic nephroma (CMN) is an unusual renal tumor, similar in some respects to Wilms' tumor and reported only in infants. We report a case of CMN in a 2-year-old boy. He was referred to us with a right abdominal mass. Excretory urogram, arteriogram and computed tomogram revealed a large malignant renal tumor on the right side. With a clinical diagnosis of Wilms' tumor of the right kidney, he received radiation and chemotherapy. After radiation and chemotherapy, tumor size was decreased. Right nephrectomy was performed on April 22, 1982. The kidney and tumor weighed 270 g and measured 12 X 7 X 5 cm. The tumor extended to the renal pelvis and the ureter with polypoid growth pattern. Microscopically, the tumor was composed of compact interlacing bundles of elongated fibromyomatous cells. Islands of entrapped glomeruli and tubules were also detected within the tumor. He was well and had no signs of recurrence of tumor at the age of 3 years.

[A case of recurrent cyst of Müllerian duct].

A case of recurrent cyst of Müllerian duct is reported. The patient had complained of dysuria and underwent resection of Müllerian duct cyst in May, 1977. Five years later (in July, 1982) at 50 years old, he again visited our clinic with the complaint of urinary retention and perineal mass formation. Recurrent multilocular cyst of Müllerian duct was diagnosed by CT and transrectal ultrasonotomography. Transabdominal extirpation of Müllerian duct cyst, combined with perineal approach, was performed under general anesthesia. The cyst was multilobular, 165 g in weight and 17 X 6.5 X 4 cm in size. Transparent yellow fluid of the cyst was about 2,000 ml in volume. Histologically it was fibroadenomatous cyst. The differential diagnosis and surgical treatment of Müllerian duct cyst is discussed.

An improved method for analysis of urinary hydroxyproline by an automated analyzer.

An improved method of analysis has been devised for estimating the amount of hydroxyproline excreted into the urine. This study has been performed to overcome the difficulties accompaning mass-examinations which are indispensable in epidimiological studies. Urine samples are pretreated with a resin-charcoal mixture for elimination of interfering substances or color inhibitions, and the supernatant is treated by evaporation to avoid complications followed by pH adjustment. The residue is reconstituted with diluted HCL. After pretreatment, samples a analyzed with an automated analyzer. Linearity of standard calibration curve, and recovery and reproducibility of this assay are satisfactory. This simple procedure of analysis requires little time. This method is also useful for routine clinical examinations.

Rapid induction of carcinoma in situ in dog urinary bladder by sequential treatment with N-methyl-N'-nitrosourea and N-butyl-N-(4-hydroxybutyl)-nitrosamine.

A highly atypical intraepithelial proliferation, which was interpreted as carcinoma in situ, developed in the urinary bladder of 2 beagle dogs after repeated intravesical instillations of N-methyl-N'-nitrosourea followed by oral administration of N-butyl-N-(4-hydroxybutyl)nitrosamine. The latent period was 37 weeks. The technique may provide a useful model to study the natural history of bladder cancer, particularly carcinoma in situ.

[Prepenile scrotum: report of a case].

A case of prepenile scrotum associated with hypospadias and syndactyly is reported. The patient was a full-termed first baby of the 25-year-old mother who had no history of congenital malformation. The clinical laboratory study including chromosomal check and I.V.P. revealed no abnormalities. Two-step operative repair was performed scrotoplasty at 2 years old and urethroplasty at 4 years old. As far as we have reviewed, 18 reports of this malformation have been published in Japan.

Epidemiological study on the hydroxyproline: creatinine ratio as an index of growth rate.

A number of studies has suggested that urinary hydroxyproline can be used as an index of collagen metabolism and urinary hydroxyproline excretion is closely related to growth hormone. This report is a cohort study to demonstrate the relationship between the urinary hydroxyproline: creatinine ratio and growth rate for four years. The pattern of increased urinary hydroxyproline: creatinine ratio in adolescence appeared to be closely ralated to the general pattern of growth rate measured in school children. In has been shown that the urinary hydroxyproline: creatinine ratio is the most suitable index of the growth rate.
