Chronic bronchobronchiolitis obliterans associated with Stevens-Johnson syndrome.

We report a case of bronchobronchiolitis obliterans associated with Stevens-Johnson syndrome diagnosed using bronchography and bronchoscopy in a 25-year-old man with dyspnea. Eighteen months prior to admission, the diagnosis of Stevens-Johnson syndrome due to anticonvulsant therapy was made at another hospital. On admission, the patient was wheezing and had overinflation of the lungs, with marked obstructive pulmonary dysfunction. Ventilation and perfusion scintigrams revealed multiple ventilation and perfusion defects but not mismatch. Bronchography revealed central bronchiectasis and obstruction of the peripheral bronchi. Occlusion of the fifth right basal bronchus was observed with fiberoptic bronchoscopy. Bronchography and bronchoscopy provide useful information for the diagnosis of this disease.

Production and properties of mouse monoclonal anti-adriamycin antibody.

Eight mouse IgG1 monoclonal anti-Adriamycin antibodies were produced in culture and in ascites in BALB/c nude mice. The binding constant and specificity was measured by an inhibition ELISA method. The assay was an indirect method using horseradish peroxidase-labeled goat Fab' antibody and 2mM ABTS (2,2'-azino-bis (3-ethylbenzthiazoline-6-sulfonic acid) containing 2.5 mM H2O2 as a color reaction reagent. The binding constant of the antibodies was 10(6) to 10(8) M-1. The antibodies possessed different specificities and affinities to the adriamycin. Though adriamycin is a low molecular weight (M.W. 579.99) hapten antigen, it possesses several epitopes as antigenic determinant sites. The antigenic epitopes of the adriamycin, which until recently were unknown, were analyzed by the monoclonal antibodies.

[A chronic case of summer type hypersensitivity pneumonitis].

A 59-year-old woman complaining of progressive dyspnea on exertion was admitted to our hospital in August 1992. In 1983 she had experienced exertional dyspnea from August to November, then improved spontaneously. In August 1984, she was admitted to our hospital due to the recurrence of the symptom. She was diagnosed as summer type hypersensitivity pneumonitis based on the following findings, 1) detection of non-caseous granuloma by transbronchial lung biopsy, 2) detection of precipitating antibody to Trichosporon cutaneum in the serum, 3) induction of the symptom by provocation test with the fungi, 4) isolation of the fungi from her house. In spite of moving from her house after discharge, she continued to visit her former house once a week to clean it. During the period, fibrotic changes on the chest roentgenogram were slowly progressive. In August 1992, ten years after the onset, she was re-admitted to our hospital with a complaint of exacerbated dyspnea on exertion. Open lung biopsy revealed granulomatous lesions with fibrotic changes, which was compatible with chronic type hypersensitivity pneumonitis. This patient was diagnosed as a chronic case of summer type hypersensitivity pneumonitis because of the clinical course and findings of open lung biopsy as well as laboratory data at the first admission. We consider that this case offer various information in considering the mechanism of chronic progress of summer type hypersensitivity pneumonitis.

[Postoperative pelvic lymphocele: demonstration by lymphoscintigraphy].

Eleven patients who had 19 postoperative lymphocele were examined by lymphoscintigraphy using 99mTc human serum albumin (99mTc-HSA). Fourteen postoperative lymphocele were demonstrated, and 5 lymphocele were not. All lesions of 2 cm or greater in short diameter on computed tomography were detected by lymphoscintigraphy, and detectability was not achieved for lesions less than 2 cm in short diameter. The lymphocele can be divided into two groups according to the site of occurrence, the external and internal groups. But we have not detected any characteristic pattern to differentiate the two groups. And delayed scan was more useful in the detection of postoperative lymphocele than early scan.

The technical problems shoot for the production of anti low molecular weight hapten antibody.

Four kinds of small molecular weight haptens, Desmosine, Adriamycin, 4'-carboxycotinine and 5'-Fluorouracil were used as the immunizing antigens to obtain rabbits polyclonal antisera. All of these haptenes were conjugated to Bovine Serum Albumin (BSA) using 1-ethyl-3(3-dimethylaminopropyl)-carbodiimide (EDCl) reagent. The functional amino or/and carboxyl group in hapten molecules was utilized for conjugation with BSA by EDCl reagent. The hapten conjugated with BSA was mixed with Freund's Complete or Incomplete Adjuvant, and immunized to rabbits abdominal subcutaneous regions. More than 3 times immunized rabbit's antisera were evaluated the titer, and the specificity was investigated by ELISA method. The specific antisera against desmosine and adriamycin were produced. Anti-desmosine antibodies had no cross-reaction to molecular structure resemble pyridinoline. Anti-adriamycin antibodies reacted well to adriamycin and it could distinguish the differences of -epi type of adriamycin. The ELISA assay systems used here had such sensitivity. However the specificity possessing antisera against 4'-carboxycotinine and 5-Fluorouracil could not produce even though the antisera titers were elevated. The reasons why and what kinds of problems were there are discussed in this paper. For the production of the specific affinity possessing antisera, the length and distance between the carrier protein and the hapten and the shape of the functional groups were the important factors for production of the specific antisera which recognize free form hapten molecule.(ABSTRACT TRUNCATED AT 250 WORDS)

[An epidemiological study of allergic conjunctivitis].

An epidemiological study focusing on the home environment and lifestyle was conducted in order to analyze the differences between a group of patients with allergic conjunctivitis only and a group with other allergic diseases associated with allergic conjunctivitis. Subjects were five hundred and sixty-four patients who visited the Ophthalmologic Clinic of Tokai University Hospital during 1990, and for controls 200 households were randomly selected from an elementary school list in Yokohama city. RAST score was measured in 102 patients. A mailed questionnaire was sent to 240 patients chosen at random and to 200 controls. Sixty patients and 139 controls responded to the questionnaire. RAST score showed no statistical difference between patients with allergic conjunctivitis only and patients with other allergic diseases associated with allergic conjunctivitis. Analysis of associations between RAST score and seasonal symptoms, showed very strong positive reactions to pollens in patients with seasonal symptoms. On the other hand, in patients with constant symptoms during the year, reaction was very strong to house dust and mite. Epidemiological factors causally associated with allergic conjunctivitis were compared among the controls, patients with allergic conjunctivitis only (Group I), patients with other allergic diseases (Group II) and all patients (Group III). Compared to controls, Groups II and III had significantly higher prevalence of past history of skin itching and inflammatory symptoms (p < 0.05 and p < 0.01, respectively). Group II subjects showed more frequent use of carpets, when compared to the controls (p < 0.05). The use of Japanese mattress (futon) was more frequent in Group II and III than controls (p < 0.05 for both comparisons).(ABSTRACT TRUNCATED AT 250 WORDS)

Wood stove effects on indoor air quality in Brazilian homes: carcinogens, suspended particulate matter, and nitrogen dioxide analysis.

The effects of wood burning stoves on indoor air quality was investigated in a rural community of southern Brazil, during the winter season of 1991. The concentrations of polycyclic aromatic hydrocarbons (PAHs), nitrogen dioxide (NO2) and suspended particulate matter (SPM) were assessed in houses with wood stoves and the results compared with levels found in houses with gas stoves. Strikingly higher (p < 0.01) levels of PAHs, and much higher (p = 0.07) levels of SPM were found in the kitchens with wood stoves. In contrast, NO2 concentrations in the kitchen as well in personal exposure, were found to be slightly higher in houses with gas stoves. All these differences were minimally affected by smoking, outdoor air pollution or other emissions from indoor combustion products. These findings appear to support the hypothesis that domestic wood burning stoves are risk factors for some upper digestive and respiratory tract cancers in Brazil.

Neoadjuvant therapy for locally invasive bladder cancer: results of randomized trials in 40 patients.

The present investigation was conducted to examine the effect of a neoadjuvant cyclophosphamide, doxorubicin and cisplatin (CAP) regimen with radiotherapy for locally invasive bladder cancer as a well-controlled randomized trial. Since 1986, a total of 40 patients with primary transitional cell carcinoma of the urinary bladder have been randomized into two groups: neoadjuvant CAP plus radiation-treated group and control group. Of 18 patients who received neoadjuvant chemotherapy, complete and partial responses were observed in 52.9% of 17 measurable and evaluable patients and downstaging was observed in 92.9% of 14 evaluable patients. The 3-year survival rates of the neoadjuvant-treated and control group were 93.8 and 83.6%, respectively. No statistical significance was achieved in the survival rates. These results indicated that neoadjuvant CAP would be useful in the management of invasive bladder cancer.

Personal exposure to nitrogen dioxide and its association with respiratory illness in Hong Kong.

In 1985, 362 primary schoolchildren and their 319 mothers were surveyed in Hong Kong to study the possible relationship of air pollution to respiratory illnesses. Using nitrogen dioxide (NO2) measured by personal samplers as a measure of air pollution, the study aimed to identify the major sources of NO2 in the indoor environment and see whether its increased presence was associated with respiratory symptoms. The levels of NO2 among the mothers was found to increase by 21% if dust exposure was reported from the workplace, 18% if they used such cooking fuels as liquid petroleum gas or kerosene, 11% when kitchens did not have ventilating fans, and 10% when incense was burned at home. In terms of respiratory symptoms, an increase in NO2 levels of 19% was reported among those with allergic rhinitis and 18% among those with chronic cough. The levels of NO2 among children were correlated with levels measured in classrooms, all of which had opened windows so that the NO2 came from outdoors. No association was found between children's NO2 levels and respiratory symptoms. With the exception of smoking by the father and the children's NO2 levels, no association was found between smoking at home and NO2 levels.

Production of monoclonal antibody against human prostatic adenocarcinoma and its immunohistochemical properties.

We have generated a monoclonal antibody against prostatic adenocarcinoma, PC-Ab (IgM) was derived from a fusion using the fresh prostatic tissue of adenocarcinoma as the immunogen. Initial screening was performed by enzyme-linked immunosorbent assay (ELISA) on the soluble fraction of the immunogen. The specific analysis was performed by the avidin-biotin-complex immunoperoxidase method on paraffin-embedded sections of normal, benign and malignant neoplastic tissues from the prostate and various organs. PC-Ab reacted with well differentiated adenocarcinoma (83.3%), moderately differentiated adenocarcinoma (92.2%), and poorly differentiated adenocarcinoma (90.2%), respectively. According to classification by stage, the reaction rates with stage T0, T1, T2, T3 and T4 were 77.3%, 80.0%, 95.2%, 91.3% and 92.9%, respectively but no significant differences in the stages were seen among these groups. PC-Ab reacted with the epithelium of the normal prostate and benign prostatic adenoma, and human fetal tissues. Molecular weight and isoelectric point of the antigen recognized by this PC-Ab was estimated to be 57,000 daltons and 7.0, respectively. These results indicate that PC-Ab reacts with the antigen associated with human prostatic adenocarcinoma.

Guanine levels in house dust as a means of estimating the mite population.

House dust has recently been incriminated as an important causative allergen in bronchial asthma and allergic rhinitis. In particular attention is being focused on the mites contained in house dust. Previously, the number of mites contained in house dust was determined by counting under a microscope. This method is not appropriate, however, for epidemiological investigations which require large numbers of specimens, as it is time-consuming and labor intensive. Guanine makes up about 0.3% of the body weight of mites and is present in large amounts in mite excrement, and by using an improved method for quantifying guanine in house dust we obtained a correlation coefficient of 0.776 between the number of mites and amount of guanine. In addition, because virtually no guanine is contained in human nails and hair, thus eliminating possible sources of error, it became clear that the amount of guanine in house dust can be used to estimate the total amount of mite allergen resulting from the bodies and excrement of the acarids.

Epidemiological study of mites and guanine in house dust.

The total number of mites, and the amount of guanine contained in house dust was determined in samples obtained from homes with and without asthmatic patients in Yokohama City. The data were quantified and their possible association with various factors within the home was examined. The largest number of mites and the greatest amount of guanine were most strongly associated with carpet floor coverings, followed by tatami mats, and then by wooden flooring. A multi-variate analysis revealed that the major factors influencing the number of mites and the amount of guanine in homes were the living room flooring, and the age of the house. Correlation to such living factors ranged from 60 to 70 percent. In the families with asthmatics the frequency of cleaning was an important additional factor. This likely was a result of the strict instructions given by doctors to asthmatic patients. It was shown that the amount of guanine contained in house dust is an indicator of not only the number of mites but also of the amount of allergens contained in home dust.

[Local hyperthermia using a device made of temperature-sensitive ferrite. The first report: temperature, output characteristics].

The basic study of intracavitary and interstitial hyperthermia was made with the Soft-Heating Method. In this method, a hybrid device composed of non-magnetic metal ring and ferrite rod is used as a heater. Being exposed to external magnetic field, the device produces heat, and its temperature rises to the Curie Temperature of the ferrite rod with self-regulation. The accuracy of the regulation is +/- 0.2 degrees C near the Curie Temperature which can be set up almost freely. In addition to that, the frequency of magnetic field is not so high (20-50 KHz) that there is no damping nor reflection of the field in a human body. Therefore the system seems to be very useful for hyperthermia of deep seated tumor. In this paper, experimental discussion of the possibility of local hyperthermia by the system is made. From the results, it is concluded that the system will be applied to clinical uses.

Promoting effect of 2,4-diaminoanisole sulfate on rat thyroid carcinogenesis.

Two experiments were conducted to examine the effects of 2,4-diaminoanisole sulfate (2,4-DAAS) on thyroid function and carcinogenesis in male Wistar rats. In experiment 1, feeding with 2,4-DAAS resulted in significantly elevated levels of thyroid stimulating hormone (TSH), reduced thyroxine (T4) and triiodothyronine (T3) in the serum, although the latter had almost recovered to normal levels by week 6. However, skin application did not affect serum levels. In experiment 2, administration of 0.5% 2,4-DAAS in the diet for 19 weeks, a week after a single 210 mg/100 g body weight i.p. injection of N-bis(2-hydroxypropyl)nitrosamine (DHPN), significantly increased the incidence and numbers of preneoplastic lesions (focal hyperplasia), adenomas and carcinomas developing in the thyroid gland. Histologically, brown pigment was usually observed within follicular epithelial cells in non-tumorous regions, but not in the tumors themselves.

[An epidemiological study on the relationships among HDL-cholesterol, smoking and obesity].

The relationships among high-density lipoprotein cholesterol (HDL-C) and other serum lipid items, and active smoking, passive smoking, obesity, alcohol drinking and working status were investigated. This study was performed both in healthy adults and in schoolchildren. The adult group was sampled at random from those who were diagnosed to be healthy by medical examination at the Automated Multiphasic Health Testing and Services Center in Tokai University Hospital, and the schoolchildren group was selected with regular health examinations in a primary school in Tokyo. In the case of adults, it was found that the greater the value of triglyceride (TG), the greater the number of cigarettes smoked. On the other hand, HDL-C decreased with increasing cigarette smoking. Obesity level increased with increasing values of TG, total cholesterol (TC) and low density lipoprotein (LDL-C) but in the case of HDL-C, the value decreased. Alcohol drinking made the HDL-C level increase. Although the HDL-C level increased slightly with obesity, the relationship between HDL-C and passive smoking was not clear in the case of schoolchildren.

Effects of health of automobile exhaust and environmental tobacco smoke (ETS) in areas alongside main road.

During a period of eight years from 1977, pupils in F primary school in S ward, Tokyo were monitored each year with a general medical check-up to identify and eliminate any children with urine abnormalities and then the hydroxyproline: creatinine ratio (HOP ratio) in the urine of 4,375 children chosen at random was measured. The children's area of residence was classified, according to distance, into three areas. In addition, the children were classified into four groups depending on their estimated degree of exposure to Environmental Tobacco Smoke (ETS). The relationship between ETS and the HOP ratio was observed annually. The HOP ratio was greatest in schoolchildren living in the area nearest the main roads. The influence of ETS also tended to increase when ventilation rates in their rooms were lowered. The strength of these associations was considered significant depending on the magnitude of the HOP ratio, the correlation coefficient, a two-way layout and relative risk (R.R.). In addition, the consistency and coherence observed in measurements each year suggested a causal relationship. The influence of ETS and area distinctions on the HOP ratio was totally independent and showed virtually no interaction. However, the changes in patterns of indoor smoking from 1980 onwards were not sufficiently accounted for by the previous interview and questionnaire surveys so we introduced an additional bias, namely misclassification of pupils. Subsequently, the possibility of underestimation of the influence of ETS was considered, thereby establishing the need for a revision of the relevant questionnaires.

An enzyme-linked immunosorbent assay (ELISA) for the quantitation of urinary desmosine.

An enzyme-linked immunosorbent assay (ELISA) method has been developed for the quantitation of the elastin cross-link desmosine in urine. The system employs Rabbit antisera directed to the conjugate of desmosine and bovine serum albumin which was conjugated using carbodiimide reagent. The ELISA was done in microtiter plates which were coated with a desmosine-gelatin conjugate. And the assay system is based on an inhibition immunoassay. With this assay system, desmosine could be detected in a range between 0.4-400 ng/ml. Recovery of desmosine (DES) added to urine was 90.6-117.0% as measured by this method. Anti-DES antisera obtained from rabbits, showed no cross-reaction to 19 standard amino acids, two elastines nor mouse acetone liver power (which contained degradated elastin). But iso-desmosine cross-reacted with the antisera 13-45% at the isodesmosine concentration range of 40-400 ng/ml. Column purification of the urinary desmosine with CF-1 cellulose will not be necessary for desmosine measurement in ELISA assay. This paper described the detail procedures for sample preparation and the desmosine measurement in urine. Desmosine measurement can be an effective marker for screening the lung elastin degradation caused by cigarette smoking and environmental pollution to human lung.
