[A case of hepatoma treated successfully with a percutaneous intratumoral injection of CDDP-PC-lipiodol suspension].

Reported is a case that was treated successfully with a percutaneous intratumoral injection of a CDDP-PC-Lipiodol Suspension (CPLS) of the viable tumor cells of a hepatoma after arterial injection therapy. Although the size of the viable tumor cells remained almost unchanged, a marked reduction in the level of the plasma AFP (484 ng/ml-35 ng/ml) was obtained after the percutaneous injection. No major complication was encountered except for minimal vomiting and mild transient abnormalities of the liver functions.

[Assessment of reproducibility of transcranial Doppler velocimetry].

In recent years, transcranial Doppler velocimetry has been widely used as a noninvasive method for measurement of blood flow velocity in basal cerebral arteries. As for the reproducibility of this method, however, there have been only few reports so far. In the present study, we attempted to evaluate the reproducibility of this method by changing the examiners and the timing of the examination. Fifteen healthy volunteers were investigated by using a pulsed wave Doppler ultrasound device with a 2 MHz transducer. Both peak and mean spatial frequencies calculated from the frequency spectrum of the Doppler signals from the middle cerebral artery (MCA) and the basilar artery (BA) were used as blood flow parameters for assessing the reproducibility. The arterial blood pressure and the end-tidal carbon dioxide partial pressure (PECO2) were also monitored during the ultrasonic examinations. The intra- and inter-observer reproducibilities were evaluated by obtaining the coefficient of variation (CV) of the differences and/or analyzing the correlation between the values from the two measurements by the same examiner and by two separate examiners, respectively. The intraobserver reproducibility in separate days was also evaluated. Both in the mean arterial blood pressure and in PECO2, no significant changes were observed between the two measurements in each subjects. In the present examination, the CV and r values ranged from 6.7 to 19.5% and from 0.69 to 0.95, respectively, showing good reproducibility of the transcranial Doppler method for measurement of blood flow velocity, though some differences were observed as follows. The intraobserver reproducibility was better than the interobserver reproducibility.(ABSTRACT TRUNCATED AT 250 WORDS)

[Phrenic nerve conduction disorder after open heart surgery--electrophysiological study].

In 32 consecutive adult patients undergoing heart surgery, the induced diaphragmatic muscle action potential was measured. Phrenic nerve conduction disorder was defined as disappearance of muscle action potential (Edi) and conduction time (CT). Phrenic nerve conduction disorder was observed in 10 patients (31%); 8 patients on the left side and 2 patients on both sides. In non-conduction disorder group (22 patients), Edi and CT were measured. Edi of the right side decreased significantly from preoperative value of 705 +/- 318 microV to 445 +/- 285 microV at 1-3 days after operation (stage I) and to 559 +/- 314 microV at 7-10 days after operation (stage II) (p less than 0.05). CT of the right side prolonged significantly from 7.1 +/- 0.7 msec before operation to 7.44 +/- 0.97 msec at postoperative stage I and to 7.40 +/- 0.21 msec at postoperative stage II (p less than 0.05). For the left phrenic nerve, Edi showed significant (p less than 0.05) decrease from 803 +/- 338 microV before operation to 429 +/- 251 microV at the postoperative stage I and 620 +/- 350 microV at the postoperative stage II. In the conduction disorder group, incidence of atelectasis, diaphragm elevation and pleural effusion as documented by chest roentgenographic findings were higher than those of non-conduction disorder group (p less than 0.01). Moreover, the lowest temperature of the myocardium during operation was significantly (p less than 0.05) lower for conduction disorder group as compared to non-conduction disorder group. We believed that it is necessary to develop a innovative method for preventing the phrenic nerve from cold injury.

[A case of massive hepatoma which responded to SMANCS/Lipiodol regimen with intra-arterial infusion].

Transcatheter arterial chemotherapy (SMANCS/lipiodol) was applied to massive hepatoma, which had a high AFP 213,000 ng/ml, A-P shunt, tumor thrombosis and metastatic lung cancer. After 3 months, the AFP value reduced to 18 ng/ml, massive hepatoma and the A-P shunt disappeared, but AFP-negative nodular hepatoma recurred around initial hepatoma. Each time, we injected SMANCS/lipiodol to the recurring hepatoma. The therapy in the initial stage was not so effective. The portal vein was not observed in the initial stage, but appeared after the second dosage. Metastatic lung cancer was declining in the initial dosage and 23 months later disappeared after the third dosage. The massive hepatoma occupied entirely the rt. lobe of the liver. The patient lived for 4 years, had total admission periods of 190 days and could return to life in society. In this case, we considered that transcatheter arterial chemotherapy (SMANCS/lipiodol) had remarkable effects.

[Principle and therapeutic effect of lipophilic anticancer agent [SMANCS/lipiodol]: selective targeting with oily contrast medium].

Lipiodol, an oily contrast medium, is utilized to deliver the anticancer agent SMANCS to the target tumor in which the tumor selective delivery of 2,500 fold more than plasma was confirmed with prolonged retention in the tumor tissue. This unique tumor targeting is accomplished by the arterial injection of the oily formulation of the drug. The method utilizes unique vascular properties of tumor tissue. SMANCS is a derivative of neocarzinostatin conjugated with copolymer of styrene and maleic acid. It has much propronounced lipophilicity, stability against various harsh environments and exerts a potent cytotoxicity. Therapeutic effect of the drug to unresectable primary hepatoma is much better than the conventional method. For Child A category patients with intrahepatic metastasis in no more than three area, a 3 yr survival rate is more than 87%. When the Child's A and B are combined with no distant metastasis, 1-, 2- and 3-year survival rates are 87%, 50%, and 35%, respectively. The side effect of this treatment [SMANCS/Lipiodol, i.p.] is minimal; transitory low grade fever is the commonest one (40-50% of cases) which can be controlled by a routine protocol. No liver or marrow toxicity was observed. Procedural limitations for the lung cancer etc. are discussed.

Macroscopic changes seen in human erythrocytes heated in a suspension and the resulting effect on their blood group activity.

Changes in the macroscopic appearance and in the activity of various erythrocyte antigens were observed by heating human erythrocytes of known blood groups at 60-260 degrees C for 15, 30, and 60 min in a suspension that was sealed in an airtight test tube. While the erythrocytes underwent severe hemolysis after heating at 60 degrees C, this phenomenon was not macroscopically observed on heating at 80 and 100 degrees C and the cellular sludge in the tested became brown in color. With further temperature increases, the supernatant became pale yellow at 120-140 degrees C, brown at 160 degrees C and dark brown at 180 degrees C and above, whereas the sediment hardened and became darker in tone. Both A and B activity in the cellular sludge remained active until a 30-min heating at 240 degrees C, as did the O(H) activity until a 15-min heating at 220 degrees C. Both Lewis type activity was detected until a 30-min heating at 200 degrees C and a 15-min heating at 220 degrees C. The P1 activity remained specific after a 30-min heating at 180 degrees C and false-positive reactions were observed after 60 min at 180 degrees C. Among the Rh antigen activity, D activity remained until a 60-min heating at 140 degrees C, C and E until a 30-min heating at 140 degrees C, c until a 15-min heating at 140 degrees C and e until a 30-min heating at 100 degrees C.(ABSTRACT TRUNCATED AT 250 WORDS)

A new method for the determination of ethanol in the blood and urine by pulse heating.

We have established a new method for the determination of ethanol and other gaseous substances in even minute amounts of biological material by utilizing a Curie-point pyrolyzer (Model JHP 3, Japan Analytical Industry) connected to GC or GC-MS. No pretreatment of the biological material is needed and the procedure is based on the principle that volatile or gaseous substances can be evaporated from biological materials by pulse heating and introduced for analysis directly to a GC or GC-MS. The blood from rats that had been administered ethanol-d6 orally, as well as blood and urine collected from healthy volunteers that had ingested alcoholic drinks and blood and tissue specimens taken from autopsied cadavers were examined for ethanol concentrations by both this new method and by the usual head space-GC method. To test this new method, one microliter of the blood or urine was injected into a tubularly folded ferromagnetic alloy (Pyrofoil) and heated pulsatively five times at 160 degrees C (pulse heating). The vapor that evaporated from the sample then was directly introduced to the GC or GC-MS, and the time required to complete the analysis took approximately 5 min. In parallel, for comparison purposes, portions of the same samples (0.2-0.5 g) were examined by means of the head space method as well. The measured values of ethanol-d6 and ethanol in the rat blood and in the human whole blood, blood plasma, and urine that were obtained by this pulse heating procedure correlated almost perfectly with those obtained by head space method. These results show the accuracy and reproducibility of this new method.(ABSTRACT TRUNCATED AT 250 WORDS)

Applicability of an immuno-microsphere technique for a forensic identification of ABO blood types: the use of fluorescent microspheres.

In order to develop a direct micro-method for the ABO blood typing of forensic samples, an attempt has been made to utilize fluorescent immuno-microspheres. Microspheres (Covaspheres MX and CX particles, Duke Scientific) were coupled with partially purified antibodies from commercial mouse monoclonal anti-A, anti-B, and anti-H reagents, as well as with affinity purified UEA-I. The reactivity and specificity of the immuno-microspheres were checked with fresh erythrocytes of known blood groups, after which the spheres then were applied to the typing of hemolyzed and thermo-changed erythrocytes and bloodstains. The microspheres coated with the monoclonal antibodies and the UEA-I showed specific and distinct reactions with fresh, hemolyzed and thermo-changed erythrocytes, and bloodstains. Further, by combining particles labelled with different dyes, the possibility of a simultaneous double-labelling of the group antigens was indicated on the fresh cells and on the hemolyzed and thermo-changed cells and cell fragments.

[Three cases of endobronchial hamartoma].

We reported three cases of endobronchial hamartoma. There were some difficulties in diagnosis of the disease by bronchoscopic examination, because the tumors were covered with normal bronchial epithelium. But X ray film and CT film contributed to the diagnosis when calcification of intra-bronchial tumor was observed. Three different modes of operation were performed as extirpation of tumor by wedge resection of upper lobe bronchus, sleeve resection of left S6 segmental bronchus, and right middle lobectomy.

[Application of omental pedicle to tracheal allotransplantation: an experimental study using adult mongrel dogs].

Methods and results of experimental tracheal allotransplantation using adult mongrel dogs are described in this paper. Omental pedicle flap was used to reconstruct the vascularity of ischemic tracheal allograft. Twenty out of 22 allografts were found to be viable with almost normal tracheal structure and no granulation or stricture. The allografts were proved to be well revascularized from the omental pedicle flap by injection of Indian ink from the right gastroepiploic artery.

[Evaluation of cerebral hemodynamics by 123I-IMP in cerebral AVMs before and after operation by using SPECT].

To evaluate cerebral hemodynamics by N-isopropyl-p-[123I]-iodoamphetamine (123I-IMP) preoperative and postoperative (after 1 day, and after 7-10 days) regional cerebral blood flow scan, 6 arteriovenous malformations (AVMs) which performed total removal operations were studied by using a ring type SPECT "HEADTOME". We performed the dynamic scan just after the intravenous injection of 222 MBq (6 mCi) of 123I-IMP, then the static scan 20 minutes after the intravenous injection. In preoperative dynamic scans of all cases, only the first frame image showed the increased activity on the nidus probably because of the blood pool. Preoperative static scans of all cases showed the remarkable decreased activity on the nidus, and decreased activity surrounding the nidus probably because of the peripheral steal phenomenon. In postoperative scan of the next day, 4 out of 6 cases showed the transient decreased peripheral steal, particularly 2 out of those 4 cases showed the transient hyperperfusion probably because of the normal perfusion pressure breakthrough (NPPB). And, 2 out of 6 cases showed transient peripheral low perfusion on much larger area than those of the preoperative scans probably because of the focal brain damages and edemas. We conclude that 123I-IMP SPECT on AVM is very useful to decide the indication of the removal operation, and to estimate the postoperative risk, and to evaluate the preoperative and postoperative cerebral hemodynamic changes in the peripheral area of AVM.

[SPECT measurement of cerebral hemodynamics in transient ischemic attack patients--evaluation of pathogenesis and detection of misery perfusion].

To evaluate the cerebral hemodynamics and the pathogenesis by regional cerebral blood flow (rCBF) and regional cerebral blood volume (rCBV), 42 transient ischemic attack (TIA) patients and 9 normal volunteers were studied using SPECT. We classified these patients into Group A (n = 23: no occlusion or stenosis of the internal carotid or middle cerebral artery; non large vessel disease) and Group B (n = 19: chronic occlusion or severe stenosis of the internal carotid or middle cerebral artery; large vessel disease). We obtained rCBF with 133Xe inhalation and rCBV with 99mTc-red blood cells. Of 9 normal volunteers aged 43-70 yr (mean age 59.8 +/- 8.3 yr), the mean rCBF was 45.8 +/- 5.1 (ml/100 g brain/min), the mean rCBV was 4.0 +/- 0.4 (ml/100 g brain). The examination was done by comparing the values of the affected hemispheres of Group A and Group B patients with the mean rCBF and the mean rCBV of normal volunteers. Eleven out of Group A patients and 15 out of Group B patients showed decreased rCBF. But of those patients, no patients of Group A showed increased rCBV and 6 out of 19 Group B patients showed increased rCBV. Thromboembolic mechanism which is of Group A patients and Group B patients without increased rCBV, and hemodynamic mechanism which is of Group B patients with increased rCBV were considered as the main cause of TIA. Decreased rCBF and increased rCBV in Group B patients can be assumed as the misery perfusion as reported in PET studies.(ABSTRACT TRUNCATED AT 250 WORDS)

[Surgical treatment of interrupted aortic arch in infants under three months of age].

From March, 1982, through March, 1988, 8 infants less than 3 months of age underwent surgical treatment of interrupted aortic arch. Five patients had IAA type A and 3 patients had type B. Seven patients with associated VSD underwent staged operations and 1 infant with aortopulmonary window underwent primary repair off cardiopulmonary bypass. Aortic arch reconstruction was by subclavian aortic anastomosis (6) or large tube graft (8 mm) interposition (2) combined with pulmonary artery banding (5). All seven patients with associated VSD survived the first-stage operation. One patient subsequently underwent pulmonary artery banding, two underwent patch aortoplasty and four patients underwent intracardiac repair and removal of a pulmonary artery band, with six survivors (86%). The operative death occurred in an infant in whom modified Damus-Kaye-Stansel operation was carried out for severe subaortic stenosis, which rerouting all left ventricular blood through VSD and the main pulmonary artery into the ascending aorta and reconstructing right ventricular-distal pulmonary artery connection by a valved external conduit. All six surviving patients are doing well at present (mean follow up of 3 years) without any significant pressure gradient between the ascending and thoracic aorta. Based on these data we conclude: (1) Aortic arch reconstruction and pulmonary artery banding can be reliably performed even in critically ill infants with IAA and isolated VSD. (2) The primary repair will provide better chance of survival in infants with IAA associated with significant LVOTO, truncus arteriosus or aortopulmonary window that do not readily lend themselves to pulmonary artery banding.

[Relationship between an inclination of mandibular plane and a morphology of symphysis].

In order to investigate the relationship between inclination of mandibular plane and morphology of symphysis, 225 adult subjects' cephalograms were measured. Measurements of the mandibular and facial morphology were: SN-MP, gonial angle, SNA, SNB, SNP, ramus angle, SN-GN, L1 to MP, U1 to L1 angle, overbite, and overjet. Measurements of the symphysis were: thickness of symphysis, length of symphysis, height of symphysis, grade of mental protrusion, inclination of symphysis, curve of symphysis, and sectional area of symphysis. From these measurements, the inclination of mandibular plane and morphology of symphysis were compared. Then the following results were found: 1. There is negative correlation between mandibular plane angle (SN-MP angle) and thickness of symphysis at point B. There is more negative correlation between mandibular plane angle and thickness of symphysis at the apex. 2. There are no correlations between mandibular plane angle and thickness of symphysis at the base. 3. There is positive correlation between mandibular plane angle and height of symphysis. There is positive correlation between mandibular plane angle and length of symphysis. 4. There is positive correlation between mandibular plane angle and grade of mental protrusion. 5. There is a slightly positive correlation between mandibular plane angle and inclination of symphysis. There is a slightly positive correlation between mandibular plane angle and curve of symphysis. 6. There are no correlations between mandibular plane angle and sectional area of symphysis (bone volume of symphysis).

[Clinical study of norfloxacin in the treatment of acute epididymitis].

The efficacy and safety of Norfloxacin were studied in the treatment of 20 patients with acute epididymitis. Norfloxacin was orally administered at a dose of 200 mg 3 times a day for 14 days. Clinical efficacy rate on the 7th day was 95% (19/20), excellent in 9 cases, moderate in 10 cases and poor in 1 case, and on the 14th day was 95% (18/19), excellent in 12 cases, moderate in 9 cases and poor in 1 case. On the 14th day, fever, pain and swelling had disappeared in 8 cases (in 2 cases on the 7th day). The efficacy on the 14th day was further investigated compared to that on the 7th day. Before treatment with Norfloxacin, in 13 of the 20 patients, pyuria was observed. Cultivating the bacteria was isolated in 4 of the 13 patients. Norfloxacin remarkably affected the treatment of these patients with pyuria and bacteriuria. In the treatment with Norfloxacin, the count of leukocytes, erythrocyte sedimentation rate and CRP were obviously improved. Side effects and abnormal clinical laboratory findings were not observed. From these results, Norfloxacin 600 mg/day, t.i.d was considered useful and safe in the treatment of acute epididymitis.

Tumoritropic and lymphotropic principles of macromolecular drugs.

The advantages and disadvantages of macromolecular drugs, particularly on synthetic polymer-protein conjugates, are described in this article. Improvements in protein drugs, after tailoring with polymers, are as follows: increased plasma half-life, loss of antigenecity, lymphotropism, and, especially, preferred tumor-targeting efficiency and long-term retention in the tumor tissues. Hydrophobic character can make a drug also possible for oily formulation. Explained are the rationales of macromolecular drugs to preferential delivery to the tumor and lymphatic tissues based on our finding on pathological/anatomical differences of the blood and lymphatic vasculatures. Enhanced vascular permeability, which facilitates the macromolecular drug-leakage out of the blood vessel, is discussed. A model compound, which is discussed in detail, is smancs--styrene-co-maleic acid conjugated neocarzinostatin (MW 16 K). Some data on polymer-conjugated enzymes as potential therapeutic agents are described as well.
